Effect of Coverage Expansion Policy for an Ultrasonography in the Upper Abdomen on Its Utilization: A Difference-in-Difference Mixed-Effects Model Analysis (상복부초음파검사 급여확대에 따른 의료이용의 변화: 이중차이 혼합효과모형 추정방법을 이용하여)

  • Background: Korea has gradually expanded the coverage of medical care services in its national health insurance system. On April 1, 2018, it implemented a policy that expanded the coverage for an ultrasonography in the upper abdomen. In this study, we aimed to investigate the effect of the policy on the utilization of the ultrasonography in the upper abdomen in tertiary care hospitals. Methods: Using the dataset of the Health Insurance Review and Assessment Service, we explored changes in the utilization of the ultrasonography in the upper abdomen in tertiary care hospitals from July 1, 2017 to November 30, 2018 through the difference-in-difference (DID) mixed-effects-model method. Facility factor, equipment factor and personnel factors, type of hospital, the total amount of medical care expenses, and geographic region were considered as control variables. Results: On average, the utilization of the ultrasonography in the upper abdomen increased by 228% after the coverage expansion policy. However, the results of DID mixed-effects-model method analysis showed that the utilization increased by 73%. As for the number of beds, the utilization was higher with a group of 844-930, 931-1,217, and 1,218 or greater compared with a group of 843 or fewer, while the utilization of the number of ultrasonic devices was lower with a group of 45-49 compared with a group of 44 or fewer. The utilization decreased with the number of interns and the number of nurse assistants. Besides, relative to Seoul, the utilization was lower in the other metro-cities and provinces. Conclusion: The coverage expansion policy in the national health insurance system increased service utilization among people. Future research needs to investigate the degree to which such coverage expansion policy reduces the unmet medical care needs among the deprived in Korea.

Experimental study on the suppression of fire fighting by using Compressed Air Foam system (압축공기포(Compressed Air Foam) 소화시스템을 이용한 구난역 열차 화재 진압에 관한 실험적 연구)

  • Since the Daegu subway fire accident, people's perception of safety has increased, and all materials inside the train have been changed to incombustible materials. However, there is still a lack of development of fire extinguishing systems. Train components are mostly made of steel plates, and therefore it is very difficult to extinguish the train fire by using general fire extinguishing equipment. In this regard, this paper investigated rapid and easy methods of extinguishing the train fire by using compressed air foam systems through full-scale fire tests. To extinguish the fire of train at rescue station, window breakers were used to quickly destroy the train windows, and the compressed air foam system was inserted inside the train. As a result, the train windows were destroyed in 5 seconds, and the 11.88-MW fire was put out in 30 seconds by the compressed air foam discharged from the compressed air foam system inserted inside the train. For the future work, there is a need for further experimental studies to prevent the spread of fire and protect tunnel structures with the use of compressed air foam systems.

Developing Evaluation Index and Item for Water Environment Improvement of Gyeongin ARA Waterway (경인 아라뱃길의 물환경 개선을 위한 오염원인 평가항목 및 지표 개발)

  • This research has developed the criteria and index for systematically and objectively assessing the quality of river water by fixing the various factors that affect Gyeongin ARA waterway's water quality through analysis with the Delphi Technique and analytic hierarchy program (AHP) Method. Based on the results, the highest criteria are, in order of importance, physical and environmental factors 28%, administrative factors 26%, natural fixed factors 26% and finally, cultural and social factors 20%. The three dimensions of the criteria show that for the internal physical and environmental factors, the most important are the loss of self-purification capacity, and the external factors are Gulpocheon and the sludge deposit due to Gyulhweon-weir the bridge. The facility factor in management was affected by the coagulation and waste water disposal facilities. The problem for the policy and institutional factors was seen in the regulatory area. The aquatic ecology/ point pollution source for the natural fixed factors show that it is due to the polluted water of Gulpo-cheon and the living environment/ non-point pollution source is shown through the inflow water from other rivers. Cultural and social factors show that the economical causes were due to the cargo and passenger flight operations and the external factors of having a lack of sewage treatment equipment have an importance effect. In order to estimate the order of priority through logical evidence and objectivity, future research must be continued on the evaluation indexes to measure the specific methodology and technique needed to improve the Gyeongin ARA Waterway.

Study on the Gas Tight Shut-off Valve of NBC Shelter using Positive Pressure Measurement and Chemical Detection Module (양압측정 및 화학탐지 모듈을 적용한 화생방 방호시설의 가스차단밸브에 관한 연구)

  • One of the most frightening aspects of weapons of mass destruction (WMD) is their ability to cause death in very small quantities without being visible to the public. The military authorities are making considerable effort to ensure the survivability of the combatants in the event of NBC(Nuclear, Biological and Chemical) contamination. Therefore, in this study, modules were developed for the measurement of the positive pressure and for the detection of the chemicals used for the control of the various shut-off valves used in an NBC shelter. In addition, a high performance gas tight shut-off valve was developed that can overcome the disadvantages associated with manual manufacturing, such as the occurrence of defective products and high manufacturing cost. By applying the positive pressure measurement and chemical detection modules, this valve was able to be used to control the facility. The developed gas-tight shut-off valve maintained airtight characteristics at a pressure loss of 28[Pa] at the prescribed wind velocity and an internal pressure of 30[kPa]. It is expected to be possible to control the gas-tight shut-off valve through the remote measurement of the positive pressure, thereby ensuring the foreign independence of import substitution and defense related technology in the future. In addition, by installing these valves in all of the intake ports or exhaust ports connected to the outside of the NBC shelter, it is possible to prevent the damage resulting from the rapid inflow of the storm pressure caused by conventional weapons and nuclear explosions, thereby protecting the people and equipment in the shelter.

Seismic Fragility Evaluation of Cabinet Panel by Nonlinear Time History Analysis (비선형시간이력해석을 이용한 수배전반의 지진취약도 도출)

  • Earthquakes are almost impossible to predict and take place in a short time. In addition, there is little time to take aggressive action when an earthquake occurs. Therefore, there are more casualties and property damage than with other natural disasters. Recently, earthquakes have been occurring all over the world. As the number of earthquakes increase, studies on the safety of structures are being carried out. On the other hand, there are few studies on the electric facilities, which are relatively non - structural factors. Currently, electrical equipment in Korea is often not designed for earthquake safety and is quite vulnerable to damage when an earthquake occurs. Therefore, in this study, modeling was conducted through ABAQUS similar to an actual cabinet panel and 3D dynamic nonlinear analysis was performed using a natural seismic. According to seismic zone I and normal ground rock conditions of the power transmission and transmission facility seismic design practical guide, the maximum response acceleration of the performance level was 0.157g. In this study, however, it was not safe to reach the limit state of 30% of the analytical result at 0.1g for the general cabinet panel. From the results, the seismic fragility curve was derived and analyzed. The derived seismic fragility curve is presented as a quantitative basis for determining the limit state of the cabinet panel and can be utilized as basic data in related research.

Scaling Up Fabrication of UO2 Porous Pellet With a Simulated Spent Fuel Composition (모의 사용후핵연료 조성의 UO2 다공성펠렛 제조 스케일 업)

  • Processing and equipment were tailored for engineering scale fabrication of $UO_2$ porous pellets, a feed material for the electrolytic reduction process in the PRIDE (PyRoprocessing Integrated DEmonstration) facility at KAERI (Korea Atomic Energy Research Institute). The starting materials, $UO_2$ powder and pre-milled surrogate oxide powders, were proportioned to simulate the chemical composition of spent fuel (so-called Simfuel). The Simfuel powders were homogenized by mixing, compacted into a pellet shape, and finally heat treated using a tumbling mixer, rotary press, and sintering furnace. After sintering at $1450^{\circ}C$ for 24 h in $4%\;H_2-Ar$, the average bulk density of the $UO_2$ Simfuel pellets was $6.89g{\cdot}cm^{-3}$, which meets the standard of the following electrolytic reduction process. In addition, the results of a microstructural analysis demonstrated that the sintered Simfuel $UO_2$ porous pellets accurately simulate the properties of spent fuel in terms of the formation of second phases. These results provide essential information for the massive fabrication of $UO_2$ porous pellets for engineering scale pyroprocessing research.

On Enhancing Safety of Train-Centric Train Control System using Model-Based Development (차상중심 열차제어시스템 개발에서 모델기반 접근을 통한 안전성 향상에 관한 연구)

  • The train control system is a facility to ensure model-based design and safe train operation, and its safety is the most important factor for system introduction, complexity of the design information and traceability etc. Therefore, the model-based design and safety activities regarding the way-side equipment of a train control system is also highlighted. To solve this problem, In this paper, model-based design was carried out first to develop an effective train control system, which is represented by SysML(System Modeling Language). The test scenarios that can take advantage of the design model were created to improve the train safety control system. Case studies of a model-based design of a train-centric train control system were applied to the test scenarios; the results demonstrated its usability. The improved activity over the test highlighted the safety improvement approach, and it is expected to reduce the cost and time in the conceptual design of a future development model-based train control system.

A Study on the Applicability of IoT for Container Terminal (컨테이너 터미널의 사물인터넷(IoT) 적용가능성에 관한 연구)

  • The Internet of things (IoT) has been applied to a variety of industrial uses such as public service sectors, medical industries, automotive industries, and so on. Led by smart cities, this is typical. However, from a logistics perspective, the level of application is insufficient. This study examines the applicability of IoT-related technology in a container terminal, an object of the present invention, to derive an applicable plan. Analytic network process (ANP) analysis reveals the following results for IoT applications in container terminals: operating systems (26.7%), safety/environmental/security systems (26.4%), equipment maintenance systems (25.3%), and facility maintenance systems (21.6 %). The second ANP analysis reveals the following results: Economy (40.2%), productivity (21.1%), service level (19.5%), and utilizing technology level (19.2%). The application or standard of evaluation is important when applying IoT technology to container terminals; however, it is not concentrated in a certain area. It is desirable to build each container system with linkage and efficiency from a macroscopic view.

Suggestion for Integrated Process Quality Control for Facility Management of Smart City at Construction Stage (Smart City 시공단계 시설물 통합품질관리 프로세스 제안)

  • Korean government is promoting "K-Smart City" to overseas market which is an integrated solution of construction industry with ICT(Information and Communciations Technologies) industry. Due to nature of Smart City, construction quality and the development quality of the facilities need to be established to improve the overall quality. However, guidelines and regulations to initiate quality control for Smart City are behind the actual demand. This deficiency is bringing quality control for construction and ICT to be controlled separately causing lack of synergy and resulting in overall quality degradation. This research is designed to improve the construction quality of Smart City during its establishment stage by integrating ICT system with on-site construction (Integrated control center and on-site equipment). The adoption of this research to a real Smart City case had resulted in 22% reduction of construction inspection failure (Audit), and also allowed Construction Company to pre-align quality control of all purchased items of ICT Infra that resulted in 18% reduction of nonconformity, thus contributing to an overall quality improvement. This research is expected to be used widely among all construction industry of Smart City.

Development of $^{192}Ir$ Small-Focal Source for Non-Destructive Testing Application by Using Enriched Target Material (고농축 표적을 이용한 비파괴검사용 $^{192}Ir$ 미세초점선원 개발)

  • A $^{192}Ir$ small-focal source has been developed by using the HANARO reactor and the radioisotope production facility at the Korea Atomic Energy Research Institute (KAERI). The small-focal source with the dimension of 0.5 mm in diameter and 0.5 mm in length was fabricated as an aluminum-encapsulated form by a specially designed pressing equipment. For the estimation of the radioactivity, neutron self-shielding and ${\gamma}-ray$ self-absorption effects on the measured activity was considered. From this estimation, it is realized that $^{192}Ir$ small-focal sources over 3 Ci activities can be produced from the HANARO. Field performance tests were performed by using a conventional source and the developed source to take images of a computer CPU and a piece of a carbon steel. The small-focal source showed better penetration sensitivity and geometrical sharpness than the conventional source does. It is concluded from the tests that the focal dimension of this source is small enough to maximize geometrical sharpness in the image taking for the close proximity shots, pipeline crawler applications and contact radiography.
