비효율적인 대처기능의 장애가 있는 가족

  • This study was carried out to investigate agreement, content and demand for home nursing care of hospital inpatients in brain, spain and musculoskeletal diseases. The data was collected by interviewing with 242 patients who were hospitalized in university hospital on Taegu, from September 6,60 October 9, 1993. Of 242 patients, 66.1% agreed to home nursing care system and rate of agreement was highest between 30 years to 49 years of age as 40.4% in general characteristics. The rate of agreement according to type of diagnosis was highest inpatients with spinal diseases as 75.6% according to functional status was the highest in patients who had daily living activity freely as 69.4% according to prognosis in patients at terminal stage as 80.0% and the rate of agreement to home nursing care of patients who wanted early discharge was 73.9% The first-ranking reasons of agreement to home nursing care was asking for continuous relationship with doctor as 37.3% and there was statistically significant difference in reasons of agreement to home nursing care according to functional status of patients. The first-ranking reason of early discharge among patients who wanted early discharge(74.8%) was because of long time stay in hospital. Among 23 items of nursing activity that patient wanted, the first-ranking item was recovery promotion, prevention of complication, education and counseling for health as 76.4%, drug management was 2nd-ranking item as 62.1% and the third was regular checking of vital signs as 55.9%, The lowest item of demand for home nursing care was hospice care(3.9%) and airway keep(9.1%).

In Vitro and in Vivo Effects of Nitrofurantoin on Experimental Toxoplasmosis

  • Toxoplasma gondii is an important opportunistic pathogen that causes toxoplasmosis, which has very few therapeutic treatment options. The most effective therapy is a combination of pyrimethamine and sulfadiazine; however, their utility is limited because of drug toxicity and serious side effects. For these reasons, new drugs with lower toxicity are urgently needed. In this study, the compound, (Z)-1-[(5-nitrofuran-2-yl)methyleneamino]-imidazolidine-2,4-dione (nitrofurantoin), showed anti-T. gondii effects in vitro and in vivo. In HeLa cells, the selectivity of nitrofurantoin was 2.3, which was greater than that of pyrimethamine (0.9). In T. gondii-infected female ICR mice, the inhibition rate of T. gondii growth in the peritoneal cavity was 44.7% compared to the negative control group after 4-day treatment with 100 mg/kg of nitrofurantoin. In addition, hematology indicators showed that T. gondii infection-induced serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels, biochemical parameters involved in liver injury, were reduced by nitrofurantoin significantly. Moreover, nitrofurantoin exerted significant effects on the index of antioxidant status, i.e., malondialdehyde (MDA) and glutathione (GSH). The nitrofurantoin-treated group inhibited the T. gondii-induced MDA levels while alleviating the decrease in GSH levels. Thus, nitrofurantoin is a potential anti-T. gondii candidate for clinical application.

Application and Prospects of Molecular Imaging (분자영상의 적용분야 및 전망)

  • In this paper, we study to classify molecular imaging and applications to predict future. Molecular imaging in vivo at the cellular level and the molecular level changes taking place to be imaged, that is molecular cell biology and imaging technology combined with the development of the new field. Molecular imaging is used fluorescence, bioluminescence, SPECT, PET, MRI, Ultrasound and other imaging technologies. That is applied to monitoring of gene therapy, cell tracking and monitoring of cell therapy, antibody imaging, drug development, molecular interaction picture, the near-infrared fluorescence imaging of cancer using fluorescence, bacteria using tumor-targeting imaging, therapeutic early assessment, prediction and therapy. The future of molecular imaging would be developed through fused interdisciplinary research and mutual cooperation, which molecular cell biology, genetics, chemistry, physics, computer science, biomedical engineering, nuclear medicine, radiology, clinical medicine, etc. The advent of molecular imaging will be possible to early diagnosis and personalized treatment of disease in the future.

Effect of Preexisting Musculoskeletal Diseases on the 1-Year Incidence of Fall-related Injuries

  • Objectives: People who have chronic diseases, as well as gait imbalance or psychiatric drug use, may be susceptible to injuries from falls and slips. The purpose of this study was to evaluate the effect of musculoskeletal diseases on incidental fall-related injuries among adults in Korea. Methods: We analyzed data from the 4th Korea National Health and Nutrition Examination Survey (2007-2009), which are national data obtained by a rolling survey sampling method. The 1-year incidence of fall-related injuries was defined by health service utilization within the last year due to injury occurring after a slip and fall, and musculoskeletal diseases included osteoarthritis, rheumatoid arthritis, osteoporosis, and back pain. To evaluate the effects of preexisting musculoskeletal diseases, adults diagnosed before the last year were considered the exposed group, and adults who had never been diagnosed were the unexposed group. Results: The weighted lifetime prevalence of musculoskeletal disease was 32 540 per 100 000 persons. Musculoskeletal diseases were associated with a higher risk of fall-related injury after adjustment for sex, age, residence, household income, education, occupation, visual disturbance, paralysis due to stroke, and medication for depression (odds ratio [OR], 1.41; 95% confidence interval [CI], 1.03 to 1.93). As the number of comorbid musculoskeletal diseases increased, the risk of fall-induced injuries increased (p-value for trend <0.001). In particular, patients who had any musculoskeletal condition were at much higher risk of recurrent fall-related injuries (OR, 6.20; 95% CI, 1.06 to 36.08). Conclusions: One must take into account the risk of fall-related injuries and provide prevention strategies among adults who have musculoskeletal diseases.

A Dietary Treatment for Diabetes Mellitus in Chinese Medicine (한의학(韓醫學)에 있어서 소갈증(消渴症)(당뇨병(糖尿病))에 대한 식이요법(食餌療法)의 고찰(考察))

  • A dietary treatment of Chinese medicine for diabetes mellitus was extensively studied and compared with that of western medicine. The main results are as follows: Chinese medicine is based on the following empirical dietetics. First, likeness helps likeness. When an internal organ of mankind is not functioning properly, the food or drug from the same organ of an animal will be helpful to cure it. Second, take good foods for health properly and never take any foods against body. Third, building up one's body by taking tonics is not quite the same as building up one's body through proper eating. On the other hand, western medicine is based on the experimental and scientific methods which are modernized in accordance with the development of science and technology. It emphasizes upon finding the cause of a disease. Then this disease can be cured by doing a necessary medical treatment which sometime uses a surgical operation or chemical and radiological method or both. Although there are many ways in treating a diabetic in Western Medicine, here the diabetics is supposed to be the best. The same is true for Chinese Medicine. Therefore one can easily conclude that the dietetics is the most important and effective of all irrespective of Western and Chinese Medicine as far as diabetesis concerned. In Western Medicine, a diabetic is recommened to have the minimum calories necessary for life and not to have goods containing glucide beyond a certain quantity, while in Chinese medicine a diabetic is not allowed to have foods containing more than 10% of glucide. These two facts suggest that a diabetic should pay careful attention to foods containing lots of glucide. Finally a systematic cooperation between western and Chinese medicine will cure not only diabetes but also other disease more effectively than a traditionally unilateral method.

Analysis of the Effects and Nursing Intervention of Home Health Care in Public Health Centers (보건소 방문보건사업 효과와 간호중재 분석)

  • Purpose: The purpose of this study was to investigate the types and frequencies of nursing intervention of home health care in Public Health Centers and its effects. Method: The data collection period for this study lasted from March 1 to December 31, 2003. The clients were sampled by a stratified randomized method among those who had been cared for at least 3 months. The data was analyzed by SPSS for description. ANOVA, paired t test, etc. Result: The types and frequencies of nursing intervention in major chronic health problems were significantly different. The main types of nursing intervention in hypertension and DM patients included education on disease management, regular exercise, stress management, diets, etc. CVA patients were intervened in pain control (ice or hot pack, massage), position changes, and ROM exercise. Cancer patients received imaginary or relaxation therapy, pain control (ice or hot pack, massage), hospice, etc. After the intervention, the drug compliance of hypertension (8.2 days per month) and DM patients (6.2 days per month) improved. Blood sugar levels (FBS 7.6, post partum 2hrs $21.4(mg/d{\ell})$ and blood pressure(systolic 9.8, diastolic 4.3 mmHg) lowered significantly. All aspects of QOL also improved (total 3.68). Conclusion: The types and frequencies of nursing intervention were determined by characteristics of the health problems, and home health care nursing intervention in Public Health Centers was effective to the elderly of the community in many aspects.

Concentration of elemental ions released from non-precious dental casting alloys (치과주조용 비귀금속 합금의 금속 용출 수준)

  • Purpose: This study was to assess the extents of the release of metals from the non-precious alloys used for dental casting by measuring the differences in the extents of the release of metals by types of alloys, pH level and elapsed time. Methods: Uniform-sized specimens(10 each) were prepared according to the Medical Device Standard of the Korea Food and Drug Administration(2010) and International Standard Organization(ISO22674, 2006), using four types of alloys(one type of Ni-Cr and one type of Co-Cr used for fixed prosthesis, and one type of Ni-Cr and one type of Co-Cr used for removable prosthesis). A total of 12 metal-release tests were performed at one-day, three-day, and two-week intervals, for up to 20 weeks. The metal ions were quantified using an Inductively Coupled Plasma-Atomic Emission Spectrometer. Results: The results showed that the extent of corrosion was higher in the ascending order of Jdium-$100^{(R)}$, Bellabond-$Plus^{(R)}$, Starloy-$C^{(R)}$, and Biosil-$F^{(R)}$. The lower the pH and the longer the elapsed time were, the greater the increase in metal corrosion. At pH 2.4, the release of Ni from Jdium-$100^{(R)}$, a Ni-Cr alloy, was up to 15 times greater than the release of Co from the Co-Cr alloy from two weeks over time, indicating that the Ni-Cr alloy is more susceptible to corrosion than the Co-Cr alloy. Conclusion: It is recommended that Co-Cr alloy, which is highly resistant to corrosion, be used for making dental prosthesis with a non-precious alloy for dental casting, and that non-precious alloy prosthesis be designed in such a way as to minimize the area of its oral exposure. For patients with non-precious alloy prostheses, a test of the presence or absence of periodontal tissue inflammation or allergic reaction around the prosthesis should be performed via regular examination, and education on the good management of the prosthesis is needed.

Pain and Its Major Influencing Factors in the Management of Terminal Cancer Patients (말기암 환자의 통증 양상과 통증관리에 영향을 미치는 요소들)

  • Background : Validity of WHO guideline of cancer pain management has been proven and many trials have been done to derive solutions for inadequate cancer pain management. We assessed the severity of pain of terminal cancer patients in a few different ways and patients' characteristics influencing inadequate pain management. Methods : This study was based on 100 adult oncological patients who were confirmed as terminal stage in our institution from 3/1998 to 11/1998. Medical records were reviewed and individual patients were interviewed to obtain demographic information and medical characteristics such as: daily activity performance, metastasis, and drug-adjusted pain severity. Adequacy of prescribed analgesics in accordance with WHO guidelines of pain management and patients' characteristics influencing adequacy of pain management were assessed. Results : Among those cancer patients diagnosed as terminal stage, 85% complained of pain, and 68% of those patients reported pain above moderate severity. 38% of those patients received inadequate pain management resulting in greater severity of pain; the less adequate pain relief was(p<0.01). Sex, age, primary cancer site metastasis, symptoms such as depression and anxiety, and daily activity performance were not significantly related. Conclusions : Despite WHO guidelines for pain management, majority of the terminal cancer patients received inadequate pain management. There is a necessity for education on proper pain evaluation and strict implimentation for WHO guidelines of pain management.

Pharmacists' Perception for the Current and Future Pharmaceutical Services (현재와 미래 약국서비스에 대한 약사의 인식)

  • Objective: We conducted a survey to understand the current state of pharmacy services and pharmacists' thoughts in the future of the community pharmacy setting. Methods: A questionnaire was distributed to 229 pharmacists and gathered from 95 respondents. Results: We asked to pharmacists about what the ideal pharmacy duty should be in the next 10 years at the community pharmacy setting in Korea. For this question, the respondents said drug counselling (19.4%), dispensing (13.8%), long-term care of chronic disease and healthcare education (12.8%), and OTC counselling (11.5%). When asked about how much time they were spending doing certain tasks in the pharmacy, the main tasks were dispensing (23.5%), counselling (17.1%), prescription review (10.1%), and OTC counselling (5.5%). When asked about what the most important duty of a pharmacist was, medication counselling (45.4%) was the most important task that they identified and the reasons for not being able to fulfill this role properly was time shortage (78.9%) followed by the lack of counselling spaces, up-to-date knowledges, and focus on financial gains over patient counselling. Conclusion: Korean pharmacists are mainly focusing on dispensing in their daily work. Their basic responsibilities can be easily mechanized over the next few years, but the social expectations of disease-prevention and public health promotion both in current and future can not be replaced by such mechanical measures. Therefore, pharmaceutical services in Korea should be developed in more diverse and professional ways.

A Study on Health Behaviors and Medication Compliances of Hypertensive Patients in a Rural Area (일 농촌지역 고혈압 환자들의 건강행태와 약물복용 이행도에 관한 연구)

  • This study was performed to identify the health behaviors and medication compliances of hypertensive patients in a rural area, from May 1 to July 31. 2001. The subjects were 100 hypertensive patients who were registered at the Health Center in Goeje City. The data was collected by face-to-face interviews with a 25-item questionnaire on health behaviors, and analyzed by the Chi-square test on each variable. The results were as follows: Approximately 76% of the subjects were currently taking drug medication. In the comparison of health behavior rates between male and female, there were statistically significant differences in smoking (p< .000), alcohol(p< .003), low salt diet(p< .014), and the health behavior rates of female were higher than those of male. In the comparison of socioeconomic factors by medication compliance, there was statistically significant difference in sex(p< .001), and the medication compliance rate of female was higher than that of male. In the comparison of subjects' perception by medication compliance, there were statistically significant differences in seriousness of hypertension (p< .001) and medication period for hypertension care (p< .004), The medication compliance rate of the group of subjects who took the threat on their hypertension seriously was higher than that of the group who didn't, and the medication compliance rate of the group who knew that they should take medication for the rest of their life was higher than that of the group who didn't. These results suggest that community health education programs and distribution of information must be emphasized in order to increase medication and to encourage behavior changes for promoting health.
