Reliability and Validity of Gait Assessment Tools for Elderly Person (노인의 보행에 대한 평가 도구의 신뢰도와 타당도 조사 연구)

  • Purpose: The examine the Reliability and Validity of the modified Emory Functional Ambulation Profile (mEFAP), Tinetti. Gait (TG), Timed Up & Go Tes t (TUG), Comfortable Gait Speed (CGS), Berg Balance Scale (BBS) in assessing gait function and balance in elderly person. Methods: The 45 community-dwelling subjects were participated in this study. Reliability was determined by intra-class correlation coefficient (ICC$_{3,1}$), Bland and Altman method (Standard Error of Measurement (SEM), Smallest Real Difference (SRD)). Results: Validity was examined by correlation the mEFAP, TG, TUG, CGS, BBS. The intra-rater reliability were High (ICC$_{3,1}$ : mEFAP=0.95, TG=0.96, TUG=0.94, CGS=0.96, BBS=0.92) and Absolute reliability were excellent (SEM: mEFAP=1.90, TG=0.21, TUG=0.28 CGS=0.25, BBS=0.52), (SRD: mEFAP=5.26, TG=0.58, TUG=0.77, CGS=0.69, BBS=1.44). There were significant correlations between assessment tool (r=.0.58$\sim$0.78, p<0.01) indicating good validity. Our results provide strong evidence that the assessment tool has good reliability, validity for assessing elderly person undergoing rehabilitation. Conclusion: The gait assessment tool is a useful scale for measuring walking function and recovery in elderly person.

The Effects of an Education Program for Safe Drug Use in the Rural Elderly (농촌지역 노인의 안전한 약물 사용을 위한 교육의 효과)

  • Purpose: This study was conducted to develop an education program for safe drug use in the rural elderly and to measure the effect of the program. Method: This study utilized a nonequivalent control group pretest-posttest design. The subjects of this study consisted of 40 older persons who were more than 65 years old and lived in G and C moon, Y gun, Gyeongsang-bukdo and visited the public health subcenter. Twenty were assigned to the experimental group and 20 to the control group. The education was provided for one and a half hours, once a week for 3 weeks. Data was collected before, right after, and one month after the program. Result: The first hypothesis was supported(F=79.24, p=0.000) showing that the knowledge scores of the drug use of the experimental group were significantly higher than those of the control group at post education and one month after education. The second hypothesis was supported(F=23.84, p=0.000) showing that the drug misuse and abuse prevention behavior scores in the experimental group were significantly higher than those of the control group at post education and one month after the education. Conclusion: This study suggests that the education for safe drug use is effective in promoting knowledge and behavior for safe drug use of the rural elderly.

Clinical and Radiological Outcome of Distal Femoral Fracture in Elderly Patient Group (원위 대퇴골 골절에 대한 고령 환자군의 치료 결과 비교 -임상적, 방사선학적 비교-)

  • Purpose: To report the postoperative Clinical and Radiological outcomes following distal femoral fractures in elderly patients compared with young patients. Methods: From March, 1996 to March, 2012, 83 patients who received surgical treatment for fractures of the distal femur were enrolled in this retrospective study. Ages more than 65 was named group A. Group A was 49 cases and mean age is 72.5 year(65~91year). Group B was 49 cases and mean age is 45.7 year(16~61 year). Surgical methods are retrograde IM nail, locking compression plate, cannulated screw and postoperative rehabilitation is no difference between two groups. Clinical results were evaluated using Neer scores, radiographic results and the presence of clinical complications. Results: The mean union period was 18.4(12-40) weeks in group A and 17(10-24) weeks in group B. Neer functional scores are no significant statistical difference between two groups. There are 5 cases metal breakage in group A and 1case in group B. There are 3 cases nonunion in group A and 1 case in group B. Conclusion: In the case of fractures of the distal femur in elderly patients, locking plate using minimally invasive percutaneous periosteal osteosynthesis (MIPPO) technique may be one of the most effective methods and preoperative bone stock evaluation in important.

The Geriatric Care Workers' Role Care for Elderly of Sanatorium in Korea

  • This paper is to research the difference of care workers' role in Sanatorium between urban and rural areas. Interviews have been conducted with 100 care managers with structured questionnaires in community care settings. The findings of the study are as follows. In the analysis of working with carers a care manager acting as a counsellor in rural was required rather more than any other role in working with clients' carers. In working with formal and informal networks, an administrative specialist role was also important in both areas. With resource management, there were some regional variances between rural and urban. In the urban area, a care managers as a coordinator was more required than as a broker. In the rural area, a care manager as a broker, selecting service resources for elderly clients was the most suitable role. In conclusion, in general rural care managers' roles were similar to those of many core managers in urban area. Among the many possible roles of care managers that effective continuity of care is to be provided for elderly clients in community care, two have been specified as essential roles. The first is the role of care managers that provides coordination and integration of services at the clients' levels as a care manager as an implementer, a linkman, counsellor. The second is at the system level which is possible role for coordination and linkage of programs as a characteristics of care managers, task with formal & informal network, community resources, available residential & NHS resourcesw.

  • Social welfare center is a representation agency in the local welfare system to provide the local residents with necessary services directly. The purpose of this study is to give the fundamental materials for space program in terms of planning a social welfare center by means of analyzing spatial arrangement of each section in senior welfare activity. The structure of the social welfare center should be adopted to the reality and the center of local should pay attention to enhance the quality of life in elderly. As the aspects of the programs, distinctive program should be designed. To do so, social welfare center should be adjusted to the local situation and get flexibility on operating in the local level. Each program in senior welfare activity should be connected in the spatial organization and various space should be provided in desire of elderly. So the section of dining room and medical rehabilitation is a places mostly for the elderly. Multipurpose space for the interest and leisure and the space of the senior education facility are provided in social welfare center.

Depression in family caregivers of elderly people with dementia

  • The caregivers of elderly people with dementia suffer an affectation in the psychological and social order due to the low probability of total rehabilitation of the elderly and the progressive course of this pathology. In the municipality of Guisa, Granma Province there is a health problem regarding this problem since family caregivers of the elderly with dementia constantly go to the Psychology and Psychiatry consultations, in search of specialized help because they report feeling depressed. For this reason, this research was carried out with the objective of determining the main manifestations of depression in these caregivers. The results emanating from the triangulation of methods (scientific observation) and techniques (questionnaire, interviews and inventory Beck), employees at three stages of work where it was found that the main manifestations of depression of these caregivers are: poss of interest or the ability to enjoy activities that were previously pleasurable, loss of emotional reactivity to pleasant environmental events and circumstances, sleep disturbances, marked loss of appetite, weight loss, irritability, muscular tension, feelings of sadness unmotivated that produces discomfort and sufferings, ansiety, entities of handicap, blame, loss of self-confidence and feelings of inferiority, gloomy perspective of the future.

Study on the Gait Characteristics in Knee Osteoarthritis Patients with GAITRite System Analysis (GAITRite 시스템 분석을 통한 퇴행성 슬관절염 환자의 보행특성 연구)

  • The purpose of this study was to describe and compare the temporo-spatial gait characteristics of healthy elderly people with those osteoarthritis patients. 100 patients reported knee osteoarthritis, diagnosed at the hospital or clinic located in Daegu and Kyungbuk province and 100 normal elderly subjects were participated in this study. Temporal and spatial parameters of gait were analysed for using the computerized GAITRite system. The system integrates specific components of locomotions to provide a single, numerical representation of gait, the Functional Ambulation Performance score. Differences in gait characteristics between the two groups were examined using a correlated t-test and Pearson Correlation(p<.05). Significant differences were observed between the groups for temporal parameters(step time, double support time, stance phase, mean velocity) and spatial parameters(step length, step/extremity ratio)(p<.05). Also there was difference in the functional ambulation performance score between normal elderly subjects and knee osteoarthritis patients(p<.05). Consequently, it may help detect the abnormal gait pattern indicated the main problem in degenerative knee osteoarthritis patients as well as provide data analysing the pathokinesiologic components by comparing normal elderly.

Strength-dexterity Complementariness: Comparison between Left and Right Hands in Older Female Adults (근력-기민성 보완: 여성 노인의 오른손과 왼손 비교)

  • Objective: The purpose of this study was to in this study. The maximum grip force of the elderly hand was measured using a custom-designed grasping apparatus mounted with five three-component force transducers. The Jebsen-Tayler hand function test and Purdue Pegboard test were performed to evaluate the dexterity of the hand. Method: Twenty-six elderly women participated in the left hand between the maximum grip force and the Jebsen-Taylor hand function test results (r=-.513, p=.007). A significant correlation was also shown in the hand maximum grip force and the hand Purdue Pegboard results (r=.514, p=.007). However, no significant correlation was found in the right hand. Results: We found a significant correlation investigate the relationship between hand grip strength and hand dexterity in the elderly. Conclusion: Our findings in the current study support the theory of 'Strength-dexterity complementariness' which states that improvement in dexterity is associated with the grip force strength.

The Effect of Kinesiology Taping on Suprahyoid Muscles Activity in Community-Dwelling Elderly People

  • Background: The therapeutic exercise method using kinesiology taping (KT) has been reported to be effective in activating the suprahyoid muscle in healthy adults. However, its applicability and effectiveness are not known to the physically vulnerable elderly. Objectives: To investigate the effects of kinesiology taping on the activity of suprahyoid muscles in community-dwelling elderly people. Design: Repeated measure design. Methods: We enrolled 23 healthy elderly adults (age range 60-75 years) with no history of neurologic disease. Participants performed five consecutive spontaneous swallowing actions at 5-second intervals under three conditions (KT with 80% stretch, placebo-KT, and non-KT. Activation of the suprahyoid muscles during swallowing in the three conditions was measured using surface electromyography (sEMG). In addition, a 0-10 numerical rating self-report scale was used to evaluate the required effort and the resistance felt during swallowing. Results: KT with 80% stretch were significantly higher in sEMG mean value, peak value, required effort, and resistance felt during swallowing compared to other two conditions (P<.05, all). KT with 80% stretch required the most effort during swallowing and, consequently, has a positive effect on increasing suprahyoid muscle activation. Conclusion: Our results could be taken into consideration in therapeutic exercise method for suprahyoid muscle in dysphagia rehabilitation.

Improvement measures for Satisfaction of Utilization and its' Comparison of Assistive Products in Long-term care Elderly (장기요양노인의 복지용구 이용만족도 비교와 개선방안)

  • This research is focused on improvement of satisfaction of the assistive products, which is used by long term care elderly people. We investigated 1,200 long-term cared elderly and this survey was conducted by care-giver. The average satisfaction of the assistive products are in convenience 3.43, in function and effectiveness 3.33, in safety and secure 3.29 and in adequacy cost compared to effectiveness 2.69. Total average satisfaction of utilization is 3.19. Satisfaction with the use of assistive products in using motivation does not show a statistically significant difference, but in selection factors is a difference to be statistically significant. In conclusion, expert-consulting should be enhanced for the improvement of satisfaction and the provision of assistive products should be expanded in long-term care insuance schemes. Also, the rational index and evaluation method of the satisfaction of the assistive products should be developed through continuous studies.
