Critical Review on Theory of "Eccentric Diseases are Due to Phlegm" (괴병다속담(怪病多屬痰) 이론에 대한 비판적)

  • The theory of "eccentric diseases are generally due to phlegm (怪病多屬痰)" means that phlegm is a major cause for intractable diseases including cerebrovascular accident, depression, insanity, and shock from strange things or evil sprint. It has been a key foundation for the diagnosis and treatment of phlegm in traditional Korean medicine. However, the origin of the theory is not clear and controversial. In this study, we critically reviewed the origin and developing process of the theory in the viewpoints of philology and pathology in Korean medicine. Wang Yin-Jun (王隱 君) did not claim that eccentric diseases are generally due to phlegm in Taidingyangshengzhu-lun (泰定養生主論). The miscitation by following medical literature caused the misunderstanding in the meaning of Taidingyangshengzhu-lun, that phlegm can cause variable symptoms and signs. In the Ming dynasty, some poor medical doctors had tended to diagnose any difficult case as phlegm syndrome and to use Kuntan-huan (滾痰丸) as a standard herbal formula for treating phlegm syndrome. However, the tendency to categorize delicate cases easily to phlegm syndrome is not desirable. Besides, the tendency to use Kuntan-huan as a basic formula for the phlegm syndrome might cause diverse and severe adverse effects. Thus, we cannot accept the theory of eccentric diseases is generally due to phlegm without a doubt. In conclusion, this theory might be a valuable aphorism in terms of considering the possibility of the secondary pathologic factors including phlegm and blood stasis which should be considered first in case of intractable diseases.

Types of narratives about great doctors ill literature (문헌에 나타난 명의 설화의 유형 분석)

  • Narratives are the old stories handed down for a long time, which are not personal creations but common ones for ages. They were formed from the public consciousness, passed orally and recorded in letters. There have been lots of studies on narratives about characters, whereas there have been poor in the studies on the great doctors. The author examined the narratives about great doctors in literature for successive generations, and made researches on world view and consciousness of reality reflected on the age directly or indirectly. The author inquired into the descriptions and constructions of the narratives separately, and classified the great doctors in literature into the miraculous doctors, eccentric doctors and righteous doctors. Therefore truly great doctor is the one who treats not only personal diseases but also social problems, and who tries to harmonize with human beings and nature.

Biomechanical Validation about Dumbbell Curl Exercise Effects of Virtual Environment (가상환경 변화에 따른 덤벨 컬 운동효과에 관한 운동역학적 검증)

  • Objective: The purpose of this study is to apply exercise learning effect to various subjects through training effect and information accumulation based on verification of the effect on dumbbell curl exercises applied with virtual reality. Method: To analyze the effect on the dumbbell curl exercise in the virtual environment, a total of 20 persons with 10 males and 10 females who does not have orthopedics diseases were selected. The dumbbell weight of the subjects was set to a weight of 70% strength of 1RM. At this time, the virtual environment situation was set to four types; presence/absence of virtual environment, preferred colors, and unfavorable colors to perform dumbbell curl exercise. The anaysis of muscle activity was conducted by adhering four surface electrodes (Biceps Brachii, Triceps Brachii, Brachioradialis Muscle, Extensor Carpi Radialis Longus Muscle) on the right upper limbs. Independent sample t-test using SPSS (24.0) program was carried out to analyze average values and standard deviations for each variable depending on the presence/absence of virtual environments and changes in color (preferred colors, unfavorable colors) and the level of significance was set to a=.05. Results: In the eccentric contraction, males showed high muscle activity in the Biceps Brachii under virtual reality. On the other hand, females had high muscle activity in the Biceps Brachii in the absence of virtual reality. Also, in case of a change of colors in the virtual environment, females had the high muscle activity in the unfavorable color in the eccentric contraction. Conclusion: During the dumbbell curl exercise, results of different exercises present depending on gender. When males put VR on and performs a basic dumbbell curl exercise, the effect of Biceps presents Brachii for them while exercising in unfavorable colors. However, since it is the basic research data of muscle exercise using virtual reality, it is necessary to verify whether or not it is effective for myopachynsis through long-term training rather than unity.

Primary Pulmonary Hypertension in a Maltese Dog (말티즈 견에서 발생한 원발성 폐동맥 고혈압증)

  • A 1-year-old castrated male Maltese dog(weighing 2.4 kg) was presented with primary complaints of occasional coughing, dyspnea and exercise intolerance. Based on diagnostic findings including paradoxical split S2, diastolic regurgitant murmur, marked dilation of pulmonary artery, right ventricular eccentric hypertrophy with thickening of ventricular septum, severe tricuspid and pulmonic regurgitation(5.4 m/sec and 3.4 m/sec, respectively) and the absence of any congenital intracardiac shunting, obstructive pulmonary diseases and systemic diseases associated with right ventricular pressure overload or pulmonary hyperperfusion, the case was tentatively diagnosed as primary pulmonary hypertension. The dog was treated with furosemide, aspirin and oxygen supplementation. This case report described a rare case of primary pulmonary hypertension in a Maltese dog.

  • The middle mediastinum contains several important organs and pluripotent cells. It is difficult to make a definitive diagnosis in patients with middle mediastinal tumors due to a wide range of diseases. The likelihood of malignancy is influenced primarily by the following factors: patient age, size, tumor location, and the presence or absence of symptoms. We describe a case of a middle mediastinal tumor, which was suspected on chest x-ray; chest computed tomography revealed the eccentric mass of distal esophagus. This case emphasizes the diagnostic importance of the chest x-ray to the physicians. The possible differential diagnoses are reviewed.

Are Blood Blister-Like Aneurysms a Specific Type of Dissection? A Comparative Study of Blood Blister-Like Aneurysms and Ruptured Mizutani Type 4 Vertebral Artery Dissections

  • Objective : Blood blister-like aneurysms (BBAs) resemble arterial dissections. The purpose of this study was to investigate the relationship between these two disease entities and highlight commonalities and distinct features. Methods : Among 871 consecutive patients with aneurysmal subarachnoid hemorrhage, 11 BBAs of internal carotid artery and seven vertebral artery dissections (VADs) with a short segmental eccentric dilatation (Mizutani type 4), which is morphologically similar to a BBA, were selected. The following clinical factors were studied in each group : age, gender, risk factors, Hunt and Hess grade (HHG), Fisher grade (FG), vasospasms, hydrocephalus, perioperative rebleeding rate, and treatment outcome. Results : The mean age was 47.9 years in the BBAs group and 46.4 years in the type 4 VADs group. All the BBA patients were female, whereas there was a slight male predominance in the type 4 VAD group (male : female ratio of 4 : 3). In the BBA and type 4 VAD groups that underwent less aggressive treatment to save the parent artery, 29% (n=2/7) and 66.6% (n=2/3), respectively, eventually required retreatment. Perioperative rebleeding occurred in 72.7% (n=8) and 28.6% (n=2) of patients in the BBA and type 4 VAD groups, respectively. There was no statistical difference in the other clinical factors in both groups, except for the male dominancy in the type 4 VAD group (p=0.011). Conclusion : BBAs and ruptured type 4 VADs have a similar morphological appearance but there is a distinct clinical feature in gender and perioperative rebleeding rates. Complete isolation of an aneurysm from the parent artery might be the most important discipline for the treatment of these diseases.

Effect of CLT and Sling Exercise Combined Program on Knee Pain in Patellofemoral Pain Patient: A Single-Subject Study (협응이동훈련과 슬링운동 결합 프로그램이 무릎넙다리통증 환자의 무릎통증 및 기능적 움직임에 미치는 영향: 단일사례 연구)

  • Background: Patellofemoral pain is one of the common diseases of the musculoskeletal system. Many previous studies have recommended the application of exercise therapy to patellofemoral pain patients for treatment. The purpose of this study was to investigate effect of coordinative locomotor training (CLT) and sling exercise combined program on knee pain and functional movement in patellofemoral pain patient. Methods: In this study, single-subject design (A-B-A') was conducted for 6 weeks. A repeted-measure analysis was conducted to assess results of the anterior knee pain scale (AKPS), Clarke's test (CT), eccentric step down test (ESDT). During the intervention (B), the CLT and sling exercise combined program was conducted three times a week for 4 weeks. Results: From baseline period A to intervention period B, the AKPS, CT, ESDT were improved from 61 to 48 (27%), from 8.33 to 3 (64%), from 7.67 to 3,58 (53%). From baseline period A to baseline period A', the AKPS, CT, ESDT were improved from 70.67 to 48 (47%), from 0.67 to 3 (92%), from 1.33 to 3,58 (83%). Conclusion: Based on the results of this study, we recommend the application of CLT and sling exercise combined program to improve the pain and functional movement in patellofemoral pain patients.

Changes in Distribution and Morphology of Rat Alveolar Macrophage Subpopulations in Acute Hyperoxic Lung Injury Model (고농도 산소로 유발한 흰쥐의 급성폐손상모델에서 폐포대식세포 아형군의 분포와 형태 변화)

  • Background : In acute lung injury, alveolar macrophages play a pivotal role in the inflammatory process during the initiation phase and in the reconstruction and fibrosis process during the later phase. Recently, it has been proven that alveolar macrophages are constituted by morphologically, biochemically and immunologically heterogenous cell subpopulations. The possibility of alterations to these characteristics of the alveolar macrophage population during lung disease has been raised. To investigate such a possibility a hyperoxic rat lung model was made to check the distributional and morphological changes of rat alveolar macrophage subpopulation in acute hyperoxic lung injury. Method : Alveolar macrophage were lavaged from normal and hyperoxic lung injury rats and separated by discontinuous gradients of percoll. After cell counts of each density fraction were accessed, the morphomeric analysis of alveolar macrophages was performed on cytocentrifuged preparations by transmission electron micrograph. Result : 1. The total alveolar macrophage cell count significantly increased up to 24 hours after hyperoxic challenge (normal control group $171.6{\pm}24.1{\times}10^5$, 12 hour group $194.8{\pm}17.9{\times}10^5$, 24 hour group $207.6{\pm}27.1{\times}10^5$, p<0.05). oHoHH However the 48 hour group ($200.0{\pm}77.8{\times}10^5$) did not show a significant difference. 2. Alveolar septal thickness significantly increased up to 24 hours after hyperoxic challenge(normal control group $0.7{\pm}0.2{\mu}m$, 12 hour group $1.5{\pm}0.4{\mu}m$, 24 hour group $2.3{\pm}0.4{\mu}m$, p<0.05). However the 48 hour group did not show further change ($2.5{\pm}0.4{\mu}m$). Number of interstitial macrophage markedly increased at 24 hour group. 3. Hypodense fraction(fraction 1 and fraction 2) of alveolar macrophage showed a significant increase following hyperoxic challenge ($\beta=0.379$.$\beta=0.694$. p<0.05) ; however, fraction 3 was rather decreased following the hyperoxic challenge($\beta=0.815$. p<0.05), and fraction 4 showed an irregular pattern. 4. Electron microscopic observation of alveolar macrophage from each fraction revealed considerable morphologic heterogeneity. Cells of the most dense subfraction(fraction 4) were small, round, and typically highly ruffled with small membrane pseudopods. Cells of the least dense fraction (fraction 1) were large and showed irregular eccentric nucleus and high number of heterogenous inclusions. Conclusion : In conclusion, these results suggest that specific hypodense alveolar macrophage subpopulation may play a an important role in an acute hyperoxic lung injury model But further study, including biochemical and immunological function of these subpopulations, is needed.
