Effects of Thoracic Mobility Exercise on the Range of Motion, Pain, Disability Index and Quality of Life in Middle-Aged Women with Chronic Back Pain (흉추 가동성 운동이 만성 요통 중년 여성 환자의 가동 범위, 통증, 장애 지수 및 삶의 질에 미치는 효과)

  • Background: This study investigated the effects on the range of motion, pain, disability index, and quality of life when applying thoracic mobility exercise in middle-aged women with chronic back pain. Design: pretest-posttest control group design Methods: The study subjects were 32 patients with chronic back pain of 3 months or more among middle-aged women divided 2 groups. All groups were tested thrice a week for 30 minutes per session for four weeks. Each subject was evaluated using the thoracic rotation Range of Motion, the numerical pain rating scale and pressure pain threshold, Disability Index(Oswestry Disability Index), the Quality of Life(SF-36) before and after treatment. Results: As a result of the study, the range of motion at T1 and T2 of the exercise group to which thoracic mobility exercise was applied was significantly increased in both left and right rotations(p<0.05). In the evaluation of pain, the number pain rating scale(NPRS) score decreased significantly(p<0.05), and the pressure pain threshold was significantly increased (p<0.05), but there was no significant difference from the control group. In the evaluation of the disorder index, the Oswestry disorder index decreased significantly after the experiment (p<0.05), and there was a significant difference in group comparison with the control group (p<0.05). In quality of life, there was a significant increase in PCS of SF-36(p<0.05), but there was no significant difference from the control group. Conclusion: Through this study, thoracic mobility exercise can be used as an effective exercise therapy intervention method to improve and promote physical factors of range of motion, pain and disability index, and physical components of quality of life when treating middle-aged women with chronic back pain in the future.

Parenchymal-sparing anatomical hepatectomy based on portal ramification of the right anterior section: A prospective multicenter experience with short-term outcomes

  • Backgrounds/Aims: Parenchymal-sparing anatomical hepatectomy (Ps-AH) based on portal ramification of the right anterior section (RAS) is a new technique to avoid unnecessarily transecting too much liver parenchyma, especially in cases of major anatomical hepatectomy. Methods: We prospectively assessed 26 patients with primary hepatic malignancies having undergone major Ps-AH based on portal ramification of the RAS from August 2018 to August 2022 (48 months). The perioperative indications, clinical data, intra-operative index, pathological postoperative specimens, postoperative complications, and follow-up results were retrospectively evaluated. Results: Among the 26 patients analyzed, there was just one case that had intrahepatic cholangiocarcinoma The preoperative level of α-Fetoprotein was 25.2 ng/mL. All cases (100%) had Child-Pugh A liver function preoperatively. The ventral/dorsal RAS was preserved in 19 and 7 patients, respectively. The mean surgical margin was 6.2 mm. The mean surgical time was 228.5 minutes, while the mean blood loss was 255 mL. In pathology, 5 cases (19.2%) had microvascular invasion, and in the group of HCC patients, 92% of all cases had moderate or poor tumor differentiation. Six cases (23.1%) of postoperative complications were graded over III according to the Clavien-Dindo system, including in three patients resistant ascites or intra-abdominal abscess that required intervention. Conclusions: Parenchymal-sparing anatomical hepatectomy based on portal ramification of the RAS to achieve R0-resection was safe and effective, with favorable short-term outcomes. This technique can be used widely in clinical practice.

Assessment of Treatment Response in Patients With Severe Asthma Using Visual and Quantitative Analysis of Chest CT

  • Objective: To evaluate the role of visual and quantitative chest CT parameters in assessing treatment response in patients with severe asthma. Materials and Methods: Korean participants enrolled in a prospective multicenter study, named the Precision Medicine Intervention in Severe Asthma study, from May 2020 to August 2021, underwent baseline and follow-up chest CT scans (inspiration/expiration) 10-12 months apart, before and after biologic treatment. Two radiologists scored bronchiectasis severity and mucus plugging extent. Quantitative parameters were obtained from each CT scan as follows: normal lung area (normal), air trapping without emphysema (AT without emph), air trapping with emphysema (AT with emph), and airway (total branch count, Pi10). Clinical parameters, including pulmonary function tests (forced expiratory volume in 1 s [FEV1] and FEV1/forced vital capacity [FVC]), sputum and blood eosinophil count, were assessed at initial and follow-up stages. Changes in CT parameters were correlated with changes in clinical parameters using Pearson or Spearman correlation. Results: Thirty-four participants (female:male, 20:14; median age, 50.5 years) diagnosed with severe asthma from three centers were included. Changes in the bronchiectasis and mucus plugging extent scores were negatively correlated with changes in FEV1 and FEV1/FVC (ρ = from -0.544 to -0.368, all P < 0.05). Changes in quantitative CT parameters were correlated with changes in FEV1 (normal, r = 0.373 [P = 0.030], AT without emph, r = -0.351 [P = 0.042]), FEV1/FVC (normal, r = 0.390 [P = 0.022], AT without emph, r = -0.370 [P = 0.031]). Changes in total branch count were positively correlated with changes in FEV1 (r = 0.349 [P = 0.043]). There was no correlation between changes in Pi10 and the clinical parameters (P > 0.05). Conclusion: Visual and quantitative CT parameters of normal, AT without emph, and total branch count may be effective for evaluating treatment response in patients with severe asthma.

The Effects of Pilates Breathing on Trunk Muscle Activity and Balance in Adult Females (성인여성에게 필라테스 호흡을 동반한 운동이 몸통의 근활성도 및 균형에 미치는 영향)

  • Purpose: The purpose of this study was to assess the effects of breathing techniques on trunk muscle activity and balance during Pilates reformer footwork exercises, comparing results both within and between groups before and after the intervention. Methods: Thirty-one adult women over the age of 20 were selected as subjects for this study. They were divided into a Pilates breathing group (n = 15) and a general breathing group (n = 16) using a randomized control group study design. A surface electromyogram was used to measure muscle activity within and between the groups before and after the reformer footwork exercise. Static balance measurements were taken while standing on two legs, and dynamic balance measurements were taken while standing on one leg. All measurements were taken three times, and the average values were used for analysis. Results: The results of the study showed that muscle activity increased with significant differences in the external oblique and transverse abdominal muscles after exercise in the pre-post comparison within the Pilates breathing group (p < 0.05). In the between-group comparison, there was a significant difference in the increase in muscle activity of the external oblique and transverse abdominal muscles in the Pilates breathing group (p < 0.05). In the pre-post comparison of static and dynamic balance within the Pilates breathing group, there was a significant increase (p < 0.05) after exercise. The Pilates breathing group also showed a significant increase even in the between-group comparison (p < 0.05). Conclusion: This study confirmed that reformer footwork exercise accompanied by Pilates breathing has positive effects on muscle activity and static balance ability of trunk muscles in adult women. Therefore, reformer footwork exercise accompanied by Pilates breathing can be presented as an effective exercise method to increase trunk stability and balance ability through the simultaneous activity of the trunk muscles.

  • Background and Objectives: UltimasterTM, a third-generation sirolimus-eluting stent using biodegradable polymer, has been introduced to overcome long term adverse vascular events, such as restenosis or stent thrombosis. In the present study, we aimed to evaluate the 12-month clinical outcomes of UltimasterTM stents in Korean patients with coronary artery disease. Methods: This study is a multicenter, prospective, observational registry across 12 hospitals. To reflect real-world clinical evidence, non-selective subtypes of patients and lesions were included in this study. The study end point was target lesion failure (TLF) (the composite of cardiac death, target vessel myocardial infarction [MI], and target lesion revascularization [TLR]) at 12-month clinical follow up. Results: A total of 576 patients were enrolled between November 2016 and May 2021. Most of the patients were male (76.5%), with a mean age of 66.0±11.2 years. Among the included patients, 40.1% had diabetes mellitus (DM) and 67.9% had acute coronary syndrome (ACS). At 12 months, the incidence of TLF was 4.1%. The incidence of cardiac death was 1.5%, MI was 1.0%, TLR was 2.7%, and stent thrombosis was 0.6%. In subgroup analysis based on the presence of ACS, DM, hypertension, dyslipidemia, or bifurcation, there were no major differences in the incidence of the primary endpoint. Conclusions: The present registry shows that UltimasterTM stent is safe and effective for routine real-world clinical practice in non-selective Korean patients, having a low rate of adverse events at least up to 12 months.

Comparison of endoscopic ultrasound-guided drainage and percutaneous catheter drainage of postoperative fluid collection after pancreaticoduodenectomy

  • Backgrounds/Aims: Postoperative fluid collection is a common complication of pancreatic resection without clear management guidelines. This study aimed to compare outcomes of endoscopic ultrasound (EUS)-guided trans-gastric drainage and percutaneous catheter drainage (PCD) in patients who experienced this adverse event after pancreaticoduodenectomy (PD). Methods: Demographic and clinical data and intervention outcomes of 53 patients who underwent drainage procedure (EUS-guided, n = 32; PCD, n = 21) for fluid collection after PD between January 2015 and June 2019 in our tertiary referral center were retrospectively analyzed. Results: Prior to drainage, 83.0% had leukocytosis and 92.5% presented with one or more of the following signs or symptoms: fever (69.8%), abdominal pain (69.8%), and nausea/vomiting (17.0%). Within 8 weeks of drainage, 77.4% showed a diameter decrease of more than 50% (87.5% in EUS vs. 66.7% in PCD, p = 0.09). Post-procedural intravenous antibiotics were used for an average of 8.1 ± 4.3 days and 12.4 ± 7.4 days for EUS group and PCD group, respectively (p = 0.01). The EUS group had a shorter post-procedural hospital stay than the PCD group (9.8 ± 1.1 vs. 15.8 ± 2.2 days, p < 0.01). However, the two groups showed no statistically significant difference in technical or clinical success rate, reintervention rate, or adverse event rate. Conclusions: EUS-guided drainage and PCD are both safe and effective methods for managing fluid collection after PD. However, EUS-guided drainage can shorten hospital stay and duration of intravenous antibiotics use.

  • The rapid advancement of artificial intelligence and machine learning technologies is driving innovation across various industries, with natural language processing offering substantial opportunities for the analysis and processing of text data. The development of effective text classification models requires several complex stages, including data exploration, preprocessing, feature extraction, model selection, hyperparameter optimization, and performance evaluation, all of which demand significant time and domain expertise. Automated machine learning (AutoML) aims to automate these processes, thus allowing practitioners without specialized knowledge to develop high-performance models efficiently. However, current AutoML frameworks are primarily designed for structured data, which presents challenges for unstructured text data, as manual intervention is often required for preprocessing and feature extraction. To address these limitations, this study proposes a web-based AutoML platform that automates text preprocessing, word embedding, model training, and evaluation. The proposed platform substantially enhances the efficiency of text classification workflows by enabling users to upload text data, automatically generate the optimal ML model, and visually present performance metrics. Experimental results across multiple text classification datasets indicate that the proposed platform achieves high levels of accuracy and precision, with particularly notable performance when utilizing a Stacked Ensemble approach. This study highlights the potential for non-experts to effectively analyze and leverage text data through automated text classification and outlines future directions to further enhance performance by integrating Large language models.

Clinical Applicability and Safety of Conventional Frame-Based Stereotactic Techniques for Stereoelectroencephalography

  • Objective : Stereoelectroencephalography (SEEG) is increasingly being recognized as an important invasive modality for presurgical evaluation of epilepsy. This study focuses on the clinical and technical considerations of SEEG investigations when using conventional frame-based stereotaxy, drawing on institutional experience and a comprehensive review of relevant literature. Methods : This retrospective observational study encompassed the surgical implantation of 201 SEEG electrodes in 16 epilepsy patients using a frame-based stereotactic instrument at a single tertiary-level center. We provide detailed descriptions of the operative procedures and technical nuances for bilateral and multiple SEEG insertions, along with several illustrative cases. Additionally, we present a literature review on the technical aspects of the SEEG procedure, discussing its clinical implications and potential risks. Results : Frame-based SEEG electrode placements were successfully performed through sagittal arc application, with the majority (81.2%) of cases being bilateral and involving up to 18 electrodes in a single operation. The median skin-to-skin operation time was 162 minutes (interquartile range [IQR], 145-200), with a median of 13 minutes (IQR, 12-15) per electrode placement for time efficiency. There were two occurrences (1.0%) of electrode misplacement and one instance (0.5%) of a postoperative complication, which manifested as a delayed intraparenchymal hemorrhage. Following SEEG investigation, 11 patients proceeded with surgical intervention, resulting in favorable seizure outcomes for nine (81.8%) and complete remission for eight cases (72.7%). Conclusion : Conventional frame-based stereotactic techniques remain a reliable and effective option for bilateral and multiple SEEG electrode placements. While SEEG is a suitable approach for selected patients who are strong candidates for epilepsy surgery, it is important to remain vigilant concerning the potential risks of electrode misplacement and hemorrhagic complications.

A Research Synthesis of Cooperative Learning including Students with disabilities in Korea (장애학생 협동학습 중재연구 고찰 및 효과에 관한 분석)

  • Purpose: The purpose of this study was to examine the cooperative learning studies including students with disabilities and analyze the effects of the interventions used for improving students' academic achievements, social skills, and attitude. Method: According to the research purpose, the study searched the researches related to cooperative learning and students with disabilities, specifically published from 2010 to 2018. Results: Total 19 of studies were searched. (semi)Group research design was used in 12 studies out of 19. Single subject research was used in 7 of 19. Among them, nine were cooperative studies for students with disabilities to improve their academic skills, while nine were for students with disabilities to improve their social skills and learning attitude. A single-subject studies on the cooperative learning of students with disabilities to improve their academic and social skills and learning attitudes showed large effect size. It was found that (semi)group design studies were categorized by the type of intervention, group size, and each size was different. Conclusion: According to the results, the cooperative learning was effective for not only students with disabilities but also for learners' academic achievement. Therefore, it can be suggested that educators should provide lots of opportunities of cooperative learning with smallgroup setting. As well, considerations for providing cooperative learning including students with disabilities and directions for future study are discussed.

Effects of Metacognition, Digital Literacy and Digital Addiction on Academic Achievement among Nursing Students (간호대학생의 메타인지, 디지털 리터러시, 디지털 과몰입이 학업성취도에 미치는 영향)

  • This study identifies the factors that influence metacognition, digital literacy and digital addiction on academic achievement in nursing students. This study was undertaken to determine the influence of metacognition, digital literacy and digital addiction on academic achievement in nursing students. Using a structured questionnaire, data on 198 participant characteristics, metacognition, digital literacy, digital addiction and academic achievement were collected from May 17 to May 27, 2024. Data were analyzed with the SPSS/WIN 26.0 program for descriptive statistics using descriptive statistics, pearson's correlation coefficient, and stepwise multiple regression. As a result of the study metacognition was 3.67±0.61 points, digital literacy was 3.66±0.70 points, digital addiction was 2.85±0.67 points, and academic achievement was 3.23±0.59 points. academic achievement was a significant positive correlation with metacognition (r=.610, p<.001), digital literacy (r=.468, p<.001). The determining factors affecting academic achievement in nursing students were followed by metacognition (β=.518, p <.001) and digital literacy (β=.196, p =.003). The explanatory power of these factors was about 40.7%. In order to improve the academic achievement of nursing students through the results of this study, it is necessary to develop an effective nursing intervention program that can increase metacognition.
