한국 성인의 밥 섭취 정도와 잡곡밥 섭취 유무에 따른 영양섭취실태 및 대사증후군 위험수준 평가 - 2007-2008년 국민건강영양조사 자료를 이용하여 - (Nutritional Evaluation and Its Relation to the Risk of Metabolic Syndrome according to the Consumption of Cooked Rice and Cooked Rice with Multi-grains in Korean Adults: Based on 2007-2008 Korean National Health and Nutrition Examination Survey)

  • This study was conducted to investigate the nutrient intakes of subjects by quartile of percent energy intake from cooked rice, consumption of cooked rice mixed with multi-grains and to evaluate rice consumption in relation to the risk of metabolic syndrome. The subjects were 5,830 males and females aged between 20~64 years based on 2007-2008 KNHNES data. Levels of percent energy intake from cooked rice were classified into 4 groups (Q1, Q2, Q3, Q4 groups: 25% of each) using data of 24-hour recall method from KNHNES. Using medical examination and questionnaire, subjects were classified according to diagnostic criteria of metabolic syndrome. The subjects with higher age, being married, lower education, lower economic level were more likely to take higher percent energy intake from cooked rice. Quartile Q3 of percent energy intake from cooked rice tended to show higher Index of Nutritional Quality (INQ) for fiber, calcium, iron, potassium and vitamin A. INQ of protein, dietary fiber, calcium, thiamin, phosphorus, potassium, riboflavin, niacin and vitamin C by consumption of cooked rice mixed with multi-grains was higher than that by consumption of cooked white rice when adjusted for age. No association with a risk for metabolic syndrome was found for quartile of percent energy intake from cooked rice or cooked rice mixed with multi-grains compared to cooked white rice after adjusting for energy, gender, age, BMI, alcohol, smoking, income and physical activity. In conclusion, consumption of over 54% energy intake from cooked rice or only cooked white rice showed relatively low INQs, but was not associated with a higher risk for metabolic syndrome.

과체중 및 비만 직장인 대상 모바일 건강관리 프로그램의 참여도에 따른 과정 평가: 12주 중재연구 (Process evaluation of a mobile healthcare program among employees with overweight and obesity: a 12-week intervention study investigating the role of engagement)

  • Objectives: A mobile health intervention program was provided for employees with overweight and obesity for 12 weeks, and a process evaluation was completed at the end of the program. We investigated participant engagement based on app usage data, and whether engagement was associated with the degree of satisfaction with the program. Methods: The program involved the use of a dietary coaching app and a wearable device for monitoring physical activity and body composition. A total of 235 employees participated in the program. App usage data were collected from a mobile platform, and a questionnaire survey on process evaluation and needs assessment was conducted during the post-test. Results: The engagement level of the participants decreased over time. Participants in their 40s, high school graduates or lower education, and manufacturing workers showed higher engagement than other age groups, college graduates, and office workers, respectively. The overall satisfaction score was 3.6 out of 5. When participants were categorized into three groups according to their engagement level, the upper group was more satisfied than the lower group. A total of 71.5% of participants answered that they wanted to rejoin or recommend the program, and 71.9% answered that the program was helpful in improving their dietary habits. The most helpful components in the program were diet records and a 1:1 chat with the dietary coach from the dietary coaching app. The barriers to improving dietary habits included company dinners, special occasions, lack of time, and eating out. The workplace dietary management programs were recognized as necessary with a need score of 3.9 out of 5. Conclusions: Participants were generally satisfied with the mobile health intervention program, particularly highly engaged participants. Feedback from a dietary coach was an important factor in increasing satisfaction.

국민건강증진을 위한 영양 부문의 2000년 잠정목표와 전략 (Provisional National Nutrition Targets and Strategies for Health Promotion of Koreans)

  • This study attempts to propose a priority of national nutrition targets and strategies for health promotion by the year 2000 in Korea, as a part of the task set for national health promotion objectives and strategies. Among all of the important health issues raised, ten were chosen, nutrition was one priority area. In the first part, the current status of the nutrition-related health problems and risk factors are reviewed, in conjunction with the newly arisen health phenomena, such as changes in prevalence of lifestyle disease and causes of death, changes of food consumption patterns in our country. In the second section this study suggests six feasible national nutrition targets, eight implementing strategies and current major tasks on the basis of the assessment of present status and in consideration of the other health promotion goals and strategies, with reference to that of other developed countries. The main targets and strategies are suggested as follows ; Firstly, the national nutrition monitoring and surveillance system should be established for identifying the nutritional problems for our people, and current National Nutrition Survey is a strong need for improvement to a more comprehensive and reliable one. Secondly, effective administrative mechanism should be operation at national level for the development of nutrition policy. Ministry of Health and Welfare (MOHW) as well as local health department must be remarkably renewed and strengthened the nutrition section. And it is recommended that MOHW organize and operate “The Council of Nutrition”, in which all government authorities related with foodstuffs and nutrition would incorporated. The Council of Nutrition would act as an adjustor as well as a coordinator in nutrition related policy-making. Thirdly, healthy eating pattern will be supported by activities of introducing a nutrition labeling for providing consumers with the necessary information and skills for food selection. Fourthly, nutrition education, and nutrition intervention programs will be carried out in various settings such as health centers, schools, and clinical fields and workplace. Fifthly, the current dietary guidelines shall be continuously improved in detail, and publicly circulated to particular levels of people by age group and by health condition. And finally, researches and epidemiological studies particularly in regard to diet for development of chronic diseases are needed for more investigation and up-to-date national health and nutrition data should be collected with the support and cooperation from the various medical professional teams . 유아의 우유군의 섭취가 식이의 다양성과 영양소 섭취의 적정도에 미치는 영향 (The Effects of Milk Group Intake to Dietary Diversity Score and Nutrient Adequacy Ratio among Toddler)

  • This research was conduced to 1) examine the influence of milk group intake on the dietary, considering dietary diversity and nutrient adequacy, 2) identify which foods truly contribute to the intake of specific nutrients 3) point out the dietary problems of this age group. The subjects were 664(male 357, female 307) healthy toddlers of the ages of 12-36 months, who had neither been taking any medicine-type supplements nor had any diseases that affect the dietary intakes, chosen among the outpatients of university and private hospitals in Seoul and Kyonggi province during Mach-August, 1999. The 24 hour recall method and CAN Pro(food analysis program) were used to analyze the amount of nutrient intake. The grouping of the subjects for the analysis by monthly age was based on the 98 Korean Child growth standard. The results that the milk group intake showed significant negative correlations with the grain group(r=-0.194, p<0.01), the vegetable group(r=-0.115, p<0.01) and the fruit group(r=-0.125, p<0.01). It also showed that the more the milk group intake, the lower the Dietary Diversity Score(r=-0.131, p<0.01), and the more the intake of raw milk, the more dificient the intake of iron(r=-0.211, p<0.01), vitamin A(r=-0.169,p<0.01), vitamin B$_1$(r=-0.078, p<0.05) and vitamin C(r=-0.187, p<0.01). Foods contributory to nutrient intake for of 12-14 months of were mostly infant formula, and the contribution ratio was high. In the age of 15-17 months, the subjects obtained most of the nutrients from raw milk., liquid-type yoghurt, soybean milk, cooked rice, etc. As for iron intake, the age of 21-23 month obtained the most from ionic drinks, and 24-29 months from raw milk, chocolate cakes, ionic drinks, etc. Though the NAR(Nutrient Adequacy Ratio) and MAR(Mean Adequacy Ratio) generally tended to increase accordingly with the DDs, as for nutrients Ca, vitamin A, vitamin B$_1$, and vitamin C the NAR marked the highest when the DDS marked the lowest(due to infant formula). And for iron, even when the DDS was 5 the NAR value was low. Im conclusion, the more the milk group intake, the lower was the dietary diversity. High dependency on infant formula and others from milk group over the age of 12 months proved to result in a decrease of nutritious solid food intake, iron deficiency and a low dietary diversity score, which can lead to an unbalanced diet. Therefore, nutrition education towards mothers/caretakers of toddler is of urgent need, in order to reduce the fluid milk group intake of low nutritional density and to provide various solid foods the children. 지역사회 노인을 대상으로 적용한 라이프스타일 중재의 형태와 효과에 관한 체계적 고찰 (Characteristics and Effects of Lifestyle Interventions for Community Dwelling Older Adults: A Systematic Review)

  • 목적 : 본 연구는 지역사회 노인에게 적용한 라이프스타일 중재의 형태와 효과를 알아보고자 하며, PRISMA 가이드라인를 통해 체계적 고찰을 시행하였다. 연구방법 : 2008년 1월부터 2017년 12월까지 국 내외 학술지에 개제된 논문을 국가과학기술정보센터(National Digital Science Library), RISS, PubMed, CINAHL을 통하여 검색하였다. 선정기준에 따라 최종적으로 20편의 연구를 분석하였으며, PEDro scale(Physiotherapy Evidence Database scale)을 사용하여 문헌의 질 평가를 하였다. 문헌에서 사용된 중재는 참가자들의 질환적 특성에 따라 분류하였다. 결과 : 라이프스타일 중재의 주요 구성요소는 중등도 이상의 운동과 같은 신체활동 증진과 건강한 식습관 형성을 위한 교육 및 훈련으로 확인되었다. 최종 분석된 20편 중 17편의 연구에서는 운동프로그램과 교육 또는 식단관리와 운동과 같이 두 가지 이상의 구성요소를 갖는 형태로 중재가 적용되었다. 노인의 건강 및 삶의 질을 평가하기 위해 다면적인 평가가 이뤄졌으며 그 중 생화학적 요인(biochemical factor)과 건강 및 웰빙(Health and well-being)을 평가하는 도구가 가장 많았다. 연구 분석 결과 지역사회 노인을 대상으로 한 라이프스타일 중재의 효과는 연구별로 상이하게 나타났으나, 14편의 연구에서 중재 직후 실험군과 대조군 간에 건강 및 삶의 질의 유의한 차이가 나타났다. 결론 : 본 연구는 노인의 질환적 특성에 따른 라이프스타일 중재기법을 선택하는데 도움을 줄 수 있을 것으로 사료된다. 향후 국내 지역사회 상황에 부합되는 중재의 개발 및 효과에 관한 연구를 통해, 작업치료적 라이프스타일 중재의 실행가능성에 관한 연구들이 체계적으로 이루어져야 할 것이다.

Factors Related to Dietary Fiber Intake Among Sixth Grade Elementary School Children in Daejon City

  • This study was conducted to investigate variables related to dietary fiber intake among sixth grade children in an elementary school in Daejon city. One hundred and forty-seven children completed a questionnaire for determining their socioeconomic background and their food habits. Anthropometric measurements were taken, and a 24-hour diet recall method was used to collect three-day food intakes. The socioeconomic status of the children's families belonged to the upper middle class; 53.0% of their fathers and 25.8% of their mothers had completed college or higher degrees. 27.9% of the mothers had jobs, including part-time jobs. Approximately 30% of the children skipped breakfast, 66.4% of the children preferred animal foods to plant foods, and 52.4% of the children preferred green vegetables to yellow or pale vegetables. Grilled meat dishes, such as Grilled beef rib with seasoning, Bulgogi, grilled pork belly and beef steaks, were the most popular types of food eaten outside home by the children. Daily dietary fiber intake was 14.5 g in boys and 14.5 g in girls, and these intakes are low compared to the standard guidelines. The average intakes of energy and protein of the children were 84.5% and 114.0% of the Korean Recommended Dietary Allowances (RDAs), respectively. Besides energy, riboflavin, iron and calcium intakes were below the RDAs, and especially calcium intake was only 50% of the RDAs. On the other hand, thiamin, niacin, ascorbic acid, protein and phosphorus intakes exceeded the Korean RDAs. Family income or the children's body mass index (BMI) was not directly related to dietary fiber intakes. However, higher dietary fiber intakes tended to be related to higher intakes of green vegetables and fruits. Children with higher dietary fiber intake tended to prefer plant foods to animal foods. Energy and most nutrients, except heme iron and retinol, showed positive relationships with dietary fiber intake. Especially potassium and plant origin protein and calcium were highly correlated with dietary fiber intakes(r>0.6). From these results, it is concluded that dietary fiber intakes of these sixth grade elementary school children were less than the standard reference and it is anticipated to decrease further in the future with increased incomes. Therefore, increased intakes of dietary fiber by elementary school children should be promoted through nutrition education, together with the development of cooking methods and recipes utilizing green vegetables and fruits.

일개 대학병원 간호사들의 근무형태에 따른 건강행태 (Health Behaviors of Nurses at a University Hospital according to Type of Work)

  • 이 연구는 대학병원 간호사들을 대상으로 하여 이들의 근무형태에 따른 건강행태를 파악하고 간호사들의 건강유지 및 증진을 위한 체계적이고 효과적인 건강교육의 방향 설정을 위한 기초 자료를 얻고자 실시하였다. 연구대상은 대구지역 일개 대학병원에 근무하는 간호사 340명이었고, 자료의 수집은 2006년 3월 6일부터 3월 30일까지 대상자의 일반적 특성, 업무환경특성, 건강행태, 자신의 건강에 대한 인식, 건강증진행위 실천 점수 등에 대하여 자기기입식 설문조사를 통하여 수행되었다. 교대근무 간호사(이하 "교대")와 비교대근무 간호사(이하 "비교대")의 건강행태의 항목에서 규칙적인 식사여부(p<0.01), 음주여부(p<0.05), 건강관리(p<0.05), 규칙적인 운동(p<0.01), 근무시간 외 시간 활용(p<0.05), 충분한 휴식시간(p<0.05), 피로인식여부(p<0.01)에서 통계학적으로 유의한 차이를 보였다. 대상자의 근무형태에 따른 건강증진행위 실천 점수는 교대가 115.4점, 비교대가 121.9점으로 비교대가 건강행위를 더 잘 실천하는 것으로 나타났고, 하부영역별로 실천 점수가 유의한 차이가 있는 것은 자아실현과 영양영역(p<0.01)이었으며, 자아실현영역에서 가장 점수가 높았고, 운동영역에서 점수가 가장 낮았다. 이상의 결과로 볼 때 비교대보다 교대가 교대근무로 인해 상대적으로 시간적인 여유가 부족하고 불규칙적인 생활로 인해 시간활용을 잘 하지 못하는 것으로 생각되며, 교대의 건강행위 실천율을 높일 수 있도록 여건을 개선하고 건강행위에 대한 관심을 유도하는 방법을 모색해야 할 것이다. 또한 간호사들의 생활습관의 개선과 자신에 대한 긍정적인 평가가 이루어지도록 하고, 이에 대한 교육과 홍보가 필요하다고 생각된다.

요추척추궁 절제술 환자의 표준관리지침서 개발 (Development of a Critical Pathway for Patients with Lumbar Laminectomy)

  • The purpose of this study was to develop a critical pathway for case management of patients who have received Lumbar Laminectomy because of low back pain, arm and leg numbness, and radiating pain in the leg. For this study, a preliminary critical pathway was developed through a review of the literature including five critical pathways which are currently being used in the USA. In order to identify the overall service contents required by these patients, 30 cases were analyzed. These cases were taken from medical records of those with Lumbar Laminectomy between January, 1998 and December, 1998 in the department of neurosurgery at the Pusan National University Hospital in Pusan. An expert validity test was done for the preliminary critical pathway, a clinical validity test was also done using 12 patients with Lumbar Laminectomy between October 1, 1999 and January 31, 2000. After these processes, the final critical pathway was developed. The results are summarized as follows. 1. The vertical axis of the critical pathway includes the following eight items: assessment, consultation, diet, test, medication, treatment, activity, education/ discharge planning. The horizontal axis includes the time from the start of hospitalization to discharge. Analysis of the 30 medical records was done. analysis of the service contents showed the horizontal axis of the preliminary critical pathway was set from hospitalization to the 12th post operation day and the vertical axis was set to include eight items, the contents which should have occurred, according to the time frames of the horizontal axis. 2. As a result of the expert validity test, it was found that among the 233 items, 203 showed over 88% agreement and 30 of them showed less than 88% agreement, which were then revised or deleted from the critical pathway. At the preliminary meeting for the clinical validity test, the time of hospitalization on the horizontal axis was shortened to the 10th post operation day. A clinical validity test was done with 12 patients with Lumbar Laminectomy. All the cases progressed according to the critical pathway although some variances were noted in assessment, consultation, test, medication, and treatment. 3. Based on these results, a final critical pathway was determined. In conclusion, this critical pathway is partially applicable to the care of patients with Lumbar Laminectomy and needs further investigation.

전라도 구곡순담 장수벨트지역에 거주하는 중노년층의 연령군에 따른 여름철 식품 섭취량과 식품 섭취 다양성 및 식이 패턴 평가 (Assessment of Food Consumption, Dietary Diversity and Dietary Pattern during the Summer in Middle Aged Adults and Older Adults Living in Gugoksoondam Logevity Area, Korea)

  • The purpose of this study was to investigate food intake, dietary diversity and dietary pattern during the summer in middle aged and older adults living in Gugoksundam (Gurye, Goksung, Sunchang and Damyang counties), Jeonla Province located in southern part of Korea and known as one of the representative Korean longevity areas. Food intake and dietary diversity were assessed by using the data from 2 day-24 hour recall of 1,051 subjects (394 males and 711 females) aged 45 years and older (45-93 years in male, 45-105 years in female). The average ages of males and females were 70.7 years and 71.1 years, respectively. Average total daily food intake was significantly decreased with aging in both genders, and the average animal food intake ratio to total food intake was ranged 10~14% in 3 different age groups, 45~64 years, 65~74 years and 75 years and older. The contribution of each food group, in weight, to total food intake showed the descending order of grains, fruits, vegetables and alcohols in males, and grains, fruits, potatoes and meats in females. On daily intake amount of each food item, rice, watermelon, soju, kimchi, and potato were ranked on top 5 in descending order in males, and rice, watermelon, potato, kimchi and ylmukimchi in females. On intake frequency, rice, kimchi, onion, green pepper and potato were ranked on top 5 in descending order in both genders. Dietary variety score (DVS) and dietary diversity score (DDS) for the assessment of dietary diversity and balance were significantly decreased with aging in both genders. In food group intake pattern (DMGFV), 01101 type without consumption of dairy and fruits was the most prevalent, and only 3.6% of male and 3.9% of female subjects showed 11111 type, consumed all the 5 food groups a day. Dietary pattern of subjects was analyzed by cluster analysis with 18 food groups intake. Overall dietary pattern was classified into two clusters, one was more desirable and the other was less desirable. The percentage of subjects with more desirable dietary pattern was about 15% in male and 32% in female. In conclusion, most of our subjects living in Gugoksundam area were taking very simple diet with low amount of dairy products and fruits. These results indicate that nutrition intervention and education for older people living in rural areas should be focused on various food intake including dairy products and fruits.

스켈링 실습실에 내원한 여고생의 구강건강 실태 조사 (A Study of the Actual Conditions of the Dental Health of High School Girls Visiting Scaling Practice Rooms)

  • The researcher worked with a total of 158 high school girls visiting the scaling practice room at the department of dental hygiene in Jinju Health college from November 11, 2002 to December 4, 2002, having their teeth scaled, observing their teeth and interviewing them with the aid of questionnaires. The findings can be summarized as follows. 1. The survey on the understanding of dental health and the attitudes of the subjects shows the following results : "I have never learned how to brush teeth" (65.8%: 104 girls); "I brush my teeth twice a day" (57.0%: 90 girls) : "I brush my teeth for 2 minutes"(44.3%: 70 girls): "I brush my teeth vertically and horizontally" (53.2%: 84 girls): "( brush my teeth, gum and tongue" (462%: 73 girls): "I use one brush for two months" (40.5%: 64 girls); "I don't have any floss" (582%: 92 girls): "I have never had my teeth scaled" (84.2%: 133girls) : "Smoking is bad for the teeth" (77.8%: 123 girls): "Drinking is bad for the teeth" (48.7%: 77 girls): "I have ever visited dental clinics" (92.4%: 146 girls). 2. The study on part of the teeth most exposed to caries shows the following results : Mandibular right first molar(79.8%: 126 girls): Mandibular left first molar(77.5%: 124 girls), Mandibular left second molar(70.2%: 111 girls). Mandibular right second molar(65.7%: 104 girls). Maxillary left first molar(59.5%: 94 girls): Maxillary left second molar(59.5%: 94 girls): Maxillary right first molar(58.2%: 92 girls). Maxillary right second molar(50.7%: 80 girls) 3. The statistics on the subjects' caries show the following results: DMF rate: 943%: DMFT rate: 25.61%: DMFT index: 7.17 teeth : DT rate: 4651%: FT rate: 52.25%: MT rate: 124% 4. The survey on dental caries statistics, dental health, dental health behavior, and the subjects' attitudes shows the following results: The worse the subjects' dental health was(r=-327, p<.001), the more they visited dental clinics(r=.195, p<.05), the higher DMT rate, DMFT index were; The more the subjects visited dental clinics(r =.359, p<.001), the higher DT rate was; The worse the subjects teeth went(r=-.197, p<.05), the higher MT rate was. It is necessary that systematic and substantial dental health education for high school students should be carried out in regard to plaque control, fluoride application, diet control, pit & fissure sealant as a part of developing national health policies designed for the dental health of the teenagers.
