Outcomes of Extracorporeal Membrane Oxygenation in Children: An 11-Year Single-Center Experience in Korea

  • Background: Extracorporeal membrane oxygenation (ECMO) has become an important treatment modality in pediatric patients with cardiopulmonary failure, but few studies have been conducted in Korea. Methods: We conducted a retrospective review of pediatric patients younger than 18 years who were placed on ECMO between January 2004 and December 2014 at Samsung Medical Center. Results: We identified 116 children on ECMO support. The overall rate of successful weaning was 51.7%, and the survival to discharge rate was 37.1%. There were 39, 61, and 16 patients on ECMO for respiratory, cardiac, and extracorporeal cardiopulmonary resuscitation, respectively. The weaning rate in each group was 48.7%, 55.7%, and 43.8%, respectively. The survival rate was 43.6%, 36.1%, and 25.0%, respectively. Sixteen patients on ECMO had functional single ventricle physiology; in this group, the weaning rate was 43.8% and the survival rate was 31.3%. Ten patients were on ECMO as a bridge to transplantation (8 for heart and 2 for lung). In patients with heart transplantation, the rate of survival to transplantation was 50.0%, and the overall rate of survival to discharge was 37.5%. Conclusion: An increasing trend in pediatric ECMO utilization was observed. The outcomes were favorable considering the early experiences that were included in this study and the limited supply of specialized equipment for pediatric patients.

The Here and Now of Admission Counseling at Child-Care Centers - With a Focus on the Episodes and Metaphors as Used by the Principals - (어린이집 입학상담의 지금-여기(here and now) : 원장들의 에피소드와 은유를 중심으로)

  • The purpose of study is to understand how a principal of a child-care center and parents accept different episodes and experiences in admission counselling and their use of metaphorical expressions about this, and to illuminate the current state of play by examining the requirements of parents in selecting a child-care center. The subjects were 10 principals of private child-care centers located in the Gyeongsangbuk province of Korea, and the research materials for episodes and metaphors were collected and analyzed by means of in-depth and telephone interviews during an application period. In terms of the overall climate of the admission counselling, as shown in the various episodes, our results indicated that an increase of parents' demands occurred through a primarily consumer-centered environment, and the sudden increase in a fathers' participation, the inside circumstances of admission counselling in progress throughout the year, the non-existence of respect and credibility and the continuity of conflicts and were all observed to co-exist. In terms of the emotional meaning of counselling via metaphors, the following emotions could be inferred; hopelessness, anger, lethargy, stress, fear and eagerness. It is anticipate that this study will make a meaningful contribution to better illuminating and improving the current climates of here and now admission counselling.

Moderating Effects of Personal Innovativeness on the Relationship between Perceived Usefulness, Subjective Norm and Intention to Use Mobile Internet (인지된 유용성과 주관적 규범이 모바일인터넷 사용 의도에 미치는 영향에 있어 개인 혁신성향의 조절효과에 대한 연구)

  • Recently, utilization of mobile Internet services is rapidly increasing. To properly accelerate the vitalization of mobile Internet usage, we have to know exactly about the factors affecting potential users' value recognition and intention to use. Technology acceptance model (TAM) is one of the most powerful reference framework for the individual user's adoption of IT innovations. This research borrowed core concept of TAM, and tried to explain users' intention to use mobile Internet by understanding the direct and indirect effects of subjective norm and perceived usefulness. To increase the explanatory power of the model, this paper introduced personal innovativeness as a key moderator variable. The research findings include; i) subjective norm has strong positive influence on perceived usefulness and intention to use, ii) perceived usefulness has positive influence on intention to use, iii) the influence of subject norm is greater than that of perceived usefulness, iv) positive influence of subjective norm on the perceived usefulness and intention to use are significantly stronger in the case of low personal innovativeness sub-group than the case of high personal innovativeness sub-group, and v) high personal innovativeness sub-group have shown strongly positive influence of perceived usefulness on the intention to use while there is no such significant relationship in the case of low personal innovativeness sub-group. Based on these findings, this paper proposed several guidelines to increase the adoption and the usage of mobile Internet; i) word-of-mouth is extremely important, ii) to form a good word-of-mouth, it will be very important to make early adopters and heavy users have positive direct experiences of using mobile Internet.

A Study on Factors Influencing Kindergarten Teachers' Science Anxiety (유치원 교사의 과학 교수 불안에 영향을 주는 교사 내 . 외적 요인)

  • The purpose of this study was to explore factors influencing kindergarten teachers' science anxiety. The subjects were 182 kindergarten teachers. The career, educational background, scientific literacy, science-related experience, type of kindergarten, age of classroom students, science supporting environment of kindergarten, and students' interest in science were examined as possible factors affecting teachers' science anxiety. The results showed that the science-related experience, science supporting environment of kindergarten, and students' interest in science were related to the teachers' science anxiety. However, the most important factor influencing kindergarten teachers' science anxiety was science-related experience. These results can be interpreted that positive science experiences affect decreasing teachers' science anxiety

A Study on the Health Education Need Assessment of Rural Community (농촌지역사회의 보건교육 요구도에 관한 연구)

  • At the opening of a new millennium and a new century, health promotion and education services in Korea are in the early developmental stage. The National Health Promotion Act legislated in 1995 was a milestone for initiating a national and local health promotion program in Korea. And since then local governments and health centers have been developing and providing health promotion program for the community populations. The short history of health promotion and education in Korea has meant that local governments and health centers have a limited experience and organizational capacity for health promotion and education planing and practice. This study was attempted to measure health education need of rural community and to analyze the factors for health education need assessment. Surveyors interviewed 1250 subjects randomly selected. Subjects were 2.17% of men and women in Changnyung county and older then 20 years old. Data were collected from April 17, 2000 through April 27, 2000. The questionnaire consisted of general characteristics, health educational experiences, health educational method, health educational content and health educational needs for rural community residents. The questions on the health educational needs of content consist of 36 questions in 8 fields. The statistical methods used for the analysis were $X^2$-test, t-test, F-ratio and ANOVA using SPSS program. In conclusion, despite more needs to the respondents who are in the low education level and socioeconomic state, in the old age, in the low health knowledge, they required less health education. To enjoy a more healthy life after more community residents actively understand and are interested in health education and health promotion, we certainly require a designed and systemic health education. The resources of health department in Korea are limited and the investment involved in health promotion and health education is severely reduced. Particularly this situation is more severe in the rural community. To select and perform an effective health education methods that the nature and reality of the rural community are considered, well use the resources to invest in health promotion affairs as effectively as possible and then they will take the responsibility of healthy community.

  • This study focuses on the staffs of the Governmental Organization of Construction who actually led this organization. Two main points of this study are as follows; 1) The composition and appointment of staffs, before and after the Korea-Japan Treaty in 1907, are evaluated, and 2) The working and academic career of each personnel are analyzed in connection with understanding of internal characteristics of this organization. Special attention is given to Korean personel and Japanese engineers who undertook a leading role. Korean engineering staffs were very few in number. It is thought, viewing their records of careers, that almost all of them didn't have any engineering basis, and simply they had some relations to Japan. It is certain that their role in this organization was interpreters using their Japanese language ability, and their roles were only limited to non-technical areas, if any. The early engineers from Japan were selected among the officials with comparatively rich careers of the Temporary Architectural Dept. or the Temporary Engineering Dept. of Custom House in Japan. But, in later days, the quality of Japanese engineers changed to the level of premature without practical experiences who unwillingly came to Korea to have a job under the depression of construction economy of Japan. The academic careers of Japanese engineers were various, from self-taught to regural college education, but the leading engineers were from Tokyo University. The civil engineers became high-level technical officials in the Government-General after the Japanese Annexation of Korea. But, sometimes later, many of the architectural engineers became practicing architect managing design offices on private basis.

A Comparative Study on the Health Status of the Elementary Students Living in a certain Industrial area and a certain Residential area in Busan (부산시내 일부 공단지역과 주거지역 초등학생의 건강상태 비교연구)

  • This study was conducted to suggest a necessity in student health promotion device by comparing the health status and identifying the environmental health influence of the elementary students in Busan. Five schools were selected as sample group based on the number of air pollutant emitting facilities and whole 6th grade 825 students(391 from industrial area and 434 from residential area) were selected from these elementary school. The data were collected from convenient sample based on written questionnaires from 1st to 30th, Nov, 2001. The student group who lives in an industrial area experiences more the symptoms of the eyes(p=.033) and a cold(p=.000)(p=.015), more frequent indirect smoking condition(p=.023, p=.001), and think ill of the relation about air pollution and health(p=.002), cognition of the surroundings(p=.000), and perception of subjective health status(p=.000). There were not significant differences in the number of student in asthma score by their residents, but there were significant differences at the dwelling term of the students in asthma score(P=.002). According to the above results, there must be a device of preventive treatment and early diagnosis of disease needed for the student in industrial areas because they are in relatively poor health and social economic status than residential areas.

Effects of Expanding Methods on Residual Stress of Expansion Transition Area in Steam Generator Tube of Nuclear Power Plants (원전 증기발생기 전열관 확관법이 확관부위 잔류응력에 미치는 영향)

  • The steam generator tubes of nuclear power plants are pressure boundaries, and if tubes are leaked, the coolant with the radioactive materials was flowed out from the primary system to the secondary system and polluted the plant and the air. Recently most crack defects of tubes are stress corrosion cracks and these defects are located in expansion transition area, sludge pile-up region, and U-bend area. The most effective one of crack initiation factors in expansion transition area and U-bend area is the residual stress. According to the experiences of Korea standard nuclear plants(Optimized Power Reactor-1000), they had the stress corrosion cracks at the tube expansion transition area in early operating stage and especially lots of circumferential cracks were occurred. Therefore in this study, the distributions and conditions of residual stresses by tube expansion methods were compared and the dominant reason of a specific direction was examined.

An Empirical Analysis on the Determinants of VOD Viewing Pattern of Users in IPTV Platform (IPTV에서의 VOD 시청패턴 결정 요인에 관한 실증 분석)

  • This paper empirically analyzes specific characteristics of VOD users and their practical viewing patterns, using socio-demographic data of the current IPTV subscribers in combination with actual data of genre usage and payment. The findings revealed active viewing patterns of male, the unemployed, high earners and early adopters. In terms of preferences, households with large numbers of women prefer time shift contents, whereas households composed of more men or preschoolers prefer non-time shift contents. Likewise, the households that have more women or higher income had relatively a lot of experiences of purchasing time shift contents on one hand, but the households characterized by the larger numbers of family members or unemployed householder or young householder showed much willingness to pay premium contents on the other hand. Given the utilization of correct database, the findings offer useful information conducive to service promotion and marketing strategies to maximize VOD use in the practical dimension.

Living Experience of Unmarried Mothers before Childbirth (출산 전 양육비혼모 삶의 경험)

  • The purpose of this study was to give a deep understanding of the structure and essence of the living experiences for unmarried mothers before childbirth and before pregnancy. As results, the theme 'days stained with wounds' reveals the living experience that they were abandoned or not protected by parents due to broken family relationships and function. In the theme 'daily life at stake', to drop out of school and running away from home led them to precarious life. And also they were early exposed to sexual intercourse due to indiscriminate sexual culture and poor sex awareness. The theme 'embarrassing pregnancy' shows that unmarried mothers got pregnant caused by their partners'refusal to practice contraception, their incorrect knowledge of the contraception, and the use of incorrect contraceptive methods. In the final theme 'childbirth decided on with difficulty', even if unmarried mothers had no choice in childbirth because they noticed their pregnancy too late, they rejected abortion and decided to give birth as they realize preciousness of life. Based on the results of this study, the necessity of implementing sex education to provide accurate contraceptive knowledge and methods for male, as well as strengthening sex education for unmarried mothers was discussed. With the help of information technology.
