Community-acquired Legionnaires' Disease in a Newly Constructed Apartment Building

  • Objectives: Legionnaires' disease (LD) is a severe type of pneumonia caused by inhalation of aerosols contaminated with Legionella. On September 22, 2016, a single case of LD was reported from a newly built apartment building in Gyeonggi province. This article describes an epidemiologic investigation of LD and identification of the possible source of infection. Methods: To identify the source of LD, we interviewed the patient's husband using a questionnaire based on the Legionella management guidelines from the Korea Centers for Disease Control and Prevention. Water samples from the site were collected and analyzed. An epidemiological investigation of the residents and visitors in the apartment building was conducted for 14 days before the index patient's symptoms first appeared to 14 days after the implementation of environmental control measures. Results: Legionella pneumophila serogroup 1 was isolated from the heated-water samples from the patient's residence and the basement of the apartment complex. Thirty-two suspected cases were reported from the apartment building during the surveillance period, yet all were confirmed negative based on urinary antigen tests. Conclusions: The likely source of infection was the building's potable water, particularly heated water. Further study of effective monitoring systems in heated potable water should be considered.

  • Growth hormone (GH) treatment has been increasingly widely used for children with GH deficiencies as the survival rate of pediatric patients with malignancies has increased. Both GH and insulin-like growth factor-I have mitogenic and antiapoptotic activity, prompting concern that GH treatment may be associated with tumor development. In this review, the authors examined the relationship between GH treatment and cancer risk in terms of de novo malignancy, recurrence, and secondary neoplasm. Although the results from numerous studies were not entirely consistent, this review of various clinical and epidemiological studies demonstrated that there is no clear evidence of a causal relationship between GH treatment and tumor development. Nonetheless, a small number of studies reported that childhood cancer survivors who receive GH treatment have a small increased risk of developing de novo cancer and secondary malignant neoplasm. Therefore, regular follow-ups and careful examination for development of cancer should be required in children who receive GH treatment. Continued surveillance for an extended period is essential for monitoring long-term safety.

Development of a Single Nucleotide Polymorphism DNA Microarray for the Detection and Genotyping of the SARS Coronavirus

  • Severe acute respiratory syndrome (SARS), a disease that spread widely in the world during late 2002 to 2004, severely threatened public health. Although there have been no reported infections since 2004, the extremely pathogenic SARS coronavirus (SARS-CoV), as the causative agent of SARS, has recently been identified in animals, showing the potential for the re-emergence of this disease. Previous studies showed that 27 single nucleotide polymorphism (SNP) mutations among the spike (S) gene of this virus are correlated closely with the SARS pathogenicity and epidemicity. We have developed a SNP DNA microarray in order to detect and genotype these SNPs, and to obtain related information on the pathogenicity and epidemicity of a given strain. The microarray was hybridized with PCR products amplified from cDNAs obtained from different SARS-CoV strains. We were able to detect 24 SNPs and determine the type of a given strain. The hybridization profile showed that 19 samples were detected and genotyped correctly by using our microarray, with 100% accuracy. Our microarray provides a novel method for the detection and epidemiological surveillance of SARS-CoV.

Multiple Primary Malignancies - A Retrospective Analysis at a Single Center in Turkey

  • Background: A literature review on 1,104,269 cancer patients concluded that the prevalence of multiple primary malignancies (MPM) is between 0.73% and 11.7%. MPMs seem to have higher incidence than that influenced by hazard only. The purpose of this study was to investigate clinically useful information for effective screening for synchronous and metachronous second primary cancers and to identify a potential surveillance protocol. Materials and Methods: Using statistical and epidemiological indicators we evaluated the patients with MPMs (double locations) admitted to Dr. Abdurrahman Yurtarslan Ankara Oncology Education and Research Hospital between 1981 and 2010. Results: Out of the 130 cases, 24 (18.4%) were synchronous while 106 cases (81.6%) were metachronous tumours. Mean interval time from first to second primary cancers was 4.65 years (0-27 years). The most frequent malignant associations were breast-breast, breast-endometrium and breast-ovary. Both primary and secondary tumors tended to be in an advanced stage explained by the low compliance of the patients to follow-up. Conclusions: The possibility that MPMs exist must always be considered during pretreatment evaluation. Screening procedures are especially useful for the early detection of associated tumors, whereas careful monitoring of patients treated for primary cancer and a good communication between patients and medical care teams should ensure early detection of secondary tumors, and subsequent appropriate management.

A sero-surveillance of Brucella spp. antibodies and individual risk factors of infection in cattle in Bangladesh

  • Brucellosis is a serious zoonosis, recognized worldwide. It primarily affects animals, which act as reservoirs for human infection as well as being of economic significance to the agri-food industry. Bangladesh has been reported as an endemic area for brucellosis. So a cross sectional study was conducted to determine the seroprevalence and potential risk factors of brucellosis in cattle in Dinajpur and Mymensingh districts of Bangladesh. A total of 182 cattle were examined by Rose Bengal Plate Test (RBPT) between September 2008 and October 2009. Then Positive, doubtful, and negative samples were further confirmed with slow agglutination test (SAT) and both indirect and competitive enzyme linked immunosorbent assay (iELISA and cELISA). A questionnaire was used to collect epidemiological information of the animals. The overall animal-level prevalence was 3.30%. Brucellosis seroprevalence was higher (4.76% by cELISA) in cattle above 48 months than those under 48 months. Female showed higher seroprevalence (10.67%) than male (6.25%). Higher seroprevalence was also found in cattle bred naturally (20.0%) than artificially (8.77%) and cattle that aborted or with previous abortion record (22.22%) showed higher seroprevalence than non-aborted (7.69%). The sensitivity of RBT and SAT was found 100% as compared to cELISA standard test, whereas specificity of RBT (95.35%) was higher than that of SAT (94.32%).

  • The paper gives an overview of the problems to be tackled nowadays by occupational health with regards to shift work as well as the main guidelines at organizational and medical levels on how to protect workers' health and well-being. Working time organization is becoming a key factor on account of new technologies, market globalization, economic competition, and extension of social services to general populations, all of which involve more and more people in continuous assistance and control of work processes over the 24 hours in a day. The large increase of epidemiological and clinical studies on this issue document the severity of this risk factor on human health and well being, at both social and psychophysical levels, starting from a disruption of biological circadian rhythms and sleep/wake cycle and ending in several psychosomatic troubles and disorders, likely also including cancer, and extending to impairment of performance efficiency as well as family and social life. Appropriate interventions on the organization of shift schedules according to ergonomic criteria and careful health surveillance and social support for shift workers are important preventive and corrective measures that allow people to keep working without significant health impairment.

Epidemiological characteristics on new cases of notified tuberculosis in Jeju Province, Korea, 2011~2018 (제주도 결핵 신환자 발생의 역학적 특성, 2011~2018)

  • While the notified crude incidence rates(per 100,000 persons, CIR) of new tuberculosis between 2016 and 2017 decreased in nationwide and 16 provinces, except Jeju Province, Korea. The aim of this study was to compare CIRs of Jeju Province and nationwide by calendar years and age groups in order to interpret the increasing trend of CIR in Jeju Province. The data source was the Integrated Diseases and Health Control System of the Korea Centers for Disease Control and Prevention. The selection criteria were the notified new cases as tuberculosis, who had the address as Jeju Province at diagnosis, between 2011 and 2018. The 95% confidence interval of CIR was estimated based on Poisson distribution. There was not statistically significant difference of CIRs between Nationwide and Jeju Province of 2011 and 2018, except 2016. Especially the CIR of group aged over 65 years in Jeju Province, 2016 was significantly lower compared to Nationwide CIR. But, the CIR of group aged 30~50 between 2017 and 2018 showed significantly higher compared to Nationwide CIR. Based on the results, the fact of increasing CIR from 2016 to 2017 in Jeju Province might be interpreted as an introduction of new tuberculosis screening project rather than real increased CIRs.

Epidemiologic Trends in Human Hepatitis A Virus Infection Stratified by Age between 2010 and 2018 in Cheonan, Korea

  • Although studies on Hepatitis A virus (HAV) were crucial in the establishment of the HAV infection prevention programs, no systematic investigation into HAV has been conducted since 1999. We retrospectively analyzed the data between January 2010 to December 2018 from all the patients who underwent HAV antibody tests at the Dankook University Hospital Health Care Center. Data were collected from 56,204 individuals. Overall, 34,834 (62.0%) individuals from this cohort were positive for HAV antibodies and the annual rate of anti-HAV antibody positivity was highest in 2010 (68.5%) and lowest in 2013 (54.8%). The average decline in the antibody positivity rate was 0.62% per year, showing a statistically significant difference (p < 0.001). In the over 40s age group, anti-HAV antibody positivity rates decreased from 89% in 2010 to 64% in 2018 (p < 0.001), with an annual decrease of 3.1%. In the over 30s age group, it decreased from 48.2% in 2010 to 34.7% in 2018 (p < 0.001), with an annual decrease of 1.82%. This study shows that the antibody positivity rate is decreasing across age groups but given that HAV infection poses more significant risks in older patients it is important to expand the evaluations of the current and future antibody positivity rates for HAV in various age groups.

Epidemiology of trigeminal neuralgia: an electronic population health data study in Korea

  • Background: Trigeminal neuralgia (TN) is one of the most painful disorder in the orofacial region, and many patients have suffered from this disease. For the effective management of TN, fundamental epidemiologic data related to the target population group are essential. Thus, this study was performed to clarify the epidemiological characteristics of TN in the Korean population. This is the first national study to investigate the prevalence of TN in Korean patients. Methods: From 2014 to 2018, population-based medical data for 51,276,314 subscribers to the National Health Insurance Service of Korea were used for this study. Results: The incidence of TN was 100.21 per 100,000 person-years in the year of 2018 in Korea, and the male to female ratio was 1:2.14. The age group of 51-59 years had the highest prevalence of TN. Constant increases in medical cost, regional imbalance, and differences in prescription patterns by the medical specialties were showed in the management of TN. Conclusions: The results in this study will not only help to study the characteristics of TN, but also serve as an important basis for the effective management of TN in Korea.

Reduction of Reinfection Rates with Opisthorchis viverrini through a Three-Year Management Program in Endemic Areas of Northeastern Thailand

  • To clarify the reinfection profile associated with risk factors of opisthorchiasis, we conducted an epidemiological study on the chemotherapeutic effects on reinfection with O. viverrini in the endemic areas of Northeastern Thailand for 3 years. A total of 3,674 fecal samples were collected from participants in villages of 5 provinces. They were examined microscopically using a modified technique of formalin ethyl-acetate concentration. Egg-positive residents were reexamined year (2018) by year (2019) after treatment with a single dose (40 mg/kg) of praziquantel. Health education was provided to the participants yearly. The egg-positive rate of O. viverrini was 14.3%, and was highest (22.2%) in the 20-30 year-old group in 2017. The egg positive rate was 15.3% in dogs and 11.4% cats. Human reinfection rate was 15.5% and 6.3% in next 2 years, and was highest (23.2%) among the fishermen. Relative risk factors of reinfection were significantly higher for males, over 40-year-old age, or working as fishermen or farmers, and eating uncooked fish within the preceding year. A significant difference resulting from a health education program was observed in the third year. Therefore, health education and sustainable surveillance for opisthorchiasis should be maintained to decrease the risk of reinfection.
