Effects of Early Exercise Education using Video on Exercise Knowledge and Compliance of Elderly Patients Who Undergo Total Knee Replacement Surgery (동영상을 이용한 수술 후 초기 운동교육이 슬관절 전치환술 노인 환자의 운동지식 및 운동이행도에 미치는 효과)

  • Purpose: This study was conducted to identify the effects of early exercise education using videos on exercise knowledge and compliance of elderly patients who undergo total knee replacement surgery. Methods: This is a quasi-experimental study with a nonequivalent control group non-synchronized design. The participants were 70 elderly patients who had total knee arthroplasty in the C university hospital located in G city, Korea. Thirty-five patients were assigned to an experimental group and the other patients were assigned to a control group. After the total knee arthroplasty, the early exercise education accompanied by watching video instruction was conducted on the experimental group at post-operation second and third day for 15 minutes. General exercise education was administered to the control group. The exercise knowledge and compliance of the elderly patients were measured through structured questionnaires. Conclusion: The results of this study indicate that early exercise education using videos has positive effects to improve the exercise knowledge and compliance of elderly patients after total knee arthroplasty. This early exercise education can be used going forward as nursing intervention for elderly patients who undergo total knee replacement surgery.

Effects of the NADPH Oxidase p22phox C242T Polymorphism on Endurance Exercise Performance and Oxidative DNA Damage in Response to Aerobic Exercise Training

  • We examined the effects of the NADPH oxidase p22phox C242T polymorphism on endurance exercise performance and oxidative DNA damage in response to acute and chronic exercises. One hundred three subjects were recruited, among which 26 healthy subjects (CC: 12, TC: 12, and TT: 2) were studied during rest, exercise at 85% $VO_2max$, and recovery before and after 8 weeks of treadmill running. Lymphocyte DNA damage increased significantly in response to exercise (p < 0.05). There were no significant differences in plasma MDA, SOD concentrations and lymphocyte DNA damage between CC genotype and T allele group, but significant endurance training differences were observed. Endurance training increased exercise time to exhaustion in both the CC genotype and T allele groups (p < 0.05) but no significant difference was found between groups. The results of the current study with young, healthy, Korean men are interpreted to mean that 1) the majority had the CC genotype of the NADPH oxidase p22phox C242T polymorphism (82.5%: CC, 15.5%: TC, 1.9%: TT), 2) acute exercise increased lymphocyte DNA damage, 3) endurance training significantly increased exercise time to exhaustion, and alleviated lymphocyte DNA damage, and 4) The NADPH oxidase p22phox C242T polymorphism, however, did not alter lymphocyte DNA damage or exercise performance at rest, immediately after exercise, or during recovery.

  • We investigated the effect of different exercise intensities on biomarkers of oxidant-antioxidant balance, inflammation, and muscle damage. Eighteen healthy and untrained male subjects were enrolled.Subjects were randomly and equally assigned to a moderate-intensityexercise(MIE, $65%VO_2max$) group(n=9) or a high-intensity exercise(HIE, $85%VO_2max$) group(n=9).Blood samples were collectedimmediately pre-exercise, post-exercise, and 60min post-exercisetoexamine oxidant-antioxidant balance(d-ROMs; BAP), inflammation(CRP; fibrinogen), muscle damage(CK; LDH), and lactate. Serum d-ROMs and BAP levels were significantly increased post-exercise compared with pre-exercise levels in HIE group(p<0.05). Lactate levels were significantly increased post-exercise compared pre-exercise levels in both the MIE and HIE groups(p<0.05). In addition, post-exercise serum d-ROMs and plasma lactate levels were significantly higher in the HIE group than in the MIE group(p<0.05). These results suggest that although relatively high-intensity exercises may increase oxidative stress levels in the body, they do not produce inflammatory response and/or muscle damage.

Effects of intermittent ladder-climbing exercise training on mitochondrial biogenesis and endoplasmic reticulum stress of the cardiac muscle in obese middle-aged rats

  • The aim of this study is to investigate the effects of intermittent ladder-climbing exercise training on mitochondrial biogenesis and ER stress of the cardiac muscle in high fat diet-induced obese middle-aged rats. We induced obesity over 6 weeks of period in 40 male Sprague-Dawley rats around 50 weeks old, and were randomly divided into four experimental groups: chow, HFD, exercise+HFD, and exercise+chow. The exercising groups underwent high-intensity intermittent training using a ladder-climbing and weight exercise 3 days/week for a total of 8 weeks. High-fat diet and concurrent exercise resulted in no significant reduction in body weight but caused a significant reduction in visceral fat weight (p<0.05). Expression of $PPAR{\delta}$ increased in the exercise groups and was significantly increased in the high-fat diet+exercise group (p<0.05). Among the ER stress-related proteins, the expression levels of p-PERK and CHOP, related to cardiac muscle damage, were significantly higher in the cardiac muscle of the high-fat diet group (p<0.05), and were significantly reduced by intermittent ladder-climbing exercise training (p<0.05). Specifically, this reduction was greater when the rats underwent exercise after switching back to the chow diet with a reduced caloric intake. Collectively, these results suggest that the combination of intermittent ladder-climbing exercise training and a reduced caloric intake can decrease the levels of ER stress-related proteins that contribute to cardiac muscle damage in obesity and aging. However, additional validation is required to understand the effects of these changes on mitochondrial biogenesis during exercise.

The effect of muscle strength, endurance and flexibility on Graded Low back exercise and education of chronic low back pain patents. (만성요통환자의 점증적 요통 운동과 교육이 근력과 근지구력, 유연성에 미치는 효과)

  • The purpose of this study was to examine the effects of graded low back exercise program with patient education about low back pain care on abdominal strength, endurance and flexibility of waist of chronic low back pain patents. For this study 13 patients aged 40 to 60 were selected as subjects, who suffered from low back pain and got diagnosed by physician. The exercise program and the education were executed for total 6 weeks, posture education were executed with physical exercises over first 1 to 4 weeks period and over 5 to 6 weeks the exercises were executed. The effect of the exercises and the education were evaluated through weights, WHR, fat(%), flexibility by sit-and-reach and trunk extension test, strength by sit-up test and back sit-up test. Weights were, decreased after physical exercise and education, but there was no statistical significance. Wasit hip ratio(WHR) and fat(%) were decreased after physical exercise and education, but there were no statistical significances. Sit-and-reach was significantly increased from $13.68{\pm}5.59cm$ to $19.45{\pm}3.81cm$ after education and physical exercise(P<0.05). Trunk extension was significantly increased from $30.31{\pm}11.34cm$ to $40.88{\pm}6.16cm$ after education and physical exercise(P<0.05). Sit-up and Back sit-up were increased after physical exercise and education, but there were no statistical significances. These results suggest that graded low back exercise program with patient education about low back pain care increase the abdominal strength, endurance and the flexibility of waist in low back pain patients.

Effects of Stretching Exercise Included Health Education on Physical Health Index, Self-reported Symptoms, and Self-efficacy of Exercise in Elderly (건강교육을 포함한 신장운동이 노인의 생리적인 지표와 자가증상 보고 및 운동 자기효능에 미치는 효과)

  • This was one experimental study, conducted to identify the effects of stretching exercise included health education on physical health index, self-reported symptoms, and self-efficacy of exercise in Elderly. Subjects included 28 women elderly who were residents of H Dong, Dondae moon-Gu, Seoul. The treatment intervention was applied during total 8 weeks as 5 times/week for stretching exercise with 2 times/weeks for health education. Data collection were from February to April, 2004. SPSS Window program was used by aims of this study for data analysis. The results were as following: 1. 75 old age above (39.3%) was the most of age in subjects. None education (42.9%) was the most of subjects in this study. Perceived health state was the most as 71.4% in moderate and good health state. 2. Stretching exercise included health education, significantly effected on the total Cholesterol, HDL-cholesterol, triglyceride (p=.009*; p=.043*; p=.006*). However, the body fat weight was no significantly on the effect of stretching exercise included health education (p=.991). Also, stretching exercise included health education, significantly effected on total self-reported symptoms (p=.001*) with joint pain, joint rigidity, fatigue, indigestion, elimination disorder, and sleep disturbance. However, indigestion (p=.129) was no significantly on the effect of stretching exercise included health education. Self efficacy of exercise (p=.000*) was significantly on the effect. As according to this results, physical health index (total Cholesterol, HDL-cholesterol, triglyceride) and self-reported symptoms (joint pain, joint rigidity, fatigue, elimination disorder, and sleep disturbance) were reduced through stretching exercise included health education. Self efficacy was improved by this exercise program. Therefore, it is confirmed that stretching exercise included health education is an effective nursing intervention for physical, mental, and psychological health management in elderly. Accordingly, authors are proposing that variously effective health management exercise programs must be developed for elderly, at the same time, the application and following up on the programs will be more important in the future.

  • To investigate the effects of conjugated linoleic acid (CLA) and/or exercise on body fat mass and weight, college women of normal weight (2130%) were recruited for this study. The participants were divided into 4 groups: placebo-no exercise, placebo-exercise, CLA-no exercise, and CLA-exercise groups. Three grams of either a placebo (corn oil) or CLA were taken every day for 12 weeks, and the exercise groups performed 80 minutes of aerobic and anaerobic exercise three times a week for 12 weeks. There were no differences in nutrient intakes among the groups. The CLA-exercise group had significantly lower body weight, BMI and body fat mass compared to the placebo-no exercise group. In addition, the HDL-cholesterol levels of subjects in the CLA-no exercise and CLA-exercise groups significantly increased compared to those in the placebo-no exercise group. These results suggest that a combination of CLA supplementation with exercise could efficiently reduce body fat mass and body weight compared to CLA supplementation or exercise alone.

Analyses of physiological wrist tremor with increased muscle activity during bench press exercise

  • [Purpose] To date, there have been no studies on the response of wrist tremor to increased muscle activity during exercise. This study aimed to evaluate the wrist tremor response with increasing muscle activity during bench press exercise. [Methods] Triceps muscle activity and wrist tremor response were measured by electromyography and an accelerometer, respectively, during bench press exercise in 11 healthy men without weight-training experience. Subjects performed bench press at 30% repetition maximum (RM), and the rating of perceived exertion (RPE) and lactate concentration were measured before and after exercise. One week later, an equivalent number of bench presses at 30% RM was performed without weight load as a control trial (CT). [Results] RPEs and lactate concentrations significantly increased after resistance exercise (30% RM) from 7.4 to 14.3 and 1.7 to 4.9, respectively (P<.01), but no such difference was observed in the CT. Muscle activity linearly increased during the 30% RM exercise, and wrist tremors were shown to linearly decrease. A strong negative correlation was observed between the two variables (r=-0.88, P<.001). [Conclusion] We found that wrist tremors during resistance exercise, as measured using an accelerometer, can be used to predict muscle activity.

The effect of short-term creatine intake on blood lactic acid and muscle fatigue measured by accelerometer-based tremor response to acute resistance exercise

  • [Purpose] The purpose of this study was to investigate the effects of short-term creatine intake on muscle fatigue induced by resistance exercise in healthy adolescent men, i.e., lactic acid concentration and wrist and head tremor measured by an accelerometer. [Methods] Twelve healthy adolescent men who had no experience with creatine intake were included. The subjects were randomly assigned to the creatine group and the placebo group, followed by 5 days of creatine and placebo intake, and 5 times of 5 sets of leg press, leg extension, bench press, and arm curl exercises at 70% repetition maximum (RM). The lactic acid concentration before and after exercising, rate of perceived exertion (RPE), and accelerometer-based wrist tremor and head tremor during exercise were measured. Subsequently, after 7 days to allow for creatine washout, the same exercise treatment and measurement were performed in each group after switching drug and placebo between the groups. [Results] The level of lactic acid before and after the acute resistance exercise trial was significantly lower in the creatine group than in the placebo group (P <0.05). The mean RPE during the resistance exercise was significantly lower in the creatine group than in the placebo group (P <0.05). There was no difference between the two groups in the mean wrist tremor during resistance exercise, but the mean head tremor values were significantly lower in the creatine group than in the placebo group in the arm curl, the last event of the exercise trials (P <0.05). [Conclusion] Short-term creatine intake reduces the blood fatigue factor increased by resistance exercise, and is thought to suppress fatigue, especially in the latter half of resistance exercise. Therefore, these findings indicate that short-term creatine intake can have an improved effect on anaerobic exercise performance.

Effects on Exercise Behavior and Health Status of Exercise Education Program in a Public Health Center (운동교육 프로그램이 보건소 내원자의 운동습관과 건강상태에 미치는 영향)

  • Objectives: This study was performed to evaluate the effects of exercise education for the people living in the area where health promotion program has been operated. Methods: The research had been investigated at a public health center located in ChungNam-Do from April to September in 2003. The subjects of the research were the visitors at a public health center who were willing to obtain the exercise education program and the visitors at other one. 142 people learned the health program at the health center and 72 people didn't do that. These two groups had taken tests such as health condition(SF-36). blood pressure and blood test for six months. Results: The educated group got better marks in the area of knowledge and attitude than non-educated one. The educated group showed increased exercise frequency by second survey. The health status of the educated group looked better after education than the non--educated group, but it was not significant statistically. The grade of knowledge and attitude for exercise was higher in the exercise group than in non-exercise group. The exercise frequency was increased in the exercise group at second survey. The change of health status was not different between exercise group and non- exercise group. Conclusions: According to the research the author found that the exercise education program was helpful for population to improve their exercise habits. It had supposed that the education of exercise and the exercise itself had benefit on health status. But It was not significant in this study with any methodological limitations. If the exercise education would performed more adequately and more persistently, its effects on health status will be more positive.
