입원환자 일반촬영 이용량 및 피폭선량: 2018년 입원환자데이터 (General Radiography Imaging Usage and Effective Dose of Inpatients: Based on Data from Inpatients in 2018)

  • In this study, we analyzed the use of general radiography imaging and effective dose in inpatients. Our aim is to help reduce national medical radiation exposure doses and develop rational health-care financial policies. The effective dose for each general radiography was calculated using the ALARA-GR program for 53 types (total: 260 codes) general radiography codes selected from 'National Health Insurance Care Benefit Cost'. The usage of general radiography was analyzed in the 2018 inpatient patient data of the Health Insurance Review and Assessment Service, and the effective dose for each general radiography was analyzed. 89.00% of inpatients undergo general radiography imaging at least once, with an average of 12.63 scans per person and an effective dose of 1.00 mSv. Those who received support from Medical Aid showed a higher value compared to those who were insured by National Health Insurance, with 17.39 cases and 1.43 mSv (p<.001). Chest had the highest usage rate at 23.12% for general radiography imaging, while L-spine had the highest effective dose at 24.53%. It is estimated that 420 inpatients patients undergo 121 to 820 general radiography imaging procedures per year, and 233 inpatients are estimated to have an annual effective dose of >20.00~58.25 mSv. Rational use of health-care finances and the practice of medical radiation safety management are essential for the well-being of individuals, the enhancement of quality of life, and the improvement of health-care quality.

  • We will provide basic data on the evaluation of patient dose in terms of DECT quality control by comparing the equipment-provided dose with the measured dose according to the configuration method of the X-ray generator by the manufacturer of the dual-energy CT unit. For computed tomography (CT) equipment, Discovery 750HD, Aquilion ONE GENESIS Edition, and Somatom Definition Flash were used. The $CTDI_{vol}$ value was measured by inserting the Unfors Xi ion chamber into a 32 cm PMMA acryl Phantom. The results of estimated $CTDI_{vol}$ DECT and measured $CTDI_{vol}$ showed that the dose difference between DECT 80 + 140 kVp of G company was at least 0.51% and -1.90% max, and measured $CTDI_{vol}$ was slightly lower (p<0.05). The difference of 80 + 140 kVp of S company was the minimum of 5.84% and the maximum of 7.52% (p<0.05). The measured $CTDI_{vol}$ was less than estimated $CTDI_{vol}$. The C company's 80 + 135 kVp showed a difference of at least 7.58% and a maximum of 13.58% (P<0.05), and all of measured $CTDI_{vol}$ was less. The linearity of exposure dose for all DECT equipment was very linearly reflected with $R^2$ being 0.97 or above, and the measured dose of the ionization chamber was less than the predicted dose of the monitor.

우리나라 응급의료센터 응급구조사의 직업적 방사선 노출 (Occupational Radiation Exposure of Emergency Medical Technicians in Emergency Medical Centers in Korea)

  • Objectives: This study aims to investigate the occupational radiation exposures of emergency medical technicians(EMTs) in emergency medical centers in Korea. The results will provide a basis for developing prevention programs to minimize adverse health effects relating to radiation exposure among emergency medical technicians working in this area. Methods: Radiation exposure doses were measured for twenty-two EMTs working in six emergency medical centers. Thermo Luminescent Dosimeters(TLD) were placed on three representative body parts, including chest, neck, and a finger. Measurements were conducted over the entire working hours of the participants for foor weeks. Dosimeters were analyzed according to a standard method by a KFDA-designated lab. Detection rate, annual radiation exposure dose, and relative levels to dose limit were derived based on the measured doses from the dosimeters. SPSS/Win 18.0 software(IBM, US) was used for statistical analysis. Results: Detection rates were 45.5%, 36.4%, and 45.5% for the dosimeters sampled from chest, neck, and a finger, respectively. The average annual doses were $2.39{\pm}3.44mSv/year$(range 0.38-10.0 mSv/year) for the chest, $2.72{\pm}3.05mSv/year$(2.00-11.34) for the neck, and $20.98{\pm}17.57mSv/year$(1.25-53.50) for the hand dose. The average annual eye dose was estimated to $3.61{\pm}2.37mSv/year$(1.50-8.34). The exposure dose levels of EMTs were comparable to those of radiologists, who showed relatively higher radiation dose among health care workers, as reported in another study. Conclusions: EMTs working in emergency medical centers are considered to be at risk of radiation exposure. Although the radiation exposure dose of EMTs does not exceed the dose limit, it is not negligible comparing to other professionals in health care sectors.

Radiation Dose Distribution of a Surgeon and Medical Staff during Orthopedic Balloon Kyphoplasty in Japan

  • Background: The present study investigated the radiation dose distribution of balloon kyphoplasty (BKP) among surgeons and medical staff, and this is the first research to observe such exposure in Japan. Materials and Methods: The study subjects were an orthopedic surgeon (n = 1) and surgical staff (n = 9) who intervened in BKP surgery performed at the National Hospital Organization Disaster Medical Center (Tokyo, Japan) between March 2019 and October 2019. Only disposable protective gloves (0.022 mmPb equivalent thickness or less) and trunk protectors were used, and no protective glasses or thyroid drapes were used. Results and Discussion: The surgery time per vertebral body was 36.2 minutes, and the fluoroscopic time was 6.8 minutes. The average exposure dose per vertebral body was 1.46 mSv for the finger (70 ㎛ dose equivalent), 0.24 mSv for the lens of the eye (3 mm dose equivalent), 0.11 mSv for the neck (10 mm dose equivalent), and 0.03 mSv for the chest (10 mm dose equivalent) under the protective suit.The estimated cumulative radiation exposure dose of 23 cases of BKP was calculated to be 50.37 mSv for the fingers, 8.27 mSv for the lens, 3.91 mSv for the neck, and 1.15 mSv for the chest. Conclusion: It is important to know the exposure dose of orthopedic surgeons, implement measures for exposure reduction, and verify the safety of daily use of radiation during surgery and examination.

QUANTITATIVE DATA TO SHOW EFFECTS OF GEOMETRIC ERRORS AND DOSE GRADIENTS ON DOSE DIFFERENCE FOR IMRT DOSE QUALITY ASSURANCE MEASUREMENTS

  • To quantitatively evaluate how setup errors in conjunction with dose gradients contribute to the error in IMRT dose quality assurance (DQA) measurements. The control group consisted of 5 DQA plans of which all individual field dose differences were less than ${\pm}5%$. On the contrary, the examination group was composed of 16 DQA plans where any individual field dose difference was larger than ${\pm}10%$ even though their total dose differences were less than ${\pm}5%$. The difference in 3D dose gradients between the two groups was estimated in a cube of $6{\times}6{\times}6\;mm^3$ centered at the verification point. Under the assumption that setup errors existed during the DQA measurements of the examination group, a three dimensional offset point inside the cube was sought out, where the individual field dose difference was minimized. The average dose gradients of the control group along the x, y, and z axes were 0.21, 0.20, and 0.15 $cGy{\cdot}mm^{-1}$, respectively, while those of the examination group were 0.64, 0.48, and 0.28 $cGy{\cdot}mm^{-1}$, respectively. All 16 plans of the examination group had their own 3D offset points in the cube. The individual field dose differences recalculated at the offset points were mostly diminished and thus the average values of total and individual field dose differences were reduced from 3.1% to 2.2% and 15.4% to 2.2%, respectively. The offset distribution turned out to be random in the 3D coordinate. This study provided the quantitative data that support the large individual field dose difference mainly stems from possible geometric errors (e.g., random setup errors) under the influence of steep dose gradients of IMRT field.

A study on pressurizer cutting scenario for radiation dose reduction for workers using VISIPLAN

  • The operations in the design lifecycle of a nuclear power plant targeted to be decommissioned lead to neutron activation. Operations in the decommissioning process include cutting, decontamination, disposal, and processing. Among these, cutting is done close to the target material, and thus workers are exposed to radiation. As there are only a few studies on pressurizers, there arises the need for further research to assess the radiation exposure dose. This study obtained the specifications of the AP1000 pressurizer of Westinghouse and the distribution of radionuclide inventory of a pressurizer in a pressurised water reactor for evaluation based on literature studies. A cutting scenario was created to develop an optimal method so that the cut pieces fill a radioactive solid waste drum with dimensions 0.571 m × 0.834 m. The estimated exposure dose, estimated using the tool VISIPLAN SW, in terms of the decontamination factor (DF) ranged from DF-0 to DF-100, indicating that DF-90 and DF-100 meet the ICRP recommendation on exposure dose 0.0057 mSv/h. At the end of the study, although flame cutting was considered the most efficient method in terms of cutting speed, laser cutting was the most reasonable one in terms of the financial aspects and secondary waste.

치과 파노라마 장치의 X선 공간선량분포 측정을 통한 두경부 피폭영역 조사에 대한 연구 (Study on the Exposure Field of Head and Neck with Measurement of X-ray dose Distribution for Dental Panoramic X-ray System)

  • 최근 고령화 사회에 접어들고 있는 의료현장에서 치아건강에 대한 관심이 커지면서 진단을 위한 치아 방사선 검사의 횟수가 증가하고 있다. 이는 국민 전체의 방사선피폭량 또한 증가하고 있다고 볼 수 있다. 또한 치과방사선에 대한 국민들의 방사선 피폭에 대한 관심도 증가하고 있어 치과 파노라마 촬영장치에 대한 기본 데이터 확보와 이에 대한 조사 및 선량의 측정이 필요하다. 본 연구에서는 ALOKA PDM-117 선량계를 이용하여 치과파노라마장치(VATEC Pax-400)에서 발생되는 2차원적인 선량분포도를 측정하고 그 분포에 대한 평가를 환자의 방사선피폭 차원에서 확인하였다. 치과파노라마장치의 선량분포는 치아부분 이외에도 턱과 안면부위에서 높았으며 산란선의 영향까지 고려한다면 방사선에 민감한 수정체에까지 불필요한 방사선의 피폭됨을 알 수 있었다. 본 연구 결과는 다양한 크기의 검사체와 선량 측정위치에서 보다 정확한 선량평가를 하는데 매우 유용하게 이용될 것으로 사료된다.

자궁경부암 강내 방사선 조사장치에 의한 직장 및 방광의 피폭선량 평가 (Dose Distribution of Rectum and Bladder in Intracavitary Irradiation)

  • The intrauterine irradiation is essential to achieve adequate tumor dose to central tumor mass of uterine malignancy in radiotherapy. The complications of pelvic organ are known to be directly related to radiation dose and physical parameters. The simulation radiation and medical records of 203 patients who were treated with intrauterine irradiation from Feb. 1983 to Oct. 1983, were critically analized. The physical parameters to include distances between lateral walls of vaginal fornices, longitudinal and lateral angles of tandem applicator to the body axis, the distance from the external os of uterine cervix to the central axis of ovoids were measured for low dose rate irradiation system and high dose rate remote control afterloading system. The radiation doses and dose distributions within cervical area including interesting points and bladder, rectum, according to sources arrangement and location of applicator, were estimated with personal computer. Followings were summary of study results ; 1. In distances between lateral walls of vaginal fornices, the low dose rate system showed as $4\~7cm$ width and high dose rate system showed as $5\~6cm$. 2. In horizontal angulation of tandem to body axis, the low dose rate system revealed mid position$64.6\%$, left deviation $19.2\%$and right deviation $16.2\%$. 3. In longitudinal angulation of tandem to body axis, the mid position was $11.8\%$ and anterior angulation $88.2\%$ in low dose rate system but in high dose rate system, anterior angulation was $98.5\%$. 4. Down ward displacement of ovoids below external os was only $3\%$ in low dose rate system and $66.7\%$ in high dose rate system. 5. In radiation source arrangement, the most activities of tandem and ovoid were 35 by 30 in low dose rate system but 50 by 40 in high dose rate system. 6. In low and high dose rate system, the total doses an4 TDF were 50, 70 Gy and 141, 123, including 40 Gy external irradiation. 7. The doses and TDF in interesting points Co, B, were 93, 47 Gy and 230, 73 in high dose rate system but in low doss rate system, 123, 52 Gy and 262, 75 respectively. 8. Doses and TDF in bladder and rectum were 70, 68 Gy and 124, 120 in low dose rate system, but in high dose rate system, 58, 64 Gy 98, 110 respectively, and then grades of injuries in bladder and rectum were 25, $30\%$ and 18, $23\%$ respectively.

코발트-60 조사야 밖의 장기에 미치는 2차선의 영향 (Effect of Scatter ray in Outside Telecobalt-60 Field Size)

  • Radiation dose outside the radiotherapy treatment field can be significant and therefore is of clinical interest estimating organ dose. We have made measurements of dose at distances up to 70 cm from the central axis of $5{\times}5$, $10{\times}10$, $15{\times}15$, and $25{\times}25$ cm radiation fields of Co-60 ${\gamma}-ray$, at 5 cm depth in water. Contributions to the total secondary radiation dose from water scatter, machine (collimator) scatter and leakage radiation have been seperated. We have found that the component of dose from water scatter can be described by simple exponential function of distance from the central axis of the radiation field for all field sizes. Machine scatter contributes 20 to 60% of the total secondary dose depending on field size and distance from the field. Leakage radiation contributes very little dose, but becomes the dominant componant at distance beyond 40 cm from the central axis. Then, wedges can cause a factor 2 to 3 increase in dose at any point outside the field compared with the dose when no wedge is used. Adding blocks to a treatment field can cause an increase in dose at points outside the field, but the effect is much smaller than the effect of a wedge. From the results of these measurements, doses to selected organs outside the field for specified treatment geometries were estimated, and the potential for reducing these organ doses by additional shielding was assessed.

저준위 방사선 노출의 암 유발에 관한 국내 원전종사자 코호트 연구 (A Cohort Study on Cancer Risk by Low-Dose Radiation Exposure among Radiation Workers of Nuclear Power Plants in Korea)

  • 원전 종사자를 대상으로 경시적인 코호트 연구를 통하여 저준위 방사선 노출과 암 발생 위해도를 규명하고자 하였다. 방사선 노출에 관한 정보는 한국수력원자력(주)의 방사선 관리 DB에서 수집하였고, 암 발생에 관한 정보는 한국인 중앙 암 등록 자료를, 암 사망에 관한 정보는 통계청 사망원인 자료를 이용하여 수집하였다. 방사선 노출과 암 발생 위해도는 표준화 암 사망비(SMR)와 표준화 암 발생 비(SIR)로 평가하였다. 노출 군에서 암 발생에 대한 상대위험도는 1.08로 평가되었으나 전체 암에 대한 SIR은 0.81로 유의성이 관찰되지 않았다. 암 사망에 대한 상대위험도는 1.21 이었으나 전체 암에 대한 SMR도 0.86으로 역시 유의성은 관찰되지 않았다. 암 유형별 양상은 우리나라 일반인과 유사한 결과를 보였으며, 방사선량 증가에 따른 양-반응 관계 또한 확인되지 않았다.
