Numerical Modeling of Compression-Controlled Low-level Laser Probe for Increasing Photon Density in Soft Tissue

  • Various methods have been investigated to increase photon density in soft tissue, an important factor in low-level laser therapy. Previously we developed a compression-controlled low-level laser probe (CCLLP) utilizing mechanical negative compression, and experimentally verified its efficacy. In this study, we used Bezier curves to numerically simulate the skin deformation and photon density variation generated by the CCLLP. In addition, we numerically modeled changes in optical coefficients due to skin deformation using a linearization technique with appropriate parameterization. The simulated results were consistent with both human in vivo and porcine ex vivo experimental results, confirming the efficacy of the CCLLP.

Immortalization of Primary Keratinocytes and Its Application to Skin Research

  • As a major component of the epidermal tissue, a primary keratinocyte has served as an essential tool not only for the study of pathogenesis of skin-related diseases but also for the assessment of potential toxicities of various chemicals used in cosmetics. However, its short lifespan in ex vivo setting has been a great hurdle for many practical applications. Therefore, a number of immortalization attempts have been made with success to overcome this limitation. In order to understand the immortalization process of a primary keratinocyte, several key biological phenomena governing its lifespan will be reviewed first. Then, various immortalization methods for the establishment of stable keratinocyte cell lines will be explained. Finally, its application to a three-dimensional skin culture system will be described.

센텔라아시아티카정량추출물의 리포좀 캡슐기술 개발과 피부개선효과 (Skin Improvement Effects and Development of Liposome Capsule Technology Using Centella Asiatica Extract Powder)

  • 본 연구에서는 의약품과 화장품에 사용되는 고순도의 센텔라아시아티카정량추출물을 리포좀화하는 방법과 이를 이용한 크림을 개발하여 주름개선에 대한 in-vivo 임상평가에 대하여 연구한 결과를 보고한다. 리포좀 라멜라 베지클에 센텔라아사아티카정량추출물을 안정화시킨 센텔라좀-10EX을 만들기 위하여, 5%의 하이드로제네이티드레시틴과 2%의 수크로오스다이스테아레이트를 사용하여 완성시킬 수 있었다. 센텔라좀-10EX의 외관은 저점성의 크림상, 색상은 엷은 황색, 냄새는 원료 본연의 고유취가 있었다. pH는 약 6.12 이었으며, 비중은 1.09, 산가는 약 0.35이었다. 리포좀 베지클에 함유된 센텔라아시아티카정량추출물의 주성분의 함량은 10,800ppm의 아시아틱애씨드, 10,900ppm의 아시아티코사이드, 6,000ppm의 마테카식애씨드, 1,600ppm의 마데카소사이드가 함유하고 있으며, 장기간 보관 시에도 변색이 없었고, 열역학적으로 주성분이 안정하게 유지되고 있음을 알 수 있었다. 센텔라좀-10EX의 리포좀 베지클의 구조를 메커니즘 적으로 분석하기 위하여, 투과전자현미경 (Cryo-TEM)으로 관찰한 결과 다중층막이 형성되어 수분이 채워져 있고, 그 주변에는 10~60여층의 다중층의 리포좀 라멜라 베지클 형성되었음을 확인하였다. 임상시험(in-vivo)으로써, 센텔라좀 크림의 5주동안 바른 후의 보습효과는 28.3%로 플라세보 보다 유의차 있게 증가되었다. 피부탄력효과는 13.6%로 플라세보 보다 유의차 있게 보습력이 증가되었다. 눈가의 피부잔주름 개선효과는 플라세보 크림보다 23.52%가 개선되었다. 이 연구 결과를 통하여 개발된 센텔라좀-10EX의 리포좀 베지클에 봉입 안정화하는 처방과 제조방법에 대하여 연구할 수 있었으며, 이를 이용한 크림에 대한 주름개선효과 등의 임상연구를 통하여 얻어진 결과들이 화장품 산업에 있어서 피부과학을 연구하고, 고 효능을 가진 고품격 화장품을 개발하는데 폭넓게 활용될 수 있을 것으로 기대한다.

Alleviation of Atopic Dermatitis through Probiotic and Mixed-probiotic Treatments in an Atopic Dermatitis Model

  • The use of lactic-acid bacteria (LAB) is effective for preventing and curing immune disorders by activating the immune system in the digestive tract and the consequent immune response in the blood. In this study, LAB and mixed LABs were used in an atopic dermatitis (AD) mouse model. Alleviation of AD was observed based on the change in cytokine level and immunohistochemical staining. An ex vivo test showed that immunoglobulin-E and interleukin (IL)-4 levels were significantly lower in all groups treated with LAB than in the group treated with only 1-chloro-2,4-dinitrobenzene. Results of an in vivo test based on the ex vivo results showed that the scratch score decreased in all groups treated with the LAB and particularly decreased in the group treated with mixed LABs. Additionally, the T helper (Th) 1 cytokines interferon-gamma and IL-12p40 were upregulated by the LAB and mixed-LABs, whereas levels of the Th2 cytokine IL-4 were downregulated in a mouse model of AD-like skin lesions. Furthermore, hematoxylin & eosin and immunohistochemical staining of the dorsal area of the mice in each group showed that AD improved in the LAB-treated groups. These results suggest that LAB and mixed LABs inhibit the development of AD in NC/Nga mice by suppressing the Th2 cell response and increasing the Th1 cell response. Our results indicate that mixed LABs are better than LAB for treating AD-like skin lesions.

손소독제(겔형, 액제형, 와이프형)의 효능 평가법 개선: 평가 전략 연구 사례 및 손 균총 정보 활용 등 최근 동향 (Improvement of the Efficacy Test Methods for Hand Sanitizers (Gel, Liquid, and Wipes): Emerging Trends from in vivo/ex vivo Test Strategies for Application in the Hand Microbiome)

  • 피부를 대상으로 한 살균을 목적으로 하는 외용소독제의 경우 식품 취급자에 오염된 미생물의 사멸 또는 제거를 목적으로 활용될 수 있으며, 최근 개인위생에 대한 관심 증가에 따라 제품 소비 증가와 제품 다양화가 두드러지게 나타나고 있다. 살균 효능은 소독제의 핵심 품질 평가 요소로서 수행 절차 및 조건에 따라 상이한 결과가 나타날 수 있기 때문에 시험법의 효율성과 정확성을 높이기 위한 연구가 필요하다. 이에 본 총설논문에서는 주요 제형별(겔형, 액제형, 와이프형) 시험법 개발 현황을 파악하고 시험법별 특장점 분석 결과와 최근 관련 연구를 통하여 제시된 시사점을 기반으로 향후 효능 평가 체계의 발전 방향을 제시하고자 하였다. 인체 대상 시험법의 경우 시험 유형에 따라 소독제를 시험 대상 피부 표면에 처리하는 조건이 다양화되어 있어 시험법 간 동등성에 대한 평가를 통해 소독제 제품의 성분이나 특성에 따라 최적의 시험 유형을 파악하고 그에 대응되는 적절한 평가 체계 및 관련 규제의 표준화의 필요성을 시사하였다. 특히 와이프형 소독제의 경우 처리 방식이 미생물 제거 및 살균에 직접적으로 영향을 미침에도 불구하고 피부에 노출하는 손 대상 처리를 위한 사용 패턴의 표준화 사례가 부족 하였다. 한편 [전처리 - 소독제 노출 - 미생물 회수] 등 각 시험 절차별로 결과에 영향을 미치는 주요 결정 요인을 발굴하는 연구의 지속 수행을 통해 기존 시험법을 개선하고 신규 시험법을 개발하고자 하는 노력이 요구된다. 최근 활발하게 개발되고 있는 ex vivo 시험법은 인체 시험의 제한적인 연구 재현성과 같은 한계를 극복하면서도 인간 피부 환경을 구현하기 위한 기술의 적용을 통해 연구 결과의 신뢰도를 확보할 수 있을 것으로 판단된다. 한편 손 피부를 대상으로 한 균총 연구 등 소독제 처리 전후 미생물의 특성과 분포 분석 관련 연구가 최근 다수 보고되고 있어 이를 활용한 미생물 군집 단위의 소독제 효능 평가 시험법의 확립이 기대된다. 본 연구를 통해 제시된 소독제 효능 시험법의 현황 기반 발전 전략은 보다 효과적인 개인위생 관리 확립을 통해 손을 통해 교차 오염되는 미생물에 의한 감염성 질병 발생을 최소화하여 공중보건 및 식품 안전성 향상에 기여할 수 있다.

The Effect of an Optical Clearing Agent on Tissue Prior to 1064-nm Laser Therapy

  • Background and Objectives Although lasers have been widely applied in tissue treatment, the light penetration depth in tissues is limited by the tissue turbidity and affected by its absorption and scattering characteristics. This study investigated the effect of using an optical clearing agent (OCA) on tissue to improve the therapeutic effect of 1064 nm wavelength laser light by reducing the heat generated on the skin surface and increasing the penetration depth. Materials and Methods A diode laser (λ = 1064 nm) was applied to a porcine specimen with and without OCA to investigate the penetration depth of the laser light and temperature distribution. A numerical simulation using the finite element method was performed to investigate the temperature distribution of the specimen compared to ex-vivo experiments using a thermocouple and double-integrating sphere to measure the temperature profile and optical properties of the tissue, respectively. Results Simulation results showed a decrease in tissue surface temperature with increased penetration depth when the OCA was applied. Furthermore, both absorption and scattering coefficients decreased with the application of OCA. In ex-vivo experiments, temperatures decreased for the tissue surface and the fat layer with the OCA, but not for the muscle layer. Conclusion The use of an OCA may be helpful for reducing surface heat generation and enhance the light penetration depth in various near-infrared laser treatments.

Enhancing Skin Delivery of 5-Aminolevulinic Acid with Transferosome Using Lyso-Phospholipid and Surfactant

  • In order to enhance the clinical efficacy of 5-aminolevulinic acid-induced photodynamic therapy (ALA-PDT), liposomal formulations using bulk hydrogenated phospholipids from soybean were introduced. Three types of lipids, S75-3, S100-3, and SL80-3 were used for formulating ALA. The pH of all the liposomal ALA is 4.5~5.5 and the size is 50~200 nm. All the liposomal formulations gave better ex vivo ALA skin penetration using nude mice skin in Franz cell than free ALA did. Among them, SL80-3 including 22% of lyso-phosphocholine achieved excellent ALA penetration when compared with those of S75-3 and S100-3 which have only 1~2% of lyso-phospholipids. S100-3 showed a little better results than S75-3 did. Addition of humectants (glycerine, propylene glycol, butylene glycol, betaine) in liposomal ALA formulated with SL80-3 produced little enhancing effect in ALA penetration. On the other hand, addition of surfactants (Tween 20, 60, Brij 72, 76, 78) in same liposomal system produced significant increase in ALA penetration. Among them, transferosomal system of lyso-phospholipid, SL80-3 and the surfactant, Brij76 showed the highest ALA penetration. Furthermore, this system also established the highest in vivo PpIX biosynthesis in hairy mice skin of C57BL/6. These results concluded that the transferosome of SL80-3 and Brij76 produced the best results in both ALA penetration and PpIX biosynthesis, and proved good correlation between them.

Effect of Layer-by-Layer (LbL) Encapsulation of Nano-Emulsified Fish Oil on Their Digestibility Ex Vivo and Skin Permeability In Vitro

  • Omega-3 rich fish oils are extremely labile, thus requiring control of oxidation and off flavor development. A recently proposed emulsification method, layer-by-layer (LbL) deposition, was found to be a plausible method to enhance the characteristics of bioactive ingredients, especially lipids. The present work was designed to test the possibility of enhancing the uptake and utilization of omega-3 fatty acids present in fish oil. The bioavailability of nano-emulsified fish oil was monitored in terms of intestinal absorption as well as skin permeability by using the everted intestinal sac model and Franz cell model. The skin permeability and intestinal absorption characteristics was significantly improved by LbL emulsification with lecithin/chitosan/low methoxypectin. Multilayer encapsulation along with nano-emulsification can be a useful method to deliver biologically active lipids and related components, such as fish oil. The protective effect of this tool from lipid oxidation still needs to be verified.

Laboratory Experiments for the Force and Load with Pseudo-Dynamic Test: Ex-vivo Study for the Manual Therapy

  • Background: Because of the lack of accurate values for applied forces in manual therapy, manual therapists relies on the magnitude of the individual's perception during applying the force. However, excessive loading maneuvers carry risks for patients. Objective: To establish the relationship between the maximal force applied to swine skin with the specific region, sex, and baseline parameters of the subject. Design: Ex-vivo Study and laboratory Experimental research Methods: 3.5 kg of Korean pork sirloin that is a piece of swine was handled and it was set 3 dimensions; #A; #B; #C. Forty-seven participants who has no experience in physical therapy randomly carried out the experiment, indicated to push each place of the pressure spots with same posture and process under supervision from the instructor who has over 15 years of manual therapy, and we measured the pressure force in each time. Results: The biggest pressure force was recorded in spot #A, and #B was represented after #C. Pressure on #A showed certain statistic relation with height (r=.317, p<.05) and weight (r=.434, p<.01); pressure on #B showed certain relation which has statistical meaning with only height (r=.401, p<.01); pressure on #C emerged to have statistic relationship with height (r=.308, p<.05)and weight (r=.428, p<.01). The age aspect revealed relation with pressure on #A, #B and #C, but that was not statistically significant. Conclusions: It can be inferred that there is the most loss of pressure in the area where cartilage is like an island in the middle.

Antipruritic effect of curcumin on histamine-induced itching in mice

  • Itching is a common clinical symptom of skin disease that significantly affects a patient's quality of life. Transient receptor potential vanilloid 1 (TRPV1) receptors of keratinocytes and peripheral nerve fibers in skin are involved in the regulation of itching as well as pain. In this study, we investigated whether curcumin, which acts on TRPV1 receptors, affects histamine-induced itching in mice, using behavioral tests and electrophysiological approaches. We found that histamine-induced itching was blocked by topical application of curcumin in a concentration-dependent manner. In ex-vivo recordings, histamine-induced discharges of peripheral nerves were reduced by the application of curcumin, indicating that curcumin acts directly on peripheral nerves. Additionally, curcumin blocked the histamine-induced inward current via activation of TRPV1 (curcumin $IC_{50}=523nM$). However, it did not alter chloroquine-induced itching behavior in mice, which is associated with transient receptor potential ankyrin 1 (TRPA1). Taken together, our results suggest that histamine-induced itching can be blocked by topical application of curcumin through the inhibitory action of curcumin on TRPV1 receptors in peripheral nerves.
