Analysis of Resistive Exercise Characteristics for Newly Developed Sonic Vibration Leg Press (새로이 개발된 음파 진동 레그프레스의 저항 운동 특성 분석)

  • Many studies have emphasized the importance of resistive exercise to maintain a healthy human body, particular in prevention of weakening of physical strength. Recently, some studies advocated that an application of vibration as a supplementary means in a regular training was effective in encouraging physical strength. Aim of the current study was, therefore, to identify if an application of vibration in a resistive exercise was effective in encouraging physical strength as that in a regular training. A 3-dimensional virtual lower extremity model for a healthy male and virtual leg-press model were generated and synchronized. Dynamic leg-press exercises on a slide machine with/without extra load and on a footboard with vibration as well as on a slide machine with extra load were analyzed. The results of the current indicated that the application of the vibration on the dynamic leg-press exercise might be not greatly effective in encouraging physical strength, compared with the dynamic leg press exercise with extra load. It was, however, thought that the application of the vibration might be helpful to elderly individuals because the reduced maximum muscle strength appeared by the effect of the vibration may avoid a muscular spasm, which can be driven from a high muscle strength sometimes produced during the leg-press exercise with extra load.

Epidemiologic and Socioeconomic Status of Bladder Cancer in Mazandaran province, Northern Iran

  • Background: Bladder cancer is one the most common malignancies of the genitourinary tract. The present study aimed to assess the epidemiology, of bladder cancer in Mazandaran, a large province in northern Iran as high-incidence cancer area, during a 2-year period. Methods: The data for this study were obtained from the population-based cancer registry of the Vice-Chancellory for Health Affairs of Mazandaran University of Medical Sciences and Mazandaran hospitals between March 1, 2010 and March 1, 2011. Demographic data, including sex, age, residency and symptoms were investigated through careful review of medical records. Using a questionnaire protocol, several variables were assessed for these cases such as smoking, history of opium, vegetable consumption habits, and history of other cancers. Results: A total of 112 cases were analyzed, 98 (87.5%) in men and 14 (12.5%) in women (mean age of $68.0{\pm}14.6$ years). Urban and rural residence were 60.7% and 39.3%. Tobacco and opium use were found in 45.5% and 21.4% of patients, respectively. Approximately 60% consumed vegetables an average of fewer than one time per day. Hematuria was the first symptom in these cases which were mainly diagnosed as having bladder cancer by ultrasonography. Conclusion: The results showed that bladder cancer tends to be found in the elderly and the male to female ratio is high. Macroscopic hematuria is a very important symptom for indicating probably urothelial tumor that should be followed up patients with transabdominal ultrasonography as a routine modality.

Diet and Lifestyle Risk Factors of Benign Prostatic Hyperplasia (전립선비대증에 영향을 미치는 식생활 및 생활습관 요인)

  • Benign prostatic hyperplasia (BPH) is one the most common prostate diseases in middle aged and elderly men. This study was conducted to investigate diet and lifestyle risk factors for benign prostatic hyperplasia in a community-dwelling free-living population group. The dietary data were collected from the 601 male subjects aged 50-79 years using the 24-hour recall method. The mean age of the BPH group ($63.0{\pm}7.9$ years) was significantly higher than that of the non-BPH ($58.8{\pm}7.4$ years). Among many nutrients, the amount of animal fat intake was increased while that of carbohydrate intake decreased in subjects with BPH compared to those with non-BPH. In BPH subjects, the proportion of energy from fat was also greater than in subjects with non-BPH. The logistic regression analysis on the food consumption data showed that the consumption of total animal food was increased while that of mushrooms was decreased in patients with BPH compared to the subjects with non-BPH. The age-adjusted odds ratios and 95% confidences limits for BPH incidence in subjects whose milk and milk products, beverages and alcoholic liquors intake was greater than the median were 1.796 (1.167-2.782) and 1.738 (1.129-2.676) respectively, compared to those in subjects whose intakes were below the median. These results may be applicable in the development of a nutrition intervention and education program toward a reduction in the risk for benign prostatic hyperplasia.

Antidiuretic Hormone Levels in Men with Burning Mouth Syndrome: A Pilot Study

  • Purpose: Burning mouth syndrome (BMS) is a disabling pain that mostly occurs in elderly women, but rarely in men. It is characterized by an unremitting oral burning sensation and pain without detectable oral mucosal changes. We investigated the clinical and hematologic features of middle-aged men with BMS, and compared the results to those of men with oral mucositis. Methods: Five men with BMS ($48.60{\pm}6.19years$) and five age-matched controls with oral mucositis ($49.80{\pm}15.26years$) underwent clinical and psychological evaluations and blood tests. Psychological status was evaluated using the Symptom Checklist-90-Revised. Cortisol, estradiol, progesterone, testosterone, follicle-stimulating hormone (FSH), luteinizing hormone (LH), adrenocorticotropic hormone (ACTH), and antidiuretic hormone (ADH) levels and erythrocyte sedimentation rate (ESR) were determined from the blood samples. Results: ADH level was significantly lower in men with BMS than in the controls. ADH levels correlated with testosterone (p<0.01), and ACTH levels strongly correlated with ESR (p<0.05). Progesterone level positively correlated with FSH and LH levels. Pain intensity on a visual analogue scale correlated with estradiol level only in men with BMS. Among psychological factors, the obsessive-compulsive disorder, interpersonal-sensitivity, and anxiety scores were higher in men with BMS than in the controls (p<0.05). However, no correlations were observed between the psychological and hematologic factors in both groups. The BMS symptoms presented only on the tongue, with the lateral border being the most prevalent area. Conclusions: Men with BMS may experience dysregulated endocrinologic or psychoneuroendocrinologic interactions, which might affect oral BMS symptoms, aggravating the severity of the burning sensation.

Development and evaluation of estimation model of ankle joint moment from optimization of muscle parameters (근육 파라미터 최적화를 통한 발목관절 모멘트 추정 모델 개발 및 평가)

  • Estimation of muscle forces is important in biomechanics, therefore many researchers have tried to build a muscle model. Recently, optimization techniques for adjusting muscle parameters, i.e. EMG-driven model, have been used to estimate muscle forces and predict joint moments. In this study, an EMG-driven model based on the previous studies has been developed and isometric and isokinetic contraction movements were evaluated to validate the developed model. One healthy male participated in this study. The dynamometer tasks were performed for maximum voluntary isometric contractions (MVIC) for ankle dorsi/plantarflexors, isokinetic contraction at both $30^{\circ}/s$ and $60^{\circ}/s$. EMGs were recorded from the tibialis anterior, gastrocnemius medialis, gastrocnemius lateralis and soleus muscles at the sampling rate of 1000 Hz. The MVIC trial was used to customize the EMG-driven model to the specific subject. Once the subject's own model was developed, the model was used to predict the ankle joint moment for the other two dynamic movements. When no optimization was applied to characterize the muscle parameters, weak correlations were observed between the model prediction and the measured joint moment with large RMS error over 100% (r = 0.468 (123%) and r = 0.060 (159%) in $30^{\circ}/s$ and $60^{\circ}/s$ dynamic movements, respectively). However, once optimization was applied to adjust the muscle parameters, the predicted joint moment was highly similar to the measured joint moment with relatively small RMS error below 40% (r = 0.955 (21%) and r = 0.819 (36%) and in $30^{\circ}/s$ and $60^{\circ}/s$ dynamic movements, respectively). We expect that our EMG-driven model will be employed in our future efforts to estimate muscle forces of the elderly.

Changes in Diseases of Physical Therapy Patients and Medical Department on Two General Hospitals in Taegu (대구시내 2개 종합병원 물리치료환자의 진료과 및 질병 변화 $(1989\~1991)$)

  • This study was conducted to investigate the changes diseases of physical therapy patient. Through the analysis, of total 2,902 cases in one university hospital and one general hospital in Taegu, of which 1,619 cases for 1989 and 1,283 cases for 1981. The physical records were analyzed in terms of sex, age, pattern in PT diseases, fee, and medical department of PT. The international classification of Diseases, 9th revision was used fer the study. Major results are as follows : 1. The ratio of male me female nae 1.51 to 1 in 1989, 1.53 to 1 in 1991. The proportion of the elderly over 60 was $15.6\%$ in 1989, $22.0\%$ in 1989. And the age groups of 50-59 years ranked the first an years. 2. As to the PT patients of medical department, Orthopaedics$(50.3\%)$, Neurosurgery$(28.1\%)$. Neuromedicine$(8.0\%)$. Plastic surgery$(4.4\%)$, and Dentry$(3.2\%)$ in that order in 1989. On the other hand, Orthopaedics$(51.2\%)$, Neurosurgery$(22.1\%)$, Neuromedicine$(9.6\%)$, Plastic surgery$(6.5\%)$, and Internal medicine$(6.5\%)$ in that order in 1991. 3. No significant difference was observed by season of PT patients, but winter (December, January and February) ranked the first all years. 4. No significant difference was observed changes in diseases as for the 56 international classification of diseases of PT patients, Diseases of the musculoskeletal system and connective tissue occupied the largest proportion all years. Fractures increased from $21.4\%$ in 1989, $24.5\%$ in 1991. On the other hand, Diseases of the nervous system remarkably increased from $8.9\%$ in 1989, $19.7\%$ in 1991.

The Effect of Herbal Formulation KH-305 Mainly consisted of Rubus Coreanus on Benign Prostatic Hyperplasia-Induced Rat (전립선 비대증 유발 백서에서 복분자를 주성분으로 한 생약제제 KH-305가 미치는 영향)

  • Benign prostatic hyperplasia (BPH) is one of the common diseases in elderly men. Recently, the old-aged population has increased, with the interest in the clinical importance of BPH ever growing. This study is designed to investigate the effects of KH-305 on BPH induced rat. The herb formulation KH-305 is consisted of Rubus coreanus, Cornus officinalis and Cuscuta chinensis. An experimental prostatic hyperplasia was induced in male rats by the administration of testosterone propionate, 3 mg/kg SC, for 2 months. The rats were divided into 3 experimental groups: the control, BPH-induced, oral KH-305 ingestion group. After 2 months, the prostates were removed, and analyzed for their prostatic weight and histological examination. The prostate weights were measured in each group, and found to be 820${\pm}$38mg, 3140${\pm}$26mg, 1880${\pm}$21mg in the control, BPH-induced, and oral KH-305 ingestion group, respectively. The BPH induced group showed statistically significant increases in their prostatic weights compared with control group(p<0.05) but oral KH-305 ingestion group showed more significant decreases than BPH-induced group statically(p<0.05). Histologically injected testosterone lead to prostatic hyperplasia in rats, but oral KH-305 ingestion decreased this change. These results suggest that KH-305 may be effective in treatment of BPH, and complementary medicine of BPH.

Effect of Onion Flesh or Peel Feeding on Antioxidative Capacity in 16-Month-old Rats Fed High Iron Diet (양파 육질 및 껍질의 섭취가 고철분식이를 공급한 노령흰쥐의 항산화능에 미치는 영향)

  • Recently, epidemiological evidence has raised concerns that moderate elevation in body iron stores may increase oxidative stress and the risk of cardiovascular disease and cancer. Onion flesh or peel contains antioxidant such as flavonoids and alk(en)ylcysteine sulphoxides. This study was conducted to examine the effect of onion flesh or peel feeding on antioxidative capacity in aged rats supplemented with high dietary iron. Thirty-two Sprague-Dawley male 16-month-old rats weighing $618{\pm}6g$ were acclimated for 10 days with medium-iron diet (35ppm in diet), and blocked into 4 groups according to their body weights and raised for 3 months on either control diets (adequate iron-35ppm or high iron-350ppm) or experimental diets containing onion flesh/peel (5% w/w in diet) with high iron (350ppm). Rats fed high iron-onion peel diet had significantly high quercetin and isorhamnetin levels in plasma whereas rats fed high iron-onion flesh diet did not show. Plasma TBARS level was lowered by onion flesh or peel diet with high iron supplementation. However, there was no significant difference in cellular DNA damage in brain and kidney tissue among all experimental groups. We concluded that high iron diet (10 times higher than requirement) tend to increase oxidative stress and it is plausible that onion flesh or peel feeding enhances antioxidative capacity in the elderly even with iron supplementation.

Design Guideline of Height-adjustable Wash Basin for Persons with Disability (고령자 및 장애인용 높이조절 세면기의 설계 가이드라인)

  • In this study, we proposed a design guideline of height-adjustable wash basin which is an assistive product for personal hygiene for persons with disability. We performed biomechanical assessments using both of conventional and prototype height adjustable wash basin. Total five elderly subjects (all male, age $68.6{\pm}4.3yrs.$, height $169.8{\pm}5.7cm$, weight $70{\pm}7.7kg$) participated for the assessment test. Each joint angles were measured by using a 3D motion capture system when subjects use wash basins, and the lumbar moment of each subjects was estimated based on a human body model. From the assessment results, a design guideline which has the range of the height from 652[mm] to 1162[mm] was proposed. Then additional assessment tests with five healthy subjects ($25.8{\pm}1.8yrs.$, $175.5{\pm}5.8cm$, $74{\pm}15.7kg$) were performed in order to verify effectiveness of the design guideline. The results showed a height-adjustable wash basin applied the proposed design guideline was effective to reduce the lumbar moment.

  • Objective : The purpose of this study is to assess the utility of clinician administered posttraumatic stress disorder scale (CAPS) to diagnose combat related posttraumatic stress disorder (PTSD) in Korean veterans of the Vietnam War. Methods : Sixty-one Korean male veterans of the Vietnam war participated in this study. We compared the diagnostic values of CAPS, Korean version of mini international neuropsychiatric interview (MINI) against the Structured Clinical Interview for DSM-IV (SCID) in diagnosing PTSD. Results : The overall internal consistency of CAPS was 0.93. Compared to the SCID, total severity 45 (TSEV45) showed the best results among 5 CAPS scoring rules. In detail, sensitivity was 71.4%, specificity was 85.1, positive predictive value was 58.8%, negative predictive value was 90.9%, and accuracy was 82.0. Conclusion : CAPS was demonstrated as a reliable tool to diagnose combat related PTSD in the elderly. The optimum CAPS scoring was TSEV45.
