Validation of the International Classification of Diseases 10th Edition Based Injury Severity Score(ICISS) (ICD-10을 이용한 ICISS의 타당도 평가)

  • Objective : To compare the predictive power of International Classification of Diseases 10th Edition(ICD-10) based International Classification of Diseases based Injury Severity Score(ICISS) with Trauma and Injury Severity Score(TRISS) and International Classification of Diseases 9th Edition Clinical Modification(ICD-9CM) based ICISS in the injury severity measure. Methods : ICD-10 version of Survival Risk Ratios(SRRs) was derived from 47,750 trauma patients from 35 Emergency Centers for 1 year. The predictive power of TRISS, the ICD-9CM based ICISS and ICD-10 based ICISS were compared in a group of 367 severely injured patients admitted to two university hospitals. The predictive power was compared by using the measures of discrimination(disparity, sensitivity, specificity, misclassification rates, and ROC curve analysis) and calibration(Hosmer-Lemeshow goodness-of-fit statistics), all calculated by logistic regression procedure. Results : ICD-10 based ICISS showed a lower performance than TRISS and ICD-9CM based ICISS. When age and Revised Trauma Score(RTS) were incorporated into the survival probability model, however, ICD-10 based ICISS full model showed a similar predictive power compared with TRISS and ICD-9CM based ICISS full model. ICD-10 based ICISS had some disadvantages in predicting outcomes among patients with intracranial injuries. However, such weakness was largely compensated by incorporating age and RTS in the model. Conclusions : The ICISS methodology can be extended to ICD-10 horizon as a standard injury severity measure in the place of TRISS, especially when age and RTS were incorporated in the model. In patients with intracranial injuries, the predictive power of ICD-10 based ICISS was relatively low because of differences in the classifying system between ICD-10 and ICD-9CM.

The Improvements of the Food and Nutrition Field in the 4th Edition of KDC (KDC 제4판 식품영양학 분야의 수정전개 방안)

  • This study investigated general problems concerning the food and nutrition field in the KDC 4th edition based comparative analysis DOC, NDC, and Disciplinary Classification System of Korean Research Foundation, and suggested on some ideas for the improvements of them. Results of the study is summarized as follows. First, the knowledge classification of the food and nutrition field is divided by nutrition, food, and food service management. Second, the library classification of the food and nutrition field deals separately with nutrition and food, namely the nutrition is subdivided in medical science and food is subdivided in home economics. Third, the food and nutrition field in the KDC 4th edition had to be improved to use current terminology, to choose appropriate headings, and to subdivide the subjects more to introduce new topics.

A Comparative Study of Subject Headings Related to Korea, China, and Japan in the LCSH (미국의회도서관 주제명표목표의 한.중.일 관련 주제명표목의 변천과정 비교 분석)

  • The purpose of this study is to analyze the historical process and characteristics of subject headings related to Korea, China, Japan in the LCSH, from the first edition to 31th ed. The analytic results show that the headings in the 31th edition include in Korea 713, China 1,742, Japan 2,647, compared to Korea headings 4, China headings 49, Japan headings 24 in the first edition. Some subject headings considered important and essential are left out. We can also recognize the some headings are relatively too subdivided. The omitted and insufficient Korean, Chinese, Japanese subject headings are considered to be tied up with library policies of LC. Therefore our active support such as donation are being called for collecting more detailed analysis of Korea, China, Japan-related publications in LC.

A Preliminary Study for Standardizing the Fine Motor Scale of the Korean-Bayley Scales of Infant and Toddler Development, Third Edition (한국형 베일리 영유아 발달검사 3판 소근육 운동척도 예비 연구)

  • The purpose of the present study was to conduct a preliminary study for developing a Korean version of the Fine Motor Scale of the Bayley Scales of Infant and Toddler Development, $3^{rd}$ Edition(Bayley-III). The subjects for this study included 313 Korean infants and toddlers aged from 16 days to 42 months and 15 days. The results of this study were as follows. Firstly, the analysis of the degree of item difficulty indicated that the arrangement of the items corresponded with child development by age. Secondly, the analysis of the inter-item consistency, the test-retest reliability and the inter-rater reliability revealed a high degree of reliability of the scale. Thirdly, the Fine Motor Scale of K-Bayley-III score showed a significant high correlation with the Motor composite score but a significant moderate correlation with the Language composite score, providing evidence of its convergent and discriminant validity. This study demonstrated that the preliminary version of the K-Bayley-III Fine Motor Scales can be applied to assess the development of fine motor skills of Korean infants and toddlers.

Designing Augmentative and Alternative Communication (AAC) Application for Children with Severe and Multiple Disabilities (중도중복장애아동을 위한 보완대체 의사소통(AAC) 앱 설계)

  • In this study, specific elements and functions in modules of the AAC (Augmentative and Alternative Communication) application for children with severe and multiple disabilities were elicited, and screen interface was designed accordingly. As results, screen configuration, communication display edition, audiovisual output, and switch and scanning modules were defined. Screen configuration module consists of communication category, spelling board, favorites, screen lock, and setting function. The Communication display edition module includes communication categories, symbols, and favorites edition. The audiovisual output module provides the ability to adjust the pitch, intensity, speed, and tone of the voice individually in the form of auditory output. In the form of visual output, the background color and size of the frame, border color and thickness are adjusted. The switch and scanning module provides a function to select by pressing the switch when the symbol cell is highlighted audibly and visually. The development of the AAC application designed in this study is needed.

Tumor volume/metabolic information can improve the prognostication of anatomy based staging system for nasopharyngeal cancer? Evaluation of the 8th edition of the AJCC/UICC staging system for nasopharyngeal cancer

  • Purpose: We evaluated prognostic value of the 8th edition of the American Joint Committee on Cancer/International Union for Cancer Control (AJCC/UICC) staging system for nasopharyngeal cancer and investigated whether tumor volume/metabolic information refined prognostication of anatomy based staging system. Materials and Methods: One hundred thirty-three patients with nasopharyngeal cancer who were staged with magnetic resonance imaging (MRI) and treated with intensity-modulated radiotherapy (IMRT) between 2004 and 2013 were reviewed. Multivariate analyses were performed to evaluate prognostic value of the 8th edition of the AJCC/UICC staging system and other factors including gross tumor volume and maximum standardized uptake value of primary tumor (GTV-T and SUV-T). Results: Median follow-up period was 63 months. In multivariate analysis for overall survival (OS), stage group (stage I-II vs. III-IVA) was the only significant prognostic factor. However, 5-year OS rates were not significantly different between stage I and II (100% vs. 96.2%), and between stage III and IVA (80.1% vs. 71.7%). Although SUV-T and GTV-T were not significant prognostic factors in multivariate analysis, those improved prognostication of stage group. The 5-year OS rates were significantly different between stage I-II, III-IV (SUV-T ≤ 16), and III-IV (SUV-T > 16) (97.2% vs. 78% vs. 53.8%), and between stage I, II-IV (GTV-T ≤ 33 mL), and II-IV (GTV-T > 33 mL) (100% vs. 87.3% vs. 66.7%). Conclusion: Current anatomy based staging system has limitations on prognostication for nasopharyngeal cancer despite the most accurate assessment of tumor extent by MRI. Tumor volume/metabolic information seem to improve prognostication of current anatomy based staging system, and further studies are needed to confirm its clinical significance.

Korean Wechsler Intelligence Scale for Children-Fourth Edition Profiles in Child and Adolescent with Attention-Deficit Hyperactivity Disorder : Retrospective Study (주의력결핍 과잉행동장애 아동청소년의 Korean Wechsler Intelligence Scale for Children-Fourth Edition 프로파일 : 후향적 의무기록 분석)

  • Objectives : The aim of this study was to investigate Korean Wechsler Intelligence profiles and specific abilities related to attention problem of children with attention-deficit hyperactivity disorder (ADHD). Methods : The Korean Wechsler Intelligence Scale for Children-fourth edition (K-WISC-IV) and Advanced Test of Attention (ATA) were administered to 91 children and adolescents (age $8.5{\pm}2.6$ years, 73 boys) with ADHD. Pearson correlation and independent t-tests were used. Results : The means of Working Memory Index (WMI) and Processing Speed Index (PSI) showed a score of low average in K-WISC-IV. WMI scores for the K-WISC-IV showed clinically significant correlations with omission errors, commission errors, and response time variability on auditory ATA. PSI scores also showed significant correlations with response time and variability on visual ATA. In addition, significantly lower digit span backward scores were observed in hyperactive-impulsive/combined subtypes compared to inattentive subtype (t=3.60, p<.001). Conclusion : Children with ADHD showed significantly lower scores in WMI and PSI which were clinically correlated with ATA scores, and hyperactive-impulsive/combined subtypes showed poorer working memory functions in WMI. Follow-up studies are proposed.

A Study on the Korean Vernacular Script Medical Classic Danbang-Biyo-Gyeongheom-Shinpyeon Written during the Period of the Japanese Occupation (일제강점기 언해한의서 『단방비요경험신편』 연구)

  • Hae-Yong Shin was a renowned merchant during the transitional period from Korean Imperialism to Japanese Occupation, and devoted his life during the period of Japanese Occupation as a proponent of patriotic enlightenment movement and translator. He also authored many medical and scientific works; in particular, he integrated the modern Western medicine into the Korean herbal medicine in his writings. His early works include New Edition of Natural History (1907), Physiology published in six series in the YaRoe, a magazine for the patriotic enlightenment movement, and the New Edition of Zoology (1908). These writings are assumed to have deepened Shin's knowledge of and insights into human and animal physiologies and anatomies. In the Danbang-Biyo-Gyeongheom-Shinpyeon (1913), he sought to incorporate the aspects of the Western medicine while mainly adopting the approach of the Korean herbal medicine. While keeping the contents and formations of Donguibogam, he recorded many empirical prescriptions and deleted theories incomprehensible for the general population, shamanic prescriptions, and poisonous and deadly ingredients. Its most salient features are the use of the Korean vernacular script for explications and simple ingredients for prescriptions. As medicinal materials, he presented commonly found low-cost native ingredients easily obtainable and affordable for. In the disciplines of childbirth, childbearing, and first aid, he adopted Western medical treatments. Danbangshinpyeon is particularly significant in that it contributed to public health by spreading practical basic medical knowledge in the vernacular script easily applicable at home in difficult situations for obtaining medical services under the Japanese colonial rule.

Design and Implementation of J2ME-based Wired/Wireless Interworking Mobile Wallet (J2ME 기반 유.무선 연동의 모바일 전자지갑 설계 및 구현)

  • Together with the activation of wireless Internet, the interest for various integrated methods of the existing wired and wireless e-commerces is on the rise. For the integrated wired and wireless e-commerce, firstly, the optimized security and authentication under the radio communication environment should be decided. And also the user-friendly usage is important. In this paper, we designed and implemented wired and wireless interworking Mobile Wallet on terminal that can be used through both wired and wireless methods by developing electronic wallet module for electronic payment using MIDP (Mobile Information Device Profile) based on J2ME (Java 2 Micro Edition). The characteristics of mobile wallet implemented in this paper provide the stability through encoding/decoding payment information and on-line usage on wired/wireless environments through downloading electronic wallet. In addition, we also present safe and convenient payment method for e-commerce based on wired/wireless Internet.

A Study on Developing Facets for Subject Headings in Korea (한국 주제명 표목의 패싯 유형 개발에 관한 연구)

  • The subject heading is an elaborate access tool for subject browsing and searching in information retrieval environment. The purpose of this study is to suggest the applicable facets to subject headings in Korea. First, the concepts of subject and the definitions of facets were investigated in the literature review. Second, six cases including OCLC's FAST, PRECIS, "Thesaurus construction and use", CC $7^{th}$ edition, BC $2^{nd}$ Edition, and UDC $3^{rd}$ Edition were analyzed to focus on configuration of facets as case studies. Based on the results, twenty-two facets were proposed including Topical, Event, Geography, Chronology, Personal and Corporate Name, Title, Form, Genre, Language, and Person facets as 11 top facets. Also, Topical-Thing/Entity and Topical-Action/Status, Part, Kind, Property, Whole, Material, Patient, Product, By-Product and Agent facets as sub-facets of Topical facet.
