응급실 간호사의 폭력반응, 대응 및 소진간의 관계 (Relationship between Violence Response, Coping, and Burnout among Emergency Department Nurses)

  • Purpose: The purpose of this study was to identify relationships between violence response, coping, and burnout among emergency department nurses. Method: This descriptive study using self report questionnaires was conducted from October 1 to October 20, 2008. Participants were 147 general nurses working in emergency departments in hospitals having over 500 beds and located in Busan. ANOVA and Pearson's correlation coefficients with SPSS PC+ WIN 14.0. were used to analyze the data. Results: Scores for violence response, coping, and burnout were 3.08 (1-5 scale), 2.57 (1-4 scale), and 3.22 (1-5 scale) respectively. There were significant differences in violence response and burnout by education, clinical careers, clinical careers in ED, and plans to work in the future, and no significant differences in coping by general characteristics. There was a significant and highly positive relationship between violence response and burnout. Conclusion: The degree of violence response and burnout was comparatively high, while that of coping was average. Therefore, it is necessary to develop effective programs related to prevention and coping with violence for nurses in emergency departments. Institutional measures by hospital authorities are also needed to improve the nursing quality by providing a safe workplace.

한국형 폭력 반응 측정도구의 타당도 및 신뢰도 검증: 응급실 간호사 대상 (Validity and Reliability of ARQ-K (Korean Version of the Assault Response Questionnaire) for Emergency Department Nurses in Korea)

  • Purpose: To investigate the validity and reliability of the Korean Version of the Assault Response Questionnaire (ARQ-K) measuring the intensity of reaction to victimization of emergency nurses in Korea. Methods: An internal consistency reliability and construct validity using exploratory and confirmatory factor analysis were conducted using SPSS WIN (20.0) and AMOS (20.0). Survey data were collected from 321 nurses who worked in 3 levels - wide regional emergency centers, regional emergency centers, appointed emergency centers - of emergency care facilities in Busan, Korea. Results: The Cronbach's alpha values regarding internal consistency were .77~.93 for the subscales of ARQ-K. Factor loadings of the 26 items on the four subscales ranged from .59 to .84. The four-subscale model was validated by confirmatory factor analysis (${\chi}^2/df=3.85$, p<.001, RMR=.06, GFI=.80, NFI=.81, TLI=.83, CFI=.85, RMSEA=.09). Conclusion: This study shows that the Korean Version of the Assault Response Questionnaire is a valid and reliable instrument to assess nurses' reaction to victimization of emergency nurses in Korea.

  • Purpose: The purpose of this study was to examine the frequency, perceived importance and competence of pediatric emergency nursing practice (PENP) in nurses who cared for pediatric patients in the emergency department. Methods: This cross-sectional descriptive study analyzed 175 emergency department nurses caring for pediatric patients at 7 university hospitals with more than 500 beds, located in Seoul and Gyeonggi Province. The measurement tool was modified from the Classification of Standard Nursing Activities to measure the frequency, importance of PENP, and nursing competence. It comprised 143 items in 16 domains of PENP. Results: The most frequent nursing practice was the domain of 'nursing records and environmental management' and the least frequent practice was the 'research and consulting' domain. The nursing care domain perceived to be most important by nurses was 'specialized intensive nursing care'. The nursing care domain with the highest level of nursing competence was 'hygiene care', and the domain with the least level of nursing competence was the 'research and consulting'. Conclusion: These results will be utilized as basic data for future pediatric emergency nursing education initiatives and for establishing priorities of nursing policy to improve health care for children admitted to the emergency department.

응급실 간호사의 회복탄력성, 분노표현양식이 간호업무성과에 미치는 영향 (Influence of Resilience and Anger Expression Style on Nursing Work Performance in Emergency Nurses)

  • Purpose: This study aimed to identify the influence of nurses' resilience and anger expression styles on their nursing work performance in emergency departments. Methods: The study included 107 nurses from 5 hospitals in Seoul and Gyeonggi-do who consented to participate. Data ware collected from March to April 2016. Multiple regression analysis was used to identify factors influencing nurses' performance. Results: Nurses' performance was higher when resilience was high and anger expression was low. The result of the multiple regression indicated that present working years in the emergency room, work satisfaction, anger expression, anger control, and resilience predicted 39.4% (F=7.27, p<.001) of the nursing performance of hospital nurses in emergency departments. The most powerful predictor was resilience (${\beta}=.45$, p<.001), followed by anger-out (${\beta}=-.25$, p=.006). Conclusion: This study showed that the most influential variables of work performance in emergency nurses ware resilience and anger expression. To enhance the nursing performance of hospital nurses in emergency departments, it is necessary to develop and implement mentorship programs that enhance resilience and educational programs that help them manage their anger.

응급실 간호사의 임파워먼트와 감염관리 수행도의 관계 (The Relationship between Empowerment and Performance of Infection Control by Emergency Department Nurses)

  • Purpose: The purpose of this study was to identify the relationship between empowerment and performance of infection control by emergency department nurses. Method: Participants were 190 nurses working in emergency departments in 14 hospitals located in Busan. Descriptive statistics, t-test, ANOVA, and Pearson's correlation coefficients with SPSS PC+ WIN 12.0 were used to analyze the data. Results: There were significant differences in empowerment by years in ED (emergency departments) and position. There was a significant differences in performance of infection control by position. There were significant differences in empowerment by infection control-related characteristics and by regular conference for infection control. There was a significant and positive relationship between empowerment and performance of infection control in ED nurses. Conclusion: The results of this study indicate that improvement in the level of ED nurse's empowerment would lead to an increase in the performances of infection control.

응급실 간호사의 회복탄력성, 환자안전관리에 대한 인식, 선도적 행위가 표준주의 수행에 미치는 영향 (The Influence of Resilience, Awareness of Patient Safety Management, and Proactive Behavior on Performance of Standard Precautions among Nurses Working in Emergency Department)

  • Purpose: This study investigated the correlations among resilience, the awareness of patient safety management, proactive behavior, and the performance of standard precautions among nurses working in the emergency department. Additionally, it aimed to identify the factors influencing the performance of standard precautions. Methods: Data were collected through a descriptive survey conducted between June 10 and July 9, 2023. The participants included 208 nurses working in the emergency department in Korea. The collected data were analyzed using SPSS/WINdows software version 29.0, based on the frequency, percentage, mean, standard deviation, t-test, one-way analysis of variance, Scheffé test, Pearson's correlation coefficients, and multiple regression. Results: The analysis based on the participants' general characteristics showed that performance of standard precautions varied significantly according to age (t=2.10, p=.036), size of the emergency department (F=7.46, p<.001), average number of patients treated per day while working in the past month (t=-3.88, p<.001) and standard precautions training experience (t=2.16, p=.032). The main factors influencing the performance of standard precautions among nurses working in the emergency department are awareness of patient safety management (β=.62, p<.001), resilience (β=.18, p=.042), and age (β=-.15, p=.007), with a total explanatory power of approximately 46.0%. Conclusion: It is necessary to establish programs and an effective education system to increase resilience and awareness in patient safety management to improve the performance of standard precautions among nurses working in the emergency department.

응급실 간호사의 노인학대 개입의도 영향요인 (Factors Influencing Intention of Intervening towards Elder Abuse among Nurses in the Emergency Department)

  • Purpose: The purpose of this study was to examine the factors influencing intention of intervening towards elder abuse among nurses in the emergency department. Methods: A cross-sectional descriptive design was used. A convenience sample of 209 emergency room nurses working in university hospitals and general hospitals completed the questionnaires. Multiple regression analysis was performed to examine predictors of intention of intervening towards elder abuse. Result: Awareness of elder abuse, legal and institutional knowledge, attitude, subjective norm, and self-efficacy were positively correlated to the intention of intervening towards elder abuse. Factors of the intention of intervening towards elder abuse in emergency room nurses were awareness of elder abuse, followed by subjective norm and attitude. Conclusion: The findings suggest that it is necessary to educate emergency room nurses in order to improve awareness and knowledge of elder abuse. From the nursing practice and hospital organization level, specific interventions and strategies to increase the assessment and intervention of elder abuse are needed.

응급실 간호사의 건강신념이 낙상예방활동에 미치는 영향 (Effects of Health Belief on Fall Prevention Activities of Emergency Room Nurses)

  • Purpose: The purpose of this study was to analyze factors affecting fall prevention activities of emergency room (ER) nurses based on their health belief factors (perceived susceptibility, perceived benefits, perceived severity, perceived barriers, and cues to action). Methods: The study design was a descriptive survey using questionnaires which were given to 127 emergency room nurses from two regional emergency medical centers, four local emergency medical centers, and two local emergency medical facilities. Data were analyzed using descriptive analysis, t-test, one-way ANOVA with LSD test, Pearson correlation, and multiple regressions. Results: ER nurses' fall prevention activities had a mean of $3.78{\pm}0.50$. Eight individual characteristics and health belief factors accounted for 30.8% of the fall prevention activities. Fall prevention activities were found to be positively affected by emergency medical facilities, perceived benefits, and cues to action and negatively affected by factors, such as ER career and perceived severity. Conclusion: The results indicate that it is necessary to formulate a plan for enhancing perceived benefits and cues to action to improve fall prevention activities. In addition, fall prevention activities should be encouraged for ER nurses who have worked in local medical institutions for less than 1 year or more than 5 years.

응급실 간호사의 전문직 삶의 질: 공감만족, 소진, 이차 외상성 스트레스 (Emergency Nurses' Professional Quality of Life: Compassion Satisfaction, Burnout, and Secondary Traumatic Stress)

  • Purpose: Professional quality of life is the quality one feels in relation to their work as a helper. The purpose of this study was to describe professional quality of life among emergency nurses. Methods: A total of 15 emergency rooms were selected in three cities. Among 263 nurses working at these emergency rooms, 178 nurses consented to participate in this cross-sectional survey. Professional Quality of Life (ProQOL) Scale version 5 was used to measure compassion satisfaction, burnout and secondary traumatic stress. Cluster analysis was used to classify nurses according to professional quality of life. Results: The mean scores (SD) for compassion satisfaction, burnout, and secondary traumatic stress were 32.12 (5.45), 28.27 (4.28), and 28.20 (5.07), respectively. The result of cluster analysis according to standardized score of compassion satisfaction, burnout, and secondary traumatic stress yielded three clusters. Over 50% of participants showed low professional quality of life. Nurses who were included in a cluster representing low professional quality of life were younger, had shorter periods of nursing experience, and perceivedlower social support than other cluster. Conclusion: Education or support programs for emergency nurses are needed to enhance their professional quality of life.

응급실 간호사의 중증도 분류 역량에 대한 개념분석 (Concept Analysis of Triage Competency in Emergency Nursing)

  • Purpose : This concept analysis identified attributes and defined triage competency among emergency nurses. Method : Walker and Avant's approach was used to guide the concept analysis. A literature review was completed including 26 studies, 5 reports of related associations, and 5 books. Results : The concept of triage competency in emergency nurses was identified as five attributes: clinical judgment, expert assessment, management of medical resources, timely decision, and communication. Antecedents of the concept were triage education and emergency room experience. The consequences of the concept were efficiency of care, patient rating, and safety. Triage competency in emergency nurses was defined as the comprehensive ability to prioritize patients' urgency and allocate limited medical resources. Conclusion : This study is meaningful since it clarified triage competency among emergency nurses. The attributes and empirical indicators of this study will likely lay the foundation for development of triage competency metrics.
