호스피스.완화의료 표준교육 프로그램 시범교육의 효과 (The Effects of the Pilot Program in Standard Curriculum of Hospice and Palliative Care Education)

  • 목적: 호스피스 완화의료 전문인력을 위한 기본단계 교육과정으로 개발된 표준교육 프로그램 시범교육의 결과를 평가하기 위함 방법: 표준교육 프로그램을 개발하기 위하여 미국의 Education in Palliative and End-of-life Care (EPEC)교육을 벤치마킹하여 국내 실정에 맞게 수정 보완하였다. 표준교육 프로그램은 총 19개의 세부 주제로 구성하였으며, 강의와 쌍방향식 강의, 소그룹 토론, 역할극 등의 다양한 교육방법을 적용하도록 설계하였다. 개발한 표준교육 프로그램의 내용과 교육방법의 타당성을 검증하기 위하여 3개 주제를 선택하여 호스피스 고위과정을 수료한 23명을 대상으로 시범교육을 실시하였다. 결과: 표준교육 프로그램 시범교육 대상자들은 피교육자 중심의 교육방법과 현장에서 활용할 수 있는 교육 내용에 대하여 만족하였으며, 강의를 위주로 하는 기존의 교육보다 효과적인 교육이라고 평가하였다. 결론: 표준교육 프로그램 시범교육은 그 효과가 긍정적이었으며 호스피스 완화의료 교육의 표준화와 교육 방법의 발전에 기여할 것으로 기대된다. 앞으로 표준교육 프로그램을 성공적으로 실행하기 위해서는 교육자 (Trainer)를 위한 교육이 실행되어야한다.

  • 본 연구의 목적은 요양보호사의 직무스트레스 및 직무소진 완화와 치유를 목적으로 ACT 집단상담 프로그램을 개발하고 그 효과를 확인하는 데 있다. 이를 위해 프로그램의 목표 및 이론적 모형, 내용, 운영 및 평가를 구성하였으며 절차에 따라 적용하였다. 주요 연구결과는 다음과 같다. 첫째, 연구대상의 직무스트레스를 측정한 결과 프로그램 참여 집단에서 긍정적인 변화가 있었다. 둘째, 연구대상의 직무소진 수준을 측정한 결과 프로그램 참여 집단에서 긍정인 변화가 있었다. 셋째, 연구대상의 역할갈등 수준을 측정한 결과 프로그램 참여집단에서 긍정적인 변화가 있었다. 넷째, 연구대상의 역할과다 수준을 측정한 결과 프로그램 참여 집단에서 긍정적인 변화가 있었다. 다섯째, 연구대상의 역할모호 수준을 측정한 결과 프로그램 참여 집단에서 긍정적인 변화가 있었다. 본 연구는 실무적 시사점으로서 각종 요양보호사 교육과정에 ACT이론 및 실천방안을 내용에 포함시킴으로서 직무 소진과 직무 스트레스에 대한 대응력을 높이고 궁극적으로 요양보호사의 심리적 유연성을 높임으로서 일과 삶을 대하는 태도의 본질적 변화의 기회를 제공할 것에 대해 기술하였다.

가정간호 사업에 대한 의사, 간호사, 진료관련부서 직원 및 환자의 인식 비교 (A Study on Differences of Opinions on Home Health Care Program among Physicians, Nurses, Non-medical personnel, and Patients.)

  • The government has adopted a policy to introduce Home Health Care Program, and has established a three stage plan to implement it. The three stage plan is : First, to amend Article 54 (Nurses for Different Types of Services) of the Regulations for Implementing the Law of Medical Services; Second, to tryout the new system through pilot projects established in public hospitals and clinics; and third, to implement at all hospitals and equivalent medical institutions. In accordance with the plan, the Regulation has been amend and it was promulgated on January 9,1990, thus establishing a legal ground for implementing the policy. Subsequently, however, the Medical Association raised its objection to the policy, causing a delay in moving into the second stage of the plan. Under these circumstances, a study was conducted by collecting and evaluating the opinions of physicians, nurses, non-medical personnel and patients on the need and expected result from the home health care for the purpose of help facilitating the implementation of the new system. As a result of this study, it was revealed that: 1. Except the physicians, absolute majority of all other three groups - nurses, non-medical personnel and patients -gave positive answers to all 11 items related to the need for establishing a program for Home Health Care. Among the physicians, the opinions on the need for the new services were different depending on their field of specialty, and those who have been treating long term patients were more positive in supporting the new system. 2. The respondents in all four groups held very positive view for the effectiveness and the expected result of the program. The composite total of scores for all of 17 items, however, re-veals that the physicians were least positive for the- effectiveness of the new system. The people in all four groups held high expectation on the system on the ground that: it will help continued medical care after the discharge from hospitals; that it will alleviate physical and economic burden of patient's family; that it will offer nursing services at home for the patients who are suffering from chronic disease, for those early discharge from hospital, or those who are without family members to look after the patients at home. 3. Opinions were different between patients( who will receive services) and nurses (who will provide services) on the types of services home visiting nurses should offer. The patients wanted "education on how to take care patients at home", "making arrangement to be admitted into hospital when need arises", "IV injection", "checking blood pressure", and "administering medications." On the other hand, nurses believed that they can offer all 16 types of services except "Controlling pain of patients", 4. For the question of "what types of patients are suitable for Home Health Care Program; " the physicians, the nurses and non-medical personnel all gave high score on the cases of "patients of chronic disease", "patients of old age", "terminal cases", and the "patients who require long-term stay in hospital". 5. On the question of who should control Home Health Care Program, only physicians proposed that it should be done through hospitals, while remaining three groups recommended that it should be done through public institutions such as public health center. 6. On the question of home health care fee, the respondents in all four groups believed that the most desireable way is to charge a fixed amount of visiting fee plus treatment service fee and cost of material. 7. In the case when the Home Health Care Program is to be operated through hospitals, it is recommended that a new section be created in the out-patient department for an exclusive handling of the services, instead of assigning it to an existing section. 8. For the qualification of the nurses for-home visiting, the majority of respondents recommended that they should be "registered nurses who have had clinical experiences and who have attended training courses for home health care". 9. On the question of if the program should be implemented; 74.0% of physicians, 87.5% of non-medical personnel, and 93.0% of nurses surveyed expressed positive support. 10. Among the respondents, 74.5% of -physicians, 81.3% of non-medical personnel and 90.9% of nurses said that they would refer patients' to home health care. 11. To the question addressed to patients if they would take advantage of home health care; 82.7% said they would if the fee is applicable to the Health Insurance, and 86.9% said they would follow advises of physicians in case they were decided for early discharge from hospitals. 12. While 93.5% of nurses surveyed had heard about the Home Health Care Program, only 38.6% of physicians surveyed, 50.9% of non-medical personnel, and 35.7% of patients surveyed had heard about the program. In view of above findings, the following measures are deemed prerequisite for an effective implementation of Home Health Care Program. 1. The fee for home health care to be included in the public health insurance. 2. Clearly define the types and scope of services to be offered in the Home Health Care Program. 3. Develop special programs for training nurses who will be assigned to the Home Health Care Program. 4. Train those nurses by consigning them at hospitals and educational institutions. 5. Government conducts publicity campaign toward the public and the hospitals so that the hospitals support the program and patients take advantage of them. 6. Systematic and effective publicity and educational programs for home heath care must be developed and exercises for the people of medical professions in hospitals as well as patients and their families. 7. Establish and operate pilot projects for home health care, to evaluate and refine their programs.

요양보호사의 직업의식과 영향요인 (The Factors Influencing of Professional Consciousness of Long-term Care Workers)

  • 본 연구의 목적은 노인장기요양보험제도로 신설된 요양보호사의 직업의식 정도와 영향요인을 파악하여 요양보호사의 직업의식을 높이기 위한 간호중재 개발에 기초자료를 제공하는데 있다. 서술적 연구로써 D시, C도 G시, C도 C시, K도 O시에 소재하고 있는 요양원 3곳, 재가복지센터 3곳, 요양병원 3곳의 요양보호사 185명을 대상으로 2009년 11월 12일에서 4주 동안 설문지를 이용하여 자료수집 하였고, 서술적 통계, t-test, ANOVA, Pearson's correlation coefficients, Multiple regression으로 분석하였다. 연구결과, 요양보호사의 평균연령은 48.1세, 대다수 여성(95.1%)이며, 기혼자로서 배우자가 있고(79.5%), 고등학교 이상 졸업자(76.8%)가 많았다. 요양보호사의 자기효능 정도는 평균점수 3.68점, 소명의식 3.41점, 외적 통제위 성격 3.23점으로 보통 이상이며, 내적 통제위 성격은 4.12점으로 내적 통제위 성격이 강한 것으로, 교육훈련 유효성은 3.71점, 직업의식은 3.97점으로 다소 높은 것으로 나타났다. 직업의식과 관련 요인들의 관계에서, 직업의식과 내적 통제위 성격은 낮은 정 상관관계(r=.378, p=.000)를, 외적 통제위 성격은 낮은 역 상관관계(r=-.356, p=.000), 자기효능(r=.420, p=.000), 소명의식(r=.636, p=.000), 교육훈련 유효성(r=.441, p=.000)은 보통 정도의 정 상관관계를 보였다. 요양보호사의 직업의식에 영향을 미치는 요인으로는 소명의식(B=.329, p=.000)이 가장 높은 영향을 주는 것으로 나타났으며 교육훈련 유효성(B=.250, p=.000), 내적 통제위 성격(B=.216, p=.000), 외적 통제위 성격(B=-.165, p=.002), 요양보호사 선택시 자기 특성 고려(B=.207, p=.004) 순으로 전체 설명력은 57.5%로 나타났다. 추후 요양보호사에게 소명의식 제고 교육을 위한 체계적인 교육과 내적 통제위 성격 강화훈련, 요양보호사의 역할과 업무를 정확히 규명 하는 것이 필요하며 이러한 요인들을 활용하여 직업의식을 높일 수 있는 교육과 연구를 제언한다.

공공도서관 이용자의 소비자건강정보(CHI) 리터러시 향상을 위한 교육프로그램 개발 및 성과측정연구 (The Development and Performance Measurements of Consumer Health Information(CHI) Educational Programs to Improve Health Literacy among Public Library Users)

  • 국민들의 건강정보에 대한 관심이 증가하고 있으나 정확하고 신뢰할 수 있는 건강정보원을 탐색할 수 있는 이용자의 비율은 매우 낮은 것으로 분석되고 있다. 일반 이용자들과 최접점에 있는 공공도서관은 그들의 건강정보활용능력을 향상시키기 위한 교육프로그램을 제공함으로써, 공공도서관 이용자들이 건강정보탐색능력과 분석능력, 정보원의 정확성과 신뢰성을 판단할 수 있는 능력, 올바른 건강관련 의사결정을 내릴 수 있는 능력을 갖출 수 있도록 해야 한다. 이에 본 연구에서는 국내 공공도서관 이용자에게 적합한 건강정보활용 교육프로그램을 개발하고 이를 현장에 적용함으로써 그 효과를 측정하고자 하였다. 그 결과, 공공도서관 이용자의 CHI관련 용어에 대한 친숙도 증가, 건강관련 정보를 인터넷에서 찾을 수 있는 능력 향상, 건강관련 사이트 및 정보원에 대한 지식 향상 등의 측면에서 유의한 수준의 교육효과가 나타났다. 또한 공공도서관에서 다양한 건강정보원에 대한 세미나를 제공해 주기를 요구한다고 응답하였으며, CHI관련 교육을 공공도서관의 정규교육과정으로 포함할 것을 요구하였다. 그러나 본 연구는 공공도서관 이용자들을 대상으로 하여 수행된 국내 최초의 CHI관련 교육프로그램으로서 강사진 구성, 주제의 다양성, 방법론 등에서 한계가 있었다. 향후 보다 다양한 CHI관련 이용자교육프로그램이 개발되어 현장에 적용될 수 있기를 바란다.

Project Approach in the Organization of Scientific and Methodological Work by Applying Information Technology in Higher Education Institutions

  • The article is devoted to studying the development of scientific and methodological work and its impact on the quality of students' vocational training in higher pedagogical education institutions by applying information technology. The article aim is to development of the organizational methodological support and pedagogical diagnostics of the effectiveness of the project «Modelling scientific and methodological work in a higher education pedagogical institution by applying information technology » realization in the framework of increasing the level of scientific and methodological work in a higher education pedagogical institution as a factor contributing to enhancing the quality of pedagogical education. The research program of the project activity envisages stating and substantiating the problem of scientific and methodological work by applying information technology in the framework of increasing the level and quality of educational activities in a higher pedagogical education institution through the implementation of the project approach, developing a model for the system of organizational and methodological support of the project implementation as well as monitoring the process and evaluating the results of the project implementation in terms of developing teachers' scientific, methodological, information competency and enhancing students' progress in studying. The set of criteria were developed to evaluate the level of formation of scientific and methodological competency as a result of implementing the project for the development of scientific and methodological work. The scientific and methodological work by applying information technology in the academy was carried out in accordance with the following principles: systematic character, consistent diagnostics, practical focus, scientific organizational and methodological support.

중학생의 규칙적인 운동이 기초체력에 미치는 영향 (Effects of Regular Exercises on Basic Physical Fitness for Middle School Students)

  • The main purpose of this thesis was to provide a basis to investigate the effectiveness of regular exercises on basic physical fitness for middle school students. 40 second graders from a middle school were chosen randomly and divided into 4 groups-wrestling group, basketball group, jogging group, and control group. The first three groups have been trained 4 times per week for 18 weeks after class to be compared with the comparison group. Pre and post measurements of physical fitness of the subjects were processed by means of the SPSS program. The average and the standard deviation of the measurements of each group were covered with the ANOVA test, according to the periods and the types of training. Duncan's Multiple Range Test was performed to identify the significance of the relationship between training period and improvement in basic sports ability. The conclusions as follows; 1. In the 50 meter sprint, only the wrestling group showed statistical significance, while the other groups showed gradual progress in general. 2. In the standing long jump, there is no group that showed any improvement. 3. In the forward flexion of the trunk, no significance was to be found. 4. In 30-second sit-ups, the wrestling group showed statistical significance. The wrestling group made greater progress than the other groups, and the basketball group and the jogging group also showed rather great progress. 5. In push-ups, statistical significance was found in the groups of wrestling, basketball, and jogging. And the wrestling group came in first, the basketball group second, and the jogging group third on the progress level. 6. In the distance run, the jogging group showed statistical significance and the jogging group made considerable progress. Steady improvement was to be found in the groups of wrestling and basketball.

  • 학술연구정보서비스는 해당 정보를 필요로 하는 연구자를 대상으로 하기 때문에 대중적이지 않으며 정확히 원하는 정보를 찾아 제공하기 위한 정보 특성별 고유한 검색방법을 제공하므로 일반적인 웹 검색 및 인터넷 포털 서비스와 달리 생소하고 어려운 편이다. 따라서 학술연구정보서비스 제공자들은 신규 고객을 유치하고 서비스를 원활히 이용하도록 하는데 더 많은 노력이 필요하다. 본 연구에서는 일반적인 웹 검색 및 인터넷 포털 서비스와 달리한국과학기술정보연구원의 과학기술정보 통합서비스 NDSL에 대한 2008년 방문교육 결과를 통해 온라인 정보서비스의 방문교육 효과를 알아보고자 하였다.

디지털 치료제가 뇌성마비 아동의 균형 및 보행에 미치는 효과: 체계적 고찰 (Effect of digital therapies on balance and gait in children with cerebral palsy: A systematic review)

  • Background: Digital therapeutics are software medical devices that provide evidence-based treatments to prevent, manage, and treat disease. Digital therapies have recently been shown to be effective in motivating children with cerebral palsy as a tool in neuropsychological therapy. Digital therapies improve postural control, balance and gait in children with cerebral palsy. Therefore, this study aims to investigate the effects of digital therapies on balance and gait in children with cerebral palsy and to provide guidelines for prescribing digital therapies for children with cerebral palsy. Design: A Systematic Review Methods: This study searched for English-language articles published in medical journals from January 2000 to July 2023 using PubMed and MEDLINE based on the year of initiation of the digital therapy. The search terms used in the study were 'digital technology' OR 'digital therapeutic' OR 'mobile application' OR 'mobile health' OR 'virtual reality' OR 'game' AND 'cerebral palsy', 'balance' 'gait' as the main keywords. The final article was assigned an evidence level and a Physiotherapy Evidence Database (PEDro) score to assess the quality of clinical trials studies. Results: The digital therapies applied to improve balance and gait in children with cerebral palsy are game-based virtual reality training and the Nintendo Wii Fit program. Both digital therapy interventions had a significant effect on improving balance in children with cerebral palsy, and virtual reality training significantly improved balance and gait. However, there were no significant improvements in balance and gait within two weeks of treatment, regardless of the type of digital intervention. Conclusion: The study suggests that this data will be important in building the evidence base for the effectiveness of digital therapies on balance and gait in children with cerebral palsy and in advancing clinical protocols.
