Analysis of the Characteristics of Ambulatory Care Sensitive Conditions in Patients Visiting the Emergency Departments: Focused on Health-care Delivery System (외래의료 민감 질환으로 응급실을 내원한 환자의 특성 분석: 의료전달체계 중심으로)

  • This study analyzed, the characteristics of ambulatory care sensitive conditions(ACSCs) in patients visiting emergency departments(EDs) and compared characteristics according to two age groups (adults aged 19-64 years and, elderly people aged ≥65 years). By accessing data from the National Emergency Department Information System(NEDIS) from January 1 to December 31, 2018, we examined the proportions of different ED types and ACSCs, length of stay(LOS) in the ED, LOS hospital, and hospital admission rates. Regarding the types of EDs, we found that the proportion of local emergency medical centers was high(P<0.001). Regarding the rates of different ACSCs, 31.7% of adults were treated for gastroenteritis, a high proportion of the elderly people(48.2%) were diagnosed with and treated for pneumonia(P<0.001). The LOS in the ED was longer in elderly people for all diseases categories, except for congestive heart failure and diabetes(P<0.001). The LOS in the hospital was also significantly longer in elderly people for all ACSCs(P<0.05), and the admission rate was significantly higher in elderly people for all diseases, except for diabetes(P<0.01). Thus, analyzing the ED visits made by patients with ACSCs will need to strengthen the health-care policy to induce treatment centered on outpatient.

A Comparative Study on the Pharmaceutical Properties of Rectal Suppository containing Omeprazole Complexes (오메프라졸복합체 함유 직장좌제의 특성비교)

  • Omeprazole(OMP) complexes such as inclusion complexes of OMP with $hydroxypropyl-{\beta}-cyclodextrin$(HPCD) and ${\beta}-cyclodextrin({\beta}-CD)$, OMP-cholestyramine(CHL) and OMP-ethylenediamine(OMP-ED) were prepared, respectively. The partition coefficients in Witepsol H-15 /pH 7.4 phosphate buffer solution of OMP complexes$(OMP-HPCD;\;3.69{\pm}0.26,\;OMP-{\beta}-CD;\;4.08{\pm}0.21,\;OMP-CHL;\;4.36{\pm}0.25\;and\;omeprazole\;sodium(OMP-Na);\;3.64{\pm}0.37)$ were higher than that of OMP $(2.66{\pm}0.47)$. OMP was not completely dissolved until even 3 hrs, but all the OMP complexes studied were released about 100% in 20 min. The rectal suppositories containing OMP or each above OMP complex were prepared using Witepsol H-15 base, and their dissolution and stability were examined, and pharmacokinetic study were investigated after their rectal administrations to the rabbits. While the suppository containing OMP was released only less than 60% in 150 min, $OMP-{\beta}-CD$, OMP-CHL, OMP-Na and OMP-ED suppositories were all released about 65% in 20 min. Especially, OMP-HPCD suppository released OMP about 70% in 10 min. All the additives such as sodium laurylsulfate, eglumine, arginine and PVP increased drug release from OMP-HPCD suppository to some extent. The decomposition rate constants of OMP in the suppositories were $9.117{\times}10^{-3}\;day^{-l}$ for OMP suppository, $2.121{\times}10^{-2}$ for OMP-HPCD, $1.607{\times}10^{-2}$ for $OMP-{\beta}-CD$, $9.26{\times}10^{-3}$ for OMP-Na, $6.769{\times}10^{-3}$ for OMP-CHL and $5.58{\times}10^{-3}\;day^{-l}$ for OMP-ED suppository, respectively. Additives such as arginine, eglumine and ED had some stabilizing effect for OMP-HPCD, OMP-CHL and OMP-Na suppositories, respectively. After 6 month-storage at $30^{\circ}C$, 75% RH, OMP-CHL suppository was most stable. The values of Tmax for OMP-HPCD and OMP-Na suppositories were $11.7{\pm}2.36\;and\;11.4{\pm}2.56\;min$, respectively. The values of Cmax for OMP-HPCD and OMP-CHL suppository were $2.31\;{\mu}g/ml\;(p<0.01)\;and\;1.89\;{\mu}g/ml\;p<0.01)$, respectively. The values of AUC for OMP and $OMP-{\beta}-CD$ suppository were $61.9{\pm}25.79\;and\;68.6{\pm}29.48\;{\mu}g\;{\cdot}\;min/ml$, and the corresponding values for OMP-HPCD and OMP-CHL were $106.1{\pm}43.16\;(p<0.05)\;and\;127.3{\pm}42.52\;{\mu}g\;{\cdot}\;min/ml(p<0.01)$, respectively. The above results indicate the OMP-HPCD and OMP-CHL suppositories have the excellent bioavailabilties in vivo study.

Antihypertensive Effect of Amlodipine Adipate, a Novel Salt of Amlodipine, in Hypertensive Rat Models

  • The vascular relaxant effect of amlodipine adipate, a new salt of amlodipine, was evaluate in isolated rat aorta, and compared with that of amlodipine besylate. Furthermore, antihypertensive effects were measured in hypertensive rat models, such as spontaneously hypertensive rats (SHR) and rena1 hypertensive rats (RHR). Amlodipine adipate concentration-dependently inhibited $Ca^{2+}$-induced contraction of rat aorta with a very slow onset of action (reached its maximum at 3.5 h;$IC_{50}$: 3.76 nM), having a pattern and a potency similar to those of amlodipine besylate ($IC_{50}$: 4.01 nM). In SHR and RHR, orally administered amlodipine adipate produced a dosedependent and long-lasting (>10-24 h) antihypertensive effect ($ED_{20}$: 2.48 and 1.57 mg/kg, respectively), with a pattern and a potency similar to those of amlodipine besylate ($ED_{20}$: 2.50 and 1.99 mg/kg in SHR and RHR, respectively). These results suggest that amlodipine adipate is a potent and long-lasting antihypertensive agent and that its antihypertensive effect is not significantly different to that of amlodipine besylate.

  • The evidence for the existence of a common pathway for health issues in men is presented in this review. Several epidemiological studies have shown that conditions like cardiovascular diseases (CVD), metabolic syndrome, diabetes, lower urinary tract symptom (LUTS), erectile dysfunction (ED), prostate cancer, hypogonadism, depression and suicide can be associated as risk factors for each other. Thus, the risk of CVD is significantly increased in men with metabolic syndrome, ED, hypogonadism, prostate cancer and/or LUTS. In addition, the above mentioned conditions are more prevalent in atherosclerotic patients. In addition, growing evidence indicates that low androgen levels can cause metabolic syndrome. In addition, obesity, dyslipidaemia and diabetes can further reduce androgen levels potentiating their adverse effect. Low testosterone levels are also associated with a higher incidence of aggressive prostate cancer on biopsy and on definitive pathology, and lower probability of abiraterone response in the metastatic setting. Several recent studies point towards diffuse endothelial dysfunction and dysregulated pro-inflammatory state as the biological link between all these disorders. Our current hypothesis is that oxidative stress caused by these dysfunctions explains the pathogenesis of each of these conditions.

Electrodialysis with a channeled stack for high strength cadmium removal from wastewater

  • In this study, high concentrations of cadmium-containing wastewater were treated by electrodialysis (ED) with a channel stack. The limiting current density (LCD), cadmium removal efficiency, and current efficiency were investigated under each experimental condition according to the Reynolds number (Re), membrane area, and pH. With the increase in the film area to 111, 333, 555, and 777 cm2 at Re (109.1), LCDs decreased to 408.11, 44.45, 35.32, and 13.64 A/m2, respectively. The highest cadmium removal efficiency (99.6%) and current efficiency were obtained for the membrane area of 111 and 777 cm2, respectively. Under changing Re in the pH range of 1 to 4, Re and LCD were proportional under the same pH condition, and pH and LCD tended to be inversely proportional under the same Re condition. Cadmium removal rate was the best at the pH range 3 - 4. It has been found that ED with channeled stacks can be successfully applied to treat wastewater containing high concentrations of cadmium.

Effects of Various Diets on Growth and Body Composition of Juvenile Olive Flounder, Paralichthys olivaceus (배합사료 종류가 넙치 Paralichthys olivaceus 치어의 성장 및 체성분에 미치는 영향)

  • The 7-week feeding experiment was conducted to investigate the effects of one experimental diet (ED) and five different commercial diets (CDs) on growth and body composition of juvenile olive flounder, Paralichthys olivaceus. An ED was formulated to contain 50.0% crude protein (CP) from fishmeal, casein, zein and wheat flour and 15.0% crude lipid (CL) from squid liver oil. Five CDs for seawater fish were two domestic E commercial diet (DECD) and C commercial diet (DCCD), three imported H commercial diet (IHCD), M commercial diet (IMCD) and O commercial diet (IOCD) containing 53.1~58.0% CP and 4.8~12.7% CL, respectively. Each diet was fed to triplicate groups of juvenile olive flounder initially weighing $29.1{\pm}0.8g/fish\;(mean{\pm}SD)$ in a flow-through seawater system with a water temperature of $23.4{\sim}28.0^{\circ}C$. Weight gain (WG) was significantly greatest in fish fed the IMCD; intermediate responses were observed for fish fed the DECD, DCCD, and IOCD, while the IHCD and the ED produced the lowest WG values. Feed efficiencies (FE) were similar to WG excluding fish fed the DCCD; FE was also greatest in fish fed the DCCD. Survival with no significant difference approached 100% for fish fed the all six diets in this experiment. Whole-body crude protein and ash contents were not affected excluding moisture and crude lipid by the different type of diets. Therefore, type of diets appeared to be important factor in influencing WG, FE and whole-body moisture and crude lipid of juvenile olive flounder; the best diet for juvenile olive flounder was determined to be the imported commercial M diets containing intermediate protein (55.9%) and lipid (12.7%) in natural seawater based on highest WG, and FE, respectively. This study indicates that the one commercially formulated diet containing intermediate protein and lipid used in this experiment could be a practical diet for juvenile olive flounder; these differences in growth performance between ED and CDs may be due to different dietary protein and lipid levels.

In Vitro Sensitivity to Metalaxyl of Phytophthora parasitica var. nicotianae Isolates from Burley Tobacco in Korea

  • In disease surveys from 1986 ti 1998, disease incidence of tobacco black shank was gradually increased in burley tobacco from 1996. To study the causes of the disease occurrence, one hundred and fourteen isolates of Phytophthora parasitica var. nicotianae (Ppn) were collected from burley tobacco-growing areas in the southern part of Korea during 1996-1997, and tested in vitro for meatlaxyl sensitivity which was determined by measuring the mycelial growth on corn meal agar (CMA) amended with metalaxyl. Of the tested isolates, 78.1% showed sensitive to metalaxyl, having $\textrm{ED}_{50}$ values less than 1.0 $\mu\textrm{g}/$\textrm{ml}, while 1.7% was resistant weth $\textrm{ED}_{50}$ greater than 100 $\mu\textrm{g}/$\textrm{ml}. Ppn isolates from three provinces, Chungnam, Chonbuk and Chonnam showed similar distributions of metalaxyl sensitivity. Metalaxyl-resistant isolates were not significantly different from metalaxyl-sensitive ones in mycelial growth rate, chlamydospore formation capacity and size of the spore, and pathogenicity on tobacco plant (cv. Burley 21). These results suggest that the metalaxyl-resistant Ppn in burley tobacco may be one of the major factors to cause the higher occurrence of the tobacco black shank in the burley tobacco-growing area.

An Acetophenone Derivative, Clavatol, and a Benzodiazepine Alkaloid, Circumdatin A, from the Marine-Derived Fungus Cladosporium

  • The crude extract of the mycelium of Cladosporium was found to exhibit antimicrobial activity against the Staphylococcus aureus, methicillin-resistant S. aureus, and multidrug-resistant S. aureus. Bioassayguided fractionation of an organic extract led to the isolation of an acetophenone derivative, clavatol (2',4'-dihydroxy-3',5'-dimethylacetophenone) (1), and a benzodiazepine alkaloid, circumdatin A (2). Compound 1 showed moderate antibacterial activity against S. aureus, methicillin-resistant S. aureus, and multidrug-resistant S. aureus with minimum inhibitory concentration (MIC) values of 62.5, 62.5, 31.0 $\mu$g/mL, respectively, but compound 2 was inactive. Compounds 1 and 2 exhibited UV-A protection activity with ED$_{50}$ values of 227.0 and 82.0 $\mu$M, respectively, indicating that they were more potent than the positive control, oxybenzone (ED$_{50}$ 350 $\mu$M), a common sunscreen agent.

  • Cholesteryl p-[3-(2-chloroethyl_-3-nitrosoureido] phenylthioaccetate (2) was synthesized : an intermediate, p-[3(2-chloroethyl_-3-nitrosoureido] phenylthioacetic acid (1) is a congener of an antitumor chlorambucil which both the -CH$_{2}$CH$_{2}$-linkage and -N(CH$_{2}$CH$_{2}$Cl)$_{2}$ group of chlorambucil molecule is doubly modified into the respective -S-linkage and -NH-CO-NNO-CH$_{2}$CH$_{2}$CL group. The attackment of cholesterol moiety as a carrier group to p-[3-(2-chloroethyl-3nitrosoureido] phenylthioacetic acid was accomplished through the esterification of cholesterol with p-[3-(2-chloroethyl-3-nitrosoureido] phenylthioacetyl chloride which was obtained from the treatment of p-[3-(2-chloroethyl)-3-nitrosourei-do] phenylthioaceticacid with SOCL$_{2}$, p-[3-(2-chloroethyl)ureido]]-phenythioacetic acid was nitrosated with NaNO$_{2}$ IN 98-100% HCOOH to give exclusively p-[3-(2-chloroethyl_-3-nitrosoureido] phenylthioacetic acid. Refractive Index Dispersion of Aluminate Glasses on the Addition of $SiO_2$ ($SiO_2$ 첨가에 따른 알루미네이트 유리의 굴절률 분산 특성)

  • The refractive index and dispersion in the (100-x)(0.6CaO.0.4Al2O3).xSiO2(x=0~30) glasses were investigated. As the amount of SiO2 increased, the refractive index decreased. The change of refractive index was attributed to the change of the molar refraction rather than the molar volume. When the amount of SiO2 was smaller than 20 mol%, the average electronic transition energy gaps(E0) and the electronic oscillator strengths(Ed) were about 10.9($\pm$0.1) nd 18($\pm$0.5)eV, respectively. However E0 and Ed of the glass (CAS30) with 30 mol% SiO2 increased to 12.63 and 19.89eV, respectively. The similar results was observed in the variation of Abbe Number. Abbe number of the glass in the range of 0~20 mol% SiO2 was about 46 and that of CAS30 increased to 60. The zero-material dispersion wavelength({{{{ lambda }}0) of pure calcium aluminate glass was 1.8 ${\mu}{\textrm}{m}$. As the amount of SiO2 increased, the zero-material dispersion wavelength shifted to a shorter wavelength. {{{{ lambda }}0 of CAS30 was 1.5 ${\mu}{\textrm}{m}$, that is currently using for the optical telecommunication system.
