한국노인여성의 검열과 안구돌출의 상관관계 (Correlation between Palpebral Fissure and Exophthalmos of The Elder Women in Korea)

  • Purpose: This study was to investigate the relationship between the palpebral fissure and the exophthalmos in an elder women population in Korea subjects with normal eye status. Methods. From November 2014 to July 2015, The author analyzed the morphological values in 81 Korean elder women population. The clinical test was performed using the Hertel exophthalometer and verner califer. Analysis of data were described using the spss 20 and Pearson's test. Results. The mean age of the 81 subjects (162 eyes) was (73.938+/8.585)mm. The mean value of palpebral fissure was (5.673+/-2.065)mm in width and (24.179+/-4.409)mm in height. The mean value of exophthalmos was (5.537+/-2.151)mm. There were a significant relationship(P>0.01) in the mean size between the width and height in palpebral fissure. Also, there were a significant relationship(P>0.01) in the mean size between the palpebral fissure width and exophthalmos. Also, there were a significant relationship(P>0.01) in the mean size between the palpebral height width and exophthalmos. However, there were not significant relationship(P>0.01) in the mean size between the palpebral fissure width and exophthalmos in right eye. On the other hand, The relationship between right eye and left eye of the palpebral fissure and exophthalmos was similar results. Conclusions. In this study, these results suggested that the palpebral fissure and exophthalmos was a significant relationship in the mean value of morphologic characters in Korean elder women population.

  • Purpose. This study was to investigate the relationship between the palpebral fissure size and the exophthalmos in a population of Korean adult women subjects with normal eye status. Methods. From November 2012 to September 2013, The authors analyzed the morphological values in 73 Korean adults population. The test was performed using the Hertel exophthalmometer and verner califer. Analysis of subjects data were recorded using the spss version 20.0 a statistical program (IBM Co, Armonk, NY USA). Results. The average age of the 73 subjects (146 eyes) was (21.219+/-0.261)mm. the average value of palpebral fissure was (25.110+/-1.646)mm in width and (8.096+/-1.464)mm in height. The average value of exophthalmos was (15.002+/-1.922)mm. There was a significant relationship(P>0.05) in the average size between the width and height in palpebral fissure. Also, there was a significant relationship(P>0.05) in the average size between the palpebral fissure width and exophthalmos. On the other hand, there was a significant relationship(P>0.01) in the average size of palpebral fissure height and exophthalmos. Conclusions: In this study, these results suggested that the palpebral fissure and exophthalmos had a significant relationship in the average value of size in Korean adult women population.

안구 돌출을 동반한 비부비동의 미만성 큰 B세포 림프종 1예 (A Diffuse Large B-Cell Lymphoma in Nasal Sinuses Accompanying with Exophthalmos:A Case Report)

  • Lymphomas of the nasal sinuses are relatively uncommon and represent less than 1% of all head and neck malignancies. Most common symptoms are nasal obstruction, epistaxis, rhinorrhea. We report a case of primary extranodal diffuse large B-cell non-Hodgkin's lymphoma in nasal sinuses accompanying with exophthalmos. A 70-year-old male patient was referred to our hospital with exophthalmos and severe septal deviation. We conducted endoscopic biopsy with septoplasty and biopsy result was diffuse large B-cell lymphoma. He received chemotherapy(R-CHOP) and radiotherapy. At follow-up, he remained free of disease.

두개조기유합증, 안와격리증, 안구돌출증 환자의 동시 교정 치험례 (Experience of Single Stage Treatment of Caniosynostosis, Hypertelorism, Exophthalmos Patient)

  • The simultaneous correction of the hypertelorism and exophthalmos combined with craniosynostosis is very rarely performed operative procedures in the world. The craniosynostosis is the congenital anomaly that designates premature fusion of one or more sutures in either cranial vault or cranial base. Hypertelorism is not a distinct clinical syndrome in itself, but is a physical finding secondary to facial and cranial maldevelopment and it is defined as a increase in the distance between the medial orbital walls. Exophthalmos can occur following the decrease in the size of the orbit in patients with developmental skeletal disorders such as craniofacial synostosis. The authors experienced 9-year-old male patient, who has complex cranio-facial abnormality. The craniosynostosis was oxycephaly type and primary fronto-orbital advancement surgery had been performed in other hospital. The abnormal cranial vault combined with hypertelorism and exophthalmos due to maldeveloped both orbital walls. Surgical correction was obtained by various cranio-fronto-orbital remodeling technique such as calvarial bone craniotomy, fronto-orbital advancement, paramedian resection, medial canthopexy, Tessier-Wolfe three wall orbital expansions. We achieved a quite satisfactory result both functionally and aesthetically in a complex cranio-facial deformity patient by combination and modification of previously developed various cranio-facial plasty technique and hereby report the case with brief discussion and review of literature.

안와내 가성종양 1예 - 증 례 보 고 - (A Case of Intraorbital Pseudotumor - Case Report -)

  • Intraorbital pseudotumors are inflammatory lesions of the orbit of unknown etiology, which initially mimic an orbital neoplasm. The authors report a case of intraorbital pseudotumor presenting with painful exophthalmos. A 45-year-old male patient had a two-week history of acute progressive exophthalmos and diplopia. Right orbit was explored through transcranial route and the mass was biopsied, which revealed dense inflammatory cells and fibrosis. He subsequently underwent corticosteroid therapy and symptoms improved gradually. The literatures were reviewed.

Infantile Dural Arteriovenous Fistula of the Transverse Sinus Presenting with Ocular Symptoms, Case Reports and Review of Literature

  • Dural arteriovenous fistula (DAVF) of the transverse sinus with ophthalmic manifestations in young children are rare. We reviewed two cases of direct AVF of the transverse sinus with ocular manifestations managed at our institution. The first, a 2.5 years old male child presented with left exophthalmos. Angiography revealed AVF between the occipital artery and the transverse sinus. The second, a 2 years old female child, complained of left exophthalmos. Imaging studies showed bilateral direct AVFs of the transverse sinus with bilateral dysmaturation of the sigmoid sinus. Transarterial embolization was done in both cases. Clinical and radiological follow up revealed complete cure. This report suggests that DAVF of the transverse sinus supplied by the external carotid branches can present with ophthalmic manifestations especially if there is distal venous stenosis or obliteration involving sigmoid sinus. Transarterial embolization using coils and liquid embolic agents could be safe and feasible to obliterate the fistula.

Intraorbital Encephalocele Presenting with Exophthalmos and Orbital Dystopia : CT and MRI Findings

  • A 15-year-old female patient with progressive pulsatile exophthalmos caused by intraorbital encephalocele was evaluated with computed tomography (CT) and magnetic resonance imaging (MRI) in our clinic. She had no history of trauma or reconstructive surgery. When she was a little girl, she had undergone surgery for congenital glaucoma on the right eye. On the three-dimensional image of CT, a hypoplasic bone defect was observed in the greater wing of the right sphenoid bone. MRI and CT scan showed herniation through this defect of the arachnoid membrane and protruded cerebral tissue into the right orbita. Intraorbital encephalocele is an important entity that can cause pulsatile exophthalmos and blindness.

Salvage Approaches for Maintaining the Eye Globe in a Glaucomatous Patient with Severe Unilateral Exophthalmos

  • An eight-year-old Shih-Tzu dog was referred to Kyungpook National University Veterinary Medical Teaching Hospital because of severe glaucoma and exophthalmos. The symptoms included severe buphthalmos, conjunctival hyperemia, focal dried cornea and discomfort in the affected eye. Although enucleation was considered as a potential intervention measure in light of the severity of symptoms, it was possible to save the globe and relieve symptoms through evisceration, intraocular prosthesis implantation, and lateral canthoplasty. Our experience with this case suggests that symptomatic salvage therapy, rather than enucleation, is an appropriate approach for resolution of exopthalmos and other complications associated with glaucoma surgery.

Computed tomographic features of third eyelid gland adenocarcinoma in a dog: a case report

  • A 15-year-old Schnauzer, showing right exophthalmos, was diagnosed as adenocarcinoma originated from the third eyelid gland. On computed tomography, a normal right third eyelid gland was not observed. Instead, a heterogeneous cystic mass invaded the retrobulbar space and displaced the eye globe dorsolaterally. In addition, lysis of the bony nasolacrimal duct was found, which was considered the tumor invasion. These findings indicated that third eyelid gland adenocarcinoma should be considered when a retrobulbar mass is found ventromedial to the globe without observation of the normal third eyelid and accompanies osteolysis of the bony nasolacrimal duct in dogs showing exophthalmos.

Reconstruction of Large Orbital Floor Defect Caused by Maxillary Sinus Mucocele

  • Maxillary sinus mucocele can occur due to many medical factors such as chronic infection, allergic sinonasal disease, trauma, and previous surgery. However, it occurs mainly after Caldwell-Luc operation, usually more than 10 years after surgery. There are a few cases of maxillary sinus mucocele with ocular symptoms. Also, a case causing ocular symptoms because of invasion to the orbital floor is rare. Therefore, we report a case of a 55-year-old male patient who underwent Caldwell-Luc operation about 30 years ago. Then, symptoms such as exophthalmos, diplopia, and visual disturbance developed suddenly 3 months prior to admission. Computed tomography showed a cyst invading the orbital floor which resulted in eyeball deviation. The orbital floor defect measured approximately $2.5{\times}3.3cm$. Maxillary sinus mucocele was removed through an endoscopic approach. After this, we reconstructed the orbital floor through a subciliary incision. Observation was carried out after three years, and ocular symptoms such as diplopia and exophthalmos did not recur.
