Real-world Adverse Events Associated with Fluconazole and Itraconazole: Analysis of Nationwide Data Using a Spontaneous Reporting System Database (의약품부작용보고시스템 데이터베이스를 이용한 fluconazole 및 itraconazole 관련 이상사례 분석)

  • Objective: This study aimed to investigate the occurrence and types of the adverse events (AEs) associated with oral fluconazole and itraconazole and factors associated with specific types of AEs. Methods: We analyzed AEs reported by community pharmacies nationwide over 10 years using the Korea Adverse Event Reporting System database. Various AE terms were categorized into 18 types, and concomitant medications were classified by drug-drug interaction (DDI) severity. The relationship between the specific type of AE and age, sex, and number of concomitant medications was investigated using multiple logistic regression analysis. Results: A total of 879 AE reports of fluconazole and 401 reports of itraconazole were analyzed; of these reports, 321 and 83 reports of fluconazole and itraconazole, respectively, described concomitant drug administration categorized as DDI severity of contraindicated or major. Women had a higher risk of psychiatric AEs associated with fluconazole use (OR, 1.587; p=0.042). Polypharmacy increased the risk for psychiatric AEs (OR, 3.598; p<0.001 for fluconazole and OR, 2.308; p=0.046 for itraconazole). In dermatologic AEs, the mean age of patients who received itraconazole was lower than that of patients who received fluconazole (46.3±16.8 vs. 54.9±15.4; p<0.001). Co-administration of fluconazole with 1-3 drugs increased the risk of neurological AEs (OR, 1.764; p=0.028). Conclusion: When using fluconazole and itraconazole, psychiatric AEs should be noted, particularly in women and in case of polypharmacy; moreover, when fluconazole is co-administered with other drugs, attention should be paid to the occurrence of neurological AEs.

Management Strategy For Health Functional Food Containing Several Functional Ingredients (두 가지 이상 기능성원료를 복합하여 함유하고 있는 건강기능식품의 안전 관리 방안)

  • In this study, we analyzed the database for items reported to Korea Food and Drug Administration as for manufactured health/functional food during 2010. There were 183 health functional food products manufactured in domestic having over 2 functional ingredients (hereinafter, combinational health functional food) among total 7319 products. Among 183 products, there were 177 products having over two kinds of functional ingredient and 6 products were over 3 ingredients. The most commonly used functional ingredients in the combinational health functional food were Garcinia cambogia extracts which were used in 41 products, Octacosanol and Saw Palmetto extract. When we searched the safety information for the pair of ingredients used in combinational health functional food using several database, there were no reports for safety concern. However, as there are still safety concerns when intake various functional ingredients at once, we suggested to enforce the reporting system of adverse event in order to strength safety management of health functional food. With these complement, the safety management of health functional food might be achieved including a combinational products.

  • The thesis tries to compare The Structure of World History with Anti-Oedipus in the textual context, and to re-compare in the educational context. I mean by the education an event which contrasts starkly with an essence. It adopts 5W1H, a general reporting form of an accident or event, as the distinctive features at twice comparisons. The purpose of the thesis is not evaluative but interpretive comparison. In the textual context, the thesis discusses, 1) as WHAT, the use of Marx from Kant vs. Nietzsche's point of view, 2) as WHO, the actual subjects of the exchanging human vs. the productive machine, 3) as WHEN/WHERE, the society of the modes of exchange vs. the modes of inscription, 4) as HOW, the revolutionay means of the simultaneous revolution of the world vs. the schizophrenic process, 5) as WHY, the ideal subjects of the associative human vs. the non-human of liberation of desire. In the educational context, the thesis discusses, 1) in the WHAT as educational way, autonomous morality vs. active power, 2) in the WHO as the affirmity of actual subjects, that of the ideal idea vs. that of real power, 3) in the WHEN/WHERE, as the in-between time-space of education, the incommensurable communicative situation of humans vs. the conflictive of machines, 4) in the HOW, as the educational method of achieving the ideal, the involuntary restoration of the already-had ideal vs. the now-have completion and break-through of the schizophrenic process, 5) in the WHY, as the aim of education, cosmopolitan vs. overman.

Meta-analysis and Systematic Review on the Effects of Herbal Medicine and Synthetic Drugs in Helicobacter pylori Infection: Focusing on PubMED (Helicobacter pylori infection에 대한 한약과 합성의약품 병용투여 효과에 대한 메타분석 및 체계적 문헌고찰 : PubMED를 중심으로)

  • Objectives : In clinical practice, there are many cases of co-administration of herbal medicine and synthetic drugs. This study tried to identify whether the combined administration of herbal medicine and synthetic drugs including amoxicillin increases Helcicobacter pylori eradication rate compared to the single administration of synthetic drugs or the combined administration of synthetic drugs and placebo herbal medicine through systematic review. Methods : Relevant randomized controlled trials were searched in PubMED database. The risk of biases was assessed through the Cochrane Risk of Bias criteria. Three reviewers were extracted the characteristics and outcomes of each study. Meta-analysis of eradication rate and adverse event was conducted. Results : Four RCTs were selected. In meta-analysis, the combination of herbal medicine and synthetic drugs showed eradication effect, but it was not statistically significant (Odds Ratio [OR] 0.46; 95% confidence interval [CI] 0.17 to 1.24; p=0.13; I2=56%) than administration of synthetic medicine alone. Combination of herbal medicine and synthetic medicine did not increased the incidence of adverse event(OR 1.07; 95% CI 0.72 to 1.59; p=0.68, I2=0%) compared to single administration of synthetic medicine. Conclusion : Although no significant difference was observed between the two groups in the eradication rate and the incidence rate of adverse events, it was difficult to draw a clear conclusion due to the heterogeneity between studies and the low quality of reporting. A number of studies that have overcome these limitations in the future will lead to definite conclusions.

Mobile Payment and Operation System for the Local Area Festival (지역 기반 문화축제를 위한 모바일 결제 및 운영 시스템)

  • Local area festivals have grown on a scale for the past 20 years, but have suffered ups and downs. Through the trial and error of the festival operation, problems such as prediction of the number of visitors, planning of event scale, calculation and expansion of sales volume, and management of various participants in the duration were highlighted. To solve, this study designed and developed a mobile payment system and festival operation management system for local scale festivals as a platform operating system of web and app combined. The results of this study presents four basic functions. It includes ticketing management, attendance identification and entrance control, charge of festival currency and use of payments, real-time provision and management of related information, and performance reporting for each role. It was applied to local festivals in practice as to enable local shop owners to participate in advertisements or sponsorships and confirmed their contribution to local commercial market and the revitalization of festivals through this system.

  • Career building can be challenging for young surgeons, especially when topics such as lifestyle, work-life balance and subspecialization arise. Suggestions and advice from senior colleagues is very valuable but many young surgeons do not have such opportunities or are limited to a few senior surgeons. The International Microsurgery Club (IMC), in collaboration with the World Society of Reconstructive Microsurgery, organized a combined webinar for this topic and invited world renownedmicrosurgery masters polled by the IMCmembers to join, including Prof. Peter Neligan (Emeritus from University of Washington, United States), Prof. Raja Sabapathy (Ganga Hospital, India), Dr. Gregory Buncke (The Buncke Clinic, United States), Prof. Isao Koshima (Hiroshima University Hospital, Japan), Prof. David Chwei-Chin Chuang (Chang Gung Memorial Hospital, Taiwan), and Prof. Eric Santamaria (Hospital General Dr. Manuel Gea Gonzalez, Mexico) on May 1, 2022. Prof. Joon-Pio Hong (Asan Medical Center, South Korea) and Prof. Fu-Chan Wei (Chang Gung Memorial Hospital, Taiwan) were also selected but unfortunately could not make it and were therefore invited to another event in April 2023, summarized in a recently published paper. There is ample literature reporting on different aspects of developing a microsurgical career but the goal of this session was to offer an opportunity for direct exchange with experienced mentors. Moreover, insights from experienced microsurgeons from different part of the world were more likely to offer different perspectives on aspects such as career building, failure management, and team culture. This webinar event was moderated by Dr. Jung-Ju Huang (Taiwan), Dr. Susana Heredero (Spain), and Dr. Wei F. Chen (United States).

A Study on the Revision of the Notification Form and Procedures of Marine Incident (준해양사고 통보서식 및 절차 개정에 관한 연구)

  • Accident prevention is more important than follow-up, which is based on Heinrich's law. The marine incident system is a very meaningful system that can prevent similar accidents, and was introduced in 2010 in Korea in accordance with the enforcement of the Code for the Investigation of Marine Casualties and Incidents (CI Code). Based on the CI Code, ship owners or ship operators are required to notify the Central Chief Inspector using the designated notification form in the event of a marine incident, but the number of voluntary notifications is still small. In this regard, this study intends to provide a direction for improvement by conducting an in-depth analysis focusing on the lack of notification procedures and forms of the marine incident system. To this end, we analyzed related regulations, cases of excellent overseas shipping countries such as the United Kingdom and Singapore, cases of similar domestic transportation systems such as aviation and railways, and marine incident notification procedures and forms of leading shipping companies. Major improvements in the notification process include the transition of the marine incidents to voluntary reporting, the expansion of the reporting subjects, and the identification of the security of the informer's identity. The main contents of the notification form revision include the use of the term "reporting" instead of "notification," the content of the identity guarantee in the notification form, and the increase in statistical value through the expansion of optional entries.

As USN-applied field is expanded, multiple application services are expected to be provided through one USN soon. Multiple applications be required a periodic data reporting, consecutive information monitoring, event-driven data and query-based data as the data pattern that USN is delivered. Accordingly, the mechanism which can assure QoS according to each data characteristic is required. In this paper, A proposed protocol apply the duty cycle of S-MAC protocol variably and flexible according QoS level. This method largely reduces the delay on the delay-sensitive traffic, while keeping the energy efficiency.

A Study on Worker's Perception of Patient Safety Culture in a hospital (일개 병원의 환자안전문화에 대한 인식)

  • Background : The purpose of study in to grasp the level of perception of hospital workers on the patient safety culture, consider the difference in perception of patients safety culture according to medical service and finally find out a way to establish patient safety culture in hospital. Methods : As for the data, the analysis on frequency, t-test, ANOVA and tukey test were carried out by using SPSS 12.0. Result : The results of comparison among the positive response ratios on the patients culture of hospital workers showed that the subjects had perceived the teamwork within units most positively(74.1%), and perceived most negatively on the non-punitive response to error(16.2%)and the staffing(26.2%). 68.6% of subjects answered that the medical error were mostly of always reported. when daytime working hours are longer, perception of patient safety culture ranked low. In general, departments for direct medical service than departments for indirect medical service assessed patient safety culture high. Conclusion : Organizational learning and teamwork within units, communication openness, active support of hospital management for patient safety, and cooperation across the units would be crucial to promote the overall perceptions of patients safety of hospital workers and the level of patients safety in the units and to improve the quality of the event reporting system.
