Study of the Geriatric Health Care Facility in Japan (일본의 노인보건시설에 대한 연구)

  • The objective of the present study is to review of the system, type of care and utility of the Geriatric Health Care Facility(GHCF) in Japan. Geriatric Health Care Facilities in Japan were started with subsidies from the Ministry of Health and Welfare in 1987 to encourage return of the elderly from hospitals to their homes rather than other destinations such as nursing homes or hospitals. Concerning to the type of care, there is the difference between GHCF and other geriatric care facilities(i.e., geriatric hospital and nursing home). GHCF provides both medical and nursing care. The following services are available for the GHCF's user's. As institutional care services, rehabilitation training, ADL exercise, nursing care and management of medicine, are available. For the out-patients, supplying meals, taking a bath, and rehabilitation services, are also available. The medical treatment fee at the facilities is about US $ 1,500 per month. Expenses for meals, daily necessities, shall be borne by the recipient, Those expenses are about US $ 360 per month. In anticipation of the coming of the aging society, the Goverment must be formulated consecutively several vital policies of measures, especially GHCF, for the elderly in the future few years.

Changes in pain, swelling, and range of motion according to physical therapy intervention after total knee arthroplasty in elderly patients

  • Objective: This study was carried out to compare changes in pain, swelling and range of motion (ROM) between the two groups according to physical therapy intervention. Design: Randomized controlled trial. Methods: There were a total of 20 elderly subjects ages 65-75 years old who have been admitted to the hospital for a total knee arthroplasty. After surgery, the experimental group (n=10) carried out quadriceps femoris muscle strengthening exercise, hamstring stretching exercises and gastrocnemius stretching exercise with a physical therapist for 30 minutes and additionally, received ice pack therapy for 20 minutes once a day. The control group (n=10) carried out continuous passive motion for 30 minutes and received ice pack therapy for 20 minutes once a day. The experimental group and control group carried out each intervention program for 2 weeks 3 times a week. The Visual Analogue Scale was used to assess pain, tape measurements were taken to assess swelling, and a steel goniometer was used to assess knee joint ROM. Results: As a result, the experimental group showed a statistically significant decrease in the pain and swelling, and a significant increase in knee flexion ROM after the intervention, compared to the control group (p<0.05). There was a significant improvement in pain, edema, and knee flexion and extension ROM in all subjects after intervention (p<0.05). Conclusions: According the results of this study, exercise in the experimental group is effect on the pain, swelling and ROM for total knee arthroplasty.

The Effects of Kyongrak Massage in the Elderly with Chronic Pain (경락마사지가 만성 통증 노인에게 미치는 효과)

  • Purpose: this study was implemented to develop new nursing intervention, Kyongrak massage, and to examine its effect on chronic pain in elderly women. Method: Kyongrak massage is a manipulation massaging on Kyonghyul site, and Duboo, Kyongchoo, Chucksoo, Hajee, Sangjee Kyongrak massages were used in this study. The subjects(11persons) took Kyongrak massage on head, neck, spinal cord, upper extremities and back lower extremities for 25minutes daily during 5days. The dependent variables were subjective health state, paid, blood pressure, pulse and body temperature. This study was carried out, at 10-12 o'clock, from August 7th, to 22th, 2000. Data were analyzed using frequency, percentage, mean, standard deviation, paired t-test by SPSS PC. Results: subjective health state((t=-3.560, p=.005), body temperature(t=-2.557, p=.029) were increased, and pain level(t=7.884, p=0.000), systolic blood pressure(t=2.923, p=.015) were decreased significantly by Kyongrak massage. Conclusions: The above results have informed us that this Kyongrak massage program(Duboo, Kyongchoo, Chucksoo, Hajee, sangjee) for 25minutes is a useful nursing intervention to decrease chronic pain in each life styles.

Case Series Study of Liver-Tonification Acupuncture Treatment for Muscle Cramp of Middle-aged and Elderly (중·노년층의 전근증상에 대한 간정격 활용의 증례군 분석)

  • Objectives The aim of this study was to investigate the effects of liver-tonification acupuncture (LTA) on periodic muscle cramps (MC). Methods We retrospectively reviewed medical records of 28 patients treated for periodic MC with LTA in a Korean medicine clinic. Patients were treated with (A) LTA alone, (B) LTA and treatments for myofascial meridian, or (C) LTA and treatments for other diseases. We mainly investigated effectiveness of treatments, the number of treatments, the number of relapses, and time to relapse. Results After treatments, periodic MC disappeared in all patients except two in group C. The number of treatments in group C was significantly greater than that of group A (p<0.05). Periodic MC recurred in one patient in each of groups A and C, but symptoms disappeared after re-treatments. Periodic MC did not recur for a median of 7.0, 8.5, and 5.0 weeks in groups A, B, and C, respectively. Conclusions We found some therapeutic effects of LTA treatments for periodic MC in middle-aged and elderly patients.

A Survey of the Rehabilitation Services Needs of the Elderly in Rural Area (지방도시 거주 노인들의 재활서비스에 대한 요구 조사 - 안동시 거주 노인들을 중심으로 -)

  • This study was carried out to find out general characteristics and health-related be haviors of elders aged 60 or over and factors related to their activities of daily living, and thus to contribute to their health maintenance and promotion. The subjects were 200 elders who had resided at An-Dong city and participated in this study. To collect data, the field survey was conducted from December 13 to 23, 1996 with structured questionnaires by 6 trained student interviewers. Chi-square test, t-test and Pearson's correlation were used for data analysis by use of SPSS/$PC^+$ program. The major findings were as follows; 1) In the individual characteristics of the respondents, the average age were 73.6 years old. 21.5% of the subjects were 60-69 years old, while 27.5% were 70-74 years old, 25.0% were 75-79 years old, 26.0% were 80 years old or over. 2) For the degree of the subject's perceptive health condition, the aver age 3.39 point and standard deviation 1.09. The perceptive health condition of the subjects was in the mid point range of health condition. 3) 46.6% of men and 25.8% of women practiced exercise regularly for their health maintenance and promotion. 4) For the degree of the subject's perceptive health condition, the difference by sex was not statistically significant. 5) 51.8% of men and 60.2% of women has idea for interventions to health promotion program by rehabilitation services.

A Study on the Influence of Change of Electrical Stimulation Time on Body Regions Affects on Threshold of People Over Age 60 (60세이상 노인의 신체 부위별 전기자극시간에 따른 역치 변화에 대한 연구)

  • Background: The purpose of this study was influence of change of electrical stimulation time on body regions affects on electrical current threshold. Methods: The present study is to examine the effect of silver spike point (SSP) electrical stimulation (1Hz), transcutaneous electrical nerve stimulation (TENS, 100Hz), and interferential current therapy (ICT, 50Hz) from the low back and scapulodorsal and knee joint regions on stimulation-induced current thresholds from the elderly (over sixty) people (male:72, female:91) in senior welfare center. Result: The low back region, but not scapulodorsal and knee joint region, significantly increased the TENS and ICT, but not SSP, electrical stimulation, significantly increased the time-dependent current thresholds in elderly patients. Conclusion: Therefore, these results, in part, suggest that the TENS and ICT were shown to be a more adaptable method of stimulation, and that needed of the development of senile specialized physical therapy and the utilization of senior leisure facilities such as senior welfare center the others.

  • As our society progresses rapidly toward an aged society, there is a stiff increase in the population of the aged persons who have difficulties in adjusting themselves to environments due to declines of their physical functions. Therefore, there is a great need for the concept of 'Universal Design' that demands the design of overall social structure to care for the elderly. Due to this reason, the physical function of the aged persons should thoroughly be studied for the development of rehabilitation and training equipments to help the recovery of physical functions of the elderly. Among the variety of physical functions of the elderly, visual functions, motion characteristics, hearing functions, and somatosensory functions have significant influence toward everyday lives and are physical functions to study for the construction of urgently needed physical function databases of the elderly. This study concentrates on visual functions among the variety of physical functions of the elderly. To measure various visual functions of the elderly, a measurement room for the elderly has been developed, which can mimic everyday lives and perform measurements of visual functions with subjects in seated position. The measurement items for the construction of the database were capabilities in everyday vision according to changes in arrangement of colors and light intensity and capabilities in everyday vision, color distinction, dark adaptation, and light glare against changes in contrast.

The Meditation Effects of Self-esteem on the Relationship model between Korean new elderly class's Recovery resilience and Productive aging (한국 신노년층의 회복탄력성과 생산적 노화의 관계에서 자아존중감의 매개효과 검증)

  • The purpose of this study was to investigate the meditation effects of self-esteem on the relationship model between Korean new elderly class's recovery resilience and productive aging. This study sampled preliminary elderly people in other provinces nationwide. Data from a total of effective 484 subjects was analyzed. The collected data was processed using the IBM SPSS 20.0 and AMOS 5.0, and underwent descriptive statistical analysis, confirmatory factor analysis, and structure model verification. The results are as follows; First, recovery resilience of Korean new elderly class's affect their self-esteem. Second, recovery resilience of Korean new elderly class's affect their productive aging. Third, self-esteem of Korean new elderly class's affect their productive aging. Lastly thel relationship among recovery resilience, self-esteem and productive aging of Korean new elderly class's shows partial mediation, and there is the mediation effect.

Relationship between perceived health status and health-related quality of life in the elderly: A focus on moderating effects of oral health (노인의 주관적 건강상태와 건강관련 삶의 질의 관련성 : 구강건강수준의 조절 효과 중심으로)

  • Objectives: This study aimed to validate the moderating effect of oral health on the relationship between perceived health status and health-related quality of life in the elderly and to use this information as primary data to suggest oral health policies for the aged society. Methods: This study included 3,707 subjects aged over 65 years who answered all the variables used in the study model and completed the health questionnaire and screening survey based on the sixth Korea National Health and Nutrition Examination Survey. Baron & Kenny's linear regression analysis using SPSS 22.0 and SPSS Macro Version 3.1 programs was performed to confirm the moderating effect of the number of remaining natural teeth, chewing difficulty, and cardiovascular disease on the relationship between perceived health status and healthrelated quality of life in the elderly. Results: The number of remaining natural teeth, chewing difficulties, and cardiovascular diseases affected the perceived health status of the elderly. As the number of remaining natural teeth increased, the effect of perceived health status on the quality of life in the elderly was buffered. The effect of perceived health status on the quality of life increased with chewing difficulties and the number of cardiovascular diseases. In particular, it was confirmed that chewing difficulties, rather than the number of cardiovascular diseases. had a greater effect on the quality of life in the elderly. Conclusions: Oral health policies and projects are required to ensure oral rehabilitation with dentures and implants and restore chewing function to improve the quality of life of the elderly in Korea.

An investigation of grip strength, anthropometric parameters, depression, quality of life in elders : Using Korea national health and nutrition examination survey (2016) (노인의 장악력 및 신체계측인자, 우울, 삶의 질에 관한 연구 : 2016년 국민건강영양조사를 이용하여)

  • Objective : The aim of current investigation was to standardize the grip strength of elders and to examine the correlation between grip strength, anthropometric parameters, depression and quality of life. Method : The researchers used Korea national health and nutrition examination surveys (2016) for analysis and analyzed the data of 919 elders who had completed the screening test among right-handed elderly without activity limitation. The researchers classified the age of the elders as 65~69, 70~74, 75~79, and 80 years old for standardization of grip strength. Anthropometric parameters are height, weight, body mass index, and waist circumference. Depression was measured by PHQ-9(Patient Health Questionnaire-9) and quality of life was assessed by EQ-5D. Result : The ratio of right handedness of the elderly was 87.8%, left handed 4.9%, and both handles 5.7%. In the standardization and correlation analysis, the grip strength decreased with age (p<.01, r=-.308~-.305). The grip strength showed a strong correlation with height (p<.001, r=.747~.741). There was a significant correlation between depression (p<.01, r=-.172~-.163) and quality of life (p<.01, r=.285~.267). Conclusion : The findings of this investigation suggest that height, depression, and quality of life need to be considered when assessing the level of grip strength in the elderly.
