알코올 섭취량이 혈청 hs-CRP 수준 및 대사증후군 유병률에 미치는 영향 - 2015 국민건강영양조사 자료 이용 - (Influence of Alcohol Consumption on the Serum hs-CRP Level and Prevalence of Metabolic Syndrome -Based on the 2015 Korean National Health and Nutrition Examination Survey -)

  • Elevated high-sensitivity C reactive protein (hs-CRP) levels and metabolic syndrome are considered important predictors of cardiovascular mortality. This study examined the influence of the alcohol consumption level on the hs-CRP level and the prevalence of metabolic syndrome in South Koreans. The study subjects were 3,884 participants (${\geq}19$ years) from the KNHANES in 2015. The subjects were divided into four groups according to their alcohol consumption; None (none-alcohol consumption), Low (less than 15 g/day), Medium (15~29.9 g/day), and High (over than 30 g/day). The odds ratio for high-risk hs-CRP of men was significantly lower in the Low or Medium groups compared to the None group. In women, the odds ratio for high-risk hs-CRP was not decreased in the Low or Medium groups but was significantly higher in the High group than in the None group. Compared to nondrinkers, the prevalence of metabolic syndrome was lower in the Low group and Medium group in both men and women, whereas it was significantly higher in the High group in men only. In particular, the prevalence of low HDL-cholesterol was lower in all drinking groups. When the confounding factors were adjusted, the odds ratio for low HDL-cholesterol appeared to decline in the Low groups in both men and women. On the other hand, in the Medium group, women showed an elevated odds ratios of high blood pressure (HBP) and high fasting plasma glucose but men showed an elevated odds ratios of HBP and high triglyceride levels. The overall results suggest that low alcohol consumption is more appropriate than medium alcohol consumption. Nevertheless, more study will be needed to evaluate the appropriate alcohol consumption level.

대구지역 요양병원 입원노인의 식품선호도 및 영양소 섭취 실태 조사 (Survey of the Food Preferences and Nutrient Intakes of Elderly People Hospitalized in a Nursing Hospital in Daegu)

  • The number of elderly in care facilities is increasing in parallel with a rapid increase in the elderly population in Korea. We analyzed the meal satisfaction ratings, food preferences, and nutritional intakes of 74 patients aged ≥65 years living in a nursing hospital in Daegu. Food preferences were high for cooked rice, rice gruel, meat soup, soybean paste stew, beef, frozen pollack, squid, egg, spinach, sea mustard, cabbage kimchi, apple, peanut, drinking yogurt, pickled perilla leaves, and salted squid. Preferences for duck (P<0.01) and vegetable (P<0.01) cooking method were significantly different for men and women, and preference for soybean paste stew increased with age (P<0.05). Regarding nutrient intakes, men had higher energy (P<0.01), carbohydrates (P<0.05), and zinc (P<0.05) intakes than women, while women had higher vitamin C (P<0.05) intake than men. Furthermore, energy (P<0.001), carbohydrates (P<0.001), protein (P<0.05), dietary fiber (P<0.01), vitamin B6 (P<0.01), phosphorus (P<0.05), and zinc (P<0.001) intakes significantly decreased with age. Men had a significantly higher nutrient adequacy ratios (NARs) for carbohydrates (P<0.05) and cal- cium (P<0.01) than women, while women had a significantly higher NARs for protein (P<0.05), vitamin A (P<0.01), vitamin C (P<0.05), thiamine (P<0.01), riboflavin (P<0.001), iron (P<0.001), and zinc (P<0.01). Mean adequacy ratios were 0.69 for men and 0.75 for women. In particular, NARs for carbohydrates (P<0.01), vitamin B6 (P<0.01), and calcium (P<0.05) decreased significantly with age. In summary, the study shows that supplementing vitamin C, vitamin B6, calcium, and iron, reducing sodium, and providing meals that reflect preferences will improve the nutritional statuses of elderly residents in a nursing hospital.

체중 및 건강행태변화가 혈압 및 콜레스테롤에 미치는 영향 - 철강제조업체 근로자를 대상으로 한 3년 추구연구 - (Effects of change in Obestiy and Life Style Factors on Blood Pressure and Serum Cholesterol - 3-year Follow-up among Workers in a Steel Manufacturing Industry -)

  • Objectives: We investigated the effects of changes in obesity and life style factors, such as cigarette smoking, alcohol drinking, and exercise, on the changes in blood pressure and serum cholesterol among Korean men. Methods: This study included 7,205 healthy male employees in the steel manufacturing industry. Each subject underwent health examination in 1994 and was re-examined in 1997. The study subjects were classified into four categories, according to changes in body mass index (BMI) (loss; stable; mild gain: severe gain), cigarette smoking (quitter; nor-smoker; smoker continued; smoker started), alcohol drinking (quitter; non-drinker; drinker continued; drinker started) and exercise (more exercise; continuous regular exercise; continuous irregular or no exercise; less exercise), respectively. We evaluated the relationship between the categories of change in those independent variables and the changes in blood pressure and serum cholesterol, adjusted for BMI in 1994 and age by analysis of variance. Results: The change in systolic blood pressure was positively associated with the changes in BMI (p<0.001) and drinking (p=0,001), but negatively with smoking (p=0,004), compared to the first category of each independent variables. The systolic blood pressure was significantly less increased in the continuous smoking group than quitter or hon-smoker. The changes in diastolic blood pressure and serum cholesterol appeared to have statistically significant linear relationships only with the change in BMI. The change in exercise showed a marginal significance with diastolic blood pressure (p=0.088). Conclusions: These prospective data emphasize the importance of obesity as a determinant of the changes in blood pressure and serum cholesterol. In addition, the changes in smoking and drinking habits can affect systolic blood pressure.

일반인구에서 유전자 다형성이 요중 1-hydroxypyrene 및 2-naphthol의 배설량에 미치는 영향 (Effects of the Genetic Polymorphisms on Urinary Excretion of 1-Hydroxypyrene and 2-Naphthol)

  • This study was performed to determine the effects of genetic polymorphisms, such as glutathione S-transferase ${\mu}1(GSTM1)$, glutathione S-transferase ${\Theta}1\;(GSTM1)$, glutathione S-transferase ${\pi}l (GSTP1)$, aryl hydrocarbon N-acetyltransferase 2 (NAT2), cytochrome P450 2E1 (CYP2E1), cytochrome P450 1A1 (CYP1A1) on the concentrations of urinary 1-hydroxypyrene (1-OHP) and 2-naphthol in general population with no occupational exposure to polycyclic aromatic hydrocarbons (PAHs). Study subjects were 257 men who visited a health promotion center in Susan. A questionnaire was used to obtain detailed data about age, smoking, drinking, body fat mass, intake of fat etc. Urinary l-OHP and 2-naphthol concentration were analyzed by HPLC system with a fluorescence detector. A multiplex PCR method was used to identify the genotypes for GSTM1 and GSTT1. The polymorphisms of GSTP1, NAT2, CYP1A1 and CYP2E1 were determined by the polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) method. Urinary 1-OHP concentration was higher in deleted genotype of GSTM1, increased as smoking and alcohol drinking increased. Urinary 2-naphthol concentration was also rely on the age and smoking. Neither genetic polymorphism nor drinking-related factors were significantly related to urinary 2-naphthol concentration. No significant relation was found between physical characteristics and concentrations of urinary PAHs metabolites in the subjects, but the geometric mean of urinary 1-OHP and 2-naphthol was higher in the group with higher value compared to median value. These data suggest that in general population occupationally not exposed to PAHs, urinary concentration of PAHs metabolites is influenced by smoking, alcohol drinking and deleted genotype of GSTM1 in 1-OHP and smoking in 2-naphthol.

  • 음주 시작연령을 변수로 하여 30세 이상 50세 이하의 중장년층 남성을 대상으로 뇌 백질과 회백질의 손상 유무를 파악할 수 있는 확산텐서영상을 검사하여 영상을 획득한 후 Tract-Based Spatial Statics(TBSS)방법으로 뇌 회백질 부위의 해마 영역 부위 신경섬유로의 비등방도 FA(fractional anisotropy)값을 측정 분석한 결과 모든 영역에서 음주 시작연령이 낮을수록 비등방성 측정값이 낮게 관찰되었지만 FA 값은 통계적으로 유의하였다. 본 연구에서 측정한 FA 결과 값이 나타내는 연구결과는 즉, 음주 시작연령이 빠를수록 뇌 회백질의 해마 영역의 모든 신경학적이고 해부학적인 뇌 영역의 세부적인 형태학적 변화와 신경섬유조직에 심각하게 영향을 준다고 할 수 있으며 신경섬유로를 위해 및 손상시키고 알코올과 관련된 기능적 형태학적 변이에 영향을 끼친다고 할 수 있다.

Adherence to Health-Related Lifestyle Behavior Recommendations and Association with Quality of Life among Cancer Survivors and Age-Matched Controls in Koreaaa

  • Background: This study was conducted to determine the relationship between lifestyle behavior and quality of life (QoL) among cancer survivors in Korea. Materials and Methods: Data for a total of 471 (173 men, 298 women) cancer survivors (CS) over 40 years old were obtained from the database of the $4^{th}$ Korean National Health and Nutrition Examination Survey (KNHANES IV). An identical number of subjects of the same age, sex, and education who had no restrictions in physical activity were randomly selected from the database and represented the control group (CG). Drinking, smoking, and exercise behavior were assessed. Results: The number of heavy drinkers was lower in CS (9.4%) than in CG (15.8%) (p<0.01); similarly, there were fewer smokers in CS (9.1%) than in CG (14.0%) (p<0.05). The percentage of individuals engaging in vigorous, moderate, and low-intensity exercise did not differ between CS (13.6%, 14.7%, and 50.0%) and CG (14.3%, 13.4%, and 49.7%, respectively). No differences in Euro QoL Questionnaire 5-Dimensional Classification (EQ-5D) scores on both drinking and smoking behaviors were noted. Compared to the non-exercisers, the low-intensity exercisers in CG ($0.91{\pm}0.10$ vs. $0.94{\pm}0.09$), vigorous-intensity exercisers in CS ($0.84{\pm}0.62$ vs. $0.91{\pm}0.11$), and low-intensity exercisers in CS ($0.82{\pm}0.22$ vs. $0.88{\pm}0.13$) scored higher on the EQ-5D. Conclusions: Although cancer survivors practiced more conscious health behavior in drinking and smoking, their engagement in exercise did not differ from that of non-cancer survivors. Since exercise engagement increases QoL in general, implementation of an educational program that promotes exercise engagement in cancer survivors may be required.

음주시작연령과 BMI의 연관성에 대한 융복합 연구: 성별 차이를 중심으로 (A convergence study on sex differences in association between age at drinking onset and BMI)

  • 본 연구는 성별에 따른 음주시작연령과 BMI 간의 상관관계를 파악하여 청소년기 음주행태와 성인기의 건강과의 관계에 대해 고찰하고자 한다. 본 연구는 2019년 지역사회건강조사를 활용하여 206,987명의 연구대상자를 대상으로 이분형 로지스틱 회귀분석을 실시하였다. 연구결과, 음주시작연령이 성인기 이전인 경우 성인 남성의 BMI가 비정상일 오즈값이 통계적으로 유의미하게 높았다. 이러한 연구결과는 남성의 높은 알코올 소비량과 음주에 대한 성별에 따른 차별적인 음주문화 때문인 것으로 설명할 수 있다. 본 연구는 성인기 이전의 조기음주가 야기할 수 있는 한시적인 어려움이 아닌 장기적인 건강 부작용 측면을 다룸으로써 자발적인 문제성 음주를 예방의 필요성을 강조하였다.

전통주 이용 실태 및 활성화 방안 (The Use of Korean Traditional Liquors and Plan for Encouraging It)

  • The purpose of this study was to examine the use of different sorts of Korean traditional liquor among male and female adults in Seoul and Gyeonggi province, their awareness of them and their preference in an effort to discuss what problems Korean traditional liquors were faced with and how they could gain popularity among people in general. First, It is investigated of drinking frequency by gender, 51.3 percent of the male adults drunk once or twice a week, and 33.2 percent of the female adults drunk once or twice a month. Thus, the men drunk more often than the women. By age, the adults who were in their 20s and 30s were far different from those who were in their 50s above in drinking frequency. Second, regarding what kind of liquor they enjoyed, the men enjoyed Soju the most, followed by beer and traditional liquors. The women enjoyed beer the most, followed by Soju and wine. The favorite liquor of the men was Soju, followed by beer and traditional liquor, and the women most liked for beer, followed by wine and Soju. The female adults preferred low-proof liquor more than the male adults. Third, concerning their awareness of traditional liquor, Andong-soju was most widely viewed as traditional liquor, which were followed by Munbaeju, Gyeongju-gyodongbeopju, Gyeongju-beopju, Gukhwaju, Ssal- makgeolri, and Geumsan-insamju. Overall, they were rarely aware what traditional liquor was. Fourth, as to purchase experience, the men and the older people had more experience to buy traditional drinks than the women and the younger ones. Fifth, as for anju (dishes for traditional drinks), they believed that panfried food and Kimchi should be served with coarse liquor. Panfried and streamed dishes were considered to be good complements to Takju (rice wine) and Cheongju (clear strained rice wine), and pot stew and soup were looked upon as good complements to distilled liquor. The above-mentioned findings illustrated that in order to step up the development of the traditional liquor industry, perpetual research efforts should be put into adding new tastes to unique traditional liquor drinks. And it's required to commercialize those drinks, and multiple P.R. and marketing strategies should be prepared to promote their sales.

일개 사업장 근로자들의 한방건강검진에서 체성분검사를 통한 체지방률과 복부비만 결과 분석 (A Study on the Percentage of Body Fat and Abdominal Obesity of Workers Using Bioelectrical Impedance Analysis)

  • Objectives: Obesity is chronic condition which can severely influence health. People with a high percentage of body fat (%BF) have high prevalence of hypertension and hypercholesterolemia. Abdominal obesity (AO) seems to play a major role in the development of cardiovascular disease, diabetes and hyperlipidemia. This study investigated the %BF and AO of workers in the automobile manufacturing industry. Methods: The subjects were categorized by sex, age, job class, drinking history and smoking history. %BF and AO were measured by using bioelectrical impedance analysis (Jawon Medical, Seoul, Korea). We analyzed the relationship between age, sex, job class, drinking history, smoking history and %BF and WHR into frequencies, $X^2$-test using the SPSS Ver. 12.0. Results: Regarding the relationship between sex and %BF, AO, there was statistically significant difference as men had a significant higher level of %BF and AO than women. There was a positive correlation between age and both %BF and AO. Workers in their 40s especially ran level of AO up. In job class, office workers were apt to have a higherlevel of %BF and AO than production line workers. %BF and AO of the non-drinker group showed a lower level than the drinker group. On the other hand, the ex-smoker group had higher levels of %BF and AO than the non-smoker or current-smoker groups. Conclusions: The above results showed that men, the advanced in years, office workers, drinkers and ex-smokers had high levels of %BF and AO.

커피음용이 단기간 근육작용 활성화에 미치는 영향 (Effects of Coffee on Activation of Muscular Working During a Short Period)

  • The purpose of this study was to verify the effects on fine motor, muscle strength (hand-grip), explosive muscular strength (vertical jump) and muscular endurance (sit-up) after ingesting coffee. Hence this study targeted a sample group of 38 healthy men in their twenties, without cardiac disorders and muscle disorders and none of them were hypersensitive to caffeine with symptoms such as palpitation and dyspnea. Nineteen of them ingested coffee, whereas the other nineteen men ingested decaffeinated coffee at the same amount. The amount of coffee was controlled by weight so as to regulate intake to 6 mg caffeine per kg. Research material was evaluated through O'Conner's finger dexterity test, hand-grip strength test, vertical jump test and sit-up test. The data were analyzed by means of paired t-test and ANCOVA. The material was then, analyzed by means of two-way ANOVA in order to verify the effect of one or two cups of coffee on fine motor and hand-grip strength. All parameters were measured by an independent observer. The results were as follows: There were no significant differences to fine motor, hand-grip strength, vertical jump, sit-up before and after drinking decaffeinated coffee, but there was a difference to those before and after drinking caffeine coffee. There was no significant difference to hand dexterity and hand-grip strength in one or two cups of coffee. Intake of a certain dosage of caffeine effects activation of muscles working in a short period based on the results. As a consequence, intake of certain dosages of caffeine was beneficial to enhance efficiency of activity during exercises, but it is will be difficult to obtain the desired result with only one or two cups of coffee.
