급성심근경색증 환자에 대한 병원 전 단계와 병원 단계에서의 응급처치 분석 (Analysis on emergency care to the patients with acute myocardial infarction in pre-hospital and in-hospital phase)

  • Purpose : The purpose of this study is to provide the basic data to improve pre-hospital phase emergency care for acute myocardial infarction (AMI) patients by analyzing AMI patients' clinical characteristics and emergency care situations. Methods : Data were collected through medical records of 385 AMI patients including ambulance records of 107 AMI patients transferred to the emergency medical center for three and a half years. Results : Regarding emergency care for AMI patients in pre-hospital phase, 47% of the care revealed moderate level or higher, and appropriateness of pre-hospital phase emergency care for cardiopulmonary complaints practiced by paramedics showed statistically significant improvement in recent years (p<.001). The time from onset of symptom to ballooning intervention by 119 emergency services was shorter than that in other cases. However, emergency care by paramedic was mainly basic life support. Conclusion : Since prognosis of AMI shows vast differences depending on prompt detection and medical intervention, cooperation between pre-hospital and in-hospital phase is highly required. 119 paramedics should be trained focusing on the accurate assessment and emergency care, and medical direction should be activated. In addition, regulation on 12-lead EKG, cardiac enzyme analysis, use of analgesics and thrombolytic agents should be legally implemented.

서울지역 일개 지역응급의료센터에 내원한 환자의 보험급종별 응급실 이용행태 분류 (Classification of emergency room usage patterns according to the type of insurance in patients visiting an emergency medical center in Seoul, Korea)

  • Purpose: We analyzed the characteristics and differences in patients' medical benefits and health insurance based on disease severity classification. Methods: We examined 29,139 patients who visited the emergency medical center of K Hospital from January 1,2016 to December 31, 2016. Survey items included the Korean Triage and Acuity Scale (KTAS) classification of emergency and non-emergency situations ratio and type of insurance. Results: According to KTAS classification, 76.2% of patients exhibited an emergency condition and 23.8% exhibited a non-emergency condition. Emergency patients exhibited more trauma than non-emergency patients. According to the type of insurance coverage, the duration of stay in the emergency room was longer for patients with medical care than for patients with health insurance. Additionally, 119 ambulances use was significantly higher among patients with medical care. Conclusion: Policy discussions should address alternative ways to replace the 119 ambulances used by patients in this study. Additionally, health care administrators should identify alternative care agencies as potential alternatives to emergency room visits.

IoT notification system for marine emergencies

  • Minimization of human casualties in disaster situations is of paramount importance. In particular, if a marine disaster occurs, it can be directly connected to human casualties, so prompt action is needed. In the event of a marine disaster, the route and location of movement should be identified and life tubes should be used to float on the water. This paper designs and proposes an emergency IoT notification system that can quickly rescue drowning people. The maritime emergency IoT notification system consists of four main types. First, an emergency IoT device that detects the expansion of the life tube and delivers location and situation information to the emergency IoT notification server. Second, an emergency IoT web server that manages emergency information and provides notification. Third, a database server that stores and manages emergency IoT notification information. And finally, an emergency notification app that can receive and respond to emergency notification information. The emergency IoT device consists of a TPMS(Tube Pressure Monitoring System) device that checks the pressure value of the TPMS in real time and sends it to the IoT device, and an IoT device that sends the rescuer's voice information and emergency information to the emergency IoT server. Emergency information is delivered using the MQTT(Message Queuing Telemetry Transport) protocol, and voice information is delivered to the IoT server as HTTP FormData.

일반인의 구급차 이미지에 대한 인식 유형 (Public perception of ambulances)

  • Purpose: This study aimed to obtain, categorize, and analyze baseline data regarding the subjective perception of ambulances from the consumers' perspective, which can be utilized for improving emergency medical service systems. Methods: Fifty people aged 20 to 40 years old were selected as P samples. Data from 35 statements of Q-cards and Q-sample distribution plots were gathered using Q-sort and analyzed using the PC-QUANL program for major factor analysis. Results: We found four unique characteristics which accounted for 57.57% of the total variation: "Considerate of emergency situations," "Identifying emergency patients," "Trusting emergency vehicles," and "Sympathizing with emergency patients." Conclusion: Organizations that provide emergency medical services using ambulances should plan and render patient care according to the four types of public perception of ambulances.

응급의료에서의 설명·동의 원칙과 응급의료거부죄 (Informed Consent and Refusal of Treatment in Emergency Medical Situation)

  • 이 논문에서는 현행 응급의료에 관한 법률이 규정하고 있는 응급의료에서의 설명·동의의 원칙과 응급의료거부죄를 검토함으로써 응급의료종사자의 환자에 대한 생명보호 의무가 환자의 자기결정권 보장보다 중요한 경우에 한하여 응급의료거부죄가 성립함을 제시한다. 응급의료에서도 일반 의료상황과 마찬가지로 의료행위 시행 전 환자에게 응급의료의 필요성이나 방법 등에 관하여 설명하고 동의를 받아야 함이 원칙이다. 다만, 설명·동의 절차를 예외적 방법으로 이행하거나 생략할 수 있음에도 그 절차 준수를 이유로 응급의료를 거부·기피한 응급의료종사자는 응급의료거부금지에 따른 행정처분과 행정벌을 부담하게 된다. 즉, 설명·동의 절차 생략 가능성에 관한 판단에 따라 응급의료거부죄가 성립할 수도 있는 것이다. 환자가 미성년이거나 의사결정능력이 없는 경우 그 법정대리인이 환자의 의학적 이익에 반하는 결정을 하더라도 법정대리인의 의견이 무조건적으로 존중되는 것은 아니다. 미성년 환자도 원칙적으로 자신의 신체에 관하여 결정할 권리가 있고, 법정대리인의 결정 역시 환자의 최선의 이익을 위한 것일 때 유효하기 때문이다. 환자가 치료를 거부하는 상황에서도 원칙적으로 응급의료종사자의 생명보호의무가 더 우선한다. 그러나 현행법은 여러 예외 상황에 대해 명문의 규정을 두고 있지 않아 응급의료 현장에서 그 해석에 어려움이 있다. 한편, 우리 대법원 및 하급심 판례는 응급의료종사자의 응급의료의무와 설명의무 사이의 이익형량이 불가피한 상황에서 환자의 생명상실이 문제되는 경우 설명의무보다 응급의료를 시행하여 환자의 생명을 보호하여야 할 의무가 우선이고, 예외적으로 사전에 치료 여부·방법에 대해 환자의 진지한 숙고가 있었던 경우 환자의 자기결정권이 응급의료의무와 대등하게 고려될 수 있다는 취지로 설시하고 있으므로, 이를 체계적으로 정리하고자 한다. 나아가 현행법의 해석만으로 해결이 어려운 부분에 대하여는 1) 미성년자에 대한 응급의료의무 조항 신설, 2) 응급환자의 의사결정능력 판단 기준을 의학적 내용을 중심으로 수정·보완, 3) 응급처치시 의료인의 추가 동의가 불요함을 명시, 4) 복수의 의견 충돌이 있는 경우에 대한 제도적 보완, 5) 응급의료 중단시 벌칙조항 신설 등 입법 과제를 제시한다.

  • In order to prepare for the commercialization of Korean tilting train, this study was made on the pre-analysis of risk factors that can happen during operation, and emergency measures for safe operation of tilting train. The purpose of the study of emergency measures is to help to make tilting train run safely by making counteraction procedures for unusual situations such as train breakdown or accidents during the operation of a tilting train so that the engineer can quickly take care of each situation. Based on the items of emergency measures for EMU operated by Korean Railroad(or Korail), possible breakdown items during operation were selected and prioritized considering the occurring frequency and emergency level of each breakdown. In addition to these items, by adding items to represent tilting train's typicalfunctions such as tilting function error, tilting storage battery's electric pressure drop, etc, a total of62 emergency items were finally selected and measures for each item was made.

  • Due to almost all tracks of new Kyoungbu high-speed lines(HSL) consisting of bridges and tunnels, HSR operational safety management is very important factor for passenger safety and preventing catastrophic event. Especially, it is important to approach the scene of the accident and assist in saving lives as soon as possible under emergency conditions. Therefore it is essential to prepare access methods and available resources in advance in order to support emergency correspodances promptly and effectively. This paper persents an application of geographical information system(GIS) for developing emergency management system as well as batabase system on tracks and facilities, electric/communication equipment and safety equipment required to support emergencies. The on-site information on the proximity of HSL is expressed on the numerical map, which can be helpful to call external supports such as medical service, local authorities, police offices, fire brigades, and etc. for the emergency situations.

  • 최근 시간적, 공간적, 의료기술 적용의 한계를 극복하기 위하여 의료 기술과 IT 기술의 융합을 시도하고 있다. 본 연구에서는 응급 의료 서비스를 위한 원격 의료 지도를 지원하기 위한 앱을 개발하였다. 개발된 스마트 원격 의료 지도 지원 앱은 원격지에 있는 의사가 응급 구조사로부터 응급 환자의 상황을 실시간으로 전달 받을 수 있다. 또한 환자의 상태 진단 및 응급 치료를 지도할 수 있으며 응급실에서의 대응이 신속하게 이루어질 수 있도록 할 수 있다.

Emergency Signal Detection based on Arm Gesture by Motion Vector Tracking in Face Area

  • This paper presents a method for detection of an emergency signal expressed by arm gestures based on motion segmentation and face area detection in the surveillance system. The important indicators of emergency can be arm gestures and voice. We define an emergency signal as the 'Help Me' arm gestures in a rectangle around the face. The 'Help Me' arm gestures are detected by tracking changes in the direction of the horizontal motion vectors of left and right arms. The experimental results show that the proposed method successfully detects 'Help Me' emergency signal for a single person and distinguishes it from other similar arm gestures such as hand waving for 'Bye' and stretching. The proposed method can be used effectively in situations where people can't speak, and there is a language or voice disability.

  • 최근 개인건강기록(PHR)은 환자중심의 건강정보교환 모델로 각광받고 있다. PHR 소유자는 언제 어디서나 쉽게 자신의 기록을 저장하고 회수할 수 있는 접근 권한을 누릴 수 있다. 하지만, PHR의 민감성과 신뢰성 특성 때문에 PHR은 개인이 접근할 수 있는 권한을 결정할 수 있도록 안전하게 유지되어야 한다. 본 논문에서는 응급상황에서 사용자의 PHR에 접근할 수 있는 시스템을 제안하였다. 환자가 의식이 없는 응급상황에서 응급센터요원은 PHR에 대해 미리 정의된 권한에 의하여 PHR 서버에 응급접근을 요청한 응급 정보를 사용할 수 있다. 제안된 시스템에서 PHR 사용자는 응급상황에서 좀 더 정교한 접근제어를 명세화 할 수 있다.
