Objectives : To help develop strategies to cope with the changes arising from the rapid aging process by predicting the determining factors of intention to actual use of the charged long-term care services for elderly as perceived by the middle aged who play the major role of supports. Methods : Subjects were the parents (men 177, women 507) in their 40s of the students selected from a university of Busan city. A questionnaire survey was conducted for 4 weeks in October 2003 about the knowledge for long-term care service, the intention of actual use, and the preferences about the type of service suppliers. Data analysis was performed with frequency, chi-square test, and t-test using SPSS program (ver 10.0K), along with data mining using decision tree of Enterprise Miner V8.2 by SAS. Results : About half of the subjects (53.7%) had the actual experiences of elderly supports. Intentions to use the charged services were relatively high in home visiting nursing care service (40.1%) and long-term care facilities service (40.4%), and were influenced by previous knowledge about the services. The intentions were stronger in women, those with higher education, and those with greater income levels. Actual elderly supports were mostly (80%) done by women, and the perceived burdens for the supports were bigger in women and those of lower socioeconomic level. Desired charges were about 10,000 won for the bath service, 20,000 won for the rests services per day, and about 500,000 won for the long-term care facilities service per month. From the result of decision tree analysis, the job professionalism was the most important determining factor of intention to actual use of the services with validation as $63{\sim}71%$. Health and welfare mixed type facilities were preferred, and the most important consideration was the level of professionalism. Conclusions : Intention to actual use of the charged services was largely determined by the aspects of time and cost. Polices to increase the number of service suppliers and to decrease the burdens perceived by actual supporters were strongly recommended.

The nursing-care insurance system started in April, 2000 in Japan. It was a kind of business opportunities for a lot of private entrepreneurs. They came into this care service business from another type of business rapidly. They opened the pay nursing homes with 24 hour nursing. However, the expense load of the nursing-care insurance system was large, local governments started controlling on total numbers of facilities with 24 hours nursing in 2006. So the group houses besides the pay nursing home without nursing care are paid to attention. The pay nursing home has been often managed by the nursing business. Most of the group houses are managed by community based NPO. This study is a case study by the visit and the interview form for one of the group houses. The group house "Shalom Tsukimino" in Kanagawa started operations comparatively at early time among them. Through the study we recognized that a lot of people were helping the NPO as a community service. So they can manage the group house and people who live there get the services with low cost. They show that they separate residence and nursing and use the community service efficiently. We can find a new direction in this case for the life in elderly.
Objectives : The purpose of this study was to examine the effects of an oral health promotion program. And suggest that education on oral health should be given by a professional. Methods : Sixty-four people over the age of 65 years were selected from three elderly care facility located in Gyeongsangbuk-do, A city. From June 7 to August 19, 2016, an oral health promotion program was given to two experimental groups (N=21 and 20) for a total of eight times. One program was conducted by a professional and the other by a non-professional although both were considered experts. Additionally, a control group(N=23) was a observed. Results : While both experimental groups showed a positive change from the oral health promotion program, the experimental group that was given the oral health promotion program by a professional showed more positive changes in oral cavity function. Conclusions : By using professionals, it is possible to provide a health promotion program that actively monitors the oral cavity of the elderly as well as provide elderly care services for oral hygiene. Appropriate elderly health policies and oral health business model for elderly are needed.
Purpose: The objectives of this study were to define basic data for developing silver services related to goods, medical care, finance, leisure and residence. Method: In this cross-sectional study, the subjects were 792 pre-elderly and 382 elderly. The data were collected 2005 by using a structured questionnaire from Aug. 19 to Sep. 5. Data were analyzed using descriptive statistics on the SPSS version 12.0 for windows. Result: The pre-elderly had the intention to use massage chair(36.6%), homecare service(29.8%), national pension(35.9%), sports(48.5%) and personal house(59.5%) in the ares of silver goods, medical care, finance, leisure and residence, respectively. The elderly wanted sphygmomanometer(38.7%), homecare service(27.7%), pocket money from their offspring (40.5%), sports(33.5%), and personal house(74.8%). They wanted manpower in the area of reforming of living place, leisure time and education, counseling of silver goods, and management of finance. Conclusion: Based on this study findings, the pre-elderly are thoroughly established for their future than the elderly. It is important to upgrade the silver services and to prepare the manpower for the pre-elderly and the elderly in the future of the aging society.
Purpose: The purpose of this study was to investigate the relationship between health knowledge and health promoting behavior in the elderly. Methods: The participants of this study were 114 men and women over 65 years at P-myeon, Yeongju-si, Gyeongsangbuk-do. Data were collected from March 1st to April 9th in 2011. The survey was carried out via the face to face interview using structured questionnaires. Results: Eighty nine percent of the participants responded that they were aware of the types of healthcare service. The most desirable service was 'long term care insurance system'. The average of health knowledge was 7.94 (${\pm}1.51$) out of 10 and average of health promoting behavior was 2.81 (${\pm}0.30$) out of 4. Health promoting behavior showed a positive correlation with health knowledge (r=.189, p=.044). Conclusion: The health promoting behavior in the elderly was related with health knowledge. Therefore, the enhancement of health knowledge is needed to improve health promoting behavior in the elderly. Education for self care and providing information for health care are needed for health maintenance and improvement in elderly. In addition, program development for providing health knowledge and nursing intervention for supporting health promoting behavior are in need.
This study was designed to search for the direction to develop housing service which assists the older people to live independently in their community and provides the systematization of community care. The subjects of the research include 533 older people between the age of 60 to 79 residing in Seoul and 84 welfare officials at a dong(in)-office and 67 social workers working at comprehensive social welfare institutions. The results were as follows: Both demanders and deliverers recognized that housing service for older people was necessary for community care, and, in particular, the deliverers had higher degree of necessity as people charged in welfare service. The most needed service was community alarm service which currently is proceeded partially for low-income and living-alone olders. It needed to expand the scope of the people to be served. It appeared that the need for inside safety measures rather than for housing space or structural problems. It requires less effort and cost for those who need simple repair, such as safety grips or slippery protection, and safety checks. Among financial support services, the need for tax benefits appeared the most necessary service for the older people. It suggested that more 'direct' financial support service is needed for the elderly people.
Purpose: Long-term care facilities have a responsibility to provide care service that enables residents to maintain their maximal functional capacity and quality of life. Also their needs must be reflected to the service programs. In oder to provide an adequate service, we should assess the elderly's physical, psychological and social health status and the need. In addition to this, the long-term care facilities must be defined clearly by the type of services. This study would contribute to conduct appropriate services in public long-term care policy for the older population in the future. This study would provide informations of long-term care facilities' services and older persons' needs for long-term care. Method: To achieve this objectives, this paper investigates the types, service programs of long-term care institutes and job descriptions of workers. The subjects were consisted of 150 long-term care institutes. 150 institutes of long-term care facilities were drawn from all over the country by a nonrandom, convenience sampling. The data were analyzed by frequency, percentage, $x^2$-test using SPSS program. The instruments of this study were self-reported questionnaires for long-term care institutes. The data were collected from March 1, 2004 to may 31, 2004. Results: Service programs of long-term care institutes were not enough for residents' demands. The job descriptions among nurse, social worker and physical therapist were not clearly defined. The nurse's main role was medication and checking vital sign(49.7%), that of social worker's was observation and supervising (31.2%). The most significant problems were lacking of diverse service programs for residents. Conclusion: Considering these findings and conclusion, the needs of long-term care services should be provided by individual physical and psychological level. And the professional manpower for elderly should be educated in multi disciplines.
Purpose: The purposes of this study were to explore the functional status of elderly residents and to analyze time use, and finally identify factors to predict nursing care needs in relation to functional status and health related variables. Methods: In this study a descriptive-correlational design was used. Functional status of participants was obtained through interviews, and nursing care time was examined using a 1 min time-motion study with a standardized instrument developed by Korea Long-Term Care Planning Committee (2005). Results: The mean total functional score was 65 (range 28-125) and mean total nursing care time was 144.15 min per day. There were significant positive relationships between total nursing care time, marital status, back pain, dementia, and vision impairment. Multiple regression analyses showed that a liner combination of number of illnesses, types of primary disease, ADL, IADL, cognitive function, nursing demand, and rehabilitation demand explained 42.8% of variance of total nursing time. ADL (${\beta}$=-.533) was the most significant predictor of nursing service need. Conclusion: Identifying factors that result in variations of service need has implications for adequate nursing service, estimation of optimum nurse to patient ratio, quality of care and patient safety.
In this study, the effect of chronic pain on the lives of elderly people in long-term care service was analyzed based on the mediated effect of depression. The research data was sampled from elderly people in long-term care services, 204 people participated. From mediated regression analysis, depression was the most relevant factor on the quality of life, followed by chronic pain. With chronic pain and depression as independent variables and quality of life as a dependent variable, depression was proved to have had a fully mediated effect on quality of life. The result of this study suggested that convergence of various support systems should be implemented for the elderly in long-term care services.

The purpose of this study is to examine the effect of care workers' assistance services on behavioral changes of the elderly. To achieve this, a survey was carried out on 100 elderly people, who receive the services from care workers in J area-based elderly care facilities, and elderly patient families. The results of this study are as follows. First, the effect of care workers' assistance services on the elderly's satisfaction of physical activities showed that the nursing and caregiving services have a significant positive effect on their satisfaction of physical activities(p<.05). Second, the effect of care workers' assistance services on the elderly's satisfaction of psycho-emotional activities showed that the caregiving services have a significant positive effect on their satisfaction of psycho-emotional activities(p<.01). Third, the effect of care workers' assistance services on the elderly's satisfaction of social cognition activities showed that the caregiving services have a significant positive effect on their satisfaction of social cognition activities(p<.01). Consequently, if the elderly receive the repetitive and continuous nursing and caregiving service in the limit that is forced to deteriorate the elderly's physical, psychological and cognitive development, and slow down their activities, there will be changes and promotion in their physical, psychological and cognitive behaviors. In other words, it is of use in that care workers' continuous assistance services can improve the elderly's life, driven by the changes of their physical, psychological and cognitive behaviors.
