An Analytical Study on Health Characteristics of Obesity of Adulthood (성인 비만의 건강특성에 관한 분석적 연구)

  • This study was carried out to offer the basic data for more qualitative lives of the adulthood with obesity through their health maintenance and control. So, the researcher reviewed and analyzed the health problems of the subjects according to sex, age, and positive rate on the screening tests. The subjects were 91 obese persons in K university hospital in Seoul from December 1, 2000 to January 31, 2001. A Questionnaire developed by researcher to obtain subject's general characteristics, main symptoms, medical examination, diagnosed disease was used as the tool for the study. The results of this study are as follows: 1. For the distribution of diagnosis in subjects, liver disease was most frequent and hypertension was second. 2 The most common complaints were indigestion in the digestive system, throat pain in the respiratory system, dyspnea in cardiovascular system, thirsty in endocrine system, frequent urine in the urinary system, vertigo in the hematologic system. 3. The main symptoms according to sex were for females, a statistically significant higher rate of complaints in the digestive system, musculoskeletal system as compared to males. 4. The main symptoms according to age were that in the fourth decade there were higher rate of complaints in the musculoskeletal system. 5. In screening tests according to sex, males showed a higher positive rate for Alk-Phosphatase and females, in FBS, urine-micro, VDRL. 6. In screening tests according to age, higher positive rates of BP, Chest X-ray, mammography, ESR, CRP were seen in the fourth decades. 7. Obesity was positively related to cardiovascular system, endocrine system, urinary system, musculoskeletal system, mental and nervous system in main symptoms. 8. Obesity was positively related to BP, Sonography, FBS, SGOT, SGPT in screening tests.

Structure Relationships for Diseased and Health-related Quality of Life in the Elderly (노인들의 질환별 유병여부와 건강관련 삶의 질 구조모형 분석)

  • This study conducted a secondary analysis by using original data of performed by Korea Institute for Health and Social Affairs to determine factors affecting health-related quality of life in the elderly. The results were as follows. For medical health condition, it became longer in endocrine diseases and cardiovascular diseases. In the case of functional health condition, subjects with musculoskeletal diseases showed higher functional health condition. Subjective health condition, while it became worse in digestive diseases and musculoskeletal diseases in the order, it recorded the best score when subjects did not have any musculoskeletal disease. Quality of life, while it became lower in endocrine diseases in the order, it was the highest when subjects did not have any musculoskeletal disease. Thus, increase of subjective health status for the elderly is important in the future.

Fulvestrant 250mg versus Anastrozole 1 mg in the Treatment of Advanced Breast Cancer: a Meta-Analysis of Randomized Controlled Trials

  • Objective: Most patients with advanced breast cancer experience resistance to endocrine treatment and eventual disease progression. This meta-analysis was designed to compare the efficacy and tolerability of fulvestrant 250mg with anastrozole 1mg in postmenopausal women with advanced breast cancer. Methods: Electronic literature databases (Cochrane Library, Medline, and Embase) were searched for randomized controlled trials (RCTs) published prior to August 2013. Only RCTs that compared fulvestrant 250mg to anastrozole 1mg in postmenopausal women with advanced breast cancer were selected. The main outcomes were time to treatment failure (TTF), time to progression (TTP), duration of response (DOR), clinical benefit rate, and tolerability. Results: Four RCTs covering 1,226 patients (fulvestrant, n=621; anastrozole, n=605) were included in the meta-analysis. Fulvestrant increased the DOR compared to anastrozole (HR =1.31, 95% confidence interval [CI] 1.13-1.51). There was no statistically significant difference between fulvestrant and anastrozole in terms of TTF (HR=1.02, 95%CI 0.89-1.17), complete response (RR=1.79, 95%CI, 0.93-3.43), and partial response (RR=0.91, 95%CI 0.69-1.21). As for safety, there was no statistical significance between the two groups for common adverse events. Conclusion: Fulvestrant 250mg is as effective and well-tolerated as anastrozole 1mg treatment for advanced breast cancer in postmenopausal women whose disease progressed after prior endocrine treatment. Thus, fulvestrant may serve as a reasonable alternative to anastrozole when resistance is experienced in breast cancer cases.

The Eosinophil Count Tends to Be Negatively Associated with Levels of Serum Glucose in Patients with Adrenal Cushing Syndrome

  • Background: Cushing syndrome is characterized by glucose intolerance, cardiovascular disease, and an enhanced systemic inflammatory response caused by chronic exposure to excess cortisol. Eosinopenia is frequently observed in patients with adrenal Cushing syndrome, but the relationship between the eosinophil count in peripheral blood and indicators of glucose level in patients with adrenal Cushing syndrome has not been determined. Methods: A retrospective study was undertaken of the clinical and laboratory findings of 40 patients diagnosed with adrenal Cushing syndrome at Chungnam National University Hospital from January 2006 to December 2016. Clinical characteristics, complete blood cell counts with white blood cell differential, measures of their endocrine function, description of imaging studies, and pathologic findings were obtained from their medical records. Results: Eosinophil composition and count were restored by surgical treatment of all of the patients with adrenal Cushing disease. The eosinophil count was inversely correlated with serum and urine cortisol, glycated hemoglobin, and inflammatory markers in the patients with adrenal Cushing syndrome. Conclusion: Smaller eosinophil populations in patients with adrenal Cushing syndrome tend to be correlated with higher levels of blood sugar and glycated hemoglobin. This study suggests that peripheral blood eosinophil composition or count may be associated with serum glucose levels in patients with adrenal Cushing syndrome.

Clinical Characteristics of Patients with Oral Candidiasis

  • Purpose: Oral candidiasis is the most common fungal infection in the oral cavity which is usually diagnosed from clinical findings. A retrospective study was conducted to identify risk factors for oral candidiasis and to characterize the demographic and clinical features of affected patients. Methods: From January 1, 2019 to December 31, 2019, it consisted of 90 oral candidiasis patients diagnosed based on clinical finding and treated with antifungal drugs. As a retrospective study of those people, surveys were conducted on sex, age, systemic disease, a use of dentures, complaints of dry mouth, smoking and alcohol consumption, culture on potato dextrose agar (PDA) medium, culture on chromogenic agar (CA) medium and a duration of antifungal treatment. Results: Among 90 selected patients, the male and female ratio was 41:49. Overall, female had a higher infection rate than male in all age groups. In this study, oral candidiasis was not clearly susceptible to dry mouth, smoking or drinking, wearing dentures and association with systemic disease. Among 90 patients with oral candidiasis, 83 had colonies formed on PDA medium and 53 had colonies formed on CA medium. The duration of antifungal treatment was highest between 5 and 8 weeks. In addition, there was statistical significance between the culture results in CA medium and the duration of antifungal treatment. Conclusions: Generally, old age or infants, dry mouth, smoking, a use of dentures and endocrine abnormalities are risk factors to increase oral candidiasis; however, in this study, it was mainly found in the elderly aged 60 or older regardless of sex and the incidence of oral candidiasis was not obviously related with patients with dry mouth, smoking or drinking, denture wearers and endocrine abnormalities. Interestingly, when the fungi were cultured in CA medium, the duration of antifungal treatment was increased.

Surgical treatment of thymic disease -A comparison to medical treatment in myasthenia gravis- (흉선질환의 외과적 치료 -중증근무력증 환자에서 약물치료와의 비교관찰-)

  • Thymus gland is a kind of endocrine organ which secretes thymosin and thymoprotein. There can be developed variable lesions like thymoma, thymic hyperplasia, thymic cyst, thymolipoma, and carcinoid tumor of the thymus gland. We have experienced 25 patients of thymic disease: thymoma 12, thymic hyperplasia 11, thymic cyst 1, carcinoid tumor 1. The age distribution were ranged from 3 to 66 years and the sex ratio was 1:1.8 [male to female]. Thymectomy was performed in all cases, but 3 cases with deep infiltration to the adjacent structures were not resectable completely. Malignancy [all thymoma] were 5 and the rest were benign. Two cases were died of recurrence after tumor resection. Myasthenia gravis occurred in 10 cases. Among them, 2 were thymoma and 8 were thymic hyperplasia. We could obtained the result that thymectomized cases reached 2 remission and 5 improvement. Myasthenia gravis treated medically [18 cases] had no remission and only 2 clinical improvement. In the light of these results, early radical thymectomy would be most favorable treatment in not only thymic tumor, but generalized myasthenia gravis.

Recent Advances in Radioiodine Therapy for Thyroid Cancer (갑상샘암의 방사성요오드 치료의 최신 지견)

  • Well-differentiated thyroid cancer is the most common endocrine malignancy with an increasing incidence. Most patients with well-differentiated thyroid caner have a favorable prognosis with high survival rate. While surgery and radioiodine therapy is sufficient treatment for the majority of patients with differentiated thyroid cancer, a minority of these patients experiences progressive, life-threatening growth and metastatic spread of the disease. Because there is no prospective controlled study to evaluate the differences of management of thyroid cancer, it is hard to choose the best treatment option. And there are still lots of controversies about the management of this disease, such as surgical extent, proper use of radioiodine for remnant ablation and therapy, use of rhTSH instead of withdrawal of thyroid hormone, long-term follow-up strategy, thyroglobulin as a tumor marker, etc. In this review, recent data related to these conflicting issues and recent advances in diagnosis, radioiodine therapy and long-term monitoring of well-differentiated thyroid cancer are summarized.

Directed Differentiation of Pluripotent Stem Cells by Transcription Factors

  • Embryonic stem cells (ESCs) and induced pluripotent stem cells (iPSCs) have been used as promising tools for regenerative medicine, disease modeling, and drug screening. Traditional and common strategies for pluripotent stem cell (PSC) differentiation toward disease-relevant cell types depend on sequential treatment of signaling molecules identified based on knowledge of developmental biology. However, these strategies suffer from low purity, inefficiency, and time-consuming culture conditions. A growing body of recent research has shown efficient cell fate reprogramming by forced expression of single or multiple transcription factors. Here, we review transcription factor-directed differentiation methods of PSCs toward neural, muscle, liver, and pancreatic endocrine cells. Potential applications and limitations are also discussed in order to establish future directions of this technique for therapeutic purposes.

A novel RET mutation identified in a patient with pheochromocytoma and renal cell carcinoma

  • Pheochromocytomas might be sporadic or genetic. Genetic pheochromocytoma is associated with multiple endocrine neoplasia (MEN) type 2A, MEN type 2B, and von Hippel-Lindau (VHL) disease. RET mutations are identified in more than 90% of index cases of MEN2 and familial medullary thyroid cancer and in about 4-12% of apparent sporadic cases. Here, we report a 54-year-old man presenting with pheochromocytoma and renal cell carcinoma, who was identified as having a novel missense RET mutation.

Relationship between Breast Cancer and Levels of Serum Thyroid Hormones and Antibodies: a Meta-analysis

  • The breast and the thyroid are hormone responsive organs that are closely related with changes of endocrine function and glandular disease. An association between thyroid disorders and breast cancer (BC) risk has been suggested, although the results are inconclusive. The purpose of the present study was to summarize evidence supporting a relationship between BC and the level of thyroid hormones and antibodies. The MEDLINE and EMBASE electronic databases were searched for studies published between 2000 and 2014. The pooled effects were presented as weighted mean differences (WMD) with 95% confidence intervals (CI) using fixed or random effect models. We summarized the results of 8 cross-sectional studies with 4, 189 participants. The overall pooled results showed that the levels of $FT_3$ and $FT_4$ were significantly increased in patients with BC (WMD=1.592 pmol/l; 95% CI: 0.15-3.033 and WMD=0.461 ng/dl; 95% CI: 0.015-0.906; p=0.043). The TPOAb level in patients with BC was higher than that in the control group (WMD=81.4 IU/ml; 95% CI: 78.7-84.0; p=0.000). The overall pooled results of the TgAb with random effects analyses showed that the TgAb level was significantly increased in patients with BC (WMD=101.3 IU/ml; 95% CI: 48.7-153.9; p=0.000). The present results indicated that the serum levels of $FT_3$, TPOAb and TgAb are significantly higher in patients with breast cancer than in healthy controls.
