치매노인을 돌보는 가족부양자 부양부담의 영향 요인: 치매지원센터 이용자를 중심으로 (Influencing Factors on Care Burden among Family Caregivers for Elders with Dementia: Focusing on Family Caregivers using a Support Center for Dementia)

  • Purpose: This study aimed to explore influencing factors on care burden among family caregivers for elders with dementia living at home. Methods: Participants in this study were 211 family caregivers who were taking care of elders registered at a support center for dementia located in S-gu, Seoul. Data were collected using a structured questionnaire. Data were analyzed with descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression in SPSS WIN 23.0. Results: The care burden of family caregivers was mild to moderate ($40.31{\pm}21.50$) in this study. The factors influencing care burden among family caregivers for elders with dementia were the age, behavioral problems, and dependency in instrumental activities of daily living of demented elderly, in addition to the perceived health status and resting hours of family caregivers. Overall, these factors explained 46.5% of the total variability in care burden in this sample (F=13.01, p<.001). Conclusion: Findings from this study suggest that the characteristics of demented elderly and family caregivers can influence care burden differently. Individually tailored strategies based on the various caregiving contexts need to be developed to reduce the level of care burden among family caregivers for elders with dementia.

Impact of socio-demographic factors, lifestyle and health status on nutritional status among the elderly in Taiwan

  • BACKGROUND/OBJECTIVES: Aging is an imperative problem for many countries in this century, and presents several challenges for the maintenance of good nutritional status. This study aims to assess the impact of socio-demographic factors, lifestyle and health status on the nutritional status among the elderly in Taiwan. SUBJECTS/METHODS: A cross-sectional study was carried out in Taiwan. Data were obtained from the Mei Jau Health Management Institution, which is a private health evaluation provider with multiple health screening centers in Taiwan and Asia. This study included 7947 adults aged 65 years or above. The data were extracted between 2001 to 2010. Nutritional status was assessed using anthropometric data, biochemical data and dietary intake information. RESULTS: Among the 7947 participants with mean age of 70.1 (SD = 4.5) years, 20.2%, 6.6%, 10.5% and 52.5% experienced underweight, protein malnutrition, anemia and inadequate dietary intake in the past month, respectively. Age was negatively correlated with body weight (r = -0.19, P = 0.02), body mass index (r = -0.41, P < 0.001), albumin level (r = -0.93, P < 0.001) and hemoglobin level (r = -0.30, P = 0.008). Age above 70 years, gender, unmarried status, retirement, lack of education, low family income, smoking, alcohol drinking, sleep duration of 6-8 hours, vegetarian diet, multiple medications, comorbidity and dysphagia were positively associated with malnutrition in older adults. CONCLUSIONS: Underweight and inadequate dietary intake are prevalent among the elderly in Taiwan. Vegetarian diet, multiple medications, comorbidity, dysphagia and lifestyle factors such as smoking, alcohol drinking and sleep duration of 6-8 hours are risk factors for undernutrition in older adults.

Association Between Convenience of Transportation and Unmet Healthcare Needs of Rural Elderly in Korea

  • Objectives: In rural areas of Korea, where public transportation infrastructure is lacking and alternative systems are poor, the elderly experience inconveniences in using healthcare, although their need is high. This study aimed to analyze the association between the convenience of transportation and unmet healthcare needs among the rural elderly. Methods: The data used were collected in the 2016 Community Health Survey among rural elderly individuals aged 65 or older. Dependent variable was the unmet healthcare needs, explanatory variable was the convenience of transportation. The elderly were divided into 3 groups: with no driver in the household, with a driver, and the elderly individual was the driver (the self-driving group). Covariates were classified into predisposing, enabling, and need factors. They included gender, age, education, income, economic activity, household type, motor ability, subjective health level, number of chronic diseases, anxiety/depression, and pain/discomfort. The data were analyzed using logistic regression and stratification. Results: A significant association was found between the convenience of transportation and unmet healthcare needs. When examined unadjusted odds ratio of the group with a driver in the household, using the group with no driver as a reference, was 0.61 (95% confidence interval [CI], 0.54 to 0.68), while that of the self-driving group was 0.34 (95% CI, 0.30 to 0.38). The odds ratios adjusted for all factors were 0.69 (95% CI, 0.59 to 0.80) and 0.79 (95% CI, 0.67 to 0.91). Conclusions: We confirmed a significant association between inconvenient transportation and unmet healthcare needs among the rural elderly even after adjustment for existing known factors. This implies that policies aimed at improving healthcare accessibility must consider the means of transportation available.

도시 및 농어촌 노인의 활동 수준 및 활동 정도에 따른 삶의 질 비교 (Comparison of Quality of Life Related with the Level of Activity and the Grade of Activity in the Elderly Residing among City, Farmimg and Fishing Area)

  • 본 연구는 도시노인과 농어촌노인의 활동의 수준을 알아보고 삶의 질에 영향을 주는 요인을 비교 분석하고 관련성을 규명하고자 하였다. 연구대상은 60세 이상 노인 537명이 참여하였으며, 삶의 질 평가는 SF-36을 사용하였고, 한국판 활동척도를 이용하여 활동 수준을 평가하였다. 삶의 질 비교에서 도시 노인이 농어촌 노인보다 삶의 질이 높았으며, 활동수준 비교에서도 수단적 일상생활, 저강도 고강도 신체활동, 사회활동에서 도시 노인의 활동수준이 높았다. 또한 활동수준과 삶의 질과의 상관성은 모든 활동과 관련이 있었다. 도시노인은 삶의 질 하위영역 점수, 한국판 활동척도 점수, 질환유무, 직업유무가 삶의 질에 영향을 미치는 요인이었으며, 농어촌노인은 삶의 질 하위영역 점수, 한국판 활동척도 점수, 질환유무, 연령, 교육수준이 삶의 질에 미치는 요인이었다. 본 연구 결과를 토대로 농어촌 노인이 의미 있는 활동을 즐길 수 있는 환경적 조성이 요구되며, 다양한 프로그램 개발이 진행되어야 할 것이다.

독서치료 프로그램이 노인 스트레스 감소에 미치는 효과 연구 (A Study on the Effectiveness of Bibliotherapy Program to Reduce Stress of the Elderly)

  • 독서치료가 노인 스트레스 감소에 미치는 효과를 검증하기 위하여 프로그램을 개발한 후, 60세 이상의 노인들을 모집하여 집단대면프로그램과 개별통신프로그램 두 가지 유형의 독서치료를 진행하였다. 독서 치료 이전과 이후에 실시한 스트레스인지테스트와 스트레스반응테스트를 통해 효과를 측정하여 분석하였다. 총 16개 기관에서 121명이 프로그램을 이수하였으며 분석결과는 다음과 같다. 노인을 대상으로 한 독서치료는 집단대면프로그램과 개별통신프로그램 모두 스트레스 감소의 효과가 통계적으로 유의한 것으로 나타났으며, 개별통신프로그램 보다 집단대면프로그램에서 효과가 더 크다. 성별, 연령, 학력 등 개인적 배경에 따른 독서치료 효과의 차이는 통계적으로 유의하지 않다. 사전 스트레스 수준에 따른 독서치료의 효과의 차이는 스트레스인지 부문에서 통계적으로 유의한 것으로 나타났으며, 사후검증 결과 스트레스가 높은 집단이 낮은 집단보다 스트레스 감소 효과가 큰 것으로 판명되었다. 이와 같은 결과에 기초하여, 독서치료 프로그램 운영 전략과 활성화 방안을 제언하였다.

춘천지역 60세 이상 노년층의 건강식품 섭취실태 조사 (A Study on the Intake Patterns of Health Food of the Elderly Aged over 60 Years in the Chuneheon Area)

  • This study was carried out to investigate the intake patterns of health food according to the kind of disease of the elderly aged over 60 years living in the Chuncheon area. This study was conducted by using a questionnaire from December 1999 to the March 2000. The data was analyzed using a SPSS program. The results were summarized as follows : The poorer the self- perceived health condition, the lower the level of activity of daily living(ADL) and instrumental activities of daily living(IADL) were the higher. the frequency score of health food intake. The incidence of disease, such as arthritis(44.7%), hypertension(18.8%), digestive diseases(17.1%), diabetes(10.5%) were found to be higher. than other chronic diseases among the elderly. The frequency score of health food intake was not significantly different according to self-perceived health status, disease status, or the level of ADL and IADl.. The must important source of information for health food intake was the recommendation of friends and relatives(68.7%). Twenty eight point two percent of the elderly considered health foods to be effective, however 8.3% of the elderly considered them to be ineffective. The elderly with chronic diseases had taken health floods in the descending order of neuralgia-arthritis, hypertension, digestive disease, diabetes. The elderly with digestive diseases, diabetes, renal disease, neuralgia- arthritis and respiratory disease were inclined to habitually take health floods once or twice per year. Intake of Korean traditional folk food was as high as intake of registered health food and Chinese tonic medicine. The most frequently taken health food was tonic medicine, which is fo11owed by pumpkin, ginseng products, herbal tea formula with black goat, deer antler and Kye-so-ju. Intakes of health food such as tonic medicine, blood of the deer, royal jelly, aloe were found to be higher among the elderly with chronic diseases such as diabetes, hypertension, and digestive disease. However, intakes of ginseng products, deer antler, pumpkin, herbal tea formula with black goat and Kye-so-ju were found to be higher among the healthy elderly. The elderly with neuralgia-arthritis had taken sixteen kinds of health foods. From these results, a wide consumer education program should be developed to convince people of the importance of well-balanced diet and to choose proper health foods according to the elderly's health conditions. Also, comprehensive and scientific research into Korean traditional folk foods are needed for the correct use.

Socioeconomic status, food security, and chewing discomfort of Korean elders: results from the Korea National Health and Nutrition Examination Survey

  • BACKGROUND/OBJECTIVES: The purpose of this study was to investigate the association between socioeconomic status and chewing discomfort and identify the role of food insecurity in the association's causal pathway in a representative sample of Korean elders. MATERIALS/METHODS: We conducted cross-sectional analyses of the Korea National Health and Nutrition Examination Survey (2013-2015) data for elders aged ≥ 65 years. Socioeconomic status indicators used included household income and education level. Chewing discomfort was assessed according to the self-reported presence of chewing problems. Food security was surveyed using a questionnaire based on the US Household Food Security Survey Module. RESULTS: The odds ratios of chewing discomfort in the 1st and 2nd income quartiles were 1.55 (95% confidence interval [CI], 1.15-2.10) and 1.40 (95% CI, 1.03-1.90), respectively, compared to participants in the highest income quartile. Participants with the lowest education level were 1.89 (95% CI, 1.30-2.75) times more likely to have chewing discomfort than those without chewing discomfort. After including food security in the final model, the logistic coefficients were attenuated in the income and education quartiles. CONCLUSIONS: Low socioeconomic status was associated with chewing discomfort. In addition, the results confirm that food insecurity can mediate the association between socioeconomic inequalities and chewing discomfort among the elderly.

노인의 주관적 건강상태, 종교성과 죽음에 대한 공포 (Subjective Health Status, Religiosity and Fear of Death of the Elderly in Rural Communities)

  • Purpose: This study was performed to investigate the level of subjective health status, religiosity and the fear of death of the elderly and to identify the relationships among them. Methods: This study was a cross-sectional descriptive study using a questionnaire. Fear of Death Scale that was made by Loo & Shea (1996) and translated and revised by Kim(2003) was used to measure the fear of death. The data was analyzed using SPSS. Results: The subject was 128 community dwelling elders who had a religion. Of the respondents, 50% felt 'not healthy', and 69.5% was moderately religious. The mean score of FODS was 3.18 (out of 5). The mean score of each subcategory of FODS was as follows: Death anxiety about the death of self was 3.00. Death anxiety about the dying of self was 3.18. Death anxiety about the death of others was 3.31 and death anxiety about the dying of others was 3.23. There was a statistically significance correlation between the level of religiosity and FODS (F=3.29, p= .040). Conclusion: Health professionals may make efforts to learn about healthy attitudes toward death and aid for the elderly and to comfort them. Death education programs are needed for the elderly.

노인 당뇨병 환자의 규칙적 걷기운동 프로그램이 대사증후군, 심혈관 위험도 및 우울정도에 미치는 효과 (The Effects of Regular Walking Exercise on Metabolic Syndrome, Cardiovascular Risk Factors, and Depressive Symptoms in the Elderly with Diabetic Mellitus)

  • Purpose: This study aimed to estimate the effects of a regular walking exercise program on metabolic syndrome, cardiovascular risk factors, and depressive symptoms among the elderly with diabetic mellitus (DM) based on the Theory of Reasoned Action (TRA). Methods: This study has randomized and stratified experimental design with experimental and control groups. We developed a regular walking exercise program suitable for the elderly with DM based on the guidance of AAHPERD. The experimental group participated in the regular walking exercise program, which contains walking exercise 3 times a week and 50 minutes each time for 3 months and education on controlling diet and preventing complications once a week and 20 minutes each time for 4 weeks. Post-test was conducted after 3 months to estimate metabolic syndrome, cardiovascular risk factors, and depressive symptoms. Results: The regular walking exercise program was effective for decreasing the waist size, the level of fasting blood glucose (FBG) and triglyceride (TG), cardiovascular risk factors and the severity of depressive symptoms among the elderly with DM. Conclusion: The incidence of complications would be decreased by applying a regular walking exercise program.

독거노인 자살시도 영향요인에 관한 연구: 서울 및 경기지역 노인을 중심으로 (A Study on the Determinants of Suicide Attempts among the Elderly Living Alone: Focused on Seniors in Seoul and Gyeonggi Province)

  • The purpose of this study was to examine the characteristics of suicide attempts and non-attempts of the elderly living alone and the subjects in this study were 690 elderly persons living alone in Seoul and Gyeonggi Province. For data analysis, this study used descriptive analysis, mean comparison correlation analysis, and binominal logistic regression analysis using PASW 20.0. To explain the suicide attempts of elderly persons who live alone, variables such as sex, age education level, monthly household income, religion, subjective economic status, subjective health status, ADL/IADL, depression, hopelessness, social support, and stress were used. The findings of the study were as follows: First, 9.2 percent(64) of the elderly living alone had attempted suicide and 90.8 percent(626) had never attempted suicide. Second, monthly household income, hopelessness, and social support were identified as the factors that affected their suicide attempts. In other words, the elderly living alone who had lower monthly household income, less social support, and greater feelings of hoprlessness had made more suicide attempts. As it is a cross-sectional research using data from non-probability sampling, this has a limitation in generality sampling, this has a limitation in generalizing the study results. To overcome this limitation, longitudinal research using data from probability sampling is necessary.
