An Analysis Study on stress factor of Emergency medical Students during preparing Examination for Korea Registered licence (국시를 앞둔 응급구조과 학생의 스트레스 영향 요인에 관한 연구)

  • The purpose of this study was to analyze the factors on stresses to Emergency Rescue students' who have national examination ahead. In this descriptive research, 87 3rd-degree-Emergency Rescue students from K city were selected. Collected data was analyzed by using the SPSS PC program for descriptive data, t-test, ANOVA and Pearson's Correlation. In result, stress factors are categorized by curriculum($3.67{\pm}.74$), homework ($3.39{\pm}.93$), environment($3.37{\pm}.93$), examination($3.35{\pm}.74$), employment($3.04{\pm}.75$) and total mean was ($3.36{\pm}.59$). The way to adjust stresses characterized by long-term($3.14{\pm}.54$) and short-term($2.55{\pm}.44$), also the short-term is used more than the other ways. The significant differences between general characteristics and stress factor were gender(t=4.466, p=.040), grade(F=2.53, p=.047). In the correlations between categories of the stress factor and stress degree, the examination(r=.81, p=.000), curriculum(r=.66, p=.000), homework(r=.80, p=.000), environment (r=.74, p=.000), and employment(r=.62, p=.000) were significant. In conclusion, it is needed to study for managing stress according to various subjects.

A Study on the Analysis and Improvement methods of Emergency Medical Service Systems in Disaster Areas (재해현장에서의 응급의료체계에 대한 분석과 개선방안)

  • Various disasters have been continuously occurred in Korea from 1990s to now. However, there is no substantial improvement against damages as compared with the past due to various reasons such as lack of fundamental recognition, ineffective response systems and widespread insensitivity to safety. More worse, new types of disasters have been frequently generated due to rapid changes in social structures and industrial development, unusual changes in weather and changes of international situations. These disasters request comprehensive countermeasures. In particular, while material damages by disasters can be recovered, the losses of precious lives cannot be recuperated in any ways. Thus, it is critical to set effective disaster medical plans. The first way to minimize damages by disasters is the prevention and the next is to set the disaster medical plans focusing on preliminarily activating the emergency medical system to rapidly rescue and take appropriate emergency medical services for casualties in the early stage when any disaster occurs. Nevertheless, no sufficient researches or references do not exist up to now. Even worse, effective emergency medical systems that play critical roles in increasing survival rates of casualties in actual disaster areas is not deployed. For the United States, the consistent countermeasure system is established in FEMA through a close cooperative system with relevant organizations for serious accidents including terrorists' attacks or natural disasters. For the emergency medical services in disasters, the disaster medical plan is set to cope with any disasters in perfect order by special area as operating the National Disaster Medical SystemESF#8 Role by FEMA. Accordingly, we need to set the extensive and integrated disaster prevention system for rapid and flexible operation against various kinds of serious accidents. This study identified overall problems in disaster control plans in Korea and suggested how to improve the emergency medical service system in disaster areas. Furthermore, it aims to prepare the basic data to set the effective emergency medical service plans when substantial casualties break out and more reasonable and systematic disaster control plans to cope with the future occurrence of serious disasters.

Field Triage of Severely Injured Patients and Transportation by the EMS Rescue Group of Busan and Kyungnam, Korea: Is It Appropriate? (부산·경남 지역에서 전원 의뢰된 중증외상 분석을 통한 119 구급대의 환자 분류와 이송병원 선정에 대한 고찰)

  • Purpose: The objective of this study was to determine the appropriateness of Emergency Medical Service's (EMS's) triage and transport of severely injured patients in Busan and Kyungnam, Korea. Methods: The medical records of the Emergency Medical Information Center were retrospectively reviewed from January 1, 2010 to December 31, 2010. We identified the number of patients that should have been transferred from a secondary to a tertiary hospital according to the EMS field triage protocol. Results: In a total of 472 cases requests to be transferred to a third hospital were made through the Emergency Medical Information Center. Of these, 207 patients(43.9%) should have been transferred to a tertiary hospital according to the EMS field triage protocol. Among them, thirty-three(15.9%) patients satisfied step 1, 15(7.0%) satisfied step 2, and 117(56.5%) satisfied step 3. Twenty-three(11.1%) patients satisfied both steps 1 and 3. Conclusion: We found the triage by the EMS in the transfer of severely injured patients to a tertiary hospital to be inappropriate and re-education of EMS personnel regarding the EMS field triage protocol is needed. Because many patients are transferred from a secondary to a third hospital, we suggest changing the EMS field triage protocol to expand the severe injury criteria. A need exists to authorize secondary hospitals to transfer severely injured patients directly because there are no trauma centers in Korea.

A Study on the Actual Profile of Emergency Medical Personnel's Pre-admission Infection Control - Based on Jeollabuk-do provincial area - (병원 전 응급의료종사자의 감염관리 실태 연구 - 전북지역 중심으로 -)

  • Purpose: By identifying the actual profile of emergency medical personnel's pre-admission infection control practices, this study intended to provide a basic reference material for the improvement and reorientation of pre-admission infection control measures, and thereby help establish an effective plan for pre-admission infection control activities. Methods: Total 119 EMT's working for Jeollabuk-do Provincial Fire Defense HQ were asked to join a structured questionnaire survey from June to August 2006. Results: 1. It was found that 56.1% respondents answered no guideline available on the prevention of infection. Out of our rescue brigade members who knew about relevant guideline available, 34.2% respondents answered that their department conducted quality control program for the guideline. 2. For protective outfit in emergency practice, it was found that most respondents put on gloves or nothing at all(38%), which was followed by sterile gloves(29.2%), disposable mask(26.9%), gown(4.3%) and protective goggle(1.6%). And it was noted that all respondents(100%) washed out any clothing contaminated with somatic secretion on their own. 3. For a question about any experience in emergency activities exposed to infectious diseases, it was found that most of all respondents(77.9%) answered 'No', which was followed by 'Don't Know'(18.6%) and Yes(3.9 %). 4. For a question about any experience in inquiring of patients about infectious diseases, it was found that most respondents(49.4%) answered 'Yes' and 'Sometimes'(9.1%). It was noted that 20.2% respondents had extra medical examination in medical institution in terms of whether they were exposed to infectious diseases, apart from regular medical examination. Conclusions: In order to protect 119 EMT's from infectious diseases, it will be necessary to acquire emergency medical staffs specializing in infection control and organize corresponding personnel training units to keep providing reorientation and evaluation. In addition, it will be also necessary to supply them with a full set personal protection apparatuses and other equipments required for disinfection and sterilization.

  • In the recent years, an attempts have been made to convergence medical technology and IT technology to overcome the limitations of temporal, spatial in the medical technology applications. Emergency care is primarily intended to provide primary care and stabilization to the patient, thereby saving the patient's life and minimizing the risk until hospital care. In this study, we developed an application S/W to support emergency services for emergency medical services. Paramedics can easily keep a log of emergency activities and monitor the patient's condition in real time with this application S/W on smart devices. In particular, medical signals and information emergency status of emergency patients can be measured or photographs of affected areas can be taken. In addition, this report can be delivered to remote medical doctors for rapid emergency measures and immediate hospital response.

A Study on Improvement of Emergency Medical System for forestry accident (임업 사고 응급대응체계의 개선방안에 관한 연구)

  • The estimated on-site accident rate in Forestry is relatively high. According to statistics of the accident, in the recent 5 years, from 2014 to 2018, forestry accidents have resulted in 98% of injuries and 87% of fatalities. Especially, there are significant geographical constraints to access to the scene in case of an accident. Even though the capacity of first aid capacity is notably emphasized its importance to minimize the scale of damages, the relevant employees have been educated only basic first aid, which is not considered circumstances or geographic limitations, by Occupation Safety and Health Acts. Therefore, the purpose of this study is to derive a direction for a forest emergency service system to increase forestry workers' survival and prevent secondary injury through securing 'Golden Time.' This study conducts analyzing relevant laws and regulations in domestic and international settings as well as looking at several concerned accident cases. The outcome of analysis presents an issue regarding the implementation of onsite first aid in forestry and existing risk factors depending on the working process. Finally, we suggest two ways to improve the forest emergency service that are 1) an appropriate curriculum and kit for forest first aid; and 2) a system for emergency transfer through sharing information between National Fire Agency (NFA) and emergency medical service center, and emergency and rescue mission using helicopter from NFA and Korea Forest Service.

  • Currently, there are a total of 68 emergency rooms based on Seoul, South Korea, and there is a portal site that allows you to inquire the location of the emergency room, but it is difficult to use in an actual emergency situation because it consists of selecting a gu and a self-governing dong. In addition, it may be more efficient to go to the emergency room directly because you may miss the golden time necessary for survival in a situation where you call 119 and wait for the rescue team. Therefore, in this paper, we propose a service that can quickly search the location of the emergency room based on a specific location through various functions supported by MongoDB. After downloading emergency room location data based on Seoul Metropolitan City, storing it in MongoDB, processing the data through various processing techniques, and applying a spatial index, you can query the emergency room based on distance from a specific location in real time.

Posttraumatic Stress by Work in Firefighters (소방공무원의 직무별 외상 후 스트레스)

  • To investigate a posttraumatic stress, social support and work burden and to identify high risk group and related factors which were exerted influence on posttraumatic stress of firefighters. Data were gathered from total 264 firefighters consisting of 85 fire distinguishers & rescue workers, 96 emergency medical personnel and 83 in ambulance & fire truck drivers in C province and were analyzed 22 items by IES-R, 20 work burden items by Choi(2000), 8 social support items by Oh(2006) using SPSSWIN 14.0 program. Posttraumatic stress of general characteristics by work was showed significant difference in age, marriage status, education, position, office duration, mobilization frequency and experience of traumatic events. Posttraumatic stress were showed significant difference in 3 groups and fire distinguishers & rescue workers is the highest group. Posttraumatic stress was correlated with work burden(r=.317, p<0.01) for fire distinguishers & rescue workers, social support(r=-.331, p<0.01) and work burden(r=.522 p<0.001) for emergency medical personnel and work burden(r=.454, p<0.01) for ambulance & fire truck drivers. The high risk groups are fire distinguishers & rescue workers(60%), emergency medical personnel(55.2%)and ambulance & fire truck drivers(45.8%). Related factors are the experience seeing victim's danger for fire distinguishers & rescue workers(odds ratio=1.216, 95% confidence interval:1.068-1.383), and are work burden(odds ratio=1.100, 95% confidence interval:1.043-1.159) and office duration(odds ratio=1.010, 95% confidence interval 1.001-1.018) for emergency medical personnel, and are the experience seeing victim's danger(odds ratio=1.178, 95% confidence interval:1.010-1.373), age(odds ratio=1.129, 95%confidence interval:1.020-1.249), work burden(odds ratio=1.103, 95% confidence interval:1.034-1.177) for ambulance & fire truck drivers.

A Smart Care Surveillance System supporting Various CCTV Cameras (다양한 CCTV 카메라를 지원하는 스마트 케어 관제 시스템)

  • In this paper, we introduce a smart care surveillance system which can support various CCTV cameras. In order to monitor an emergency requester in case of emergency, the server performs automatical CCTV Pan, Tilt, and Zoom control based on the location coordinates of the emergency requester. Also, a server finds and tracks the emergency requester using image processing and the updated location information. We implement a smart care surveillance system using the Genetec SDK tool to support various CCTV cameras. The efficiency of the rescue operation with the smart care surveillance system can be improved because rescuer can quickly control and monitor the requester's CCTV images.

A Characteristics Analysis of Pre-hospital Acute Poisoning in The Elderly

  • This study selected all patients with acute poisoning carried by the 119 emergency services in K-Province from January 1 to December 31, 2015 for complete enumeration. The subjects were 1,627 patients who were classified as poisoning suspected in chemicals and animal/insect poisoning by Rescue Emergency Activity Information System(Emergency Statistics) in fire-fighting portal system of K-province fire-fighting officers. The 119 activity journals were analyzed retrospectively. The major results are as follows. Regarding the demographic characteristics of the elderly and the non-elderly, the most people were unemployed in the elderly group, other job, the non elderly group about their occupations and in both groups, the most people lived in rural areas about their living places. Regarding the poisoning characteristics of the elderly and the non-elderly, the most causing substance was bee sting about the poisoning causing substances and most of patients didn't get drunken about drunken state in both group. Regarding the time factors of the elderly and the non-elderly, both groups had the most accidents in afternoon about the poisoning time and in summer about the seasonal distribution.
