졸중풍(卒中風) 후유증(後遺症)에 의한 운동장애(運動障碍) 환자(患者)의 치료(治療)를 위한 통용방(通用方)으로서 만금탕(萬金湯)과 보양환오탕(補陽還五湯)의 효과(效果)에 관한 비교(比較) 연구(硏究) (Comparative Study of the Effect of Man Geum Tang and Bo Yang Whan O Tang as a General Prescription for the Treatment of Motor Disturbance in Sequelae of Cerebrovascular Disease)

  • Cerebrovascular disease(CVD) is marked by loss of consciousness. facial palsy and hemiparesis. Although being treated properly in acute stage. CVD is accompanied by various sequelae. So treatment of sequelas is great importance as much as in acute stage. Man Geum Tang(MGT) and Bo Yang Whan O Tang(BYWOT) are representative general prescription for sequelae of CVD. This study is to clarify comparatively the efficiency of MGT and BYWOT. 110 Patients who were administrated with either MGT or BYWOT over 2 weeks for treatment of CVD divided into 2 groups:53 patients(27 males and 26 fomales. average $63.58{\pm}9.48$ years old) adminstated with MGT and 57 patients (32 males and 25 females. average $65.84{\pm}8.93$ years old) with BYWOT. In the patients administrated with MGT. 15 patients were with cerebral hemorrhage and 38 patients were with cerebral infarction. 57 BYWOT group consisted of 27 patients with cerebral hemorrhage and 30 patients with cerebral infarction. MGT with cerebral hemorrhage and 30 patients with cerebral infarction. MGT was administrated for $15.18{\pm}2.43$days and BYWOT for $14.24{\pm}1.24days$. Motor improved significantly(9〈0.005) from grade after administration in upper and lower extremities respectively. In view of comparative effeciency. the grade of motor disturbance after administration in upper extremities showed insignificant(p〉0.005) change to $1.19{\pm}1.18\;and\;0.89{\pm}1.13$ in MGT and BYWOT group respectively. In lower extremities. the grade changed insignificantly (p〉0.05) to $1.26{\pm}1.08\;and\;1.18{\pm}1.10$ respectively. In the patients group with cerebral infarction. the grade of motor disturbance in MGT and BYWOT administration groups respectively were $0.84{\pm}0.14\;and\;0.60{\pm}0.15$ in upper exemities and $0.89{\pm}0.14\;and\;0.90{\pm}0.17$ in lower extremities. The significant difference between two groups didn't appeared. In cerebral hemorrhage patients the grade of motor disturbance were in upper extremities $2.07{\pm}0.33$ in MGT group and $1.59{\pm}0.29$ in BOWOT group and $2.20{\pm}0.26\;and\;1.59{\pm}0.28$ in lower extremities respectively. The difference between two groups were insignificant(p〉0.05).

편마비 환자의 양쪽 하지 테이핑 적용이 균형 및 보행에 미치는 영향 (The effect on balance and gait when applying Kinesio Taping to the lower extremities of hemiplegic patients)

  • Purpose: The purpose of this study is to understand the effect of balance and gait when simultaneous application of Kinesio Taping on the lower extremities is done to improve the pattern of hemiplegic patients asymmetric lower extremity. Methods: This study was performed with 7 patients who have hemiplegia symptoms due to stoke. The taping on the paralyzed side was done on tibialis anterior, gluteus medius, gluteus maximus and psoas major and the taping on the unparalyzed side was done on the calf muscle and quadriceps. Berg Balance Scale(BBS) and Time Up and Go Test(TUG) was used to assessment the balance and gait of the patient after the kinesio taping was done on the lower extremities. The results of the test was analyzed by Wilcoxon Signed Rank Test on SPSS(Ver. 10.1). Results: When the taping was done on the paralyzed side, there was a significant improvement of balance and gait in BBS and TUG. When the taping was done on the lower extremities, it showed significant improvement of balance and gait, compared to when the taping was only done on the paralyzed side, according to BBS and TUG. Conclusion: The result of this study shows that the simultaneous taping of the lower extremities of the hemiplegic patients improves balance and gait by improving the pattern of patients lower extremities.

  • The periodic limb movement (PLM) disorder is a disease of motor sign mainly in the lower extremities, whereas the restless leg syndrome (RLS) accompanies sensory symptoms in the lower extremities. These two disorders may occur in the one patient, which implies possible common pathophysiological background in those disorders. The aim of this article is to review the clinical features, diagnostic criteria, electrophysiological characteristics of the two disorders and their relation to neurological disorders.

  • This paper describes an improved FES system to restorp paralyzed extremities in spinal-cord-injured patients. By using time-division method in the proposed FES system, we can reduce the size and weight of system despite of increasing channels. We designed a DC-DC converter to adapt the condition of each patient. We could reduce the cost by using easily available apparatus like an IBM-PC for the stimulus-pattern-creating system and standardized parts for the protable FES system.

  • The tube voltage in radiographic technique factors of the extremities was studied to use the acryl phantom and aluminum step wedge. It was the proper tube voltage that was over 55-60kV in the finger, over 65kV in the forearm and over 75kV in the knee joint.

폐실질내 신경초종 (A Case Report of Intrapulmonary Schwannoma)

  • Schwannomas are rare and distinctive nerve sheath tumors. They occur commonly along the course of central and peripheral nerves, especially on the skin of the head, neck and extremities. Histologically, they are characterized by Antoni A and Antoni B tissues of high and low cellularity. 2 cases of intrapulmonary Schwannomas were reported worldwide by 1980. We report a case of twenty seven-year-old male patient with intrapulmonary Schwannoma, who had multiple nodules on the skin of the both upper extremities and scrotum with their characteristic findings.

  • A retrograde intravenous regional anesthesia was applied to the extremities of 30 cows, which was showed that mean value of induction time was 13.5 seconds, duration time 74.4 minutes, frequency of injection until to keep needle into the vein 1.27, and didn't observed local and systemic complications. Under the retrograde intravenous regional anesthesia, it was possible for surgical treatment of 20 cows with various foot disorders, to resect soft tissues of extremities and claw horns, to sexarticulate 2nd phalanx without pain reactions.

The Effects of Extensor Pattern Position and Elastic Taping of Non-Dominant Hand on the Grip Strength of Dominant Hand

  • Grip strength is an objective indicator for evaluating the functional movement of upper extremities. Therapists have been using it for a long time as an excellent barometer for evaluating the therapy process, therapeutic effects and prognosis of patients with injuries in upper extremities. This study investigated the effects of extensor pattern position and elastic taping of non-dominant hand on the grip strength of dominant hand among general adults. The subjects of this study were 23 males and 7 females from physical therapy departments of 3 Universities located in Busan who agreed to participate in the experiment and the resultant data were analyzed using SPSS version 12.0. The results of the study were as follows. First, there was a significant difference between the grip strength of dominant hand when the non-dominant hand was at the neutral position and that when the non-dominant hand was at the extensor pattern position and both hands were at the maximum strength simultaneously (Bonferroni-corrected p<.001). Second, there was a significant difference between the grip strength of dominant hand when the non-dominant hand was at the neutral position and that when the elastic taping of non-dominant hand was applied (Bonferroni-corrected p<.001). Third, there was no significant difference between the grip strength of dominant hand when the non-dominant hand was at the extensor pattern position and both hands were at the maximum strength simultaneously and that when the elastic taping of non-dominant hand was applied. The irradiation effects through the extensor pattern position of non-dominant hand and application of the elastic taping to non-dominant hand showed significant results in improving the maximum grip strength of dominant hand. This finding could be suggested as the probability for the indirect treatment of the upper extremities of hemiplegia and orthopedic patients due to the long-term fixing of upper extremities.

낙상 사고에 의한 골절로 입원한 노인 환자에 대한 조사 연구 (A Study on the Elderly Patients Hospitalized by the Fracture from the Fall)

  • Purpose: To identify age, gender, medication, seasons and place of fall, and areas of the fractures from the fall among the hospitalized elderly patients in order to provide the basic data for future fall prevention program for the elderly. Methods: This study was conducted for 106 elderly patients admitted into a university hospital by fractures from the fall during the period from January 1, 1999 to December 31, 1999. Data on the age, gender, medication, season and place of the fall, areas of the fracture were collected based on their medical records. Result: The age range of the subjects were from 60 to 96 years old. The subjects were aged between 60-69 years old 49(46.2%), between 70-79 years old 31(29.2%), between 80-89 years old 24(22.6%), and over 90 years old 2(1.9%). Male patients comprised was 34(28.3%), while female patients comprised 76(71.7%). The fall occurred in Winter most frequently 34(32%). The place of the fall included room 81(76.4%), streets 13(12.3%), bathroom 6(5.7%), stair 4(3.8%), and mountain 2(1.9%). Twenty-two subjects (20.8%) had medication regularly, while 84 subjects (79.2%) had no medication. The areas of the fracture from the fall included upper extremities 20(18.9%) and lower extremities 86(81.1%). Radius fracture (7.5%) was the area where the fracture occurred most frequently in upper extremities and femur fracture (52.8%) was the area where the fracture occurred most frequently in lower extremities. A significant difference was found in the fracture area by age, season and place of the fall (p<.05). No significant difference was found in the fracture area by gender and medication. In all age groups, seasons and places of the fall, occurrence of fracture in lower extremity was significantly higher than that in upper extremity.
