Ultrasound-guided internal jugular vein catheterization in critically ill pediatric patients

  • Purpose: Continuous intravenous access is imperative in emergency situations. Ultrasound-guided internal jugular vein (IJV) catheterization was investigated in critically ill pediatric patients to assess the feasibility of the procedure. Methods: Patients admitted to the pediatric intensive care unit between February 2011 and September 2012 were enrolled in this study. All patients received a central venous catheter from attending house staff under ultrasound guidance. Outcome measures included successful insertion of the catheter, cannulation time, number of cannulation attempts, and number and type of resulting complications. Results: Forty-one central venous catheters (93.2%) were successfully inserted into 44 patients (21 males and 23 females; mean age, $6.54{\pm}1.06$ years). Thirty-three patients (75.0%) had neurological disorders. The right IJV was used for catheter insertion in 34 cases (82.9%). The mean number of cannulation attempts and the mean cannulation time was $1.57{\pm}0.34$ and $14.07{\pm}1.91$ minutes, respectively, the mean catheter dwell time was $14.73{\pm}2.5$ days. Accidental catheter removal was observed in 9 patients (22.0%). Six patients (13.6%) reported complications, the most serious being catheter-related sepsis, which affected 1 patient (2.3%). Other complications included 2 reported cases of catheter malposition (4.6%), and 1 case each of arterial puncture (2.3%), pneumothorax (2.3%), and skin infection (2.3%). Conclusion: The results suggest that ultrasound-guided IJV catheterization can be performed easily and without any serious complications in pediatric patients, even when performed by visiting house staff. Therefore, ultrasound-guided IJV catheterization is strongly recommended for critically ill pediatric patients.

  • Creep behaviors of the solder balls in a flip chip package assembly during thermal cycling test is investigated.. A material models used in the finite element analysis are viscoplastic model introduced by Anand and creep model called partitioned model. Experiment of two temperature cycles using moir$\acute{e}$ interferometry is conducted to verify the reliability of material models for the analysis of thermo-mechanical behavior. Bending deformations of the assemblies and average strains of the solder balls due to temperature change and dwell time are investigated. The results show that time-dependent shear strain of solder by the partitioned model is in excellent agreement with those by moir$\acute{e}$ interferometry, while there is considerable difference between results by Anand model and experiment. In this paper, the partitioned model is employed for the time-dependent creep analysis of the FC-PBGA package. It is also shown that the thermo-mechanical stress becomes relaxed by creep behavior at high temperature during temperature cycles.

A Smart DTMC-based Handover Scheme Using Vehicle's Mobility Behavior Profile (차량의 이동성 행동 프로파일을 이용한 DTMC 기반의 스마트 핸드오버 기법)

  • For improvement of wireless Internet service quality at vehicle's moving speed, it is advised to reduce the service disruption time by reducing the handover frequency on vehicle's moving path. Particularly, it is advantageous to avoid the handover to cell whose dwell time is short or can be ignored in terms of service continuity and average throughput. This paper proposes the handover scheme that is suitable for vehicle in order to improve the wireless Internet service quality. In the proposed scheme, the handover process continues to be learned before being modeled to Discrete-Time Markov Chain (DTMC). This modeling reduces the handover frequency by preventing the handover to cell that could provide service sufficiently to passenger even when vehicle passed through the cell but there was no need to perform handover. In order to verify the proposed scheme, we observed the average number of handovers, the average RSSI and the average throughput on various moving paths that vehicle moved in the given urban environment. The experiment results confirmed that the proposed scheme was able to provide the improved wireless Internet service to vehicle that moved to some degree of consistency.

  • The train conflict resolution problem refers to an adjustment of the train operation schedule in order to minimize the delay propagation when a train conflict is predicted or when a train conflict occurs. Previous train studies of train conflict resolutions are limited in terms of the size of the problems to be solved due to exponential increases in the variables. In this paper, we propose a train conflict resolution model using a time-space network to solve the train conflict situation in the operational phase. The proposed model adjusts the size of the problem by giving only the dwell tolerance in the time-space network only for stops at the station after a train conflict. In addition, the proposed model can solve large problems using a path flow variable. The study presents a train delay propagation analysis and experimental results of train conflict resolution assessments as well using the proposed model.

  • The microstructure evolution in hot forging process is composed of dynamic recrystallization during deformation as well as grain growth during dwell time. Therefore, the control of forging parameters such as strain, strain rate. temperature and holding time is important because the microstructure change in hot working affects the mechanical properties. Modeling equations are developed to represent the flow curve. grain size. recrystallized volume fraction and grain growth phenomena by various tests. The developed modeling equations were combined with thermo-viscoplastic finite element modeling to predict the microstructure change evolution during hot forging process. The large exhaust valve spindle (head diameter of 512mm) was simulated by closed die forging with hydraulic press and cooled in air after forging. The preform was heated to each 1080 and 1150$^{\circ}C$. Numerical calculation was performed by DEFORM-2D. a commercial finite element code. Heat transfer can be coupled with the deformation analysis in a non-isothermal deformation analysis. In order to obtain the fine and homogeneous microstructure and good mechanical properties in forging. the FEM would become a useful tool in the simulation of the microstructure development. In forging, appropriate temperature, strain and strain rate and rapid cooling are required to obtain the fine grain microstructure The optimal forging temperature and effective strain range of Nimonic 80A for large exhaust valve spindle are about 1080$\∼$l120$^{\circ}C$ and 150$\∼$200$\%$.

A COMPARATIVE STUDY ON THE DISLODGING FORCE OF MAGNETIC ATTACHMENT TO THE DENTURE RESIN BY MAGNETIC DESIGN AND FIXING MATERIALS

  • STATEMENT OF PROBLEM: Detachment of the magnetic assembly from the denture base has been a problem in magnetic overdenture patients. PURPOSE: The objectives of this study were to compare the dislodging force by the fixing materials and the designs of the magnetic assembly, and to compare the effect between the fixing materials and the designs of the magnetic assembly. MATERIAL AND METHODS: Two fixing materials, Jet denture repair $acrylic^{(R)}$ and Super-$Bond^{(R)}$ C&$B^{(R)}$ and two types of magnetic assembly designed with or without wing were used. Each magnetic assembly was fixed in the chamber of the denture base resin block ($Lucitone^{(R)}$199) with each fixing material respectively. These specimens were thermocycled 2,000 cycles in the water held at $4^{\circ}C$ and $60^{\circ}C$ with a dwell time of 1 min each time. Each specimen was seated in a testing jig and then a push-out test was performed with a universal testing machine at a cross head speed of 0.5 mm/min to measure the maximum dislodging forces. RESULTS: Comparing the fixing materials, Super-Bond C&$B^{(R)}$ showed superior dislodging force than Jet denture repair $acrylic^{(R)}$. Comparing the design of the magnetic assemblies, the wing design magnetic assembly showed better dislodging force. Combination of the Super-Bond C&$B^{(R)}$ as a fixing material and wing design magnetic assembly revealed a greatest dislodging force. The kind of fixing material was more influential than the type of magnetic assembly. CONCLUSION: The dislodging force of Super-Bond C&$B^{(R)}$ was significantly higher than Jet denture repair $acrylic^{(R)}$. And the dislodging force of magnetic assembly which have wing design was significantly higher than magnetic assembly which have no wing design.

THE STUDY ON THE MICROLEAKAGE OF THE RESTORATION WITH SELF-ETCHING PRIMING/BONDING AGENT (Self-etching priming/bonding agent를 이용한 수복에서 microleakage에 관한 연구)

  • Self-etching priming bonding system was recently developed in order to simplify the clinical skills & save chair time after continuous improvements on dentin bonding agents. To test the magnitude of microleakage of a new "self-etching priming bonding agent" using sound premolar 4th, 5th, 6th generation dentin bonding agent was applied. Measure the magnitude of infiltration to the gap of enamel-restoration interface and dentin-restoration interface. After bonding of composite resin to sample surfaces according to the manufactures direction and 500 times thermocycling on dwell time 30 second, and microleakage was measured by the ratio of the depth to the axial wall and the magnitude of infiltration. Afterward analyzed by ANOVA test. The result were as follows ; 1. Enamel groups showed lesser microleakage (Group I, II, III, IV) than dentin groups(Group V, VI, VII, VIII). (p<0.05) 2. There are no statical differences among the dentin groups, in enamel groups, group IV showed more microleakage than group I, II, III. (p<0.05). For a clinical acceptance, better enamel marginal adaptation is required.

Influence of low temperature degradation on the bond strength and flexural Strength of veneered Zirconia(3Y-TZP) (저온열화가 도재전장 지르코니아의 결합강도와 굴곡강도에 미치는 영향)

  • Purpose: The aim of this study was to evaluate the effect of pre-treatment of core and hydrothermal treatment on the bond strength and flexural strength of ceramic veneered zirconia. Methods: 3Y-TZP specimens(KaVo Zr, $25mm{\times}3mm{\times}1mm$)were prepared by five pre-treatment methods and divided into seven groups including control two groups, subsequently the specimens veneered with the E-MAX ceram according to manufacturer's information(total specimen thickness 1.5mm). Two groups from ceramic-zirconia specimens(n=105, n=15 per group)were assigned into two experimental fatigue conditions, namely storage in an autoclave at $134^{\circ}C$ for 5h, thermo-cycling(3,000cycles, between 5 and $55^{\circ}C$, dwell time 45s, transfer time 2s). A flexural strength test was performed in a universal testing machine(crosshead speed: 0.5mm/min). Data were statistically analyzed using one-way ANOVA and Tukey's test(${\alpha}$=0.05). Results: The ceramic-zirconia bond strength value for liner application group(LLW, $27.3{\pm}3.8$) were significantly lower than those of the pre-treatment groups($30.72{\pm}5.3$). The ceramic-zirconia bond strength and zirconia flexural strength was not affected by thermo-cycling(p>0.05), whereas it was affected by storage in an autoclave at $134^{\circ}C$ for 5h(p<0.05). Conclusion: The results indicated that the ceramic-zirconia bond strength and zirconia flexural strength was affected by low temperature degradation.

The complexity of opt-in procedures in mobile shopping: Moderating effects of visual attention using the eyetracker (모바일 쇼핑에서 옵트인의 절차적 복잡성 연구: 아이트래커(eyetracker) 기반 시각적 주의의 조절효과)

  • Consumers tend to feel concern about disclosure of personal information and, at the same time, to avoid inconvenience of procedural complexity caused by the privacy protections. The purpose of current paper is to investigate relationships between opt-in procedural complexity and shopping behavior using smart phones, moderating by the amount of visual attentions using eyetrackers. Therefore, we created a virtual mobile Web-site in which the complexity of opt-in procedures in our experiment is manipulated and measured. Also, we measured the dwell-time of area of interest using SMI-RED 250 instrument for tracking the real eye movement. Results indicated that the levels of procedural complexity are related to repurchase, indicating a moderating effect of the amount of visual attentions. Finally, we discussed several theoretical and practical implications of management for mobile commerce.

Estimating the storage space requirement of a container terminal considering the variance of a containership's load size (본선 작업물량의 변동을 고려한 컨테이너 터미널의 장치공간 소요량 추정)

  • The storage space requirement is a very important decision variable which determines the storage capacity of a container terminal. Generally, the storage space requirement is dependent upon such factors as ship headway, allowable dwell time of containers, loading/winding time per ship, and so on. Until now, the storage space requirement is estimated under the assumption that the factors are deterministic in several studies. However, this study proposes how to estimate a storage space requirement satisfying the required service level under the assumption that a containership's load size is probabilistic. Numerical experiments, which use a simulation show that the proposed method can estimate more adequately the storage space requirement than other methods under a probabilistic environment.
