음악 기반 슬링운동 프로그램이 치매환자의 인지, 보행 및 기능적 운동성에 미치는 효과 (Effects of Music-based Sling Exercise Program on Cognition, Walking, and Functional Mobility in Elderly with Dementia: Single-blinded, Randomized Controlled Trial)

  • PURPOSE: This examined the effects of a sling exercise based on music on the cognition, physical performance of patients with dementia. METHODS: Thirty subjects with dementia volunteered to participate in this study. All subjects were allocated randomly to either the experimental group or control group, with 15 subjects in each group. All subjects underwent the exercise program for an average of 60 minutes per day for 16 weeks. The experimental group performed sling exercise based on music, and the control group performed the general exercise program. Assessments were made using the Korean version of mini-mental state examination (MMSE-K), 10 m walk test (10MWT), Tinetti mobility test (TMT), and Katz's Index of Independence in activity daily living (KIIADL) to detect changes in the cognitive level and physical performance before and after the 16-week training period. A paired t-test was conducted to compare the within-group change before and after the intervention. An independent t-test was performed to compare the between-group difference. The statistical significance level was set to α=.05 for all variables. RESULTS: The experimental group showed significant within-group changes in the MMSE-K, 10MWT, TMT, and KIIADL (p<.05). The control group showed a significant change in only the KIIADL (p<.05). A significant difference was observed between the experimental group and the control group regarding the change in MMSE-K and KIIADL after the interventions (p<.05). CONCLUSION: A music-based sling exercise program effectively improves cognition, physical performance, and ADL in patients with dementia. Further studies with a wider range of subjects and scientific equipment will be needed to strengthen the results of this study.

At 1990 an early stage, because physical therapist and doctors of medical treatment developed country norway of north europe cooperate sling exercises therapy's concept trend spreading worldwide establishing theory newly based on scientific basic be. Musculoskeletal system, old man and young child's nervous system injurer's treatment and exercise and industry worker's rehabilitation process, athlete's rehabilitation etc. several fields introduced in early 2000s to Korea apply. Sling exercises therapy neuromuscular system disease continuous abatement as general concept of active exercise and treatment that use sling exercises equipment by purpose know can. This review article wishes to introduce about neuromuscular activation, "Neurac" technique that can speak as step developed more concept based on application principle etc, of basic sling exercises effort's change by suspension point's change that is concept, stability exercises principle, open kinetic chain exercises and closed kinetic chain exercises. Arranged about Plateau potential's characteristic and working mechanism and Neurac technique's application method that can say as Neurac technique's neuro-physiologic base, and presented treatment method of lumbar part disease and cervical part, shoulder joint part disease to application example. Because plateau potential forward player that induce using Neurac technique in part muscles that act role that keep spine surrounding stability in this article keeps muscle's contraction continuously, between global and local muscles, presented several study findings that can cause affirmative change in insufficient muscle adjustment state such as imbalance of muscles' action order. Expect that case studies that use this Neurac technique here after consist continuously.

Effects of Isokinetic Strength Exercise of the Hemiparetic Knee Joint on Lower Extremity Sensorimotor and Gait Functions in Patients With Chronic Stroke: Open Kinetic Chain Versus Closed Kinetic Chain

  • Background: After stroke, in order to improve gait function, it is necessary to increase the muscle strength and to enhance the propriocetive function of the lower extremity. Objects: This study aimed to compare the effects of open kinetic chain (OKC) versus closed kinetic chain (CKC) isokinetic exercise of the hemiparetic knee using the isokinetic equipment on lower extremity sensorimotor function and gait ability in patients with chronic stroke. Methods: Thirty participants with chronic hemiplegia (> 6 months post-stroke) were randomly divided into 2 equal groups: CKC group and OKC group. Patients from both groups attended conventional physiotherapy sessions 3 times a week for 6 weeks. Additionally, subjects from the CKC group performed isokinetic exercise using the CKC attachment, while those from the OKC group performed isokinetic exercise using the OKC attachment. The isokinetic knee and ankle muscles strength, position sense of the knee joint, and spatiotemporal gait parameters were measured before and after interventions. Results: The knee muscles peak torque/body weight (PT/BW) and hamstring/quadriceps (H/Q) ratio significantly increased in both groups (p<.01). In particular, ankle plantarflexors PT/BW, position sense of the knee, gait velocity, and spatial gait symmetry significantly improved in the CKC group (p<.01, p<.05, p<.01, and p<.01, respectively). Conclusion: CKC isokinetic exercise can be an effective therapeutic intervention for the improvement of sensorimotor function of the lower extremity and gait functions, such as gait velocity and symmetry. CKC position in isokinetic strength training is effective to improve functional ability in patients with chronic stroke.

Muscle Functional MRI of Exercise-Induced Rotator Cuff Muscles

  • The aim of this study was to provide a new assessment of rotator cuff muscle activity. Eight male subjects (24.7 ± 3.2 years old,171.2 ± 9.8 cm tall, and weighing 63.8 ± 11.9 kg) performed the study exercises. The subjects performed 10 sets of the exercise while fixing the elbow at 90 degrees flexure and lying supine on a bed. One exercise set consisted of the subject performing external shoulder rotation 50 times using training equipment. Two imaging protocols were employed: (a) true fast imaging with steady precession (TrueFISP) at an acquisition time of 12 seconds and (b) multi-shot spin-echo echo-planar imaging (MSSE-EPI) at an acquisition time of 30 seconds for one echo. The main method of assessing rotator cuff muscle activity was functional T2 mapping using ultrafast imaging (fast-acquired muscle functional MRI [fast-mfMRI]). Fast-mfMRI enabled real-time imaging for the identification and evaluation of the degree of muscle activity induced by the exercise. Regions of interest were set at several places in the musculus subscapularis (sub), musculus supraspinatus (sup), musculus teres minor (ter), and deltoid muscle (del). We used the MR signal of the images and transverse relaxation time (T2) for comparison. Most of the TrueFISP signal was not changed by exercise and there was no significant difference from the resting values. Only the T2 in the musculus teres minor was increased after one set and the change were seen on the T2 images. Additionally, except for those after one and two sets, the changes in T2 were significant compared to those at rest (P < 0.01). We also demonstrated identify and visualize the extent to which muscles involved in muscle activity by exercise. In addition, we showed that muscle activity in a region such as a shoulder, which is susceptible to B0 inhomogeneity, could be easily detected using this technique.

비대면 휘트니스 MR 플랫폼 개발을 활용한 운동 수행 효과에 관한 연구 (Study on Effect of Exercise Performance using Non-face-to-face Fitness MR Platform Development)

  • 본 연구는 기존 휘트니스 사업의 문제점을 극복하고, 코로나 시국에 대한 늘어난 수요를 충족시킬 수 있는 휘트니스 시스템을 구축하고자 수행되었다. 비대면 휘트니스 에듀테인먼트 서비스를 위한 플랫폼 기술로써 다양한 신체 부위 운동 및 네트워크형 정보 동기화가 가능한 차세대 휘트니스 운동 기구이다. 휘트니스 장비의 운동 정보를 동기화하여, MR 기반 아바타를 통한 학습형 콘텐츠로 구성하였다. 이용하는 사용자의 누적 운동 효과에 따른, LSTM 기반 알고리즘을 적용한 A.I 분석으로 운동량 분석을 통한 맞춤형 평가 시스템의 적용성을 검토하여 정량화된 결과를 도출하였다. 학계 전문가를 통한, 체계적 운동 기법 적용을 위한 모션 캡처 및 3D 가시화 휘트니스 프로그램으로 이용자의 휘트니스 지식 및 운동능력 향상에 기여할 것이라 판단된다.

코로나19 의심 또는 확진 환자가 병원 밖 심정지 발생 시 구조자의 개인 보호장비(PPE) 착용에 의한 가슴압박 효과 비교 : 무작위 교차 시뮬레이션 연구 (Effect of level D personal protective equipment on chest compression for pre-hospital arrest patients with suspected or confirmed COVID-19 : A randomized crossover simulation trial)

  • Purpose: The purpose of this study was to determine how the use of level D personal protective equipment (PPE) and cardiopulmonary resuscitation (CPR) feedback equipment affects chest compression (CC). Furthermore, this study provides basic data for developing Korean CPR guidelines that can be applied to patients with suspected or confirmed COVID-19. Methods: This randomized, single-blinded, crossover simulation trial included 26 student paramedics who performed 2-minute chest compressions using three different methods: Method A involved performing traditional CC for two minutes without donning level D PPE, Method B involved performing CC while donning level D PPE, and Method C involved performing CC with a CPR feedback device while donning level D PPE. Results: The use of a CPR feedback device during the 2-minute CC increased the exercise intensity of the subjects, but donning level D PPE didn't affect the quality of CC and the exercise intensity. The results of methods A and B showed changes in the quality of compression 80 seconds after the start of CC. No significant changes occurred in 2-minute CC when using the CPR feedback device. Conclusion: Using a CPR feedback device could prevent deterioration in the quality of CC while donning level D PPE.

불안정한 지지면 위에서의 플랭크 운동이 만성허리통증환자의 유연성, 배 근육 두께 및 통증에 미치는 영향 (The Effect of Unstable Support Surface Plank Exercise on Flexibility, Abdominal Muscle Thickness and Pain in Chronic Low Back Pain)

  • Background: The purpose of this study was to investigate the effect of Plank exercise on unstable support surfaces on flexibility, abdominal muscle thickness and pain in patients with chronic back pain. Design: Randomized controlled trial. Methods: This study was performed on 16 patients with chronic back pain of ◯◯ military unit. Sixteen subjects were randomly assigned into two groups, an upper extremity trainer group (group I, n=8) and a lower extremity trainer group (group II, n=8). The subjects in group I carried out Flank exercise applying the stability trainer to their upper extremities and ones in group II carried out the same exercise applying the stability trainer to their lower extremities for 4 weeks. In order to ascertain the difference between two groups, flexibility, abdominal muscle thickness and pain were measured before and after the exercise. The flexibility was measured by sit and reach test, the thickness of the abdominal muscle was measured by using ultrasonic imaging equipment, and the pain was measured by the visual analogue scale. A paired t-test was utilized to compare changes in pain, abdominal muscle thickness and flexibility before and after flank exercise on unstable support surfaces. Analysis of Covariance (ANCOVA) was performed for ascertaining the significant differences between groups. The significance level was set by α=.05. Results: 1) The flexibilities of two groups were increased after the exercise (p<0.05). 2) In both groups, the thicknesses of rectus abdominis, external oblique abdominis, internal oblique abdominis, and transverse abdominis were all increased after the exercise (p<0.05). 3) The pains in both groups were decreased after the exercise (p<0.05). 4) In the comparisons of two groups, there were no differences in the flexibility, thickness of external oblique abdominis, internal oblique abdominis and transverse abdominis and pain (p>0.05). Whereas only thickness of Rectus abdominis was larger in the group I than in the group II (p<0.05). Conclusion: Plank exercise on the unstable support surface for 4 weeks resulted in increased flexibility, abdominal muscle thickness and pain reduction in patients with chronic back pain. Therefore, it is considered that performing flank exercise on the unstable supporting surface is suitable for the reduction of the pain in patients with chronic back pain. However, in this study, it is considered that continuous and diverse studies are needed because there was not a large difference between the groups when the upper or lower limbs are provided unstable support surfaces.

지역주민의 학교시설 이용현황에 관한 기초적 연구 - 대구광역시 수성구를 중심으로 - (A Study on the Offer present condition of School Facilities for Local Residents - Focused on Suseong Gu in Daegu City -)

  • The purpose of the study is on offer present condition of School Facilities for Local Residents in Daegu City. This study made progress within the scope of twenty-four public schools excepting one private school and five schools refused request. but opening present condition of the equipment was stopping in negative opening more than thing that is seen in present condition(to be a hall, a computer, swimming pool, physical education market, clearinghouse, art thread, library, language study thread) of two locals that show in old research. As result that diagnose this research, The school facilities open every schools excepting three schools could not open on account of establishment of sprinkler, residents protest about noise, small playground. The opening equipment was limited the playground, hall, basketball court, Badminton court, parking lot, classroom, library. And The use present condition was used by parking, walk, jogging and light exercise, basketball, badminton, state examination, sports meeting, books inspection, early stage soccer. When local residents require equipment use, it is not negative attitude in school but have active carriage. and Must consider plan that raise practical use degree so that can use school equipment.

항공기용 추적레이더 시험을 위한 시험장비의 설계 및 제작 (A Design and Fabrication of Test Equipment for Airborne Tracking Radar Test)

  • This paper proposes a design and fabrication of the test equipment that is implemented as a part of the airborne tracking radar inspection under the environment of indoor simulation. This test equipment provides controlling the operation status of airborne tracking radar and replicating the velocity and range information of target by generating a variety of target signal. This is mainly composed of radar operation controller, target signal generator, horn antenna driving devices. Radar operation controller is able to perform the controlling of radar operation mode and monitoring in real time by serial communication. Target signal generator is generated doppler signal and range delayed signal using virtual target of RF-band. Horn antenna driving devices perform a role of target simulating exercise. In the end, the performance is demonstrated using experiment results of test equipment for airborne tracking radar.

편마비 환자를 위한 보행 재활기구 개발 (Development of the Gait Rehabilitation Equipment for Hemiplegic Patients after Stroke)

  • The aim of this study is to design and develop the gait rehabilitation equipment that judge patient's movement of his/her center of gravity using pressure sensors, and to aid hemiplegic patients to balance themselves using an automatic stepper that changes the patient's center of gravity. It is hard to bear the weight on the affected side for hemiplegic patients. The gait rehabilitation equipment detects the footing phase of hemiplegic patient during training and moves the unaffected footing side of the stepper up and moves the affected footing side down simultaneously so that the patient's center of gravity can shift from unaffected side to affected side. The gait rehabilitation system was developed and applied for hemiplegic patients during exercise. Eight hemiplegic patients and one normal adult were studied. The developed gait rehabilitation system could judge not only the normal adult's intention but also the patient's intention to move his/her center of gravity. Even though the most of hemiplegic patients exercised in automatic mode and a few hemiplegic patients exercised in manual mode, the developed gait rehabilitation system can aid the hemiplegic patients to train more easily.
