Purpose: This study was done in order to examine the differences in gender respect to care burden, fatigue, and life contentment of family caregivers of elderly with dementia. Methods: The setting of the study was a community-based dementia care center providing daycare services. One hundred ninety four caregivers were recruited for the survey. A structured questionnaire was used which included demographic information, care burden, fatigue, and general contentment scales. Caregivers were classified into 4 groups by gender relationship with care recipients. Results: The majority of the caregivers were middle-aged and elderly. More than twice as many were female rather than male caregivers. The results showed that women caregivers expressed more fatigue than men overall (p < .05). In the gender relationships, a different combination of man-woman appeared to have more burdens than that of a woman-woman group (p < .05). For fatigue, the man caregiver-woman elderly group showed a higher fatigue score than a woman-man group (p < .05). However, there was no significant difference in life contentment among groups. Conclusion: These results suggest that caregiver's gender and gender relationship with elders could be considerable factors when the nurses make a plan in the community for dementia nursing management services.
Purpose: The purpose of this study is to investigate effects of an oral self-care program on oral dryness and oral health related quality of life in the elderly. Methods: The current research design isa nonequivalent control group protest-posttest design with two groups of elders from two different senior centers in D city. Tools for the research include paper of salivary absorption (WF41-1850, Whatman, Germany), 6 items for subjective oral dryness and OHIP-14 for oral health related to quality of life. The data were analyzed to test three hypotheses by using the SPSS 22.0 program. Results: The three hypotheses were significantly accepted. The subjective oral dryness score was lower in the experimental group than in the control group (t=-2.45, p=.022). Salivary absorption was higher in the experimental group than in the control group (t=3.83, p<.001).Oral health related quality of life was better in the experimental group than in the control group (t=-2.11, p=.044). Conclusion: The oral self-care program has appeared to be effective on oral dryness and an oral health related quality of life in the elderly. Therefore, it can be suggested that the oral self-care program is applicable as an intervention program motivating the elderly to maintain oral health.
Background: Informal care is increasingly recognized as placing a significant burden on the lives of family caregivers. The purpose of this study is to investigate factors related to family caregivers' burden with the community-dwelling disabled elderly under the long-term care insurance system, using the Stress Process Model developed by Pearlin (1990). Methods: Total 1,233 family caregivers with the disabled elderly, using the long-term care services in their home from May to June 2009, completed questionnaires finally. The questionnaire of this study consists of a total of 32 questions, including 11 questions related to background and context, 17 questions related to objective stressors, and 4 questions related to coping resourses. Family caregivers' burden is measured by the Korean Revised Caregiving Appraisal Scale (K-RCAS, Cronbach's alpha=0.86). To investigate factors related to family caregivers' burden, multiple regression analysis was conducted. Results: The average score of caregivers' burden was 22.0 (${\pm}6.12$). In multiple regression analysis, there were statistically significant factors affecting on the family caregivers' burden, that are related to background and context (region, living status, education level, relationship with beneficiary), objective stressors (duration of caregiving), coping resourses (caregiver's health status). Conclusion: This study found that family caregivers experience a considerable burden. The findings suggest that policies must be taken to relieve family caregivers of their duties temporarily, and to support them with counselling and education.

Background: This study is to investigate the association between the distribution of multimorbidity and length of stay and medical expenses among inpatients in a municipal hospital to achieve an integrated care setting. Methods: We used the exploratory factor analysis and the generalized estimating equation model to analyze the data from patients living in the northeast region of Seoul, who were hospitalized from January 2017 to December 2017 in a municipal hospital. Results: As a result of the factor analysis, seven types of multiple chronic diseases were classified. Among the elderly patients admitted to municipal hospitals, the burden of medical expenses was mainly influenced by the length of stay (B=310,719, p-value <0.0001), not the type of disease (all not significant). Length of stay were mainly due to psychiatric illness (factor 1: B=4.323, p-value <0.0001) related to the brain and metabolic diseases (factor 2: B=2.364, p-value=0.003). Conclusion: This study showed that the medical expenses of the elderly patients were largely due to prolonged hospitalization, not multimorbidity. Therefore, it is necessary to develop an integrated care paradigm strategy cope with the multimorbidity of the elderly in the community and to alleviate the socio-economic burden.
Purpose: The purpose of this study was to understand the coping process that family caregivers experience during the process of care, to grasp coping behaviors used to solve problems and, finally, to develop a substantive theory by analyzing the coping process. Method: The methodology of collecting and analyzing data used in this study followed the ground theory of Strauss and Corbin(1998). Data were collected through in-depth interviews with open-ended and descriptive questions about the subjects' coping experiences, which were audio-taped and transcribed. The survey was conducted between February 2000 and February 2001. The subjects of this study were 17 women. Result: The result of the study is as follows. The coping process of family caregivers while taking care of the demented elderly was found to have six stages: problem recognition: undertaking care: struggling: mental control: burden mediation; and acceptance. These stages proceed with reciprocal action and in cycle. Conclusion: The result of this study is helpful for developing effective and individual nursing strategies suitable for each coping stage. It is necessary to develop an assessment tool that can judge caregivers' coping stage based on the result of this study. Moreover, we need continuous study to practice nursing mediation and to analyze the change about nursing effect and family members' adaptation.
This study conducted research on the actual state of community-oriented services for elderly rural inhabitants and their desire related to them to develop a local community service network model suitable to the characteristics of rural longevity villages. The research was conducted on 906 elderly people over 65 living in 20 rural longevity villages through questionnaires assessing filming and economy, economic activity, health care, learning and leisure activities as well as asking their wants and needs relative to local community services. As a result, it was found rural elderly people showed a high desire for local community services such as health, transportation and economy activity. In addition, they were mainly cultivating farm products as their economic activity and showed a high demand in the future as well. Most were found to take a walk in the healthcare field and showed a high demand for health examinations, health education, health consulting, hot spring bathing and basking in the woods. Respecting learning, social and leisure activities, they were mostly found to watch TV and do house chores, and showed a high desire for village environment repair, traditional farm music, visiting and tourism. With the above results, it is expected that the desire of rural elderly for such services can be satisfied, and the development of a local community service network model suitable to the characteristic of a local community is recommended.

This study aims to empirically confirm the effect and impact of community care design research centered on domestic space and environment on health promotion, diagnosis treatment, disease management, rehabilitation, and mitigation through the year of publication and perspective. To this end, based on 1,227 space and environment design studies from 2,144 community care design research data conducted for about 20 years from 2002 to 2022, when care services began in earnest through the long-term care system for the elderly, SPSS 26.0 was used to create a 'Multi-layer Perceptron' artificial neural network structure model was predicted and neural network analysis was performed. Research Results First, as a result of checking studies in each field of health care by year, there is a significant difference with the number of studies related to health promotion being the highest. Second, the five perspectives are region, time, dimension, function, and content perspective. As a result of inputting these variables as independent variables and analyzing their importance in the artificial neural network, the function perspective had the most influence, followed by the region > content > dimension > time perspective.

The purpose of this study was to establish planning criteria and development of type for the senior citizen who lives alone in home for the elderly at rural and fishing villages. In order to take most suitable co-housing, this analysis is progressed various building types based on aging in place and aging in health; self care, nono care, service care and community care. This paper is to suggest collective housing with 10 types in large scale and extracted 19 types in detail scale. The basis of classification are location condition; aging in place, building condition, health care condition; aging in health and possession & operation condition. In viewpoint of dwelling welfare 'Home for the Elderly' is to concerned with systematic methodology and architectural typology for senior citizen who lives alone in the rural and fishing village.
Purpose: This study was aimed at constructing and examining the effects of a laughter therapy program for elderly patients in long-term care hospitals. Methods: A prospective, two-group quasi-experimental design was used and 50 patients (25 experimental and 25 control group patients) from two long-term care hospitals in Gyeonggido province, South Korea, were study participants. The experimental group received 24 laughter therapy sessions twice a week for 12 weeks, and the control group received laughter therapy after data collection. Data were collected between May 26 and August 17, 2014. Results: The level of cognitive function for the experimental group increased(t=3.27, p=.002). The level of forced vital capacity (t=2.78, p=.008) and forced expiratory volume in 1 second (t=4.94, p<.001) increased among members of the experimental group. Conclusion: These results indicated that the laughter program was effective in improving cognitive and pulmonary functions among older patients who were receiving long-term care in hospitals. This program could be used for community-based elderly patients.
This study examined the factors affecting family caregiver's preference for utilization of community care services among those who are caring for 65+ elderly parents, and aimed to show how social eldercare services would be settled in Korea. Help-seeking behavior model developed by Anderson and Newman(1973) was used to analyze the factors affecting their preference for utilizing the community care service among 283 family caregivers. Frequency, Chi-square, and Multinominal logistic analysis on SAS 6.12 was used. According to the results, about 90% of the family caregivers have preference for community and institute care services. In community care service, about a half comprise the preference with charge while the other without charge. However, about 90% of those for institute care service show their willingness to pay for the service. Also, a majority of caregivers like to rely on social eldercare service, rather than family as exclusively responsible, against long-term care for their elderly parents. Multinominal analysis demonstrates that use versus nonuse of community care services is primarily affected by predisposing factors(including age, carer-caree closeness, and familism) and need factors (including economic or psychological burden of eldercare, and additional role for family care). Enabling factors, such as family income level, economic support from other family members and siblings, and supportive care-helpers, are mainly associated with the preferences of free versus charge in service use. These findings provide some implications and suggestions for the development of social eldercare services in our aging society.
