Objectives : The aims of this study were to identify life satisfaction of elderly living alone based on their health status and suggest measures that improve their physical, mental and oral health, which could be used in future welfare policies on the elderly. Methods : The study subjects were 307 senior citizens who lived alone and used senior citizen centers in Jeonju-si, Jeollabuk-do. Results : Life satisfaction tended to be high when the subjects had a low number of chronic diseases, no activity limitations and stress, no suicidal feelings, no tooth mobility and chewing difficulty and denture use. Conclusions : To improve life satisfaction, elderly living alone need to make effort to maintain their health however, policy programs that improve the physical, mental and oral health of the elderly need to be promoted.
Objectives: This study confirms the current status of visiting oral health-care services for the elderly to draw policy implications for revitalization of the visiting oral health care services in the future. Methods: First, a survey was conducted on health centers about the current status of the elderly visiting oral health-care service and how to revitalize it. Next, the number of oral hygiene services provided to the elderly was checked in the long-term care insurance system. Results: Oral health education (100%) was the most common practice in visiting oral health-care service for the elderly, and the most difficult thing in providing services was the lack of dental hygienists (38.9%). The status of oral health-care services in the long-term care insurance system for elderly revealed that the total number of service claims has been confirmed to be zero since the introduction of the system. Conclusions: Despite the existence of a system that provides elderly visiting oral healthcare services, to revitalize it, the law must be amended to secure a dental hygienist as the main agent of the activity and to further take responsibility for autonomous authority and performance.
본 연구는 노인들의 구강관리행태, 구강건강관리 자기효능감, 사회적지지가 주관적 구강건강수준에 미치는 영향을 알아보기 위해 실시하였다. 자료조사는 2019년 9월 17일부터 2019년 11월 22일까지 대전광역시와 충청남도 일부지역에 거주하는 노인들을 대상으로 하였으며, 수집된 자료는 𝑥2-test, Pearson 상관분석, 로지스틱 회귀분석으로 분석하였다. 조사결과 주관적 구강건강수준은 틀니를 사용하고 있지 않은 경우 3.242배, 일일 칫솔질 횟수가 3회 이상인 경우 2.339배 증가하였으며, 구강건강관리 자기효능감과 사회적 지지가 각각 1.755배, 1.192배 증가함에 따라 주관적 구강건강수준도 증가하였다. 이상의 결과를 토대로 노인들의 주관적 구강건강수준의 증진을 위해서는 틀니관리방법과 칫솔질교습 등의 구강보건교육의 기회를 확대하고, 구강건강관리 자기효능감과 사회적 지지를 강화시킬 수 있는 정책 마련이 필요하다.
본 연구는 한국 노인의 인구사회적인 특성과 구강보건행위가 구강건강상태에 미치는 요인을 조사함으로써 향후 노인대상의 구강건강사업에 필요한 기초자료를 제공하고자 한다. 본 연구자료는 2010년 1월부터 12월까지 질병관리본부에서 시행한 '국민건강영양조사 제5기 1차년도(2010) 자료'를 활용 구강검사 까지 완료한 65세 이상 노인 1419명중 검사가 미흡한 노인 707명을 제외한 712명을 최종 연구대상으로 하였다. 수집된 자료는 SPSS 19.0 프로그램을 이용하여 전산통계 처리하였으며, 분석방법은 빈도분석, 다중회귀분석을 이용하였다. 잔존치아 수에 미치는 요인은 성별이 남성일수록, 나이가 적을수록, 거주지가 동일수록, 교육수준이 높을수록, 흡연량이 적을수록, 구강건강용품 사용이 많을수록 높게 나타났고, DMFT 지수에 영향을 미치는 요인은 나이가 적을수록, 소득이 낮을수록, 교육수준이 높을수록 DMFT 지수가 낮게 나타났고, CPI 지수에 영향을 미치는 요인은 성별이 남성일수록, 나이가 적을수록, 거주지가 동일수록, 흡연량이 적을수록, 구강건강용품 사용이 많을수록 CPI지수가 낮게 나타났고, FS-T 지수에 영향을 미치는 요인은 성별이 여성일수록, 소득이 높을수록, 잇솔질 횟수가 많을수록 FS-T 지수가 높은 것으로 나타났고, T-Health 지수에 영향을 미치는 요인은 성별이 남성일수록, 나이가 적을수록, 거주지가 동일수록, 흡연량이 적을수록, 구강건강용품 사용이 많을수록 T-Health 지수가 높게 나타났다. 이상의 결과를 토대로 구강보건행위는 구강보건지수에 영향을 미치고 있음을 알 수 있었고, 향후 노인의 구강건강을 증진시키기 위해서는 체계적인 구강건강교육과 국가적 지원이 필요하다고 사료된다.
Objectives: The purpose of this study was to compare the differences in oral health-related quality of life among elderly people aged over 65 years, in terms of physical, mental and oral health status and to analyze factors affecting their oral health-related quality of life. Methods: From May 9 to June 23, 2017, we randomly visited aged-care community centers in the metropolitan area, and recruited 222 elderly, aged 65 or older. First, each participant completed a questionnaire consisting of 4 general items: 1 systematic disease, and 3 subjective oral conditions. Afterwards, the researchers interviewed the participants to assess their mental status, using MMSE-DS and recorded the responses. Finally, an oral examination was performed to determine the number of remaining teeth. The average oral health-related quality of life according to each characteristic was analyzed by t-test and ANOVA. Hierarchical multiple regression analysis and Pearson's correlation coefficient analysis were used to analyze the correlations between factors and the factors affecting oral health-related quality of life. Results: The mean oral health-related quality of life was 4.15. Participants with 20 or more remaining teeth demonstrated better oral health-related quality of life than those with 19 or less teeth. Higher oral health-related quality of life was also found among elderly without gingival bleeding, self-reported halitosis and dry mouth. In addition, positive correlation with the number of remaining teeth and negative correlation with gingival bleeding, self-reported halitosis and dry mouth, were noted. Finally, the results of the hierarchical multiple regression analysis indicated that remaining teeth, gingival bleeding, self-reported halitosis and education were influential factors in determining the oral health-related quality of life among the elderly. Conclusions: The results of this study confirmed the necessity of better policy support, and the importance of implementing delivered, elderly-centered oral health education program by professionals to prevent tooth loss and manage periodontal diseases.
본 연구는 노인들의 교육과 소득정도에 따른 구강건강상태 및 관리행태와 치과이용행태를 파악하여, 노인구강건강 증진에 대한 정책과 제도를 마련하기 위한 근거자료를 제시하고자 실시하였다. 자료조사는 2019년 8월 19일부터 2019년 10월 25일까지 대전지역에 거주하는 노인 224명을 대상으로 실시하였으며, 수집된 자료는 chi-square test와 logistic regression analysis 등으로 분석하였다. 그 결과 구강건강상태 및 관리행태는 틀니사용, 임플란트사용, 구강위생용품사용, 구강보건교육경험, 구강건강관리방법에서 교육과 소득정도에 따라 유의한 차이를 보였다. 치과이용행태도 교육과 소득정도에 따라 유의한 차이를 보였으며, 연령, 교육 정도, 임플란트사용 여부, 치과이용의 주된 이유가 치과이용에 영향을 미치는 요인으로 나타났다. 이에 본 연구를 토대로 교육 및 소득정도를 고려한 국가적 차원의 제도적 방안이 마련되어 노인들의 구강건강의 삶의 질이 향상될 수 있기를 기대한다.
Objectives: The purpose of this study was to investigate how mental health status affects oral health in the elderly. Methods: The subjects of the study provided the data for the sixth Korea National Health and Nutrition Examination Survey from 2013 to 2015. Four-hundred and twenty-one subjects were selected for the study. All data were analyzed by complex sampling frequency, chi-square test, and composite multiple regression analysis using SPSS 21.0 version. In addition, a significance level of 0.05 was considered. Results: The general factors affecting cases of periodontal disease were general characteristics, mental health status, oral health status, elderly males, household income, education level, daily brushing frequency, speaking, and stress. The effect of periodontal disease was 1.64 times higher in the male group than in the female group (p<0.001). In the household income level, the 'low' group had 1.91 times more periodontal disease, while the 'mid low' group had 1.64 times more periodontal disease than the 'high' group (p<0.05). Periodontal disease was found to be lower in the group that recognized subjective oral health status as 'good' (p<0.05). The study subjects had low levels of periodontal disease when there was no speaking difficulty (p<0.05). Conclusions: Therefore, in order to improve oral health of the elderly, it is necessary to understand the mental health condition of the elderly and prepare proper oral health education programs accordingly. Institutional devices for various oral health projects should be prepared, as well.
Objectives : This study aims to identify factors affecting satisfaction among the elderly with complete denture to help the elderly with complete denture perform oral hygienic care and to contribute to improvement in oral health. Methods : Interviews were performed with 247 people having complete denture, who were over 65 years and resided in Daejeon and South Chungcheong Province, from March 5 to 31, 2009. Results : 1. As for satisfaction in areas of complete denture by the number of brushing, chewing, pronunciation, and aesthetic functions were all high in over three times of brushing, showing statistically significant differences in satisfaction with chewing (p=0.1825) and aesthetic (p=0.005) functions. 2. As for satisfaction in areas of complete denture by the period of using denture, chewing, pronunciation, and aesthetic functions were all high in less than 7 years, showing statistically significant differences in satisfaction with chewing (p=0.030) and aesthetic (p=0.000) functions. 3. As for satisfaction in areas of complete denture by the subjective oral health state, the better subjective oral health state, the higher satisfaction with chewing, pronunciation, and aesthetic functions, showing statistically significant differences (p=0.000). 4. As for relevance between the subjective oral health state and satisfaction with complete denture, there was 30.1% explanation power in satisfaction with complete denture by the subjective health state. Conclusions : As for satisfaction with complete denture among the elderly, more than three times of brushing and less than 7 years of using denture were related to higher satisfaction; the better subjective oral health state, the higher satisfaction with chewing, pronunciation, and aesthetic functions. Therefore, it is necessary to have the elderly with complete denture receive education about oral hygienic care and perform the care at the same time.
Purpose: The purpose of this study was to investigate the denture attachment status and the denture need of elderly Koreans and to provide basic data for improving the quality of life by developing methods and education programs for the oral health of the elderly. Methods: This study selected 4,340 persons who completed questionnaires and oral examinations among elderly people aged 65 or older only from the National Health and Nutrition Examination Survey data of the 6th period (2013-2015). Results: There was a significant relationship between age, residence, and education level in complete dentures. There was a significant relationship between gender, age, residence, and education level in the partial denture wearing state. In the complete denture need, there was a significant relationship with age. The need for partial denture was found to be significant in gender, age, and residence. Conclusion : Based on the results of this study, it can be used as basic data for oral health education and dental prosthesis related to denture which can improve oral health of the elderly in the future.
본 연구는 일부지역 60대 이상 노인을 대상으로 구강건강관리행태에 따른 주관적인 구강건강인식과 구강 건강지식도의 관련성을 알아보고자 조사하였다. 설문 조사는 경북에 거주하는 60세 이상의 노인을 대상으로 2016년 9월 1일부터 12월 30일까지 실시하였고, 총 442부를 최종 분석에 사용하였다. 자료의 분석은 SPSS WIN 12.0 프로그램을 이용하여 빈도분석, t-test, ANOVA 등을 실시하였다. 구강건강관리행태에 따른 주관적인 구강건강인식은 칫솔질을 전체적으로 잘 닦을수록, 칫솔질 시간이 길수록 높았으며(p<.05), 구강건강관리실태에 따른 구강건강지식도는 칫솔질 교육을 받은 경우 높았다(p<.05). 이상과 같은 결과를 바탕으로 노인들의 구강건강증진을 위해서는 올바른 구강건강관리행태를 위한 지속적인 교육이 이루어져 구강에 대한 주관적인 구강 건강인식과 구강건강지식도를 높여주어야 할 것이다.
