Health-related Quality of Life of Patients With Diabetes Mellitus Measured With the Bahasa Indonesia Version of EQ-5D in Primary Care Settings in Indonesia

  • Objectives: Diabetes mellitus (DM) is a serious public health issue that places a heavy financial, social, and health-related burden on individuals, families, and healthcare systems. Self-reported health-related quality of life (HRQoL) is extensively used for monitoring the general population's health conditions and measuring the effectiveness of interventions. Therefore, this study investigated HRQoL and associated factors among patients with type 2 DM at a primary healthcare center in Indonesia. Methods: A cross-sectional study was conducted in Klaten District, Central Java, Indonesia, from May 2019 to July 2019. In total, 260 patients with DM registered with National Health Insurance were interviewed. HRQoL was measured with the EuroQol Group's validated Bahasa Indonesia version of the EuroQoL 5-Dimension 5-Level (EQ-5D-5L) with the Indonesian value set. Multivariate regression models were used to identify factors influencing HRQoL. Results: Data from 24 patients were excluded due to incomplete information. Most participants were men (60.6%), were aged above 50 years (91.5%), had less than a senior high school education (75.0%), and were unemployed (85.6%). The most frequent health problems were reported for the pain/discomfort dimension (64.0%) followed by anxiety (28.4%), mobility (17.8%), usual activities (10.6%), and self-care (6.8%). The average EuroQoL 5-Dimension (EQ-5D) index score was 0.86 (95% confidence interval [CI], 0.83 to 0.88). In the multivariate ordinal regression model, a higher education level (coefficient, 0.08; 95% CI, 0.02 to 0.14) was a significant predictor of the EQ-5D-5L utility score. Conclusions: Patients with diabetes had poorer EQ-5D-5L utility values than the general population. DM patients experienced pain/discomfort and anxiety. There was a substantial positive relationship between education level and HRQoL.

CRS installation performance in Korean Market (한국시장에서의 카시트 장착성 평가)

  • Motor vehicle crashes are the leading cause (70%) of children injury and fatality. 98% of Children under the age of 9 who came to emergency room with a traffic accident are not seated in CRS. Reduction in mortality when using CRS (NHTSA study), 71% for infants of 1-2 years old, 54% for 3-6 years old and 7-12 years old. It is very important and essential to use CRS for child passenger safety. But in Korean market, the CRS fitment rate is relatively low (20~40%). For European market, at the start of 2013, Euro NCAP started to check how easy it is to install the most common seats. And US market, NHTSA is planning a new consumer information program of CRS fitment for near future. This study examines CRS installation performance of 3 vehicles which are produced by GM Korea and 21 CRS which are based on sales volume in Korean market. Purpose of this study is to identify both vehicle and CRS design that causes bad interaction for CRS installation.

RESEARCH ON ULTRA LOW EMISSION TECHNOLOGY FOR LARGE DISPLACEMENT MOTORCYCLES

  • With the aim of achieving half the regulated value of EURO-3 Emission Regulations, an ultra low emission motorcycle has been developed based on a motorcycle with an 1800 $cm^3$, horizontal opposed 6-cylinder engine. For the fuel supply system, an electronically controlled fuel injection system was applied. For the emission purification system, three-way catalysts, a feedback control system with a LAF(Linear Air-Fuel ratio) sensor, and a secondary air induction system were applied. To reduce CO and HC emissions during cold starting, an early catalyst activation method combining RACV(Rotary Air Control Valve) and retarded ignition timing was applied. After the catalyst activation, air-fuel ratio was controlled to maximize the purification ratio of the catalyst according to vehicle speed. For the air-fuel ratio control system, the LAF sensor was used. Furthermore, fine adjustment by the LAF feedback control reduced torque fluctuation due to the air-fuel ratio change. As a result, smooth ride feeling was maintained. Owing to these technologies, half the regulated value of EURO-3 has been achieved without any negative impact to the large-scaled motorcycles' drivability. This paper presents the developed ultra low emission technologies including the control method using an LAF sensor.

Estimation of Real-Driving NOx Emission Characteristics from Light-Duty Diesel Vehicles with PEMS (PEMS를 이용한 소형 경유차의 실주행 NOx 배출특성 평가)

  • $NO_x$ emissions from diesel vehicles have been regarded as a main cause of high $NO_2$ concentration in metropolitan area. Recent studies have shown that the on-road $NO_x$ emissions of diesel vehicles are quite higher than the emission limits specified with the pre-defined test method for emission certification. To reduce air pollutants effectively, the discrepancy of emissions in certification and real-driving conditions should be tackled. In this study, the real-driving emissions have been estimated with portable emission measurement system (PEMS). The results of this study have shown that the on-road $NO_x$ emissions from diesel vehicles have been decreased as the introduction of stricter emission regulation, EURO-6, but additional reduction should be still required and robust technologies should be applied to control $NO_x$ in real-driving conditions. RDE-LDV (Real Driving Emission - Light Duty Vehicles) test method being developed in the European Union can represent excessive on-road $NO_x$ emissions of diesel vehicles as applied emission technologies and can be a solution to remove discrepant $NO_x$ emissions between certification and Korean real-driving conditions. Among the $NO_x$ reduction technologies for EURO-6 diesel vehicles, selective catalytic reduction (SCR) system has shown the better performance than lean $NO_x$ trap (LNT) system to control on-road $NO_x$ emissions. Implementing RDE-LDV will require vehicle manufacturers to adopt the more effective $NO_x$ reduction technology in real driving conditions.

  • $NO_X$ reducing technique such as LNT, LNC, and selective catalytic reduction (SCR) have been developed and applied, especially on heavy-duty vehicles. However, it is expected that $NO_X$ reduction techniques will also be applied to diesel passenger vehicles. The urea-SCR system is receiving attention as the most effective $NO_X$ reduction technology without a fuel penalty. Thus, many advanced countries are developing this technology. The urea-SCR system sprays an aqueous urea solution that separates $NO_X$ into $N_2$ and $H_2O$, which are harmless and emitted into the atmosphere. The urea injected in front of the SCR catalyst should be changed to 100% $NH_3$, which is required for $NO_X$ reduction in the SCR system to maximize the reduction efficiency. The purpose of this study was to determine the basic data for the urea-SCR system to maximize the $NO_X$ reduction efficiency by understanding the $NO_X$ reduction characteristics in a real passenger vehicle to comply with the post EURO-6 emission regulation.

Association between Insulin Resistance and Depression in the General Korean Population : Cross Sectional Findings from the National Health and Nutrition Examination Survey (한국 성인에서 인슐린 저항성과 우울증의 연관성)

  • Objectives : The purpose of this study was to investigate the association between the insulin resistance and depressive mood, suicidal ideation, psychological stress, and quality of life in general Korean population. Methods : We selected 3,613 subjects from the third year's data of 6th Korean National Health and Nutritional Examination Survey. Insulin resistance was evaluated with Homeostatic Model Assessment of Insulin Resistance. Questionnaires on depressive mood, suicidal ideation and psychological stress were conducted. The quality of life was assessed with the EuroQol-5 dimension index. We used correlation and regression analysis for analysis. Results : The risk of depressive mood and suicidal ideation was significantly higher in the insulin resistance group than in the control group. The risk of psychological stress was significantly higher in the male group with insulin resistance. The EuroQol-5 dimension index showed a negative correlation with insulin resistance. After adjusting for age, sex and body mass index, increased risk of suicidal ideation was identified only. Conclusions : We confirmed that insulin resistance is associated to mental health problems related with depression in Korean adults.

Optimal Issuance Price of Carbon Credits in the Energy Industry (에너지산업 분야 탄소배출권의 적정 발행가격 분석)

  • In this study, the optimal level of CER issuance price in the energy industry was estimated using a real options considering the uncertainty of emission price. As a result of the analysis, the break-even point for CDM projects in the energy industry registered by UNFCCC from December 2012 to the end of 2021 was 0.64-36.69 euros per ton of CO2 for each individual project. More closely, the emission permit price that reaches the break-even point when NPVw/o CER+ NPVCER ≥ 0 is estimated to be 12.10 euros on average, and the emission permit price that reaches the break-even point when NPVw/o CER + NPVCER ≥ option value is estimated to be 12.63 euros on average. Meanwhile, the option value using real options to reduce business uncertainty is about 19% at the 1-5 euro per ton level, about 11% at the 5-10 euro per ton level, and about 5% at the 10-15 euro per ton level. It was analyzed that there was an effect of increasing emissions prices due to uncertainty reduction. The results of this study may be useful to greenhouse gas reduction project entities, including investors, project operators, and companies with potential mandatory reductions.

  • Since 2007, the defect confirmation test for vehicles using PEMS has been enforced in USA. This test can measure emissions from on-street vehicles using a device mounted on a car. Europe has confirmed its plan for introducing this test from EURO6, 2013. Thus, the Korean government is also under pressure to adopt this method that reflects the real-world driving conditions using PEMS, considering the emission controls for domestic heavy-duty vehicles. To provide various utilizations of the PEM, this emission test has been developed in accordance with the type of driving road, DPF, ISG, and air conditioner. This research aims to provide the fundamental materials for implementing defect confirmation tests for commercial vehicles, which are appropriate for domestic emission control situations, after studying the defect confirmation test methods for heavy-duty vehicles using PEMS.

Injury Study of Older Children Anthropomorphic Test Device with CRS Harness Belt and Vehicle Level Crash Test (CRS 하네스 벨트 사용에 따른 어린이 인체 모형 상해 연구 및 실차 레벨 충돌 평가)

  • For years, Q1.5 (anthropomorphic test device for 1.5 years old infant) and Q3 (anthropomorphic test device for 3 years old infant) dummy protection has been improved considerably by the effort of EuroNCAP. ISOFIX strength of vehicle structure has increased and many child occupant protection tests have made child restraint system (hereafter CRS) optimized for child safety. However, from 2016, EuroNCAP changed the dummy which is used for the child occupant protection from Q1.5/Q3 to Q6/Q10 and these were also adopted in KNCAP from 2017. Therefore, a new method is required to secure the safety for older children In this research, child dummies were tested by using adult safety systems, and the different results from each adult restraint system were compared. Finally, dummies were tested with the CRS harness belt commonly used for infants, which has yielded significant result. In this research, mid-sized sedan and small SUV were used for the test. The researchers of this paper performed sled tests to correlate between the different adult safety belt system and child injury. Following the sled test, an actual vehicle test was conducted to gather the injury data of Q-dummy with the CRS harness belts. This paper will show the advantages of applying a pre-tensioner in the second row for child protection and the necessity of CRS which has its own harness belts to improve safety for older children.
