The Effect of Ginseng on Muscle Injury and Inflammation

  • The effect of Panax ginseng administration in muscle inflammatory process induced after eccentric exercise, that causes myofibrillar disruption, was studied. Changes in lipid peroxidation, inflammation, glycogen levels in muscle and release of myocellular proteins to blood were measured. The analyses were performed immediately after eccentric exercise and over week since this period are necessary for the muscle damage-repair cycle. The ginseng extract $(100\;mg\;kg^{-1})$ was orally administered to rats for three months, before the eccentric exercise performance. The results showed the protective role of ginseng against skeletal muscle damage. This effect could be associated with their membrane stabilising capacity since creatine kinase (CK) activity was significantly decreased 96 h post-exercise from $523{\pm}70\;to\;381{\pm}53$ and 120 h post-exercise from $443{\pm}85\;to\;327{\pm}75$ in treated animals. ${\beta}-glucuronidase$ activity, as indicator of inflammation, showed a significant reduction of about $15-25\%$ in soleus, vastus and triceps in these post-exercise times. The lipid peroxidation, measured by malondyaldehyde levels, was significantly decreased in the 24 h postexercise period in soleus and vastus intermedius muscles and on the recovery period. Finally ginseng administration reduced significantly the decrease of the glycogen levels immediately after exercise and when the regenerative process took place (72-168 h post exercise). Collectively, the results have showed that ginseng did not inhibit the vital inflammatory response process associated with the muscle damage-repair cycle but presumably ameliorate the injury.

사전 원심성 운동이 운동 유발 근 손상에 미치는 효과 (Effects of Pre-eccentric Exercise on Exercise Induced Muscle Damage)

  • Purpose: This study examined the effects of pre-eccentric exercise to the quadriceps for the prevention of delayed onset muscle soreness (DOMS) and recovery of muscular function, depending on the training intensities. Methods: Subjects were divided into one of three groups that control group, a low intensity eccentric exercise group (LIEE group) and a high intensity eccentric exercise group (HIEE group). Subjects who underwent pre-eccentric exercise undertook exercise at an intensity of 25% and 75% of maximal voluntary contractions, respectively. After undertaking pre-eccentric exercise for eight weeks, eccentric exercise was applied again to induce DOMS. Measurements were conducted to examine pain and muscular function changes before, immediately after, after the induction of DOMS, and at the first, third, fifth and seventh days after the induction of DOMS. Results: Subjects who underwent pre-eccentric exercise showed a significant difference from the control group for the changes in the visual analogue scale (VAS) pain threshold, pressure pain threshold and muscle thickness by isometric contraction from measuring DOMS, and in particular, pain threshold by isometric contraction had remarkable effect in the LIEE group of subjects. For the change of the root mean square values using mechanomyography (MMG) as the measurement of muscular function recovery, subjects who had undertaken pre-eccentric exercise group showed a significant difference as compared to the control group. Conclusion: Pre-eccentric exercise was very effective in preventing and recovering delayed onset muscle soreness and was helpful to prevent and recover from decreased muscular function. The difference based on the intensity of exercise was not great, but it was more effective in the low intensity eccentric exercise group of subjects. Therefore, it can be considered that pre-eccentric exercise has a high application value as a physical therapy intervention for prevention and rehabilitation of sports injuries.

The effect of aronia, montmorency, and Prunus mume supplementation on blood fatigue factors and muscle damage enzymes after running 10 km

  • The purpose of this study was to analyze the effects of intake of aronia, montmorency, and Prunus mume on 10 km endurance exercise by investigating changes in blood lactate, ammonia, creatine kinase (CK), and lactate dehydrogenase (LDH), and eventually to recommend them as ergogenic aids, which are effective for recovering from exercise fatigue and muscle damage. The subjects were divided into a P. mume intake group, an aronia intake group, a montmorency intake group, and a control group. Blood samples were taken from the brachial vein at rest, and at 0, 30 min, and 24 hr after running to measure recovery status. All data were analyzed by a two-way repeated-measures analysis of variance using SPSS. The results are summarized as follows: The changes in blood ammonia and LDH concentration following 10 km of running were not significantly different among the groups. However, lactate and CK recovery ratio increased significantly in the aronia and montmorency groups compared to those in the P. mume and control groups. In particular, CK concentration was significantly different in the montmorency group compared to that in the other groups. These results suggest that the effects of aronia and montmorency are associated with improved recovery from muscle fatigue and damage induced by highly intensive exercise, endurance exercise, or overtraining. In particular, montmorency is recommended as an ergogenic aid to inhibit muscle pain resulting from high-intensity endurance exercise.

과도 운동에 의해 유발되는 피로 및 골격근 산화적 손상에 대한 황기 다당체의 효과 (Effect of Polysaccharides from Astragalus membranaceus on Exercise-Induced Fatigue and Oxidative Damage in Skeletal Muscle in Exhaustive Exercise Animal Models)

  • Background: Astragalus membranaceus is a well known oriental medicinal herb. The polysaccharides of the aboveground parts (AMA) and the radix (AMR) of A. membranaceus are the most important functional constituents. Methods and Results: The aim of this study was to determine the effects of AMA and AMR on the oxidative damage induced in the skeletal muscle of rats subjected to exhaustive exercise. Sprague-Dawley rats were randomly divided into exercise and non-exercise groups; in the groups receiving the test compounds, AMA and AMR were administered orally for 30 days. Skeletal muscle samples were collected from each rat after running to exhaustion on a treadmill to determine the activities of superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), and catalase (CAT) and the concentation of malondialdehyde (MDA). The antioxidant enzyme activities of SOD and GSH-Px of skeletal muscle of AMA- and AMR-treated groups were significantly higher than those of the control and commercial sports drink (SPD)-treated groups in exhaustive exercise rats. In addition, MDA concentrations in the skeletal muscle of the AMA- and AMR-treated groups were significantly lower than those of the control and SPD-treated groups. In the present study, the effects of AMA and AMR on exercise endurance capacity were also evaluated in mice subjected to a swimming exercise test. AMA and AMR supplementation prolonged the swimming time of mice and enhanced exercise endurance capacity. AMA and AMR possess the ability to retard and lower the production of blood lactate, and prevent the decrease of serum blood glucose. Conclusions: These results showed that, AMR and AMA exerted beneficial effect in mice, increasing the activity of the antioxidant systems and inhibiting oxidative stress induced by exhaustive exercise. The compounds improved exercise performance and showed anti-fatigue effects against exhaustive exercise.

고강도 운동부하에 의한 흰쥐의 심장근육 손상에 대한 Salvianolic Acid B의 효능에 대한 연구 (Effect of Salvianolic Acid B on Cardiac Muscle Damage Following Exhaustive Exercise in Rats)

  • Objectives: This study examined the effects of salvianolic acid B (SAB) on exhaustive exercise-induced cardiac muscle damage to rats. Methods: The study was carried out with 12-week-old young adult male Sprague-Dawley rats. Thirty-six rats were divided into 3 groups; normal (n=12), exhaustive exercise group (ExS, n=12) and exhaustive exercise with SAB (ExS+SAB, n=12). Five days before exhaustive exercise, SAB were medicated for 5 days in ExS+SAB group. Rats in ExS and ExS+SAB group were forced to swim for 150 minutes and then they were sacrificed, while rats in normal group were sacrificed at rest. After that, blood was collected and cardiac muscle tissue damage indices were analyzed. Results: Serum aspartate transaminase activity and lactate dehydrogenase activity were significantly lower in ExS+SAB group than in ExS group. Serum creatine phosphokinase activity of ExS+SAB was significantly lower than ExS group. However, the content of serum creatinine had no difference between ExS and ExS+SAB group. In the H&E stained left ventricle myocardium, ExS group showed signs of myocardial damage such as sporadic fragmentation of myocardial fibers, interstitial edema, cytoplasmic eosinophilia and neutrophils infiltration. However, ExS+SAB group alleviated the severity of the signs of myocardial damage. In the myocardial dihydroethidium staining, optical density was remarkably decreased in ExS+SAB group compared to ExS group. Furthermore, the up-regulation of Bax/Bcl-2 ratio was observed in ExS+SAB group compared with ExS group. Conclusions: The above results suggest that SAB may protect cardiac muscle damage via antioxidant activity and prevention of apoptosis.

내리막 달리기 후 국소 근손상의 영상학적 비교분석 : 운동 강도의 영향 (Evaluating Quadriceps Muscle Damage after Downhill Running of Different Intensities using Ultrasonography)

  • 본 연구는 내리막 달리기(downhill running, DR) 후 초음파 영상분석을 이용해 대퇴사두근 무리(quadriceps group, QG) 내 근손상의 국소화 여부를 검증하고, DR 동안 운동 강도가 운동유발성 근손상(exercise-induced muscle damage, EIMD) 및 근육 반향세기 변화에 미치는 영향을 규명하려는 목적으로 수행되었다. 규칙적인 신체활동이 없는 건강한 남성 11명이 무작위 교차설계에 따라 서로 다른 강도[low-intensity DR session($50%HR_{max}$), LDR; high-intensity DR session($70%HR_{max}$), HDR]의 DR 운동을 수행하였다. DR 후 EIMD의 심각성은 혈청크레아틴 키나아제(creatine kinase, CK) 활성 수준 변화와 함께 신경근 기능 지수로서 무릎 신전근의 최대 수의적 등척성 수축(maximal voluntary isometric contraction, MVIC) 및 관절가동범위(range of motion, ROM) 변화를 통해 결정되었다. 회색조 분석을 적용한 근육 반향세기 평가는 DR에 따른 QG 내 국소 근육별(rectus femoris, RF; vastus lateralis, VL; vastus medialis, VM; vastus intermedius, VI) 손상 양상을 탐지하기 위해 활용되었다. 모든 세션에서 혈청 CK 활성 수준과 VL 및 VM의 근통증 정도는 운동 후 24시간째(RF의 경우 각각 LDR 24시간째와 HDR 48시간째) 최대에 이르렀으며, 혈청 CK 수준에서 운동 강도에 따른 유의한 차이(p<.05)가 나타난 반면 근통증에서 세션 간 통계적 차이는 없었다. 무릎 관절을 이용한 MVIC 및 ROM과 같은 신경근 기능 지표 및 VM을 제외한 모든 QG 근육 반향세기는 운동 직후 극적으로 감소 또는 증가 후 72시간까지 점진적 회복 양상을 나타내었다. 그러나 신경근 기능 지표에서 운동 강도에 따른 통계적 차이는 없었으나 RF 및 VL 반향세기에서 세션 내 및 세션 간 유의한 차이(p<.01)를 나타내었다. 본 연구의 결과로 ECC를 함유한 DR 운동 시 운동 강도는 DOMS 및 신경근 기능 지표에 부분적으로 영향을 미칠 가능성이 있으며, 특히 혈청 CK 수준과 함께 RF 및 VL의 근육 반향세기는 운동 강도의 영향을 직접적으로 반영한다는 사실을 알 수 있다. 또한, 현재 연구결과는 DR 동안 ECC를 겪는 QG 내 국소 근육 간 근손상 정도가 다를 수 있으며 초음파 근육 반향세기가 국소 근육의 EIMD 심각성을 차별화할 수 있는 유용한 평가기법임을 뒷받침하고 있다.

Protective effects of endurance exercise on skeletal muscle remodeling against doxorubicin-induced myotoxicity in mice

  • [Purpose] Doxorubicin (DOX) is a potent anti-cancer drug that appears to have severe myotoxicity due to accumulation. The skeletal muscle has a regeneration capacity through satellite cell activation when exposed to extracellular stimulus or damage. Endurance exercise (EXE) is a therapeutic strategy that improves pathological features and contributes to muscle homeostasis. Thus, this study investigated the effect of EXE training in mitigating chronic DOX-induced myotoxicity. [Methods] Male C57BL/6J mice were housed and allowed to acclimatize with free access to food and water. All the mice were randomly divided into four groups: sedentary control (CON, n=9), exercise training (EXE, n=9), doxorubicin treatment (DOX, n=9), doxorubicin treatment and exercise training (DOX+EXE, n=9) groups. The animals were intraperitoneally injected with 5 mg/kg/week of DOX treatment for 4 weeks, and EXE training was initiated for treadmill adaptation for 1 week and then performed for 4 weeks. Both sides of the soleus (SOL) muscle tissues were dissected and weighed after 24 hours of the last training sessions. [Results] DOX chemotherapy induced an abnormal myofiber's phenotype and transition of myosin heavy chain (MHC) isoforms. The paired box 7 (PAX7) and myoblast determination protein 1 (MYOD) protein levels were triggered by DOX, while no alterations were shown for the myogenin (MYOG). DOX remarkably impaired the a-actinin (ACTN) protein, but the EXE training seems to repair it. DOX-induced myotoxicity stimulated the expression of the forkhead box O3 (FOXO3a) protein, which was accurately controlled and adjusted by the EXE training. However, the FOXO3a-mediated downstream markers were not associated with DOX and EXE. [Conclusion] EXE postconditioning provides protective effects against chronic DOX-induced myotoxicity, and should be recommended to alleviate cancer chemotherapy-induced late-onset myotoxicity.

Effect of Prior Muscle Contraction or Passive Stretching on Eccentric-Induced Muscle Damage

  • Purpose: This tutorial review investigated the effect of prior fatigue and passive stretches on eccentric contraction-induced muscle injuries, as well as the underlying mechanisms of eccentric contraction-related injuries. Methods: Contraction-induced muscle damage is the most common disabling problem in sports and routines. The mechanisms underlying the pathology and prevention of muscle damage lessened by prior fatigue or stretches are critical in assessing musculoskeletal injuries. Even though there are treatments to reduce eccentric contraction-induced muscle injuries, fatigue negatively influences them. Therefore, we reviewed previous studies on eccentric contraction-induced muscle injuries with prior treatments using the MEDLINE and PubMed databases. Results: Prior passive stretching had a preventative and therapeutic effect, but prior lengthening contractions did not. On the other hand, prior isometric contractions involving relatively small forces may not provide a sufficient stimulus to induce protection. As a result, high force isometric contractions may be necessary. The studies supported the positive effects of prior fatigue, concluding that it was a factor in determining the amount of damage caused by eccentric exercise. This was due to a reduction in force and increased temperature. Studies that did not support the positive effects of prior fatigue concluded that a shift in optimal length to a longer length and reduced energy absorption during lengthening are evidence that fatigue is not related to muscle injuries induced by lengthening. Conclusion: The variability of the experiment models, conditions, muscles, and treatment methods make it necessary to interpret the conditions of previous studies carefully and draw conclusions without making direct comparisons. Thus, additional studies should be carefully conducted to investigate the positive effect of fatigue on lengthening.

과도한 운동 스트레스에 대한 숙성생강 추출물의 방어효과 (Protective Effects of Aged Ginger Extracts on Excessive Exercise-induced Stress)

  • 생강의 운동 스트레스에 대한 방어효과를 알아보기 위해 동결건조, 열풍건조, 숙성한 생강을 제조하여 추출물을 근육세포에 H2O2와 함께처리한 결과 생강을 숙성한 시료는 숙성하지 않은 시료에 비하여 우수한 근육세포 보호효과를 나타내었다. 또한 숙성생강 추출물을 마우스에 투여시 운동으로 인한 혈액내 LDH, lactate, GOT 증가를 억제시키는 경향을 나타내었다. 따라서 숙성생강을 섭취시 과도한 운동으로 인한 스트레스에 대한 방어효과를 기대할 수 있을 것으로 사료된다. 이러한 결과를 토대로, 향후 생강의 숙성 온도 및 시간에 따른 운동 스트레스 방어효과를 측정하여 가장 높은 활성을 나타내는 최적의 숙성 조건을 확립하는 연구를 진행할 예정이다.

운동 전 적용된 지속초음파와 맥동초음파가 운동 유발성 근육 손상의 통증과 근피로대사산물에 미치는 효과 (The effects of pulsed ultrasound and continued ultrasound intervention before an exercise on PPT, CK and LDH of EIMD)

  • Background: The purpose of this study was to investigate the effects of pulsed-ultrasound intervention and continued-ultrasound on the PPT (pressure pain threshold), CK (creatine kinase) and LDH (lactate dehydrogenase) recovery of before EIMD (exercise-induced muscle damage). Design: Randomized Controlled Trial. Methods: Thirty subjects who are student in their 20s at a university participated in this study, these subjects were assigned into three groups, a control group (n=10), experiment group I (n=10), and experiment group II (n=10). The subjects in experimental group were intervened by pulsed-ultrasound and continued-ultrasound, while ones on control group weren't by any intervention after induced EIMD. Results: First, In the comparison of the PPT, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the PPT of experimental group II was significantly larger than those of other groups (p<.01). Second, In the comparison of the CK, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the CK of experimental group II was significantly smaller than those of other groups (p<.001). Third, In the comparison of the LDH, there were significant variations with the lapse the time in three groups (p<.001) and there was a significant interaction of time and group (p<.001). In the among group comparison, the LDH of experimental group II was significantly smaller than those of other groups (p<.001). Conclusion: The above results revealed that the continued-ultrasound intervention before an exercise had a positive effect of muscle function after EIMD. Therefore we can consider the continued ultrasound as a considerable intervention method to prevent or reduce an exercise injury.
