A Study on Improvement of Safety Management System of Publicly Used Establishments Through Analysis of the UK Regulatory Regime (영국 안전규제체계의 분석을 통한 국내 다중이용업소 안전관리체계의 개선방안 연구)

  • Purpose: In response to the critical risk of fire due to the characteristic of Publicly used establishments(hereinafter referred to as PUE), 'Special Act on the Safety Control of PUE' was enacted in 2006 and is still in operation. However, in spite of numerous revisions so far, still there are problems to be resolved. This study analyses the regulatory regime of fire safety in UK to find measures which could fundamentally improve the safety management of PUE. Method: This study compares and analyses the safety management system of PUE in Korea and the case of the UK by using the comparative research method. Result: As a result of the qualitative analysis, some noticeable systems and concepts of the UK regulatory regime have been discovered(e.g. 'Responsible person', 'Fire risk assessment', 'Fire safety audit', etc.) and consequently, 'Proposal for the Fire Safety Management System for PUE' is designed based on the findings from examples in the UK and drawbacks of safety management of PUE. Conclusion: This study proposes the way to improve the safety management of PUE into a more rational and effective system by analysing the case of the UK, which reorganised the fire safety management to the private sector centered, in which the fire authority minimizes intervention.

  • The Fourth Industrial Revolution foreshadows radical changes in our lives. In the era of the fourth industrial revolution called the digital revolution, individualized learning based on ubiquitous learning is emphasized. The contents of learning will be centered on procedural knowledge rather than narrative knowledge, and fusion education in which boundaries between learning domains are broken down will be achieved. First of all, learners in the fourth industrial revolution era should have critical thinking and problem solving abilities. Metacognition based on self-control and cognitive flexibility is important for effective self-directed and active learning. Creativity-based collaborative activities, social vision skills, and social and emotional skills are also important competencies. Therefore, in order to provide individualized learning contents to learners in the fourth industrial revolution era, they should be transformed into learning paradigm based on personal characteristics such as learners' self-efficacy, interest, curiosity and creativity. In addition to this, evaluation forms should be diversified according to changing teaching and learning methods. In order to cultivate teachers to lead such educational innovation, it is necessary to reconsider the teaching capacity. Teachers should be able to construct creative lessons by skillfully exploiting technology in future learning environments. In addition to this, it should also have the ability to collaborate and cognitive flexibility to converge with other academic disciplines. Along with these discussions, we proposed the need for policy intervention along with changes in education.

The immediate effect of electrical muscle stimulation on rectus femoris thickness during resisted knee extension exercise (전기근육자극을 적용한 무릎 폄 저항운동 시 넙다리곧은근의 두께 변화에 미치는 즉각적 효과)

  • The purpose of this study was to compare the immediate effect of EMS (electrical muscle stimulation) on rectus femoris thickness during resisted knee extension exercise in healthy adults. This experiment was conducted on 20 healthy adults as pretest-posttest nonequivalent one group design. The subject's 1RM of both knee extension was measured indirectly using an elastic band, and the knee extension resistance exercise using an elastic band was applied to high intensity (80% of 1RM) on the right leg and low intensity (50% of 1RM with EMS) on the left leg, which were applied with 5 sets. Muscle measurements were performed on the rectus femoris (1/2 site, 1/4 site) using ultrasonography before and after exercise. There was a statistically significant difference on the thickness of the rectus femoris in low intensity exercise of the elastic band applied with EMS between pre-test and post-test (p<.05). The results of this study showed that elastic band low intensity exercise combined with EMS had an immediate effective in increasing the thickness of rectus femoris. Based on this result, it is also necessary to verify the effectiveness of intervention methods incorporating low-intensity resistance exercises applying EMS to elderly who cannot exercise high-intensity resistance training in the future, and to develop exercise programs for various body parts.

Information Seeking and Behavior Change for the Smoking Cessation of College Students Utilizing Mobile Applications (대학생들의 모바일 앱을 이용한 금연정보탐색과 행위변화)

  • The present study aimed to investigate the information needs and information seeking behavior of consumers who attempted to quit or maintain the ceasing of smoking and analyze the effects of the health information obtained at different stages. In particular, we examined how consumers use mobile health applications(health apps) as aids to change unhealthy behaviors and how their use of health apps influence health behavior changes. For 7 months from December 2017 to July 2018, the researchers observed changes in smoking behaviors of college students who use smoking-cessation apps and conducted face-to-face interviews. Regarding the effects of smoking-cessation apps, the participants in the action stage reported that they were encouraged to quit smoking by visualized information such as the number of days of smoking cessation, change of health status, and the saving of money due to smoking cessation. The participants in the maintenance stage highlighted that smoking cessation apps were helpful in recognizing the importance and achievement of smoking cessation by sharing experiences and social support with other attempters in the app community. The study provided theoretical implications in the field of information behavior in that it identified the particular types of information needs and information-seeking behavior of the consumers who were using mobile apps in their behavior modification process. In addition, those findings can contribute to designing the contents of the smoking cessation apps that reflect the information needs of those who attempt to cease smoking and further suggest practical insights to health information services that promote effective information intervention strategies in health behavior change.

The Association between Regular Physical Activity Types And Metabolic Syndrome in Fertile Women (가임기 여성의 규칙적 운동 및 운동종류에 따른 대사증후군 간의 관련성)

  • The purpose of this study was to identify the association between regular physical activity (PA) types and metabolic syndrome (MetS) in fertile women. Data were obtained from the seventh Korea National Health and Nutrition Examination Survey (KNHANES VII-3, 2018). The sample consisted of 4,172 fertile women over 19 years of age. The data were analyzed using an independent t-test, χ2 test, and multiple logistic regression analysis for examining the association of regular PA and MetS. The prevalence of MetS in fertile women was 3.1% (n =128), and 1,972 fertile women (47.2%) practiced regular PA. Compared to the non-regular PA group, regular PA resulted in a significantly lower systolic blood pressure (p =.002), lower waist circumference (p <.001), lower fasting blood sugar (p =.007), and higher high-density lipoprotein cholesterol (p <.001). After controlling the confounding variables, multiple logistic regression showed that the regular PA group (odds ratio 0.59, 95% confidence interval 0.35-0.98, p =.040) and the regular muscle-strengthen PA group (odds ratio 0.34, 95% confidence interval 0.14-0.80, p =.013) were associated with a decreased risk of MetS. Therefore, regular PA, including muscle-strengthen PA should be considered to develop effective MetS intervention in fertile women.

Review of Clinical Research about Acupuncture for Spastic Hmiplegia after Stroke - Research on China Academic Journal (뇌졸중 후 경련성 편마비의 침치료에 대한 최신 임상 연구 동향 - CAJ 검색을 중심으로)

  • To analyze the clinical efficacy of acupuncture for spastic hemiplegia after stroke, this study was accomplished by considering Randomized Controlled Trials. We searched for papers that performed acupuncture for spastic hemiplegia after stroke in the China Academic Journal of the China National Knowledge Infrastructure, from January 1, 2017 to June 30, 2020. In total, 23 reports were included in this review. There are 18 studies conducted with more than 50 subjects and less than 100 subjects. The largest number of treatment was 20 times. 28days(4weeks) was the largest number of treatment period, which accounted 10 studies. The most frequently used evaluation index was The Fugl-Meyer Assessment(FMA) and Clinical Efficacy, each used 21 times and 17 times. The most frequently used acupuncture point was LI3, which was used 13 times. The retention time was 30 minutes and 11 studies were conducted. Western medicine treatment was the most common control group in 15 studies. Most of studies showed result of the intervention group was statistically significant, compared with the control group. These results suggest that acupuncture for spastic hemiplegia after stroke was effective and it was statistically more significant than the control group. However, it is difficult to confirm a conclusion, because the quality of most of studies was low.

A Systematic Review and Meta-Analysis of the Effects of Simultaneous Dual-Task Training on Executive Function in Older Adults (동시적 이중과제 훈련이 노인의 실행기능에 미치는 효과: 체계적 고찰 및 메타분석)

  • Objective : The purpose of this study was to analyze the effects of simultaneous dual-task training to assess executive function in older adults. Methods : We searched the PubMed, EMBASE, Cochrane, Web of Science, and RISS databases of publicated studies in the past decade. Seven studies were selected based on the inclusion and exclusion criteria. Qualitative assessment and meta-analysis were performed for the seven studies. Results : A randomized controlled trial design was used in the selected studies, and PEDro Scores above seven were obtained. The Trial Making Test (TMT) evaluated the effects of dual-task training on executive function in four studies. The Color Trail Test (CTT) was used in two studies, and Stroop test was used in three studies. The effect size for total executive function was 0.38, which was small. The effect sizes for TMT and CTT were 0.37. Stroop Test was 0.34, demonstrating that their effect sizes were also small. Only significant effects in total executive function, TMT, and CTT showed significant effects (all p<0.05). Conclusion : This study confirmed that dual-task training was effective in improving executive function in older adults. To improve the effectiveness of dual-task training, the difficulty of the dual-task training should be considered. It is also necessary to implement assessments that can evaluate performance under dual-task conditions as well as conventional test tools for executive function. In the future, dual-task training could be used as an appropriate intervention for executive function in older adults to delay the onset of dementia.

A Systematic Review of Task-Oriented Training to Improve the Physical Function and Activities of Daily of Living of Children With Cerebral Palsy (뇌성마비 아동의 신체기능 및 일상생활 향상을 위해 적용된 과제 중심 훈련의 체계적 고찰)

  • Objective : This study aimed to analyze and classify the task-oriented training methods used to improve the daily lives of children with cerebral palsy. Methods : The search period from January 2008 to August 2020, and the CINAHL, MEDLINE and PubMed was databases were used. A total of 18 studies was selected, and the PICO method was applied to conduct the systematic review. The training methods were then classified according to task, practice, and feedback as strategies to learn or to exercise control based on prior research. Results : Of the selected studies, 13 reported significant results in the area of motor skills, two of which related to Activities of Daily Living. Discrete and closed were the most common forms of task, and the most used types of practice were whole, part, and blocked, in that order. Finally, feedback was not specifically provided in most studies, followed by extrinsic feedback. Conclusion : Based on this review, task-oriented training for children with cerebral palsy can be made more effective in clinical practice in the future through the systematic selection of techniques that promote exercise control and the presentation of specific methods.

Implementation of Patient Experience Assessment and Subsequent Changes at the Ground Level in Health Care: Patient Experience Employees' Perspective (환자경험 평가와 의료 현장의 변화: 의료기관 환자경험 업무 담당자의 관점)

  • Purpose: To examine whether the Patient Experience Assessment (PEA) has led to perceptible changes at the ground level of health care, as a way of evaluating PEA as a policy intervention for quality improvement. Methods: Four focus group discussions (FGDs) were conducted, each comprising six to eight participants who were employees responsible for patient experience at their respective hospitals. The primary focus of the FGDs was on questions such as: 1) How do hospitals respond to PEA? 2) What significant changes were observed after the implementation of PEA? 3) What were the unintended consequences of implementing PEA, if any? 4) What areas of improvement have been identified for maximizing the potential of PEA? Results: Two broad themes emerged out of the FGDs: changes observed post implementation of PEA, and areas for improvement of PEA. Four significant changes were reported by participants: changes in perception and attitude regarding patient experience in hospital employees, increased active involvement by the hospital leadership, enhanced efforts to improve patient experience, and increased cooperation between such activities. Furthermore, eight areas of improvement were identified, which have been grouped in three categories: improving the process of data collection for PEA, introducing additional catalysts to facilitate further changes, and paying attention to structure- and patient-level constraints that must be addressed in parallel. Conclusion: The implementation of PEA led to perceptible changes within hospitals, which implies that it can serve as an effective catalyst for improving patient experience. A number of areas of improvement that would aid in maximizing the potential of PEA were also identified.

A Study on the Effect of Proprioceptive Neuromuscular Facilitation Training by Meta-analysis -Focused on Balance and Gait Ability in Patients with Storke

  • Stroke results in balance disorders, these directly affect autonomy and gait ability. The aim of this meta-analysis was to determine the efficacy of proprioceptive neuromuscular facilitation on balance and gait. We included all randomized controlled trials assessing the efficacy of proprioceptive neuromuscular facilitation on balance and gait control in patients after stroke. This study was conducted according to the PRISMA guideline. Cochrane library, CINAHL, and PubMed were searched for studies published up to November 2021, and all randomized controlled trails(RCT) assessing PNF therapy were included. This analysis included only RCT. A total of 18 studies were selected from 1091 records obtained from the databases. The meta-analysis was performed using the R project for statistical computing version 4.0.2. The overall intervention effect was middle (standardized mean difference (SMD): 0.56) Additionally, berg balance scale (SMD: 0.48), functional reach test (SMD: 0.51), timed up and go test (SMD: 0.78), 10m walking test (SMD: 0.52), and dynamic gait index (SMD: 0.33) had medium effect sizes. The average Pedro scale was 6.63 out of 18, with a low risk of bias. These findings indicate that PNF is an effective therapy for improving balance gait in stroke patients.
