Development of Tuberculosis Education Model for Junior Health Care Professionals (예비보건의료인을 위한 결핵 교육 모형 구축)

  • For health care professionals(HCPs) who have relatively higher possibility of exposing tuberculosis(Tb), it is important for them to equip with proper levels of knowledge and prevention activities. In terms of establishment of Tb education model for junior HCPs, therefore, literature review was done alongside 129 junior HCPs and 14 HCPs were asked to answer questionnaires and telephone survey. The results are follows.; Firstly, six educational themes such as epidermiology of Tb, Tb transmission and patho-physiology, test and diagnosis, latent Tb, Tb treatment, and Tb prevention were identified, based on the literature review. Secondly, the junior HCPs showed lower levels of knowledge regarding epidermiology, transmission and patho-physiology and latent Tb, compared to the other themes. When education courses are planned, longer period of time should be given to the lower level of knowledge areas. Thirdly, the HCPs emphasized that definition and type of Tb should be well integrated into the education in particular epidermiology education. They also responded that hospital infection and infection cycle of Tb need to be addressed during educational session about transmission and patho-physiology. in addition, they said that specific and detailed contents about diagnosis and group examination should be carefully delivered during the education, along with diagnosis, test and application of personal protective devices during education of latent Tb. They also answered that patient education and adverse effect of Tb medication should be taught during Tb treatment session, as well as self activities of health promotion for junior HCPs and systematic TB education as key way of Tb prevention. As the findings were from limited numbers of respondents and contained the sampling bias, the result has to be carefully interpretated prior to generalization. Therefore, further survey with larger study population is required in terms of development of Tb education model.

Prevalence of the antimicrobial resistance and resistance associated gene in Salmonella spp. isolated from pigs and cattle in slaughterhouse (도축장의 소와 돼지 분변에서 분리한 살모넬라속의 약제내성 및 약제내성 유전자의 보유율)

  • This study was conducted to investigate the distribution of Salmonella spp. from pigs and cattle in slaughterhouse, the antimicrobial resistance pattern and the prevalence of resistance genes of isolates. A total of 640 fecal samples from pigs and cattle in slaughterhouse were collected for isolation of Salmonella spp.. Isolation rate was revealed as 15% in pigs and 1.6% in cattle. As result of serotyping, group B (56.6%) were identified as most common in pigs and cattle isolates, in order of group C (24.5%) and group E (15.1%). S. Typhimurium (50.9%) was most common serotype. The major serotypes were in order of S. Rissen and S. London (11.3%) and S. Riggil (7.6%). In antimicrobial test, all isolates were demonstrates susceptibility to nitrofurantoin. But isolates were revealed resistance other antibiotics in order of tetracycline (64.6%), streptomycin (68.3%), ampicillin and amoxicillin (56.3%) and spectinomycin (47.9%). With polymerase chain reaction, antimicrobial resistance gene strA (75.0%) and aadA1 (3.1%) were detected in streptomycin resistance isolates and tetA (94.3%) and tetB (11.3%) gene were detected in tetracycline resistant isolates, but tetG was not detected. Class 1 integron gene was detected in all Salmonella isolates.

  • Objectives : This cross-sectional study was performed to examine if the serum gamma-glutamyltransferase (GGT) level that is within its normal range is associated with the risk of type 2 diabetes and if the association between the waist hip ratio (WHR) and type 2 diabetes is different depending on the serum GGT levels. Methods : The study subjects were 23,436 persons aged 40 years or older and who participated in regular health check-ups at 11 hospitals (males: 5,821, females: 17,615). The gender-specific quintiles of the serum GGT and WHR were used to examine the associations with type 2 diabetes. Results : The serum GGT levels within their normal range were positively associated with type 2 diabetes only in women. The adjusted odds ratios (ORs) were 1.0, 1.0, 1.4, 2.1, and 2.5 according to the quintiles of the serum GGT ($p_{trend}$<0.01). The WHR was more strongly associated with the prevalence of diabetes among the women with a high-normal serum GGT level as compared with those with a low-normal serum GGT level (p for interaction=0.02). For example, the adjusted ORs for women with a low normal serum GGT level were 1.0, 1.2, 1.5, 2.2, and 2.4 according to the quintiles of the WHR, while those figures were 1.0, 2.4, 3.6, 5.0, and 8.3 among the women with a high normal serum GGT level. However, in men, the serum GGT was very weakly associated with type 2 diabetes and the association between the WHR and type 2 diabetes was not different depending on the serum GGT level. Conclusions : Serum GGT within its normal range was positively associated with type 2 diabetes, and central obesity was more strongly associated with the prevalence of type 2 diabetes when the serum GGT level was highnormal. However, these associations were observed only in women, which is different from the previous findings. The stronger relation between central obesity and type 2 diabetes among women with a high-normal serum GGT level can be useful for selecting a group that is at high risk for type 2 diabetes irregardless of whatever the underlying mechanism is.

A STUDY OF THE TRAUMATIC INJURIES IN THE PRIMARY DENTITION (유치열기의 치아 외상에 관한 연구)

  • The purpose of this study was to analyze the traumatized primary teeth and investigate the occurrence, types, area, cause, place of trauma to primary teeth and seasonal variation in a hospital population, and to evaluate the interval between injury and treatment. A total 411 children with 745 traumatized teeth participated in this study from 1998 to 2000. The results were as follows : 1. There were slightly more boys than girls giving a male-to-girl ratio of 1.74:1.0. The 1-2, 2-3 year old boys and girls had the highest number of traumatic injuries to their primary dentition. 2. The most common injury was concussion. 3. The majority of traumatized teeth are the upper primary central incisors. 4. Falling down was the most common cause of injury in both sexes. The most of the injuries occurred inside home. 5. 46% of the children visited the dental office within 24 hours after traumatic injury. 6. The most common month was October(12.7%), and June(12.2%), May(11.7%) were followed.

Korean molluscs as auxiliary hosts for parasites: A study of implications for pathogen transmission in a changing climate (기생충의 보조숙주로서의 한국산 연체동물 감염 실태: 기후변화에서 병원체 전파의 영향에 대한 고찰)

  • To determine the status of Korean molluscs infection and relation with climate change of Korean peninsula, references were reviewed. Wild animals serve as intermediate, reservoirs and paratenic hosts to zoonotic parasites of human beings. Trematode are common parasites of molluscs and almost all trematodes infect mollusks as the first host in the life cycle, and most have a complex life cycle involving other hosts. The significance of auxiliary hosts to the ecology of the parasite has not been proper discussion. There is increasing concern as to the impact of climate change on the epidermiology of many parasitic diseases. A total of 21 species for snail transmitted parasites from Korean molluscs has been reviewed. Among them, 15 species was aquatic mollusks and 6 species for marine mollusks. Maximum infections belonged to Semisulcospira livertina had 11 kind of parasite pathogenic organisms, including Paragonimus westermani, Metagonimus yokogawai, Centrocestus armatus, Notocotylus magniovatus, Centrocestus formosanus, incerte cercaria, nipponensis cercaria, Yoshidae cercaria, cristata cercaria, innominatum cercaria and Metagonimus sp. And 11 in Parafossarulus manchouricus including Clonorchis sinensis, Asymphylodora japonica, Cyathocotyle orientalis, Exorchis oviformis, Notocotylus attenuatus, Echinochasmus japonicus, Loxogenes liberum, Cercariae of Loxogenes liberum Type I, Cercariae of Loxogenes liberum Type II, Furcocercus cercariae (Family Sanguincolidae) and Cercaria of Mucobucaris, 10 in Semisulcospira sp. including Paragonimus westermani, Metagonimus yokogawai, Centrocestus armatus, Echinochasmus redioduplicatus, Notocotylus magniovatus, Cercaria incerte, Cercaria nipponensis, Cercaria yoshidae, paludinarum cercaria and Metagonimus sp., 7 in Koreanomelania globus including Pseudexorchis major, Cercaria of parapleurolophocercous type, Metagonimus sp. (A & B), Cercaria nipponensis, Cercaria inserta and Cercaria yoshidae. Also, Tapes philippinarum have 3 pathogenic organisms including Cercariae tapidis, Cercariae furcocercus and Parvatrema sp. In particular, under climatic extremes such as floods and drought, aquatic molluscs may play a more prominent role in parasite transmission in the future.
