삭제기구가 자가 산부식 접착제의 미세누출에 미치는 영향 (EFFECT OF MICROLEAKAGE OF A SELF-ETCHING PRIMER ADHESIVE ACCORDING TO TYPES OF CUTTING INSTRUMENTS)

  • 이 연구는 서로 다른 카바이드 버와 다이아몬드 버로 5급 와동을 형성 한 후 SE primer 접착 시스템인 Clearfil SE Bond를 이용한 복합레진 수복 시 버의 종류에 따른 법랑질과 상아질 변연에서의 미세누출 차이를 상호 비교하기 위하여 시행하였다. 4종의 삭제기구를 이용하여 각각 10개의 발거된 대구치의 협면 치경부에 5급 와동을 형성하였다. 1군은 plain-cut carbide fissure bur (no. 245)를 사용한 군, 2군은 cross-cut carbide fissure bur (no. 557)를 사용한 군, 3군은 fine diamond bur (TF-21F: 입자크기 $53\;-\;63\;{\mu}m$)를 사용한 군, 4군은 standard diamond bur (EX-41: 입자크기 $106\;-\;205\;{\mu}m$)를 사용한 군으로 분류하였다. 형성된 와동에 Clearfil SE Bond의 Primer와 Bond를 적용하고 Clearfil AP-X 복합레진을 충전하여 광중합한 후, Sof-Lex disc를 이용하여 마무리와 연마를 시행하였다. 수복된 치아를 2% methylene blue 용액에 24시간 동안 침적시킨 후 각 군의 치아를 협설로 절단하여 광학 입체현미경 하에서 법랑질과 상아질 변연에서의 미세누출 점수를 평가하여 다음과 같은 결과를 얻었다. Clearfil SE Bond와 복합레진을 이용한 5급 와동 수복 시 거친 다이아몬드 버의 사용은 카바이드 버나 미세한 다이아몬드 버에 비해 법랑질과 상아질 변연 모두에서 높은 미세누출을 나타내었다.

  • 5세대 상아질 접착제인 Adper Single bond 2.0 (3M-ESPE, USA), 6세대 상아질 접착제인 Prompt L-pop (3M-ESPE, USA), AdheSE (Ivoclar Vivadent, Liechtenstein) 를 각 상아질 접착제당 30개의 건전한 소구치 및 대구치를 선정하여 협면과 설면에 5급와동을 형성하고 복합레진을 적용하였다. 이중 절반은 열순환을 시행하지 않고 나머지 절반은 열순환을 시행하였다. 이후 메틸렌 블루를 침투시켜 시약의 침투 정도를 측정한 후 통계분석하였다. 열순환을 시행하지 않은 군과 열순환을 시행한 군을 통틀어 열순환을 시행하지 않은 상아질 군에서만 Single Bond 2.0의 미세누출의 양이 Prompt L-Pop과 AdheSE에 비하여 높았고 (p <.05) 나머지 군에서는 모두 Single Bond 2.0의 미세누출의 양이 적었다. (p <.05) 열순환 이후에 미세누출의 양이 늘어난 군은 Prompt L-Pop과 AdheSE의 상아질 군이었다.

  • This study was intended to evaluate the influence of different base materials on the marginal integrity of posterior direct composite resin restorations. Large conventional butt-joint MOD cavity preparations, with margins in enamel and dentin, were prepared in vitro in 24 extracted human mandibular molars and randomly divided into four equal groups. One of three base materials(RMGI, compomer, flowable resin) was placed on the pulpal floors, of the teeth of the groups. One group of teeth, which served as the control group, was not given any base mterial.(중략)

  • 저광도 중합법, 펄스 지연 중합법, 초고광도 중합법 등의 광조사 방식이 광중합형 복합레진 수복물에서의 중합수축 응력에 미치는 영향을 알아보기 위해 in vitro에서 미세 누출 실험을 하였다. 80개의 발거된 소구치의 협측면에 5급 와동을 형성하고, 600mW/$cm^2$로 30초간 광조사하는 보통 광도 중합군, 300mW/$cm^2$로 60초간 광조사하는 저광도 중합군, 400mW/$cm^2$로 2초간 광조사하고 5분간 기다린 후 800mW/$cm^2$로 10초간 최종 중합시키는 펄스-지연 중합군, 그리고 1930mW/$cm^2$의 광도로 3초간 광조사하는 초고광도 중합군 등의 4개의 군으로 나누어 hybrid 형의 광중합복합레진을 충전하고 각 군의 방법대로 중합한 후, 변연의 0.5mm 외부에 nail varnish를 도포하고 37$^{\circ}C$, 2% metylene blue 용액에 24시간 동안 침적시켰다. 시편을 아크릴릭 레진에 매몰한 후 수복물의 중앙에서 종절단하여 입체현미경하에서 그 단면을 관찰한 후 법랑질과 상아질 변연으로 나누어 색소의 침투도를 측정하여 다음과 같은 결과를 얻었다. 1. 펄스 지연 중합군의 색소 침투도는 법랑질과 상아질 변연 모두에서 보통광도, 저광도 및 초고광도 중합군과 유의한 차이를 보이지 않았다(p>0.05). 2. 상아질 변연에서 초고광도 중합군은 보통광도 중합군과 저광도 중합군에 비해 유의하게 높은 색소 침투도를 보였다(p<0.05). 3. 법랑질 변연에서의 4개 군의 색소 침투도는 서로 유의한 차이를 보이지 않았다(p>0.05). 4. 모든 군에서 상아질 변연에서의 색소 침투도는 법랑질군에서의 색소 침투도에 비해 유의하게 높았다(p<0.05).

복합레진 수복물의 미세누출 감소를 위한 레진 전색제의 효과 (EFFECT OF RESIN SEALANTS ON THE REDUCTION OF MICROLEAKAGE IN COMPOSITE RESTORATIONS)

  • 본 연구의 목적은 5급 복합레진 수복물에 각종 레진 전색제를 적용한 후, 수복물의 교합면측과 치은측 변연 미세누출에 대한 레진 전색제의 감소효과를 평가하기 위하여 시행하였다. 40개의 발거된 대구치의 협면 치경부에 5급 와동을 형성하고 복합레진을 충전한 후, 사용된 레진 전색제에 따라 다음 과 같이 4개의 군으로 분류하였다; 복합레진 표면과 변연부에 레진 전색제를 적용하지 않은 대조군, 복합레진 표면과 변연부에 각각 레진 전색제를 적용한 Fortify Plus 군, BisCover 군 및 PermaSeal 군으로 분류하였다. 각 군의 시편을 실온의 증류수에 24시간 동안 보관한 후, $5^{\circ}C$$55^{\circ}C$에서 1,000회의 열 순환을 시행하고 2% methylene blue 용액에 4시간동안 침적시켰다. 각 치아는 저속의 diamond disk 를 이용하여 각 수복물의 중앙부를 따라 협, 설 방향으로 양분 하고 20배율의 광학 입체현미경 하에서 각각 교합면측과 치은측 변연 미세누출을 평가하였다. 각 군의 교합면측과 치은측 변연 미세누출에 대한 유의성을 검정을 시행하여 다음과 같은 결과를 얻었다. 1. 교합면측 변연 미세누출은 사용된 레진 전색제 간에 통계학적으로 유의한 차이를 나타내지 않았다. 2. 치은측 변연 미세누출은 대조군과 Fortify Plus 군이 PermaSeal 군보다 통계학적으로 높게 나타났으며, 또한 Fortify Plus 군은 Biscover 군보다 통계학적으로 높은 변연 미세누출을 나타내었다 (p < 0.05). 3. 대조군과 Fortify Plus 군은 교합면측 보다 치은측 변연에서 통계학적으로 더 높은 미세누출을 나타내었다 (p < 0.05). 4. BisCover 군과 PermaSeal 군은 교합면측과 치은측의 변연 미세누출 간에 통계학적으로 유의한 차이를 나타내지 않았다. 본 연구를 종합하면, 5급 복합레진 수복물의 교합면측 변연 미세누출에 대한 레진 전색제의 감소효과는 크게 나타나지 않았지만, 치은측 변연 미세누출에 대한 레진 전색제의 감소효과는 사용된 레진 전색제에 따라 서로 다르게 나타났다.

  • The purpose of this study was to examine the fracture strength and characteristics of teeth with MOD cavity preparation. Freshly extracted sound maxillary premolars were cleaned and stored in normal saline solution $37^{\circ}C$ for 72 hours before experiments. The roots of teeth were embedded in a self-curing resin, and the exposed crown were maintained in a vertical position by a modelling wax in a brass ring. The MOD cavities were prepared with No. 57 carbide bur under high speed to a depth of 2.0mm and a width of 2.0mm(Fig.1). All the prepared teeth specimens were divided into 7 groups according to the mode of cavity form and restorative materials (Table 1, 2): Group I, unpreapred, intact teeth as control Group II, prepared cavity without restoration Group III, prepared teeth restored with amalgam Group IV, prepared teeth restored with composite resin (P-10) Group V, prepared teeth with beveled enamel margins restored with composite resin (P-10) Group VI, prepared teeth restored with light-cured composite resin (P-30) Group VII, prepard teeth with beveled enamel margins restored with light-cured composite resin (P-30) After placement of restorations, all of the specimens were stored in water at $37^{\circ}C$ for 72 hours before testing. All of the specimens were tested on the Instron Universal Testing machine (No. 6025) in order to evaluate the strength of fracture. One metal ball 5.0mm in diameter contacting the specimens parallel to the occlusal surface was used to in this study (Fig. 1). The fracture characteristics of the specimens were examined with naked eye and in the scanning electron microscope (JSM-20). The results obtained from this study were as follows: 1. The mean fracture strength was the highest in group VI and that in group II was the lowest. 2. The progress of crack of teeth propagated into the pulp cavity. 3. In case of the group of the restored teeth, the crack occurred to be accompanied with cuspal fracture. 4. The crack of restored teeth was initiated along the pulpo-axial line angle of the cavity.

제 2급 와동에서 구치부 복합레진의 미세변연누출에 관한 연구 (MICROLEAKAGE OF POSTERIOR PACKABLE COMPOSITE RESIN hi THE GINGIVAL MARGINS OF CLASS II CAVITIES)

  • The use of flowable composite resins as liners in class II packable composite restoration has been suggested by some manufacturers. However, the contributions of this technique are unproven. The purpose of this study was to compare the gingival microleakage in class II packable composite restorations with or without the use of flowable composite resins as liners. Slot cavities were prepared on both proximals of 80 extracted human molars and randomly assigned to 8 groups of 20 each. The gingival margins were located at 1mm above CEJ in 80 cavities (group1-4) and 1mm below CEJ in 80 cavities (group5-8). The prepared teeth were mounted in the customized tray with adjacent teeth to simulate clinical conditions and metallic matrix band (Sectional matrix) and wooden wedges were applied. After acid etching and application of Single Bond, each group was restored with the following materials using incremental placement technique: Group 1,5 (Filtek P60), group 2, 3, 4 and group 6, 7, 8 (AeliteFlo, TetricFlow, Revolution/ Filtek P60). All specimens were thermocycled 500 times between 5$^{\circ}C$ and 55$^{\circ}C$ with 1 mimute dwell time, immersed 2% methylene blue dye for 24 hours and then rinsed with tab water. The specimens were embedded in clear resin and sectioned longitudinally through the center of restoration with a low speed diamond saw. Dye penetration at gingival margin was viewed at 20 magnification and analyzed on a scale of 0 to 4. Kruscal-Wallis One way analysis and Mann-Whitney Rank sum test were used to analyze the results. The results of this study were as follows. 1. The leakage values seen at the enamel margin were significantly lower than those seen at the dentin margin(P<0.05). 2. On the enamel margin, packable composite resins with flowable liners showed lower leakage than those without flowable liners, but there were no significant differences among the four groups(P>0.05). 3. On the dentin margin, four groups demonstrated moderate to severe leakage, and there were no significant differences in leakage values(P>0.05).

수복에 따른 구치부 복합레진 수복물의 변연부 미세누출 및 적합도 (MICROLEAKAGE AND MARGINAL ADAPTATION OF POSTERIOR COMPOSITE RESIN RESTORATIONS ACCORDING TO RESTORATIVE TECHNIQUE)

  • Incidence of using esthetic composite resin in the posterior area is increasing but there were lots of inconsistent reports about their microleakage and marginal adaptation. The purpose of this study was to evaluate the differences of microleakage and marginal adaptation according to restorative techniques. 30 cavities with enamel gingival margin were prepared and restored with 3 types of composite resin [Z-100($Scotchbond^{TM}$ MP), AELITEFIL ($Onestep^{TM}$), Her culite XRV(Fuji BOND LC)] in direct technique and another 30 cavities were restored with preformed CR inlays and 3 different modern resin and resin-modified GI cements (Superbond C&B, Choice, Fuji Duet). Samples were chemically stressed in 75% ethanol for 24 hours and were thermocycled (5-$55^{\circ}C$(500 times. The degree of microleakage through proximal and gingival margins was examined by 1 % MB dye and the degree of marginal adaptation by examining the margins via SEM. The following results were obtained ; 1. In direct groups, Herculite XRV [Fuji BOND LC, 35.13 (15.50) %] group showed statistically different, less microleakage than Z-100 [$Scotchbond^{TM}$ MP, 72.91 (16.91 %] group and AELITEFIL [One-step, 93.73 (13.66) %] group (p<0.05). 2. In indirect groups, the degree of microleakage in Mean(S.D.) were: Super bond C & B [39.00 (24.35) %], Choice [57.19 (33.80) %], Fuji Duet [58.22 (40.36) %]. But there was no significant difference. 3. There was no significant difference between resin cement and resin-modified GI cement. 4. There were gaps at the interface with the tooth structure, but no gap was seen at the interface with restoration in all specimens. 5. In direct groups, Herculite XRV(Fuji BOND LC) group made little gap compared with other groups, but 40-$50{\mu}m$ thickness of bonding agent, Fuji BOND LC, looked like a cement used in indirect technique. 6. All indirect groups showed a variety of cement thickness, from less than $20{\mu}m$ to over $100{\mu}m$ and that dimension of buccal/lingual margin was less than that of gingival margin.

와동형태가 5급 와동 수복물의 변연누출에 미치는 영향 (THE EFFECT OF CAVITY CONFIGURATION ON MARGINAL LEAKAGE OF CLASS 5 RESTORATION)

  • The purpose of this study was to investigate the effect of cavity configurations on the marginal leakage of class 5 glass ionomer cement and composite resin restorations. Four types of cavities such as saucer shape. notch shape. combined shape(notch shape occlusally and saucer shape gingivally). and U shape were prepared on the buccal and lingual surfaces of 80 extracted premolars(40 cavities for each shape). Occlusal cavity margins were placed at enamel and cervical margins were placed at dentin. 10 cavities of each shape were restored with Ketac Fil as a conventional glass ionomer cement. Fuji II LC improved as a resin modified glass ionomer cement, Z 100 as a hybrid composite resin. and Tetric Flow as a flowable composite resin (40 cavities for each material). After thermocycling, teeth were immersed in 5% basic fuchsin solution for 6 hours and sectioned longitudinally in a buccolingual direction through the center of the restoration. The dye penetrations at the tooth restoration interface were examined by stereomicroscope. The Result were as follows 1. In saucer shape, notch shape and combined shape, composite resin restorations showed lesser leakage than glass ionomer restorations(p<0.05) and in U shape. Tetric Flow showed the least marginal leakage and others were decreased as Z 100. Fuji II LC improved, Ketac Fil in that order. There were statistically significant difference between Tetric Flow and Fuji II LC improved. Ketac Fil and between Z 100 and Ketac Fil(p<0.05). 2. In Ketac Fil restoration group, saucer shape showed the highest marginal leakage and U shape showed the least marginal leakage and others were decreased as notch shape, combined shape in that order. There were statistically significant difference between saucer shape and combined shape, U shape and between notch shape and U shape(p<0.05). 3. In Fuji II LC improved restoration group, U shape showed the least marginal leakage. There were statistically significant difference between U shape and other three shapes(p<0.05). 4. The cavity configuration had no significant effect on marginal leakage of composite resin restorations(p>0.05).

수종의 접착제를 사용한 아말감 수복시의 미세 변연 누출에 관한 연구 (MICROLEAKAGE EVALUATION IN AMALGAM RESTORATIONS USED WITH BONDING AGENTS)

  • The purpose of this study was to evaluate the effectiveness of copal varnish and five bonding agents(Scotchbond Multi-Purpose Plus Dental Adhesive System, Panavia 21, All Bond 2, Superbond D-Liner II plus, Fuji Duet) in reducing microleakage under amalgam restorations. Class V cavity were prepared on both buccal and lingual surface of sixty extracted human molars with cementum margin and were filled with different kinds of liners and amalgam. All teeth were stored at $37^{\circ}C$ in physiologic saline solution. After 1 week they were thermally stressed for 500 cycles between 5 and $55^{\circ}C$ in baths containing 0.5 % basic fuchsin dye. The dye penetrations were observed with a stereomicroscope. Scanning electron micrographs were taken of representative tooth sections from each group to assess the nature of the tooth/liner/amalgam interface. The statistical test were applied to the results using a one way analysis variance (ANOVA) and Duncan's multiple range test. The results were as follows ; 1. In all groups, the enamel margin showed significantly lower leakage value than the cementum margin (p<0.05). 2. At the dentin and enamel margins, the leakage value of Copalite-lined group showed significantly higher than that of no liner group (p<0.05), but showed significantly lower than that of bonding agent lined-groups (p<0.05). 3. There was no significant difference between the bonding agent lined-groups (p>0.05). 4. On the backscattered scanning electron microscopy observation, discontinuous gaps were observed between amalgam and dentin in the bonding agent-lined amalgam restorations and the bonding agents appeared to fill the gap space and were mixed with amalgam particles. At the amalgam/tooth interface, unsealed dentin and continuous gaps were found in both unlined and Copalite-lined restorations.
