A Study on the Scope of Practice of Nurses in the Prehospital Emergency Medical Service System (병원 전 응급의료체계에서 간호사의 업무범위에 대한 소고(小考))

  • Nurses are medical personnels under the Medical Service Act and perform medical practice such as medical assistance at medical institutions. The nurse, a medical personnel, provides emergency medical service to emergency patients in the pre-hospital emergency medical system as a 119 rescuer based on the Act on 119 Rescue and Emergency Medical Services. The scope of practice of nurses is comprehensively defined in the Medical Service Act and specified through precedents. In contrast, The scope of work of emergency medical technician is listed in detail. It is understood that nurses in the pre-hospital emergency medical service system have a wider scope of practice than emergency medical technician. In particular, the scope of practice of nurses as emergency medical personnel in the pre-hospital emergency medical system should be interpreted differently within the medical institution, considering the urgency of the patient, being transferred to the emergency medical institution, and the specificity of medical direction through tele-communication.

Images toward emergency medical technician perceived by secondary school teachers (중등교사의 응급구조사에 대한 이미지)

  • Purpose : The purpose of the study is investigate the images towards emergency medical technician(EMT) perceived by secondary school teachers. Methods : The data was collected from July 30 to 31, 2012. The subjects were 337 secondary school teachers. SPSS Statistics 18.0 was used for internal consistency reliability, descriptive statistical analysis and factor analysis. Results : Those who contacted with EMT consisted of 57.6% which was higher than those who had not(42.4%). Before contact, the gender and teaching subject showed a significant difference of p =.027 and p =.002, respectively. The average on the image towards EMT showed a positive points of $5.68({\pm}0.73)$ out of 7 and the most positive image was 'practical-theoretical' of $6.33({\pm}1.12)$. Factor analysis of EMT image was analyzed based on 3 factors including sincerity($5.89{\pm}0.89$), professionalism($6.02{\pm}0.89$) and familiarity($4.51{\pm}1.42$)(p <.000). The degree of familiarity showed significant differences in gender(p =.005), majoring(teaching subjects)(p =.049) and prior contact(p =.020). Conclusion : In conclusion, previous contact and recognition towards EMT could make good image making in secondary school teachers by getting well-organized promotions and professional services. This contact will make a good future counseling to the students who want to be EMTs.

Improvement Strategies of Arriving Time to the Scene by Enhancing EMTs' Recognition of Triage (구급출동지령서 개선을 통한 구급대원 현장 도착시간 단축방안)

  • The purpose of this investigation is to enhance the survival rate of patients by transporting them to the hospital within the golden hour through the operational improvement of emergency dispatch instruction. To this end, problems and improvements of current operating system were derived by carrying out a survey against paramedics of Incheon city in 2012 and analyzing the current emergency dispatch instruction. This study analyzed the emergency activity daily reports for one year from January 1 through December 31, 2012 and researched the consciousness of 119 emergency medical technician. According to the analysis of the survey, there were no meaningful differences in the on-site arrival times per triage. Therefore, the item of 'Emergency Classification' specified in the emergency dispatch instruction needs to be integrated in the scheme of "triage". Also, the feedbacks of the emergency action log and the emergency dispatch instruction are necessary for 'duty for operation' to review the adequacy to the severity after the end of emergency operation. Finally, the improvement of the system for the continuous communication between the paramedics and the command staff is necessary. This improvements as stated above are expected to contribute to raise survival rate of patients.

Work Stress and Satisfaction of Emergency Medical Technicians Working in National Emergency Management Agency and Hospital (소방방재청과 병원 근무 응급구조사의 업무 스트레스와 직무 만족도)

  • Purpose: To identify work stress and job satisfaction of the emergency medical technician (EMT) and to contribute to the improvement of emergency care under the current emergency medical law. Methods: The data were collected by questionnaire from the total 849 EMTs (635 National emergency management agency (119) workers and 214 hospital workers) throughout the whole country from January 5, to March 5, 2012. Each level of indicator calculated under 100-point-scale was compared by t-test and ANOVA. Factors affecting stress and satisfaction were analyzed by Multiple Linear Regression Analysis (${\alpha}$=.05). Results: Mean level of job stress was markedly higher among 119 workers (76.9) than those in the hospital (56.6) (p<.001). Mean level of job satisfaction was higher among 119 workers (63.2) than those in the hospital (60.2) (p<.001). Conclusion: To diminish work stress and to increase job satisfaction, efforts to support the pride and to improve working condition considering the situations of the agency and to offer more education chance to keep and strengthen the specialty are needed.

The Actual State of Hospital-based Skill Training in Level 1 Emergency Medical Technician (소방 1급응급구조사의 병원임상수련 술기 실태 조사)

  • This study is the analysis of the actual state of hospital-based training in level 1 emergency medical technicians (EMTs) in fire station. The survey was carried out to 42 fire fighters having level 1 emergency medical technician certification from May 11 to 12 in 2012. The results showed 56.5 %, that the level 1 EMTs of roles had direct experienced. Level 1 EMTs performance frequency of clinical practice that endotracheal intubation 66.9 %, laryngeal mask airway 8.4 %, intravenous access 76.6 %, nitroglycerin sublingual 61.2 %, bronchodilator inhalation 50.0 %, fluid loading 73.8 %, ventilator apply 57.6 %, dextrose injection 57.6 %. It is necessary to investigate the continuous study on the education among the level 1 EMTs and the continuous evaluation and analysis of problems, supplemented of guidebook, practical education programs and regulation upon the operation and management of a hospital-based skill training.

Analysis of direct medical control conducted to 119 emergency medical technicians in an emergency medical information center (119구급대원에게 시행한 일개 응급의료정보센터의 직접의료지도에 관한 분석)

  • Purpose : The goal of the present study is to provide the basic information to medical control which is the most important improving factor of pre-hospital medical treatment. Method : A total of 749 records of direct medical control were collected from 119 EMTs in emergency medical information center of Daejeon, Chungcheongnam-do and Chungcheongbuk-do from March 1, 2010 to February 28, 2011. Results : The 119 EMTs should record the level of qualification of EMT and general patient history taking precisely when they receive direct medical controls. The doctors should take medical controls within the task range of qualification of EMTs. Conclusion : It is necessary to establish the guideline of medical direction and protocol of prehospital emergency care. The quality improvement of pre-hospital emergency services will be possible by the guideline and protocol.

Knowledge, attitude and confidence on patient safety management among emergency medical technician students (응급구조과 학생의 환자안전에 대한 지식 및 태도가 환자안전관리 자신감에 미치는 영향)

  • Purpose: This study evaluated the effects of knowledge and attitude on confidence about patient safety management among emergency medical technician (EMT) students. Methods: A self-reported questionnaire was completed by 174 EMT students in two cities from May to June 2016. Data were analyzed using PASW WIN 20.0 for descriptive statistics, t-test, one way analysis of variance, and hierarchical multiple regression. Results: The students' confidence about patient safety management was 3.33 (on a 5-point scale). Confidence varied according to average academic grade (p=.015), experience in patient safety education, and experience in writing medical error reports. After adjusting for sex, average academic grade, and experience in patient safety education, knowledge and attitudes about patient safety increased a student's confidence in patient safety management by 19%. Conclusion: Patient safety education should be included in programs on health promotion and be provided as basic information in intervention programs for EMTs.

A Study on the Need for Emergency Care Education in Nursing Homes (노인요양시설에서 노인수발 담당자의 응급처치에 대한 교육수요도 조사 - 일개 도시를 중심으로 -)

  • Purpose: This study was conducted to evaluate the need for education for emergency care in nursing homes. Method: 312 people who were engaged in caring for the elderly at nursing homes in D-metropolitan c! ity answered the questionnaires about the experiences, the need, and the ideal methods of education for basic emergency care, from Jul. 1. 2007 to Jul. 30. 2007. Result: 69.9% were female. Only 7.1% were nurses and the rest did not have majors in the medical field. 83.0% had experienced education for emergency care, and 89.2%, 78.4% and 44.8% of them had been educated about cardiopulmonary resuscitation, airway obstruction care, and fracture/sprain/dislocation care respectively. Fifty-three who had not experienced education answered 'no opportunity' as a cause of non-education. The need for emergency care education was 4.53/5.00, and the desire to receive education was 2.81/3.00. Mainly workers wanted to be educated abo! ut airway obstruction (88.1%), and breathing assistance & cardiac massage (72.4%). Workers preferred to be educated 1-2 times per year for 2-3 subjects, through programs including both lecture and practice. Conclusion: It is essential to create an adequate program and apply it to the people engaged in caring for the elderly.

Images of Hospital Emergency Medical Technicians Perceived by Emergency Medical Service Providers (Focusing on tertiary medical institutions in Daejeon and Chungcheongnam do) (응급의료종사자가 인식하는 병원 내 응급구조사의 이미지 (대전, 충남 3차의료기관 중심으로))

  • This study surveyed the image of hospital emergency medical technicians with 122 emergency medical service providers (doctors and nurses) working at tertiary medical institutions in Daejeon and Chungcheongnam.do in order to provide basic materials for enhancing the status and professional image of hospital emergency medical technicians. According to the results of this study, the overall image was $3.27{\pm}0.34$, and by its sub.area, professional image was highest and role image was lowest. By item, 'Maintain a friendly relations with doctors' was highest, and 'Arrogant and negligent in job performance' was lowest. In order to enhance their image, hospital emergency medical technicians need to assume a sincere attitude toward patients and colleagues consistently, to strengthen their professionalism, and to carry out their duties and roles faithfully. What is more, the association should take proper measures and to establish the sound image of emergency medical technician as a professional through active campaigns and monitoring using mass media.

Proposed oath and ethics code for emergency medical technicians (응급구조사 선서 및 윤리강령의 제안)

  • Purpose: The purpose of our study was to introduce an oath and ethics code for emergency medical technicians (EMTs). Methods: The proposed oath and ethics code for the EMTs was evaluated using a modified Delphi technique. This oath and ethics code was presented at Korean association of emergency medical technician conference and was revised by experts in emergency medical services. Results: We examined the ethics codes for other allied healthcare professionals regarding the topics of human rights, health promotion, acting as an advocate, ethics, cooperation, observance, human rights, right to know, self-determination, confidentiality, and professionalism. These elements are reflected in our proposed oath and ethics code for EMTs. Conclusion: The proposed oath and ethics code would raise the professional status of EMTs.
