Effects of garlic intake on cancer: a systematic review of randomized clinical trials and cohort studies

  • BACKGROUND/OBJECTIVES: Due to the rapid increase of global cancer incidence and mortality and a high level of interest in cancer prevention, a systematic review of garlic intake and cancer risk is needed. SUBJECTS/METHODS: We implemented a systematic review to examine the effects of varying levels of garlic intake on cancer. We conducted comprehensive literature searches in three electronic databases (MEDLINE, Embase, and Web of Science) for studies published between database inception and July or September of 2018. Two investigators independently screened abstracts and full-texts, extracted data, and assessed risk of bias (RoB). A total of one medium-quality randomized controlled trial (RCT) and 13 cohort studies graded as high RoB were included. RESULTS: The 1-year follow-up results from a RCT showed that a significant decrease in the number and size of colorectal adenomas among participants with colorectal adenomas who received high-dose aged garlic extract (AGE) compared with those who received low-dose AGE (P < 0.05). The results of prospective observational studies provided inconsistent associations of colorectal cancer risk with garlic supplements and garlic intake as food. CONCLUSIONS: In summary, the AGE was effective in reducing the number and magnitude of colorectal adenomas in one RCT, but there were inconsistent associations between garlic intake and colorectal cancer in cohort studies. Therefore, we could not draw a firm conclusion regarding the effects of garlic on cancer, because the current strength of evidence is inadequate due to a lack of number of high-quality RCTs.

  • In the era of big data, not only structured data well organized in databases, but also the Internet, social network services, it is very important to effectively analyze unstructured big data such as web documents, e-mails, and social data generated in real time in mobile environment. Big data analysis is the process of creating new value by discovering meaningful new correlations, patterns, and trends in big data stored in data storage. We intend to summarize and visualize the analysis results through frequency analysis of unstructured article data using R language, a big data analysis tool. The data used in this study was analyzed for total 104 papers in the Mon-May 2021 among the journals of the Korea Institute of Information and Communication Engineering. In the final analysis results, the most frequently mentioned keyword was "Data", which ranked first 1,538 times. Therefore, based on the results of the analysis, the limitations of the study and theoretical implications are suggested.

Review Study on the Measurement Tools of Scoliosis: Mainly on Non-radiological Methods (척추측만증 평가 척도에 관한 문헌 고찰: 비방사선 방법을 중심으로)

  • Objectives: The purpose of this study is to investigate the characteristics, validity, and reliability of non-radiological assessment tools of scoliosis that have been studied so far. Methods: Electronic databases including Pubmed, Cochrane Library, CNKI, Science On, RISS, OASIS were searched by keywords including 'scoliosis assessment', 'scoliosis screening', 'physical examination', 'functional measurement', 'photography', and 'smartphone'. Results: 32 articles using radiation-free assessments were identified from 1,011 records. The mostly used non-radiological methods were Surface topography, Scoliometer, Ultrasound, Digital Infrared Thermal Imaging, and Photography. The other methods were Gait analysis, 3D depth sensor imaging, and Low intensity electromagnetic scan. Conclusions: It was found that non-radiological assessment tools might reduce the number of radiographs taken in scoliosis patients. To increase the reliability and validity, further research on the measurement tools of scoliosis will be needed.

A Review of Studies Using Syndrome Differentiation Questionnaire in Cancer Patients (암 환자 대상 변증 설문지 활용 현황에 대한 문헌고찰)

  • Objective: The aim of this review is to analyze the studies using syndrome differentiation questionnaire in cancer patients. Methods: We searched electronic databases including Pubmed, google scholar, Cochrane library, CNKI, KISS, RISS and OASIS. Key words used for searching were cancer, Korean medicine, pattern identification, and questionnaire. Studies using a symptom differentiation questionnaire to cancer patients were selected. Results: 35 studies were enrolled. A total of 17 questionnaires was used. Most of the types of included studies were observational studies, followed by randomized controlled trials (RCTs) and validation studies. The purposes of using questionnaires were rrelation analysis, outcome measurement, evaluating adverse events, subgroup analysis, and questionnaire development. The most used questionnaire was Body Constitution Questionnaire (BCQ), and it was used 8 times, Questionnaire for the Sasang Constitution Classification II (QSCC II) was used 5 times, Constitution in Chinese Medicine Questionnaire (CCMQ), TCM-Symptom Complex Differentiation Questionnaire (TCM-SCDQ), Yin Deficiency Questionnaire were used 4 times, and Qi Blood Yin Yang Deficiency Questionnaire was used twice. BCQ is a questionnaire diagnosing and evaluating yang deficiency, yin deficiency, and blood stasis. It has high reliability, validity, and optimal cut-off value. Conclusion: BCQ is the most used syndrome differentiation questionnaire in cancer-related studies. So, BCQ could be recommended in syndrome differentiation-related cancer studies.

An Integrative Review on the Contents and Effectiveness of Depression and Anxiety Interventions applied to Unmarried Mothers Living in Residential Facilities (시설에 거주하는 미혼모에게 적용된 우울 및 불안 감소를 위한 중재의 통합적 문헌고찰)

  • Purpose: The purposes of this study was to review the literature on intervention for treating anxiety and depression among unmarried mothers living in facilities, and to understand core that could promote the development of more effective interventions. Methods: Key words in English and Korean were used to search through eight electronic databases-PubMed, Cochrane Library, EMBASE, CINAHL, RISS, DBpia, NDSL, and the National Assembly Library. Results: Ten studies were ultimately selected for the integrative review and were evaluated in terms of contextual and methodological quality. The studies consisted of seven quasi-experimental studies and three case report studies. The selected studies utilized music, art, forest therapy, dancing, education, and play programs to change mothers' perceptions, emotions, and behavior and to improve their relationships with their babies or others. Conclusion: It is important to consider mothers' self-awareness and emotional expression, and to improve their relationships with their babies or others as core elements when developing intervention programs for anxiety and/or depression among unmarried mothers living in residential facilities.

Chuna Manual Therapy for Pediatric Allergic Rhinitis : A Systematic Review and meta-analysis (소아 알레르기 비염에 대한 추나요법의 효과 : 체계적 문헌 고찰과 메타분석)

  • Objectives : This review determines the evidence of effectiveness and safety of Chuna manual therapy (CMT) for pediatric allergic rhinitis. Methods : We searched 12 electronic databases (Pubmed, EMBASE, Cochrane Library, Web of Science, CAJ, J-STAGE, RISS, DBpia, NDSL, KISTI, KISS, and KMBASE) and two related journals up to the end of April, 2019. We only included randomized controlled trials (RCTs) investigating CMT for the treatment of pediatric allergic rhinitis. The methodological quality of the included RCTs was evaluated using the Cochrane risk of bias tool 1.0. Results : Twelve RCTs were eligible in our inclusion criteria. A meta-analysis of five studies demonstrated positive results for the using CMT for the treatment of pediatric allergic rhinitis. When used in conjunction with traditional Chinese medicine, CMT treatment significantly improved total efficacy rate compared with traditional Chinese medicine alone (P<0.02, n=5). Conclusions : Based on a published meta-analysis, there is reliable evidence for the use of CMT in treating pediatric allergic rhinitis. However, it should be noted that the studies included in this systematic review were heterogeneous and were of low quality, warranting further investigation using well-designed RCTs.

Effect of Chuna Manual Therapy for Peripheral Facial Paralysis in Children : A Systematic Review (소아 말초성 안면마비에 대한 추나요법의 효과 : 체계적 문헌고찰)

  • Objectives : The purpose of this study is to review clinical studies about the effect of chuna manual therapy (CMT) for peripheral facial paralysis in children. Methods : In this review, we searched 11 electronic databases (Pubmed, Cochrane Library, EMBASE, CINAHL, CAJ, Oasis, NDSL, KISS, RISS, KISTI, Dbpia); We concluded our literature search in April 23, 2019. We included only randomized controlled trials (RCTs) of testing CMT for peripheral facial paralysis in children. The methodological quality of each RCT was assessed using the Cochrane risk of bias tool. The meta-analysis was performed by synthesizing outcome data of total efficacy rate (TER). Results : After screening papers, a total of 6 RCTs were selected and analyzed. In the 6 RCTs, patients(n=15-60 per study) were randomized into groups for treatment and control. Specifically, the treatment group received CMT, while the control group was concurrently given usual care, such as acupuncture and medicine. The meta-analysis showed that the treatment group receiving CMT alone showed significant improvement in TER, compared to the control group receiving acupuncture therapy alone(P<0.05). And the treatment group receiving CMT combined with usual care showed positive results, in terms of TER, compared to the control group receiving usual care, but was not statistically significant(P>0.05). Conclusions : Our analysis suggests that CMT has therapeutic effects for peripheral facial paralysis in children. However, to confirm this result, further investigation accompanied by high quality studies is required.

  • The purpose of this study was to analyze the current status and effects of health-related intervention for children and adolescents in multicultural families and to provide a evidence of nursing educational intervention. The Scoping strategy was applied, the 12 published literature were analyzed according to the selection criterion and exclusion criterion among 604 published literature retrieved through 8 electronic databases. The results of this present study showed that most research was conducted on elementary school students, and types of intervention programs were art therapy, music activities, psychotherapy, and physical activities. This study suggests the need for intervention to prevent and promote health problems among children and adolescents in multicultural families, and the necessity of preparing intervention strategies including physical, psychological, and developmental areas.

Efficacy of Computerized Cognitive Behavioral Therapy in Individuals with Non-Cardiac Chest Discomfort : Review and Suggestions for a New Protocol (비심인성 흉부 불편감을 가진 대상자에서 컴퓨터 기반 인지행동치료의 효과성 : 문헌 검토와 새로운 프로토콜 제안)

  • Since non-cardiac chest discomfort (NCCD) can result in substantial healthcare burden and lower quality of life, interventions such as cognitive behavioral therapy (CBT) have been investigated for the relief of NCCD. In this review, we aimed to summarize the evidence on the efficacy of the CBT for the treatment of NCCD while introducing a newly-developed computerized CBT program for NCCD. Studies applying CBT to individuals with NCCD were searched for from both English and Korean electronic databases. Among 37 studies, 11 randomized controlled trials, 4 case-control studies, 1 case series, and 2 review articles were eligible for this review. Efficacy of conventional CBT for NCCD was shown in a series of studies as most of them reported improved symptom severity of NCCD or NCCD-related anxiety. However, a substantial variability existed among these studies in participants, treatment procedures and durations. High attrition rates were also reported in these studies on conventional CBT. Computerized CBT could be an alternative to the conventional CBT as it can be standardized and more easily accessible, but it was only reported in one previous study. In addition to the literature review, we presented a newly-developed computerized CBT program for NCCD which may overcome some of the limitations of conventional CBT. A computerized CBT could be an alternative treatment of NCCD, however, need further studies on its usefulness.

Recent Research Trends in Korean Medicine Treatment of Hyperthyroidism: A Review of Literature Focusing on Domestic Articles from 2000 to 2018 (갑상선 기능 항진증의 한의학적 치료에 대한 최신 연구 동향 : 2000년부터 2018년까지 국내 학술논문을 중심으로 한 문헌고찰)

  • Objective: This study was performed to research Korean medical treatment methods for hyperthyroidism by summarizing domestic clinical studies. Methods: The study was based on 14 papers published in Korea since 2000, on Korean medicine treatments for hyperthyroidism. Papers were searched on four domestic electronic databases. Results: 1. This paper analyzed 14 studies on the subject of clinical research on hyperthyroidism in Korea. 12 of them reported cases while two of them reported on clinical research. 2. The average age of the subjects was 39 years, 71.3% of women, 20-60 years of age accounted for 92%, and the average duration of disease was 22 months. 3. Of the 115 subjects, 102 had experienced Western medical treatment. So most of them chose Western medical treatment as the first treatment method, of which 24 experienced side effects. 4. Eumhuhwadong (陰虛火動) and Gieumyanghu (氣陰兩虛) appeared 3 times respectively. 5. The Korean herbal medicine, Ahnjeonbaekho-tang was the most frequent herbal medicine used to treat hyperthyroidism, with the next most frequent being Yanggyuksanhwa-tang. 6. The study, which was performed using herbal medicine alone, showed a marked improvement in clinical symptoms and thyroid function test, compared with administration of antithyroid drugs and herbal medicines. Conclusions: Large-scale studies and randomized controlled trials will be needed to evaluate the validity of Korean medical treatment for hyperthyroidism.
