Acupuncture for glaucoma: A systematic review and meta-analysis of randomized controlled trials (녹내장의 침치료 효과에 대한 체계적 문헌고찰 및 메타분석)

  • Objectives : This study aims to evaluate the effectiveness and safety of manual and electroacupuncture on glaucoma. Method : We searched 11 electronic databases using index words to identify randomized clinical trials. Meta-analysis of weighted mean difference (WMD) or standardized mean difference (SMD) were used to evaluate the outcomes. Cochrane bias risk assessment tool was used to assess the risk of bias in each clinical study. The collected data was analyzed using RevMan software (ver. 5.3). Results : At the initial stage of data retrieval, 549 papers were searched. After reviewing 37 full texts, a total of 10 RCT studies (426patients, 715 eyes) were selected and 8 RCT studies (357 people, 617 eyes) were involved in meta-analysis. Meta-analysis of 8 RCTs showed that acupuncture alone was more effective in reducing intraocular pressure(IOP) than conventional treatment (WMD = -5.73, 95% CI: [-12.30, 0.83], P = 0.09, I2 = 97%) The combination of acupuncture or electroacupuncture with conventional treatment was also effective in lowering IOP (WMD = -1.84, 95% CI: [-2.31, -1.37], P <0.00001, I2 = 45%). It was estimated that the combination of acupuncture with conventional treatment was also effective for improving visual field (VF) (WMD = -2.17, 95% CI: [-4.32, -0.02], P = 0.05, I2 = 89%) but improvement in visual acuity (VA) was not significant (MD = 0.06, 95% CI: [-0.03, 0.15], P = 0.23, I2 = 0%). Subgroup analyzes were performed only for the studies that used open glaucoma as the study's disease and combination of acupuncture or electroacupuncture with conventional therapy would have an effect on lowering intraocular pressure (WMD = -1.68)., 95% CI: [-2.46, -0.90], P <0.0001, I2 = 29%). Conclusion : This study suggests that acupuncture treatment for glaucoma may be effective in reducing intraocular pressure and helpful in improving visual field defects. However, due to the small sample size, high risk of bias and high heterogeneity in the methodology, it is expected that further studies will be needed to verify the results. Further studies in large-scale samples based on a minimized biased methodology would be necessary.

The Effect of Electroacupuncture on Upper-Extremity Spasticity of Stroke Patients. (뇌졸중 환자의 상지 경직에 대한 전침의 치료 효과)

  • Objectives : This study was executed in order to evaluate the effects of electroacupuncture on upper-extremity spasticity control in stroke patients. Methods : Eighteen patients with stroke were enrolled and classified into two groups, the study and control group. The control group did not receive any oriental medical treatment. In the study group, the electroacupuncture points were applied to Kokt'aek (PC3) and $Ch'{\u{o}}ch'{\u{o}}n$ (PC2), Naegwan (PC6) and $Ch'{\u{o}}kt'aek$(LU5) of the affected limb. H-reflexl M-response ratio (HIM ratio), modified Ashworth scale (MAS) and Fugl-Meyer motor function assessment (FMA) were used for evaluation of spasticity control before electroacupuncture, within two hours after electroacupuncture, and at two weeks. Results : In MAS, the study group declined more than the control group. but there was no statisticallysignificant consideration. In H/M ratio. the study group was more efficient than the control group. and spasticity decreased successively during the series in the study group. In FMA, motor function in the study group improved more than in the control group and motor function in the study group increased successively during the series. Conclusions : These results showed that electro acupuncture was a useful method to decrease upper-extremity spasticity in patients with stroke. Further studies are needed to explore more cases and the long-lasting carryover effects on upper-extremity spasticity in electroacupuncture.

Improvement of Tetraplegia and Respiratory Symptoms with Complex Korean Medicine Treatment After Traumatic Cervical Spinal Cord Injury: A Case Report

  • This study reports the effectiveness of traditional Korean medicine in treating tetraplegia and respiratory symptoms (including dyspnea) after traumatic cervical spinal cord injury surgery. The patient was treated with complex Korean medical treatment including electroacupuncture, pharmacopuncture, and herbal medicine. The manual muscle test (expanded Medical Research Council system) was used to measure the patient's muscle strength. The Korean version of the modified Barthel index and the functional independent measure were used to evaluate the patient's independence in performing daily activities. Following 2 hospitalizations and treatments, the patient's muscle strength improved > 4+ score for all joints, and the Korean version of the modified Barthel index and functional independent measure scores increased from 26 to 79 and 56 to 95, respectively. Symptoms of dyspnea, coughing, and sputum gradually improved and finally disappeared during hospitalization. This study suggests complex Korean medicine treatment may be effective in the rehabilitation of post-surgical cervical spinal cord injury patients.

  • The aim of this study was to examine the effects of electroacupuncture(EA) at the PC6 (Naegwan) on normal humans using KarhunenLoeve decomposition method. Electroencephalogram(EEG) is a multi-scaled signal consisting of several components of time series with different dominant frequency ranges and different origins. EEG KarhunenLoeve decomposition method exibit site-specific and state-related differences in specific frequency bands. In this study, KarhunenLoeve decomposition method was used as a measure(D2) of complexity. 30 channel EEG study was carried out in 10 subjects (10 males; $age=21.4{\pm}0.5$ years). Results : We found that the average values and standard deviations of D2 at FP1, FP2, FTC1, FTC2, TT1, TT2, T4, TCP1, P3, P4, T6, OZ channel (p<0.05) were higher than during the acupuncture treatment, and the average values and standard deviations of D2 at F3, F8 channels(p<0.05) were lowered than during the acupuncture treatment. However, the comparison with that before and after the treatment shows no significant differences in all channels.

  • Objectives : The aim of this study was to examine the effects of electroacupuncture(EA) at the PC6(Naegwan) and the TE5 (Oegwan) on nounal humans using power spectral analysis. Methods : EEG power spectrum exhibit site-specific and state-related differences in specific frequency bands. In this study, power spectrum was used as a measure of complexity. 30 channel EEG study was carried out in 30 subjects(30 males ; age=23.7 years). Results : In ${\alpha}$(alpha) band, the power values at F7 channels(p<0.05) during the PC6-acupoint treatment were significantly were decreased. In ${\beta}$(beta) band, the power values at Fp1, Fz, TT1, T5, P3, P4, Po1, P02, O1, Oz, O2 channels(p<0.05) during the non-acupoint treatment and at Fp1, F4, F8 channels(p<0.05) during the TE5-acupoint treatment significantly were increased. In ${\theta}$(theta) band, the power values at Fp1 channels(p<0.05) during the non-acupoint treatment and at Oz channels(p<0.05) the TE5-acupoint treatment significantly were increased. but, the power values at F7 channels(p<0.05) during the non-acupoint treatment were significantly were decreased. In ${\delta}$(delta) band, the power values at TCP1, TCP2, CP1, T5 channels(p<0.05) during PC6-acupoint treatment were increased and the power values at F7, TT2 channels(p<0.05) during non-acupoint treatment were increased. but, the power values at the TE5-acupoint treatment significantly was decreased than the before-acupuncture treatment.

  • Canine medial patellar luxation can cause hindlimb lameness in toy breeds and surgical operation needs when the lameness does not improve or worsen. In this study, the 2 dogs that showed postoperative non-weight bearing lameness were treated with electroacupuncture (EA) and the effects of postoperative EA were examined. Xue Hai (Sea of Blood), Du Bi (calf's nose, 2 acupoint), Zu San Li (leg 3 miles) acupoints were used in acupuncture. Xue Hai acupoint was connected to positive pole and Zu San Li was negative for 20 min with 2~5 volt, 5~30 Hz in EA. In case 1, EA were performed eight times for 26 days after 29th postoperative day and walking was improved from 6th EA. In case 2, EA were conducted five times for 17 day after 26th postoperative day and walking was improved from 4th EA. It may be possible to improve walking by EA treatment in the dogs that show post-operative non-weight bearing lameness, but the relationship between the effects of EA and the injury of stifle joint should be inspected.

  • The Foot drop is led by the conditional palsy of dorsal flexor according to some causes. In this case, it is caused by peroneal nerve palsy. It brings the difficulty in waking and standing up. We observed 2 cases of patient with Foot drop. Two patients were treated by the same method and treatment was performed by means of electroacupuncture(2-4Hz frequency, intensity was adjusted so that localized muscle contraction could be seen). As the result, symptoms are improved.

  • Background: A number of studies have identified the effect of Korean medicine treatment for oculomotor nerve palsy, but few have reported treatment of subarachnoid hemorrhage. This paper reports two cases of oculomotor nerve palsy with ptosis and limitation of eye movement after subarachnoid hemorrhage whose conditions were improved by Korean medicine treatment. Methods: Two patients with ptosis and limitation of eye movement after subarachnoid hemorrhage were treated with Korean medicine, including acupuncture, electroacupuncture, herbal medicine, cupping, moxibustion, and pharmacoacupuncture. Clinical symptoms were measured as the distance of the interpalpebral fissure and eyeball movement (adduction, abduction). Results: After treatment, their clinical symptoms showed improvement. Conclusion: Korean medicine treatment, including pharmacoacupuncture and electroacupuncture, could be adopted as a treatment method for oculomotor nerve palsy after subarachnoid hemorrhage.

  • Objective : To evaluate the effect of Oriental Medical Treatment on a patient with Hypoxic Ischemic Encephalopathy followed by Cardiopulmonary Resuscitation Method : We applied various methodology of Oriental Medical Treatment including Acupuncture, Electroacupuncture, Physical treatment, Herbal Medicine, Moxibustion treatment and Western medication as well. Result: Herbal medicine was applied on the basis of the patient's history. We applied formular to remove phelgm as a pathogenic factor after Hypoxic Ischemic Encephalopathy. At the same time, considering the patient spent more than a month in ICU lacking appropriate nutrition, we used formuli on the basis of 'Deficiency of Spleen' focusing to vitalize the function of digestive system. As the condition of the patient changed, we also adapted formular accordingly. We prescribed Herbal medication to strengthen Yin and Yang equally as she got hospitalized for long time. Also we applied Acupuncture treatment and Moxibustion treatment to control Qi flow. The general condition of the patient got better with successful removal of Foley catheter and elevated Glasgow Coma scale. We used Electroacupuncture, Physical treatment and Western medication at the same to get maximized effect on relaxing the contracted muscle. According to the Modified Ashworth Scale (MAS), we have some changes in muscle spasticity but later, the effect was not that significant. Conclusion : We had a patient with Hypoxic Ischemic Encephalopathy followed by Cardiopulmonary Resuscitation. In the management of Hypoxic Ischemic Encephalopathy, Conservative treatments are the mainstream but there are not many alternatives. Therefore, We suggest that Oriental medical approach may contribute to the management of Hypoxic Ischemic Encephalopathy.

  • Objectives: This case study examined the effectiveness of Korean medical treatment for pediatric Crohn's disease. Methods: A 15-year-old female Korean patient with Crohn's disease received acupuncture, electroacupuncture, herbal medicine, moxibustion, and aromatherapy treatment for 1 month in a hospital. Results: Decreases in the Pediatric Crohn's Disease Activity Index score (from 22.5 to 7.5) and the size of two abscesses (from 39.97 mm to 33.36 mm, and from 28.14 mm to 18.71 mm) according to an abdominal CT were observed following treatment. Nausea and vomiting disappeared, stool condition improved, and weight increased (from 30 kg to 33 kg) following treatment. Conclusion: It would be worth examining the long-term effectiveness of Korean medical treatment for pediatric Crohn's disease.
