Use of Economic Evaluation in the Listing and Pricing of Pharmaceuticals (의약품 보험 급여 및 가격 결정과 경제성 평가의 활용)

  • To curb a rapid increase in expenditures for pharmaceuticals, the Korean government introduced a positive list system and a negotiation process for drug prices at the end of 2006. Economic evaluation of pharmaceuticals has begun to have a pivotal role in the listing and pricing of drugs for the Korean National Health Insurance. There are some points to discuss regarding the use of economic evaluation in the listing and pricing in the context of the Korean system. First, the listing and pricing processes have been fragmented, evoking complaints from pharmaceutical companies and delaying the access of new drugs to patients. Second, there is a concern that the positive list system may limit the range and availability of drugs for patients to choose for treatment. Third, the time schedule for de-listing of existing drugs may not be realistic. Fourth, it is not always easy to provide reliable evidence of cost-effectiveness due to a lack of materials. Fifth, there is no consensus on the range of the ICER (incremental cost-effectiveness ratio) acceptable to the Korean society. In conclusion, in the near future, it will be necessary to evaluate the achievements that the economic evaluation has provided to the Korean society.

Review of Programs for Improving Patient's Access to Medicines (의약품 접근성 향상을 위한 방안 고찰)

  • Objective: This study presented the analysis period, the complexity of combined therapy and comparator choice as the key limitations in the economic evaluation of new drugs, and discussed programs for coping with these limitations. Methods: This study evaluated the post-evaluation, risk-sharing agreement, extra funding program, and flexible incremental cost-effectiveness ratio (ICER) threshold as actions or programs that would increase accessibility to costly new drugs. The study also presented the cases of other countries. The application of the post-evaluation was considered to deal with high uncertainty regarding new drugs. Results: The risk-sharing agreement was introduced in European countries as well as South Korea and has been responsible for the shift from using the financial schemes to outcome-based schemes. The drug funding program has had troubled in securing stable extra funds. The application of higher ICER in the economic evaluation of expensive and innovative oncology drugs was criticized because of the inequity between oncology patients and patients with other diseases. Conclusion: Therefore, introducing and applying actions that would increase the accessibility to costly new drugs in South Korea have been deemed necessary after careful reviews and discussions with various stakeholders (insurer, policy makers, pharmaceutical companies and patients).

  • The purpose of this study is prove the effectiveness of an energy subtraction image for the detection of pulmonary nodules and the effectiveness of multi-resolutional filter on an energy subtraction image to detect pulmonary nodules. Also we examine influential factors to the accuracy of detection of pulmonary nodules from viewpoints of types of images and evaluation methods. As one type of images, we select energy subtraction X-ray images, at the same time is done ▽$^2$G filter and multi-resolutional filter. Here select two evaluation methods and make clear the effectiveness of multi-resolutional filter on an energy subtraction image.

A Rational Development Way of Performance Evaluation Measures for the Informatization Program by means of the Coincidence Analysis (부합성 분석을 이용한 정보화지원사업 성과평가지표의 합리적 도출 방안)

  • The measurement of informatization program effectiveness continues to be a central concern of both academics and practitioners. However, most studies of it have been inclined too heavily toward testing validity and reliability of evaluation measure to strengthen the theoretical rigor. Drawbacks of this approach have been pointed out as being unsuitable for evaluating the effectiveness of informatization program due to lack of practical relevance. The paper comprises four sections to solve the problem. 1) Through literature review of theoretical conceptual models in IS effectiveness and the domestic and foreign cases on informatization program, we establish the evaluation framework which consists of evaluation phases, 6 evaluation areas, and 22 evaluation items, and developed evaluation measures of each items. 2) Different researchers have addressed different aspects of attribute of measure. With comparing them to each other, 3 mutually exclusive criteria on coincidence analysis(usability of measurement. reliability of measurement. and validity of content) are identified. 3) Data are gathered from respondents in informatizaiton program-related organizations such as policy agency, management agency, support agency, and recipient agency. 4) The degree of coincidence of each evaluation measure are analyzed by course of an coincidence analysis procedure, which includes the reliability test, arithmetical test, and criteria validity test. The academic implication of this study is that researchers can gain an insight into the coincidence analysis to approach the problem of deciding on an indicator of performance for informatization program. There is also practical implication for applying the potential means of improving the practical relevance of conducting evaluating the informatization program.

  • Evaluation of education and training is the act of measuring and observing contents related to an educational program and evaluating its quality; and thereby providing the basis for the changes in the curriculum. The National Library of Korea (NLK) has urgently needed to introduce an evaluation method that can enhance the performance and quality of its continuing education program. Therefore, this study is aimed at developing an evaluation model that can objectively and comprehensively measure the effectiveness of NLK's job training program for its librarians. To this end, the study applied Kirkpatrick's 4-Level Training Evaluation Model and conducted review of preceding researches, investigation of NLK's librarian continuing education program, along with analysis on the stakeholders' needs. Based on the results, the study suggested evaluation system to establish the directions of the evaluation system. The findings of this study are expected to be used as a practical evaluation system for NLK's librarian education program.

A Study on the Effectiveness of Evaluation Indicators for the Small Library (작은도서관 평가지표의 유효성에 관한 연구)

  • This study was implemented to verify the validity and effectiveness of the current evaluation indicators by analyzing the content and result of the current survey for evaluating a small library, based on the achievement rate of each index. To this end, questions, included in evaluation indices for the small library were reviewed, and survey data from a total of 19,476 libraries during the past 3 years from 2018 to 2020 were investigated to analyze the evaluation scores, differences and achievement rates of each item. It was proposed to delete or add some indicators of the current evaluation category, and to raise supplementally the need for qualitative assesment. Based on the research results, the following implications are suggested. First of all, the evaluation of small libraries should focus on the quality of the services they provide rather than the library environment. Second, demonstrable systematic indicators should be designed and then clearly explained to resolve the ambiguity of the terms used. Third, for better analysis, it is necessary to distinguish evaluation items between public and private small libraries and the assessment process should be done by the local government. Lastly, qualitative evaluation items that can measure users' satisfaction should be added.

Mission Effectiveness Model Applicable For Military System's Evaluation and Test Design

  • Mission effectiveness, which is the probability of successfully completing the assigned mission, is introduced as an appropriate measure of effectiveness for a military system. The model of mission effectiveness is developed for a system which is required to carry out various types of a mission. Each mission type is characterized by the maximum allowable time that determines the success of a given mission type. A given type of a mission is successful if and only if (i) the system is available at the start of a mission and (ii) the system completes its mission within the maximum allowable duration of time that this given mission type specifies without any failure during this period. Both analytic and simulation approaches are employed. Difficulties involved in the anayticl approach are discussed. The model is proposed as a useful tool for consistent system evaluation and optimum test design.

  • The recent notion of Network Centric Warfare (NCW) emphasizes the ability to distribute right information at the right time to maximize the combat effectiveness. Accordingly, in the modern combat system the importance of non-physical element, such as communication system is increasing. However, an NCW-support communication network system is expensive. Therefore, it is essential to develop a proper combat system evaluation method to establish an efficient NCW-support combat system. Traditionally, combat system effectiveness is measured in terms of physical elements such as men and fire power. Obviously, such method is hardly applicable to a modern combat system To overcome this difficulty, we propose an evaluation model based on CA (Cellular Automata) simulation. A set of preliminary combat simulations show that CA simulation may be promising in evaluating non-physical element of a modem combat system.

Evaluation of Organizational Effectiveness as to Types of Maternal-Child Health Management Program in Public Health Centers (보건소 모성과 어린이 건강관리사업의 조직유형별 효과성 평가)

  • Purpose: The purposes of this study are; to classify executive organizations engaged in MCH programs at 23 advanced public health centers according to each characteristic; evaluate the effectiveness of organizations engaged in MCH programs. Method: This study analyzes me data on the present conditions and evaluates the organizational effectiveness of MCH workers at 23 advanced public health centers. The organizational effectiveness of MCH organization is assessed by both MCH workers and clients who have received MCH services at the three health centers selected from me 23 advanced public health centers. Finally, this study seeks to obtain consensus among experts in the field of MCH. Results: The results are as follows. The executive organizational systems for MCH services are classified into three types: Type I (provides MCH and HP services by MCH org.); Type II(provides MCH and HP Services by HP org); and Type III(provides MCH Services by MCH org. and HP services by HP org.) at leading MCH Health Centers. The evaluation of the organizational effectiveness of MCH org. shows that Type II is the most effective in terms of teamwork, autonomy, service quality, and resource utilization. According to clients' evaluation of organizational system in the field of MCH, Type II is also me most effective. Conclusion: This study suggests that the executive organization for the MCH and HP services for maternity and children should be established in accordance with lifetime health programs in order to efficiently utilize the limited MCH and HP resources at public health centers.
