A Study on the Consciousness Survey of Improvement of Emergency Rescue Training -Based on the Fire Fighting Organizations in Gangwon Province- (긴급구조훈련 개선에 관한 의식조사 연구 -강원도 소방조직을 중심으로-)

  • Purpose: Fire-fighting organizations are the very first agencies that take actions at a disaster scene, and emergency rescue training is carried out for prompt and systematic response. However, there is a need for a change due to the limitations in emergency rescue trainings such as perfunctory trainings or trainings without considering regional or environmental characteristics. Method: This study is to conduct theoretical review with regard to emergency rescue training and present a measure to improve the emergency rescue training through attitude survey targeting fire-fighting organizations in Gangwon area. Result: Facilities that cause difficulties when doing emergency rescue activity were mostly hazardous material storage and processing facilities. In terms of the level of emergency rescue and response task, most respondents answered that the emergency rescue was insufficient. The respondents answered that the effectiveness of emergency rescue training was helpful, but some responses showed that the training was not helpful because of scenario-based training, seeming training, similar training carried out every year, unrealistic training, and lack of competent authorities' interest and perfunctory participations. Most respondents answered for the appropriateness of emergency rescue training and evaluation that they were satisfied, however, they were not satisfied with the evaluation methods irrelevant to the type of training, evaluation methods requiring unnecessary training scale, and evaluation methods leading perfunctory participations of competent authorities. Lastly, respondents mostly answered that training reflecting various damage situations are necessary regarding the demand on the improvement of emergency rescue training. Conclusion: The improvement measures for emergency rescue training are as follows. First, it is necessary to set and prepare various training contents in accordance with regional characteristics by reviewing major disasters occurred in the region. Second, it is necessary to revise the emergency rescue training guidelines and manuals for appropriate training plan for each fire station, provide education and training for working-level staff members, and establish training in a way that types, tactics, and strategies of emergency rescue training could be utilized practically. Third, it is necessary to prepare a scheme that can lead participation and provide incentive or penalty from the planning stage of training in order to increase the participation of supporting and competent authorities when an actual disaster occurs. Fourth, it is necessary to establish support arrangements and cooperative systems by authority through training by fire stations or zones in preparation for disaster situations that may occur simultaneously. Fifth, it is necessary to put emphasis on the training process rather than the result for emergency rescue training and evaluation, pay attention to the identification of supplement points for each disaster situation and make improvements. Especially, type or form of training should be considered rather than evaluating the execution status of detailed processes, and the evaluation measure that can consider the completeness (proficiency) of training and the status of role performance rather than the scale of training should be prepared. Sixth, type and method of training should be improved in accordance with the characteristics of each fire station by identifying the demand of working-level staff members for an efficient emergency rescue training.

Improvement of Night Pharmacy Service by Automated Dispensing Cabinet System Implementation in Emergency Medical Center (응급의료센터 자동약품분배 캐비닛시스템 운영으로 인한 야간 약국업무개선)

  • Objective: An automated process for medication preparation and dispensing is essential to improve the quality of work. To reduce night pharmacy workload, a new automated dispensing cabinet system was implemented in a hospital emergency medical center. The purpose of this study is to verify that implementation of an automated dispensing cabinet system will influence the efficiency of night pharmacy work. Methods: To evaluate the new system implementation, a retrospective study and survey was performed in the Ewha Womans University medical center. We compared the dispensing and near-miss error rates between the automated dispensing cabinet system and a night pharmacy. The degree of satisfaction of night shift workers with the new system was surveyed. Results: This study showed significantly reduced dispensing rates of night medications (56.1% and 37.3%; p < 0.01) and near-miss night medications (0.27% and 0.17%; p<0.01). Thirty-two persons responded to the survey, and the satisfaction score for the new system was 4.0 (${\pm}0.8$). The scores were high in order of efficiency, management, and convenience. Time requirement was also reduced because of the simple step of only reviewing in the pharmacy with the new system. Conclusion: Due to system implementation, workload was reduced and time was saved for not only night shift workers but also patients receiving emergency discharge medicine. It was suggested that this will have a positive effect on pharmacist medical service and patient safety.

  • The construction of an emergency generator's diagnosis system for the preparation of emergency in nuclear plant is vital. To construct a knowledge base of the diagnosis system, the classes and a causality model should be designed. In order to design those elements, at first. object of the diagnosis system should be defined. After the investigation of normal and abnormal states. the external knowledge such as entities and activities is extracted, that the operational principle of the system. For the conversion of the extracted external knowledge to the internal one, the entities are defined as classes and the activities converted into the causality. Through the recursive configuration of the causality and proper examination, the diagnosis knowledge applicable to the knowledge base is completed. In this paper, it is possible to construct a knowledge base with high portability since the independence of design model is considered through the decision table.

Exploring Near-Future Potential Extreme Events(X-Events) in the Field of Science and Technology -With a Focus on Government Emergency Planning Officers FGI Results -

  • This study aims to predict uncertain future scenarios that may unfold in South Korea in the near future, utilizing the theory of extreme events(X-events). A group of 32 experts, consisting of government emergency planning officers, was selected as the focus group to achieve this objective. Using the Focus Group Interview (FGI) technique, opinions were gathered from this focus group regarding potential X-events that may occur within the advanced science and technology domains over the next 10 years. The analysis of these opinions revealed that government emergency planning officers regarded the "Obsolescence of current technology and systems," particularly in the context of cyber network paralysis as the most plausible X-event within science and technology. They also put forth challenging and intricate opinions, including the emergence of new weapon systems and ethical concerns associated with artificial intelligence (AI). Given that X-events are more likely to emerge in unanticipated areas rather than those that are widely predicted, the results obtained from this study carry significant importance. However, it's important to note that this study is grounded in a limited group of experts, highlighting the necessity for subsequent research involving a more extensive group of experts. This research seeks to stimulate studies on extreme events at a national level and contribute to the preparation for future X-event predictions and strategies for addressing them.

A Study on the Major Standards and Actual Conditions of Evacuation Safety in Child Care Facility (보육시설의 피난안전 관련 주요 기준 분석 및 실태조사에 관한 연구)

  • Recently, the number of child care facility and social demand for high quality child care service increase suddenly. Especially, social atmosphere about fire safety is more sensitive in Korea. The majority of the residents in a child care facility are composed of early childhood having difficulty to evacuate, and therefore the number of stories of a building and the appropriateness of a evacuation equipment play an important role in the evacuation safety. In this study, problems and improvements for the evacuation safety are dealt with the present conditions of child care facilities, the comparative analysis between domestic and foreign standards, the research on the actual condition of evacuation equipments, and so on. The study brings to a conclusion that it is necessary to systematize the regulations regarding fire safety, to connect fire-fighting facilities for design standards and operations of emergency preparation equipments, to improve the regulations for fire protection system such as the sprinkler system, and to restrict the facilities having fire hazards.

  • The leakage of toxic chemicals impact seriously on human being and environment, therefore during their treatment process, a proper management system is necessary to control their toxic effect. This study was designed to suggest the management regulation that supports business managers and facilities management. There are no extra regulation to control emergency accidents and terrors in chemical facilities. Developed countries like USA operate the management standards to control the toxic chemical and facilities according to their toxicity and processes. In order to solve this problem, we have analysed the advanced nations standard methods of security in chemical plants to study the new security management regulation which helps to prevent the chemical accidents. Especially, in USA, CCPS (Center for Chemical Process Safety), SVA (Security Vulnerability Assessment) and RBPSs (Risk-Based Performance Standards) of DHS (Department of Homeland Security) were invest I gated. On the basis of the results, we have suggested the application methods of the security and safety regulation in Korea.

  • This study is to develop a mobile type environmental radiation measurement system for emergency response or environmental radiation monitoring of local governments near nuclear facilities. A mobile radiation measurement system can monitor radiation by field beyond the spatial constraints of a fixed environmental radiation monitor. If installed in local government infrastructure such as public transportation, environmental radiation can be monitored without additional manpower and measurement work. In addition, it is designed to enable monitoring and measurement of radiation from low to high doses as well as the environment in preparation for radioactive disasters such as nuclear power plant accidents. It is expected that this system will be utilized not only in normal times but also in the event of a radiation accident to improve the disaster prevention capabilities of local governments.

Status and Issues on Disaster Preparation Programs in Public Health Center (보건소의 재난관련 대비-대응 사업의 실태와 과제)

  • Objectives: The purpose of this study was to review the disaster preparation and response programs and the status of disaster preparation in public health center. Methods: In depth interview was performed in September 2017 using 5 open questions to the persons who are in charge of disaster response services in 5 public health centers of different levels in Korea. The questions included general characteristics of public health center, disaster programs and future issues. The research hired a quality method. Results: In general, the persons in charge recognized the cooperative agency of local government in disaster management. There were no disaster preparation programs developed by the public health centers. Most of the preparation were passive activities such as emergency support, crisis management on communicable disease and quarantine, participation in biological disaster response training, and education etc. The persons in charge emphasized necessity of disaster preparation programs. Conclusions: Disaster preparation and responsiveness is an evolving issue in public health centers in Korea. Medical support system and communicable disease management system are being set up in the national level. A comprehensive system covering health management, nutritional support, mental health, environment management of shelter, and volunteers supports on public health center level needs to be developed along with a easy-to-follow manual.

The Effect of Working Time Reduction in Hospital Management (근로시간 단축에 따른 병원경영 영향 분석)

  • The study was designed to investigate what effect of working time reduction which will be gradually expanded to corporate size from June, 2004 could give to medical industries, and to provide basic information for hospitals and government to prepare the policy. 276 hospitals were surveyed about medical service income and manpower realities during the first half of 2003, using a structured survey tool. Response rate was 8% and 20 hospitals were finally analyzed. The effect of working time reduction in hospital management was different to the size of hospitals and the alternative. Income to existing service income was decreased by $2.2{\sim}4.6%$ in tertiary hospitals, by $3.2{\sim}5.7%$ in general hospitals with more than 300 beds, and by $3.7{\sim}6.0%$ in general hospitals with less than 300 beds. In preparation against such decrease in income, government is required to raise insurance payment, to calculate added service charge for day-off on Saturday forenoon, to retain emergency care payment, to expand emergency care facilities, to secure duty doctors, and to support middle and small sized hospitals. Hospitals are required to give self improving efforts such as fortifying of weekday care, development of weekend care program, strengthening of care capacity and function of emergency care center, and making manpower operation efficient.
