Factors Affecting Radiation Protection Behaviors among Emergency Room Nurses (응급실 간호사의 방사선 방어행위 영향요인)

  • Purpose : This study aimed to examine factors affecting radiation protective behaviors among emergency room nurses by assessing knowledge, attitude, and environment for radiation protection. Methods : The study employed a cross-sectional design. Subjects were a convenience sample of 129 nurses working in emergency rooms of three general hospitals. Data were collected using self-report questionnaires and analyzed using t-test, ANOVA, Pearson correlation coefficients, and multiple regression. Results : The more the nurses received radiation safety education (t=2.26, p=.026), used protective gear (t=4.40, p<.001), and took health screenings (t=2.65, p=.009), the higher their levels of radiation protection behavior. There were significant relations between radiation protective behaviors and attitude (r=.27, p=.002), and radiation protective behaviors and environment for radiation protection (r=.55, p<.001). The factors affecting radiation protective behaviors were protective environment (β=.53, p<.001), protective attitude (β=.32, p<.001), and the use of protective gear (β=.24, p=.002). Conclusions : The government, hospital administrators, and radiation protection-related organization should adopt the following measures to protect emergency room nurses from radiation: research and development of shield instrument, medical examination for emergency room nurses, protocol development of radiation protection behaviors, extension of education chances of radiation protection, and encouraging the use of protective equipment.

Influence of Job Stress and State of Sleep on the Depression of Emergency Medical Technicians (응급구조사의 수면상태와 직무 스트레스가 우울에 미치는 영향)

  • Purpose: The purpose of this study was to identify the relationship of job stress, state of sleep, and depression and to identify factors influencing depression of emergency medical technicians. Methods: The research was a cross-sectional, descriptive study. Data collection were done using self-report questionnaires with 162 emergency medical technicians working in hospitals who agreed to participate in the study. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficients and multiple regression with the SPSS/WIN 18.0 Program. Results: Depression had significant negative correlations with state of sleep, (r=-482, p<.001) and positive correlations with job stress (r=.196,p=.013). State of sleep had significant negative correlations with job stress (r=-.168, p=.033). The variance for life satisfaction was 23.7% (F=25.964, p<.001). Variable of state of sleep significantly predicted depression of emergency medical technicians. Conclusion: Considering these results, for emergency medical technicians' job stress, state of sleep, and depression, further studies are needed. Furthermore, there is a need to develop and implement new strategies to improve depression among emergency medical technicians.

Emergency Nurses' Perceived Competency and Frequency of Educational Intervention (응급실 간호사의 교육중재 수행능력과 수행빈도)

  • Purpose: This study was done to investigate emergency nurses' perceived competency and frequency of 17 educational interventions. Methods: A mail survey was administered to a convenience sample of 744 nurses in 143 emergency departments across the nation. Results: The mean score for overall competency was 2.90 out of 4. The competency score for staff education (2.40 out of 4) was lower than that of patient/family education (3.40 out of 4). The mean score for overall frequency was 3.34 out of 5. The frequency score for staff education (2.27 out of 5) was lower than that of patient/family education (4.39 out of 5). Emergency nurses' perceived competency was significantly correlated with frequency of educational interventions. Overall competency score was different according to the nurses' age, education, position, hospital experience, emergency experience and the type of emergency department. Conclusions: Although the overall competency is high, the competency on some areas of educational intervention remains insufficient. Educational programs to reinforce emergency nurses' educational competency and evidenced based protocols on education should be developed.

Correlation Among Organizational Culture, Fatigue for Infection Control, and Infection Control Compliance of COVID-19 Among Emergency Nurses (응급실 간호사의 감염관리 조직문화, 감염관리 피로도와 코로나19 감염관리 수행도와의 상관관계)

  • Purpose: The purpose of this study was to investigate the levels and the relationship of emergency nurses' organizational culture and fatigue for infection control, and infection control compliance of COVID-19. Methods: Data was collected from 114 nurses caring for patients suspected of COVID-19 at two emergency medical centers in Gyeonggi-do and Seoul June 18-July 23, 2021. Results: Emergency nurses caring for patients suspected of COVID-19 suffered from high fatigue for infection control. The mean scores of the organizational culture and fatigue for infection control, and infection control compliance of COVID-19 were 51.80 ± 8.37 of 70 points, 78.46± 12.28 of 100 points and 71.02± 7.84 of 80 points, respectively. The higher infection control compliance of COVID-19 is significantly related to the higher organizational culture for infection control (r= .42, p< .001). Conclusion: In the COVID-19 pandemic, the formation of a positive organizational culture for infection control may be a priority as a major strategy to improve the infection control compliance of emergency nurses. It is also necessary to manage the high level of fatigue for infection control among emergency nurses.

Effects of Simulation-Based Education for Emergency Patient Nursing Care in Korea: A MetaAnalysis (응급환자 간호를 위한 시뮬레이션 교육효과: 메타분석)

  • Purpose: The purpose of this review was to evaluate the effects of emergency nursing simulation program on nursing students and nurses. Methods: This systematic review was performed as per the guidelines of the Preferred Reporting Items for Systematic Reviews and Meta-Analyses and using the R program meta package (version 4.9-2). RISS, KISS, and DBpia Library databases were searched for studies published between June 2000 and August 2018 using the following key words: ($Emerge^*$ OR nursing) AND ($nurs^*$ OR simulation). Selected studies were assessed for methodological quality using Risk of Bias for Non randomized Studies. Results: 7 studies were identified and all of them met the inclusion criteria. The outcome variables were significant clinical performance, self-efficacy except knowledge, and problem-solving ability. Conclusion: This review provides updated evidence of the simulation-based education program in emergency nursing. Further studies are required to increase generalizability using randomized population, research design and controlled trials with sufficient sample size. Moreover, valid measurements are needed to assess the main outcomes.

Emergency Department Nurses' Recognition of and Educational Needs for Forensics Nursing Education (응급실 간호사의 법의간호 교육에 대한 인식과 요구도)

  • Purpose: The purpose of this survey was to investigate emergency department nurses' recognition of forensics nursing and their educational needs for forensics nursing education. Methods: Data were collected via questionnaires from 167 emergency department nurses who were working in 7 university hospitals. Nurses' experiences of trauma or accidents and their recognition of and educational needs for forensic nursing education were assessed. Results: All subjects reported having suffered bodily injury due to falling or serious traffic accidents; 92.2% reported having suffered bodily injury caused by suicide attempts; 91.6% reported having experienced physical violence or abuse; 76.0% reported having experienced sexual assault or abuse; 68.9% reported having experienced some difficulties during their nursing care due to lack of forensics knowledge; and 88.6% reported never having been trained in forensics nursing. The educational needs score for "forensics nursing" was 3.61; the needs score for "abuse- and violence-related education" was 3.65; the needs score for "incident data collection related education" was 3.47; and the needs score for "forensics theory related education" was 3.34. Conclusion: The findings of the study underscore a strong need to develop an educational program on forensics nursing for emergency department nurses.

Estimation of nursing cost for selected special nursing services;operative nursing, emergency nursing, and ambulatory nursing (임상특수분야 간호원가 산정;응급실, 수술실, 외래를 중심으로)

  • Purpose: A cost analysis for nursing services in operative nursing unit, emergency nursing unit, and ambulatory nursing unit was performed using patient classification system by nursing intensity in order to determine an appropriate nursing fee schedule. Method: The data were collected from 4 secondary hospitals and 5 tertiary hospitals from November 14th 2000 to January 15th 2001. The study was conducted through four phases as follows: 1) Nursing hours of each nursing service in special nursing units were measured using three kinds of patient classification systems by nursing intensity. 2) The nursing cost of nursing services in operative nursing unit, emergency nursing unit, and ambulatory nursing units was estimated based on patient classification system by nursing intensity. Results: As a result, nursing hours by nursing intensity of each special nursing unit were measured, and every nursing cost by nursing intensity in operation room and emergency room was estimated, meanwhile, the cost of nursing services in ambulatory care units was estimated only per visit as shown in chapter 4. Conclusion: Future research on nursing cost should be extended to other special nursing units such as various intensive nursing care units, delivery room, and so on. In addition, the patient classification system should be refined for its appropriateness to apply all levels of medical institutions.

Educational Needs Based on Analysis of Importance, Frequency and Difficulty of ICU Nursing Practice for ICU Nurses (중환자실 간호실무의 중요도, 수행 빈도 및 난이도 분석을 통한 중환자실 간호사의 교육요구도)

  • Purpose: The purpose of this study was to assess the educational needs of ICU nurses based on an analysis of importance, frequency, and difficulty for ICU nursing practice. Method: A cross-sectional survey was conducted using a questionnaire with 80 questions in 14 ICU nursing categories. Data were collected from August to October 2009. A total of 295 ICU nurses from five hospitals who had minimum of one year clinical experience participated. Data were analyzed with using descriptive statistics. Results: For importance, emergency care had the highest score, followed by physical assessment, communication, cardiovascular care, and ICU basic nursing. Regarding the frequency, physical assessment had the highest score, followed by communication, medication, ICU basic nursing, and respiratory care. Cardiovascular care was the most difficult task, followed by neurological care, emergency care, other ICU related nursing care, diagnostic test, and communication. Conclusion: The findings indicate a high educational need in the areas of communication, medication, physical assessment, diagnostic test, emergency care, and cardiovascular care. Thus the development of educational programs on communication, medication, physical assessment, diagnostic test, emergency care, and cardiovascular care are needed for ICU nurses.

Effects of Health Belief on Fall Prevention Activities of Emergency Room Nurses (응급실 간호사의 건강신념이 낙상예방활동에 미치는 영향)

  • Purpose: The purpose of this study was to analyze factors affecting fall prevention activities of emergency room (ER) nurses based on their health belief factors (perceived susceptibility, perceived benefits, perceived severity, perceived barriers, and cues to action). Methods: The study design was a descriptive survey using questionnaires which were given to 127 emergency room nurses from two regional emergency medical centers, four local emergency medical centers, and two local emergency medical facilities. Data were analyzed using descriptive analysis, t-test, one-way ANOVA with LSD test, Pearson correlation, and multiple regressions. Results: ER nurses' fall prevention activities had a mean of $3.78{\pm}0.50$. Eight individual characteristics and health belief factors accounted for 30.8% of the fall prevention activities. Fall prevention activities were found to be positively affected by emergency medical facilities, perceived benefits, and cues to action and negatively affected by factors, such as ER career and perceived severity. Conclusion: The results indicate that it is necessary to formulate a plan for enhancing perceived benefits and cues to action to improve fall prevention activities. In addition, fall prevention activities should be encouraged for ER nurses who have worked in local medical institutions for less than 1 year or more than 5 years.

Projected Workforce of Certified Emergency Nurses in Korea (응급전문간호사 인력 수요추계에 관한 조사연구)

  • Purpose: The purpose of this study was to suggest the projected workforce of certified emergency nurses in Korea. Method: The methods used in this study are 1) demand & ratio model was used for the projected workforce of CEN, 2) Index functional formula was used for the suggestion of the number of general hospitals and hospitals, 3) Experts in Emergency care were contacted to get an opinion and information about the criteria of distribution and scope of CEN, 4) National and international internet data were collected. Result: The demand of CEN were analyzed by two ways; demand of Emergency centers only and demand of emergency centers including community centers. The number of CENs needed to meet the demands in 2002, 2005 and 2010 was estimated at minimum of 1,512 to maximum1,576, 1,640 to 1,704 and 1,892 to 1,956 respectively. The projected number of CENs for the 2002 was 1% total available nurses in Korea, and 3.2 CENs per 100,000 population. Conclusion: It is really desirable that CENs not only work for hospital emergency centers but also for emergency related centers in community.
