Surgical Treatment of a Submitral Left Ventricular Aneurysm and the Patient Present with Recurrent Ventricular Tachycardia (재발성 심실성 빈맥이 동반된 승모판하 좌심실류의 외과적 치료)

  • Submitral left ventricular aneurysm (SMLVA) is a rare disease entity that exclusively occurs in the black population. We herewith report on a surgical case of SMLVA in a 68-year-old male who presented with ventricular tachycardia. He underwent surgical repair under standard hypothermic cardiopulmonary bypass. In the state of apical elevation, a vertical ventriculotomy was made directly over the aneurysm, which was located at the posterobasal aspect of the left ventricle. Radiofrequency ablation was performed on the endocardium all around the neck of the aneurysm and then patch endoaneurysmorrhaphy was carried out with particular care not to injure the mitral valve and subvalvular structure. His postoperative recovery was uneventful. There has been no dysfunction of the mitral valve or recurrence of the ventricular tachycardia at 2 years' follow-up.

The Structure of the Ventricle in the Heart of the Pond Smelt, Hypomesus nipponensis (Osmeridae) (바다빙어과 빙어 (Hypomesus nipponensis)의 심실 구조)

  • The structure of the ventricle in the heart of Hypomesus nipponensis was investigated by light and scanning electron microscope. The heart consisted of four consecutive chambers, the sinus venosus, atrium, ventricle and bulbus arteriosus. The wall of the ventricle was divided into endocardium, myocardium, subepicardium and epicardium. The valves were observed in the artrioventricular and bulboventricular junctions. The ventricular myocardium was an entirely spongy without coronary vessels. The trabecular network was formed with lumina included a central lumen and the trabecula was cylindrical shape. Collagen distribution was apparent in the subepicardium, artrioventricular valve and bulboventricular valve. But in the trabeculae, collagen distribution was observed partly in the base of the ventricle. Especially, the endocardial bridges were observed between trabeculae. These results might be considered that the structure of the ventricle in the heart of pond smelt is adapted to sedentary habit associated with its habitat and lifestyle.

A Study on Tracking and Quantitative Analysis of Regional Left Ventricular Wall Motion in Echocardiography (심초음파에서 국소 좌심실벽 운동 추적 및 정량적 분석에 관한 연구)

  • The two dimensional echocardiography is widely used to evaluate regional wall motion abnormality, because of its abilities to depict left ventricular wall motion. A number of researches have been processed for evaluation and quantitative analysis of left ventricular wall motion functions. In this paper, we proposed an algorithm which detects automatically and analyze quantitatively endocardial wall motion during systole. The echocardiograms were obtained in the short-axis views in normal subjects. Automated edge detection and endocardial contour tracking algorithm was applied to each frames, quantitative analysis based on segmentation was performed, pre-defined color overlays superimposed on the gray scale images, and the images was animated. The proposed algorithm provided automated, quantitative diagnosis of regional wall motion abnormality.

Implementation of Nuclear Medicine Dynamic Cardiac Phantom for Clinical Application (임상적용을 위한 핵의학 동적 심장팬텀의 구현)

  • In the field of nuclear medicine, the various static phantoms of international standards are used to assess the performance of the nuclear medicine equipment. However, we only reproduced a fixed situation in spite of the movement of the cardiac, and the demands for dynamic situations have been continuously raised. More research is necessary to address these challenges. This study used flexible materials to design the dynamic cardiac phantom, taking into account the various clinical situations. It also intended to reproduce the images through dynamic cardiac flow to confirm the usefulness of the proposed technique. The frame of dynamic cardiac phantom was produced based on the international standard phantom. A nuclear medicine dynamic cardiac phantom was produced rubber material and silicone implemented by 3D printing technique to reproduce endocardium and epicardium movement. Therefore we compared and evaluated the image of a cardiac phantom made of rubber material and a cardiac phantom made of silicone material by 3D printing technique. According to the results of this study, the analysis of the Summed Rest Score(SRS) showed abnormalities in the image of a cardiac phantom made of rubber material at 10, 20, and 30 stroke rates, but the image of a cardiac phantom made of silicone material by 3D printing technique showed normal levels. And the analysis of the Total Perfusion Deficit(TPD) showed that TPD in the image of a cardiac phantom made of rubber material was higher than that of the image of a cardiac phantom made of silicone material by 3D printing technique at 10, 20, and 30 stroke rates. The potential for clinical application of the proposed method was confirmed in the dynamic cardiac phantom implemented with 3D printing technique. It is believed that the objective information secures the reliability of inspection equipment and it contributes to improve the diagnostic value of nuclear medicine.

The Radioprotective Effect and Mechanism of Captopril on Radiation Induced-Heart Damage in Rats (방사선 조사 후 발생한 흰쥐 심장손상에서 Captopril의 방어역할과 기전)

  • Purpose : Captopril (angiotension converting enzyme inhibitor) is known to have a radioproptective effect in the lungs, intestines and skin, but its effect in the heart is unclear. To investigate the radioprotectlve efiect and mechanism of captopril on the heart, the histopathological changes and immunohistochemical stains were compared with radiation alone, and radiation combined with captopril, in the rats. Materials and Methods : The histopathological changes and immunohistochemical stains ($TNF{\alpha}$, $TGF{\beta}1$, PDGF and FGF2) were examined in the radiation alone and the combined captopril and radiation groups, 2 and 8 weeks after irradiation. Each group consisted of 8 to 10 rats (Sprague-Dawley). Irradiation (12.5 Gy) was given to the left hemithorax in a single fraction. Captopril (50 mg/Kg/d) mixed with water, was given orally and continuously from the first week prior to, up to the 8th week of the experiment. Results : In the radiation alone group, the ventricle at 2 weeks after irradiation showed prominent edema (p=0.082) and fibrin deposit (p=0.018) compared to the control group. At 8 weeks, the edema was decreased and fibrosis increased compared to those at 2 weeks. The histopathological changes of the combined group were similar to those of the control group, due to the reduced radiation toxicity at 2 and 8 weeks. The endocardial fibrin deposit (p=0.047) in the atrium, and the interstitial fibrin deposit (p=0.019) and edema (p=0.042) of the ventricle were reduced significantly in the combined group compared to those in the radiation alone group at 2 weeks. The expressions of $TNF-{\alpha}$, $TGF-{\beta}1$, PDGF and FGF-2 in the radiation alone group were more increased than in the control group, especially in the pericardium and endocardium of the atrium at 2 weeks. At 8 weeks, the pericardial $TNF-{\alpha}$ and $TGF-{\beta}1$ in the radiation alone group continuously increased. The expressions of $TNF-{\alpha}$, $TGF-{\beta}1$ and PDGF were decreased in the combined group at 2 weeks. At 8 weeks, the expressions of $TNF-{\alpha}$ in the atrial and ventricular pericardia were markedly reduced (p=0.049, p=0.009). Conclusion : This study revealed that the early heart damage induced by radiation can be reduced by the addition of captopril in a rat model. The expressions of $TNF-{\alpha}$, $TGF-{\beta}1$ and PDGF were further decreased in the combined compared to the radiation alone group at both 2 and 8 weeks. From these results, it may be concluded that these cytokines probably play roles in the radioprotective mechanism of captopril from the radiation-induced heart toxicity, similarly to in other organs.

Treatment of Atrial Fibrillation with Microwave (Microwave를 이용한 심방세동의 치료)

  • Backgound: Cryoablation and radiofrequency ablation have been used to treat the atrial fibrillation. Some reports insisted that the microwave ablation Is a better method for a deep and extensive lesion. Material and Method: From December 2001 to July 2002, we peformed 8 microwave ablations in patients who needed mitral valve surgery (7 MVR, 1 MVR+AVR). There were 3 men and 5 women, and their mean age was 43.4$\pm$8.3 years and mean follow up period was 5.6$\pm$2.4 months respectively. The microwave was applied on endocardium or epicardium by Lynx (Afx, inc.) using a power of 45 watts for 25 seconds. We studied the left atrial dimension, the left atrial function and the sinus conversion with echocardiography and electro-cardiography at three times; 1) before the operation, 2) immediately after the operation, and 3) 6 months after the operation. Result: There was no complication and no mortality. The mean aortic clamping time was 104.6$\pm$25.0 minutes, and the mean total bypass time was 130.5$\pm$28.7 minutes. The rate of sinus conversion was 75%, A wave across the mitral valve was a mean of 77.0$\pm$24.8 cm/sec, and the AVE was a mean of 0.46$\pm$0.17 at 5.6 months postoperatively Conclusion: There was no difference in the early result of microwave ablation compared to other methods. The microwave ablation was an acceptable method due to its convenient application especially in beating heart.
