Objective : Craniovertebral junctional anomalies constitute a technical challenge. Surgical opening of atlantoaxial joint region is a complex procedure especially in patients with nuchal deformity like basilar invagination. This region has actually very complicated anatomical and functional characteristics, including multiple joints providing extension, flexion, and wide rotation. In fact, it is also a bottleneck region where bones, neural structures, and blood vessels are located. Stabilization surgery regarding this region should consider the fact that the area exposes excessive and life-long stress due to complex movements and human posture. Therefore, all options should be considered for surgical stabilization, and they could be interchanged during the surgery, if required. Methods : A 53-year-old male patient applied to outpatients' clinic with complaints of head and neck pain persisting for a long time. Physical examination was normal except increased deep tendon reflexes. The patient was on long-term corticosteroid due to an allergic disease. Magnetic resonance imaging and computed tomography findings indicated basilar invagination and atlantoaxial dislocation. The patient underwent C0-C3-C4 (lateral mass) and additional C0-C2 (translaminar) stabilization surgery. Results : In routine practice, the sites where rods are bound to occipital plates were placed as paramedian. Instead, we inserted lateral mass screw to the sites where occipital screws were inserted on the occipital plate, thereby creating a site where extra rod could be bound. When C2 translaminar screw is inserted, screw caps remain on the median plane, which makes them difficult to bind to contralateral system. These bind directly to occipital plate without any connection from this region to the contralateral system.Advantages of this technique include easy insertion of C2 translaminar screws, presence of increased screw sizes, and exclusion of pullout forces onto the screw from neck movements. Another advantage of the technique is the median placement of the rod; i.e., thick part of the occipital bone is in alignment with axial loading. Conclusion : We believe that this technique, which could be easily performed as adjuvant to classical stabilization surgery with no need for special screw and rod, may improve distraction force in patients with low bone density.

Evaluation on the test of actual concrete member to confirm the fire resistance of the concrete member using ultra-high strength concrete is required. However, test equipment which has large loading capacity is needed to the actual member experiment. So, many researchers evaluated the fire performance through analytical studies using the material models. This study experimentally evaluated strain properties on ultra-high-strength concrete of 80, 130 and 180 MPa with heating and examined to apply the existing strain model about ultra-high-strength concrete. As a results, constants are drawn by method of least squares applying experimental values and calculated values by the existing strain model, it proposed strain model that can be applied to ultra-high-strength concrete.

There were huge damages of human beings and their properties in many areas of the basin of the Nak-Dong river by the unusual weather and the localized downpour recently. In this research against disasters, we want to know strength of the cemented sand that is mixed with cement and poor-graded sand, to estimate CSG(Cemented Sand and Gravel) method used coffer dam in Japan, which is the materials of riverbed in the basin of the Nak-Dong river for levee's construction. For that, we want to provide the fundamental data which need in the examination of adaptation of levee's material, design and analysis by investigating compressive strength by curing period and cement content, elastic modulus and stress by transformation from compaction test, CBR test, unconfined compression test and triaxial compression test as changing cement content from 2% to 8% at two sites in the basin of the Nak-Dong river.
Purpose: We analyzed the ankle bony abnormality of patients with marked ankle instability who had chronic ankle sprain more than 3 years. Materials and Methods: We evaluated the chronic ankle sprain (more than 3 years) patients with marked ankle instability tested by varus stress test and anterior draw test from March 2000 to December 2005. Eighty-nine patients (104 ankle) were evaluated and there were 38 males and 51 females. The mean age of patient at the time of diagnosis was 34.5 (range, 18 to 56 years). The average duration of morbidity was 7 years and 3 months (range, 3 years and 3 months to 21 years). The patients who had history of dislocation, fracture, malalignment, operated patients, and rheumatoid ones were excluded. Plain radiographs of AP, lateral, oblique and mortise view were checked. Results: Radilologic abnormalities were found at 74 ankles (71%) among 104 ankles. Frequent sequences of location were anterior talotibial osteophyte, medial malleolar osteophyte, Os subfibulare, lateral malleolar osteophyte. Posteior osteophyte, ankle arthritis, talar articular defect were rarely found. Conclusion: Seventy-one percent among patients with chronic ankle sprain and marked ankle instability showed more than one radiologic abnormalities. Thus, more exclusive and accurate ankle examination should be performed in these patients.

Taxol, a natural product with significant anti-tumor activity, stabilizes microtubules and arrests cells in the G2/M phase of the cell cycle. It has been reported that taxol has additional effects on the cell such as an increase in tyrosine phosphorylation of proteins and activation of mitogen-activated protein (MAP) kinase. This phosphorylated kinase translocates into the nucleus and phosphorylates its substrate c-jun, c-fos, ATF2, and ATF3. The MAP kinase family is comprised of key regulatory proteins that control the cellular response to both proliferation and stress signals. First examination was cytotoxicity and apoptosis-induced concentration with paclitaxel in HeLa cell. A half-maximal inhibition of cell proliferation ($IC_{50}$) occurred at 13 nM paclitaxel. When DNA fragmentation was analyzed by agarose gel electrophoresis, a nucleosomal ladder became evident 24 h after a taxol (50 nM) addition to the cells. In addition, an apoptotic body was detected by electron microscopy. Taxol-treated cells were arrested at the S phase at 10 nM. Treatment of 50 nM taxol activated the extracellular signal-regulated protein kinase (ERK1), and a fraction of the activated MAP kinases entered the nucleus. It was also discovered that nucleus substrates c-jun was phosphorylated and activated in the cell. The activated ERK1 could subsequently translocate into the nucleus and phosphorylate its substrate c-jun as well. This study suggests that taxol-induced apoptosis might be related with signal transduction via MAP kinases.
Objectives: This study was performed to develop an assessment tool for middle aged women's health status based on dietary patterns, which will have practical applications in the working field of health and hygiene, aiming at improving the middle aged women's quality of life through their health improvement. Methods: As a first step, a literature review was conducted and the original data of '2008~2009 Korea Health and Nutrition Examination Survey' were reanalyzed. This analysis identified 65 preliminary questions that may be relevant to the study. After verifying the content validity by experts, the 65 questions were reduced into 51 questions. In order to secure higher validity of the candidate items, verification of their clinical validity was conducted among women aged between 45 and 60 years. Finally, an assessment tool was developed by applying weight and scoring. Results: Selected 51 questions were used to verify clinical validity and the results showed that 20 questions were relevant, nine questions ('regular meal time', 'regular amount of meal', 'intake frequency of dairy products', 'intake frequency of fruits', 'intake frequency of meat products', 'intake frequency of high cholesterol foods', 'intake frequency of salty foods', 'appetite', 'eat breakfast everyday') were related to dietary life. Eleven other questions ('self-rated health status', 'deep sleep', 'smoking', 'frequency of drinking', 'stress levels', 'health-related fitness levels', 'pounding of the heart', 'strange feelings on the skin', 'interfere with daily life', 'menopause will bring you a chance to see the life in a different perspective', and 'body mass index') were selected as valid questions. For the response scale for each question, 5 point Likert scale was used to make total 100 point score. Conclusions: This study is the first attempt to develop a health status assessment tool for middle aged women based on their dietary patterns. We conclude that this tool is expected to be a useful and practical tool in the field.

To define the causes of cladding degradation which can take place during the operation of nuclear power plants, it is required to develop the new fracture toughness test of spent fuel cladding. The fracture toughness of Zircaloy-4 cladding was estimated using the recently developed KAERI embedded Charpy (KEC) specimen. Axially notched KEC specimens cut directly from unirradiated fuel claddings, were tested in a way similar to the standard toughness test method of a Single Edge Bending (SEB) specimen. The results of KEC fracture toughness test at room temperatures were discussed and compared with those of the previous other studies. In conclusions, even though the KEC fracture toughness test of nuclear fuel claddings was easier and more reliable than those developed earlier, the results from the cladding fracture tests were not the material characteristics but the specific fracture parameters which were deeply related to the specification of claddings. In addition, the phenomenon of a thickness yielding was not observed from the fracture surface. It was closely related to the fact that the plane strain condition of the KEC specimen was changed to the plane stress condition during crack advancing. It was also supported by the fractographic evidence that the formation of ductile dimples at the crack initiation became the similar appearance such as a quasi-cleavage after the sufficient crack advancing.
Recipients are an integral part of embryo transfer and they are expensive to maintain as a good recipient. Recipient management is one of the most important components in a successful embryo transfer program. Management includes selection and subsequent care of the animals. A good recipient is basically on "open" cows or heffers whose reproductive tract is capable of receiving one or two embryos and incubating it to term. Potential recipients should be always be healthy and cycling normally ranging from 18 to 23 days. A thorough veterinary examination is recommended for candidate of recipients and cattle for questionable health should be eliminated from the recipient herd. Age and size of recipients are particularly important considerations when heifers are used, because of most embryos available for transfer are from large dams and sires. Body condition can influence a recipient's production, reproduction and health. Obese and underconditioned cattle should be avoided for use. Transfer of fresh embryos especially requires precise synchronization of donors and recipients. For estrus synchronization, PGF$_2$$\alpha$ is injected twice 10 to 12 days apart and short4erm progestagen treatment is applied to potential recipient cattle by coil into vagina (PRID) or ear implant (Synchro-Mate-B). The highest pregnancy results are achieved in recipients at exact synchrony with donors or 12 to 24 hr earlier than donors. Estrus detection is a major factor in breeding efficiency. High accuracy can be achieved by use of heat mount detection alds or by obserbing cattle for 30-minute peroids 3 times daily. Assay progesterone in milk can be used to discrIminate between pregnant and nonprenant recipients. Rectal palpation on day 35 to 70 after is an accurate and safe method of pregnancy diagnosis. Embryonic mortality in recipients may be associated with factors such as high environmental temperature and nutritional or lactational stress in early lactation period. Achievement of short calving interval requires concentrated management activity during the first 90 days following calving. Acceptable candidate for a recipient should be routinely vaccinated for infectious diseases. Proper nutritional programs according to NRC requirements and body condition scoring system for recipient cattles are vital to the ultimate success of an embryo transfer program.r program.

Recently vascularized fibular transfer has been used in the treatment of bone tumor that are more than six centimeter in length. With refinements in microsurgical techniques and understanding of the biological and biomechanical characteristics of vascularized bone graft, the success rate of this procedure was increased. Fifteen bone tumor patients, sixteen cases seen from Apr. 1979 to Jun. 1995 were managed by means of vascularized bone graft at Kyung Hee University Hospital. Ten cases were done intercalary graft and the others were done osteoarticular graft. the ratio of male and female was 6 : 9, and mean age was 20.4 years old at operation. Mean follow up period was 5 years 4 months(range 17 months to 16 years 2 months) and mean graft length was 13.8cm. Duration for union was 5.3 months(range 3 months to 1 year) and over-all rate of union at the last follow up examination was 93.8%. Sufficient hypertrophy of grafted bone was obtained in all cases at the time of last follow up as compared to initial size of grafted bone. Several complications were found such as stress fractures, recurrence. Vascularized fibular transfer for the treatment of bone tumor is a valuable procedure in appropriately selected patients.
