Analysis of Relevant Factors and External Quality Assessment in Small-Medium Sized Hospital (중소의료기관의 외부정도관리 시행과 관련요인 분석)

  • Korean hospitals implement external quality assessment (EQA) schemes to improve the quality of clinical tests. However, smaller hospital do not implement EQA due to lack of appreciation and cost burdens. The purpose of this study was to analyze the results of EQA from the perspective of clinical chemistry carried out at a small-to-medium sized hospital (SMH) and to investigate factors influencing quality control. The medical institution concerned had performed EQA on 21 items annually for three years and had analyzed measurement methods, variation coefficients, and anomalous results as defined by the SMH. No significant change in test methods or variation coefficients, which exhibited a high level of variation, were observed for any test item over the 3-year period. The results obtained showed that anomalous test results were significantly more frequent when EQA was not conducted (p<0.05), internal quality control was not conducted daily (p<0.01), and when tests were less frequently performed (p<0.01). Small-to-medium sized hospitals need to be more aware of the benefits of EQA and provide tailored education to staff.

Quality of Same-Day Sputum Smears Microscopy and Presumptive Tuberculosis Patients Drop-out at Health Facilities of Addis Ababa, Ethiopia

  • Background: Same-day sputum microcopy is recommended in areas where sputum smear microscopy external quality assessment (EQA) is effectively implemented and sturdy. In Addis Ababa, the status of EQA and drop-out of same-day sputum smear microcopy has not yet been assessed. The objective of this study was to assess the quality of same-day sputum smear microscopy and diagnostic drop-out of presumptive tuberculosis (TB) patients in health facilities (HFs) across Addis Ababa, Ethiopia. Methods: Amulti-analysis was conducted from September 2016 to July 2017 to determine the status of external quality assessment and diagnostic drop-out of presumptive TB patients registered for same-day sputum smear microscopy. Data was coded and entered in Microsoft Excel, and subsequently transferred and analyzed using SPSS version 20.0. Results: The drop-out of same-day sputum smear microscopy was 209 (6.2%). More than 33% of the specimens collected for purposes of same-day sputum smears were of poor quality. Among the selected HFs for the study: 13 (46.4%) used filter reagents prior to sputum smear staining while 75% of the selected HFs for the study used smear microscopy services interruption in a year. The sensitivity and specificity of the HFs participating in regional quality assessment scheme for the diagnosis of TB was 97.4% and 99.6%, respectively. Conclusion: The diagnostic drop-out of same-day sputum smear microscopy was high in Addis Ababa. Strengthening EQA, competency-based laboratory professionals training on sputum smear microscopy might reduce the reading errors in sputum smear. Awareness creation of the community on the benefits gained from completion of specimen provision for the same-day approach decreases diagnostic drop-out and enhances TB control program.

  • The production, processing, sale and service of food materials are not subject to attainment via a single country. In accordance with internationalization of the world, issues related to food safety have emerged as critical international concerns and they are closely associated with the health and interest of domestic consumers, producers, manufacturers, and distributors. As a third party certification standards, ISO 22000 is in progress International Organization for Standardization(ISO) and the Draft International Standard(DIS) has already been presented on December 2004. The purpose of this study is to analyze the international standards, guidelines and legislation in regard of Food Safety Management System(FSMS) including ISO 9001, Hazard Analysis Critical Control Point(HACCP) and Product Liability(PL) so as to present Korean-model Food Safety Management System requirements and system establishment model.

Some Suggestions to Improve the Korean Quality Awards Systems through the Comparison among Asian Countries' Awards Systems (한국 국가품질상 개선방안에 관한 연구: 아시아 국가들과의 비교를 중심으로)

  • Many Asian countries operate the national quality award systems. Most of the evaluation criteria of the award systems are generally based on the MalCom Baldrige National Quality Award System of U.S.A. which started since 1987. But their operation systems for awards are different by countries. Comparing the differences among them can give a good benchmark for Korean Quality Award systems. There are poor empirical results that the Korean Quality Awards systems improve the performances of the organizations awarded. This paper proposes some suggestions for the effective operations of Korean quality award systems through the comparison among Asian countries' awarding systems.

Six Sigma Analysis of Vitamin D Measurement Using External Quality Assessment Program (외부정도관리 프로그램을 이용한 비타민 D 검사의 6 시그마 분석)

  • Standardization of vitamin analysis continues around the world, and much effort has been made to improve the accuracy of the results. This study analyzed the sigma metrics of the vitamin D test using the external quality assessment (EQA) program. Sigma metrics is used for quantitative tests performed in the laboratory, and the test results can be objectively visualized in terms of quality. This analysis was performed based on the accuracy of the College of American Pathologists (CAP) using the results of the 2019 accuracy-based vitamin D (ABVD) survey, and about 300 laboratories participated in the survey. Reference values were obtained by the Center for Disease Control and Prevention (CDC) reference laboratory. At six different concentrations, the sigma metrics were analyzed to be 1.00, 1.85, 2.42, 1.01, 1.54 and 0.78, respectively. An average of 1.43 sigma metrics was determined. In particular, only positive biases for ABVD-16 and 17 were shown in the liquid chromatography tandem-mass spectrometry (LC-MS/MS), which is the standard method for vitamin D determination when compared to the reference values. The causes of the difference can be explained by cross reactivity to various vitamin D metabolites. Laboratories need to improve their overall performance.

Research on Technopark Management Performance Comparison Based on National Quality Awards Appraisal Standard by Countries (국가별 국가품질상 평가기준에 따른 테크노파크 경영실적 비교 연구)

  • Purpose: Most major countries have their own set of qualifications called national quality awards to measure the quality of companies and organizations. This study analyzes 3 different national quality awards and compare with the result from Korean quality awards conducted by Ministry of knowledge and Economy and Korea institute for advancement of technology. Methods: We tested 17 technoparks out of 18 technoparks in Korea and see how different the results can be depends on the value weights. We closely looked at each qualifications and tables of different countries' awards and compared with one used in Korea. Finally we proposed some suggestions to use not only domestic model but also international ones to be objective and add efficiency to organizations. Results: Depend on similarity of qualifications and weights, there were countries with different results and these caused score and ranking changes. Nevertheless, there was a comparison that did not make any changes on both score and ranking. Conclusion: We recognized the limitation that a standardized quality variation cannot be enough sources to test and analyze technoparks with different size and criteria. Integrating global standards and flow would be the first step to help grow technoparks and organizations placed in Korea in days to come.

  • Purpose: Propose model to diagnose and assess National Defense Industry and quality management by investigating and analyzing established standard model. Methods: Research on established internal model including MB, EQA and JQA model and make standard index for quality index extraction and quantitative index to test objectively for internal state. Results: Extract advantage and disadvantage by performance of National Defense Industry level diagnose and build foundation for quality management policy and road map. Conclusion: Due to result of diagnostic assesment of quality management of national defense industry, dependability, SCQM and safety part shows vulnerability and require improvement and support.

The Evaluation of Proficiency Test between Radioimmunoassay and Chemiluminescence Immunoassay (방사면역측정법과 화학발광면역측정법간의 숙련도 비교평가)

  • Purpose: To establish accurate external quality assurance (EQA) test, cross institutional and modality tests were performed using WHO certificated reference material (CRM) and same pooled patients serum. Materials and Methods: Accuracy and precision were evaluated using CRM and pooled patients' serum for AFP, CEA, PSA, CA 125, CA 19-9, T3, T4, Tg, TSH. To evaluate the accuracy and precision, recover test and coefficient variation were measured. RIA test were performed in major 5 RIA laboratory and EIA (CLIA) test were done in 5 major EIA laboratory. same sample of CRM and pooled serum were delivered to each laboratory. Results: In 2009, mean precision of total tumor marker of RIA was $14.8{\pm}4.2%$ and that of EIA(CLIA) was $19.2{\pm}6.9%$. In 2010, mean precision of 5 tumor marker and T3, T4, Tg, TSH was $13.8{\pm}6.1%$ in RIA and $15.5{\pm}7.7%$ in EIA (CLIA). There was no significant difference between RIA and EIA. In RIA, the coefficient variations (CV) of AFP, CEA, PSA, CA 125, T3, T4, TSH were within 20%. The CV of CA 19-9 was over 20% but there was no significant difference with EIA (CLIA) (p=0.345). In recovery test using CRM, AFP, PSA, T4, TSH showed 92~103% of recovery in RIA. In recovery test using commercial material, CEA, CA 125, CA 19-9 showed relatively lower recovery than CRM but there was no significant difference between RIA and EIA (CLIA). Conclusion: By evaluating the precision and accuracy of each test, EQA test could more accurately measured the quality of each test and performance of laboratory.
