A Case Report on Exfoliative Dermatitis (박탈성피부염(剝脫性皮膚炎) 증례(證例) 보고(報告) 1례(例))

  • Exfoliative Dermatitis is a name of a group of symptoms which is characterized by red rash, itching and scale in almost whole body. The patients are suffered from hypoalbuminemia, general weakness and fever sign in addition to characteristic skin lesions. So in the treatment of this disease, adequate symptomatic treatment is important with adequate topical skin management. We experienced a 69 year old Korean male patient diagnosed as Exfoliative Dermatitis based on clinical symptoms and a medical certificate. We concluded that the symptoms of this patient are Sun-symptoms(癬症) and gave him Bangpungtongsung-san(剝脫性皮膚炎) and Geunsun-bang(乾癬方). After taking Oriental Medicine(the hebal medicine of internal use), clinical improvement was noticed. Based on this experience, internal use of Oriental Medicine have a good effect in Exfoliative Dermatitis.

  • Exfoliative dermatitis is a name of a group of symptoms which is characterized by red rash, itching and scale in almost whole body. The patients are suffered from hypoalbuminemia, general weakness and fever sign in addition to characteristic skin lesions. So in the treatment of this disease, adequate symptomatic treatment is important with adequate topical skin management. We experienced a 67 year old Korean male patient diagnosed as exfoliative dermatitis based on clinical symptoms. We concluded that the symptoms of this patient are Sun-symptoms(癬症) and gave him Sunpunghwangi-san(宣風換肌散) and Chungdae-san(靑黛散). After taking herbal medicine, clinical improvement was noticed. Based on this experience, both internal and external use of oriental medicine have a good effect on exfoliative dermatitis.

  • Dapsone (4,4'-diaminodiphenylsulfone, DDS), a potent anti-inflammatory agent, is widely used in the treatment of leprosy and several chronic inflammatory skin diseases. Dapsone therapy rarely results in development of dapsone hypersensitivity syndrome, which is characterized by fever, hepatitis, generalized exfoliative dermatitis, and lymphadenopathy. Here, we describe the case of an 11-year-old Korean boy who initially presented with high fever, a morbilliform skin rash, generalized lymphadenopathy, hepatosplenomegaly, and leukopenia after 6 weeks of dapsone intake. Subsequently, he exhibited cholecystitis, gingivitis, colitis, sepsis, aseptic meningitis, disseminated intravascular coagulation, syndrome of inappropriate antidiuretic hormone secretion, pneumonia, pleural effusions, peritonitis, bronchiectatic changes, exfoliative dermatitis, and acute renal failure. After 2 months of supportive therapy, and prednisolone and antibiotic administration, most of the systemic symptoms resolved, with the exception of exfoliative dermatitis and erythema, which ameliorated over the following 4 months. Agranulocytosis, atypical lymphocytosis, aseptic meningitis, and bronchiectatic changes along with prolonged systemic symptoms with exfoliative dermatitis were the most peculiar features of the present case.

Study on Exfoliative Cytology of Keratinization and Morphology of Oral Mucosal Epithelium in Adult Diabetic Patients (당뇨병 환자에서의 구강점막각화도 및 세포상에 대한 박리세포학적 연구)

  • This study was undertaken to study the changes of epithelium of oral mucosa caused by diabetic disease in terms of keratinization and morphology of epithelial cells of oral mucasa, and to diagnose the oral diseases caused by systemic origns in terms of oral exfoliative cytology as a diagnostic tool. The author has studied the changes of epithelial cells on the cheek mucosa and upper antirior gingiva of 20 adult diabetic patients by Oral Exfoliative Cytology. And 50 healthy adults were selected as control group. The cytologic smears were stained by Papanicolaou method. The results were as follows : 1. In diabetic patient's cheek mucosa, Yellow staining cells were reveald as 11.8%, which was higher than the control group(4.2%). 2. In diabetic patients' upper anterior gingiva, Yellow staining cells were reaveale as 12.4%, which was lower than the control group(68.2%) 3. The changes of nucleus and cytoplasmic changes were not significantly different in diabetic patients and control group.

Detection of Second-Layer Corrosion in Aging Aircraft

  • The Compton backscatter technique has been applied to lap-joint in aircraft structure in order to determine mass loss due to exfoliative corrosion of the aluminum alloy sheet skin. The mass loss of each layer has been estimated from Compton backscatter A-scan including the aluminum sheet, the corrosion layer, and the sealant. A Compton backscattering imaging system has been also developed to obtain a cross-sectional profile of corroded lap-splices of aging aircraft using a specially designed slit-type camera. The camera is to focus on a small scattering volume inside the material from which the backscattered photons are collected by a collimated scintillator detector for interpretation of material characteristics. The cross section of the layered structure is scanned by moving the scattering volume through the thickness direction of the specimen. The theoretical model of the Compton scattering based on Boltzmann transport theory is presented for quantitative characterization of exfoliative corrosion through deconvolution procedure using a nonlinear least-square error minimization method. It produces practical information such as location and width of planar corrosion in layered structures of aircraft, which generally cannot be detected by conventional NDE techniques such as the ultrasonic method.

Detection and Identification of $\beta$-lactamase, Enterotoxin and Other Exotoxins Genes of Staphylococcus aureus by PCR

  • Staphylococcus aureus is a major pathogen for cattle, causing various forms of subclinical and clinical mastitis and could be a causative agent of food poisoning, it produces various superantigenic exotoxins which have a great public health significance. A total of 72 S. aureus clinical isolates from dairy farms located in Kyunggi Province Korea were examined for the species identification by biochemical method, and for the detection of $\beta$-lactamase, enterotoxin and other exotoxins genes by PCR. The results of species identification by biochemical method agreed with those of PCR done with species specific primer STA-AU. $\beta$-lactamase is an enzyme closely associated with the resistance to antibiotic penicillin, which is an important means of treatment of mastitis, all the isolates were positive for the presence of genes encoding $\beta$-lactamase, which were reproduced in penicillin susceptibility disc assay. Six types of toxin genes, Staphylococcal enterotoxin (SE)A, SEB, SEC, SEE, toxic shock syndrome toxin (TSST-1) and exfoliative toxin A (ET A) were detected in 72 isolates by PCR associated genotypic method in this study, none of the isolates carried the genes for enterotoxin D (SED) and exfoliative toxin B (ETB). The occurrence rate of exotoxin genes rated as 12.5%, and the precision of the PCR identification results has been confirmed using the reference strains.

Detection and characterization of potential virulence determinants in Staphylococcus pseudintermedius and S. schleiferi strains isolated from canine otitis externa in Korea

  • Background: A recent increase in the occurrence of canine skin and soft tissue infections, including otitis externa and pyoderma, caused by antimicrobial-resistant Staphylococcus pseudintermedius and S. schleiferi has become a significant public and veterinary health issues. Objective: We investigated the virulence potentials associated with the occurrence of canine otitis externa in S. pseudintermedius and S. schleiferi. Methods: In this study, the prevalence of genes encoding leukocidins, exfoliative toxins, and staphylococcal enterotoxins (SEs) was investigated using previously characterized S. pseudintermedius (n = 26) and S. schleiferi (n = 19) isolates derived from canine otitis externa. Susceptibility to cathelicidins (K9CATH and PMAP-36) and hydrogen peroxide (H2O2) was also examined in both staphylococcal species. Results: A high prevalence of genes encoding leukocidins (lukS/F-I, lukS1/F1-S, and lukS2/F2-S), exfoliative toxins (siet, expB, and sset), and SEs was identified in both S. pseudintermedius and S. schleiferi isolates. Notably, S. pseudintermedius isolates possessed higher number of SE genes, especially newer SE genes, than S. schleiferi isolates harboring egc clusters. Although no significant differences in susceptibility to K9CATH and H2O2 were observed between the two isolate groups, S. pseudintermedius isolates exhibited enhanced resistance to PMAP-36 compared to S. schleiferi isolates. Conclusions: These findings suggest that high a prevalence of various toxin genes together with enhanced resistance to cathelicidins may contribute to the pathogenicity of S. pseudintermedius and S. schleiferi in canine cutaneous infections.

Cytologic Findings of Colon Lavage Fluid in Colon Cancer (대장암에서 장세척물의 세포학적 소견)

  • Although exfoliative colonic cytology for the diagnosis of colorectal cancer has been largely abandoned due to the widespread use of colonoscopy, some authors still insiston the usefulness of colon lavage fluid. We tried evaluating the diagnostic feasibility of colon lavage fluid cytology using an orally administered balanced electrolyte solution. We collected colon lavage fluids in 106 patients prior to colonoscopy and reviewed the slides. Cytologic examination revealed neoplastic cells in 7 of 16(44%) cases of endoscopically proven adenocarcinoma patients. Therefore, we thin cytologic study of colon lavage fluid may be considered as one of the noninvasive diagnostic tools in colorectal cancer.

A Study of the Exfoliative Cytology of the Human Oral Mucosa of Cigarette Smoking (끽연이 구강점막에 미치는 영향에 관한 박리세포학적 연구)

  • The purpose of this study was observing the keratinization of human oral mucosa on smoking & non-smoking persons. The results are as follows : 1. In the oral mucosa of the smoker, it reaveald the increase of keratinization than the non-smoker. 2. The keratinization of the oral mucosa revealed the increase in proportion to the duration of smoking. 3. The keratinization of the oral mucosa increase in proportion to the amounts of smoking. 4. Between the male and the female in smoking, it didn't show the sensible difference in the keratinization of the oral mucosa. 5. Between smoking and non-smoking, there was not a sensible difference in the change of nucleus and cytoplasm of the oral mucosa.

  • Cytopathologic findings of two cases of adenoid cystic carcinoma of the trachea are reported. The carcinomas grew as an intratracheal mass. By bronchial washing, brushing and/or post-bronchoscopic sputum cytology, large cohesive sheets, lobulated clusters, or three dimensional ball-like structures were obtained. They had numerous cyst-like spaces containing characteristic globular basophilic material. The tumor cells were uniform and had a small amount of cytoplasm. Nuclei were small and hyperchromatic, Nucleoli were occasionally observed. The cytological diagnosis was confirmed by bronchoscopic biopsies. Since the cytomorphology of adenoid cystic carcinoma is characteristic, review of these cytologic features will enhance the diagnostic accuracy in exfoliative cytology of the respiratory tract.
