Enhanced Recovery after Cancer Surgery using Dokhwaljihwang-tang-gami : A Case Series (독활지황탕가미를 이용한 암 수술 후 조기 회복 : 연속 증례 보고)

  • Objectives: Fatigue, malnutrition, and weight loss are common complaints after undergoing surgery for cancer. These symptoms can be associated with Yin-Deficiency (陰虛) of Exhaustion syndrome (虛勞). Therefore, this study aimed to determine if it is possible to use Dokhwaljihwang-tang, which originates from Yukmijihwang-tang, for enhanced recovery after cancer surgery. Methods: After cancer surgery, Dokhwaljihwang-tang-gami was administered three times a day, in 80-120 ml doses at a time. Results: In four cases of gastric, colorectal, and thyroid cancer, enhanced recovery was achieved using Dokhwaljihwang-tang-gami to treat general weakness, fatigue, and pain in the surgical site after cancer surgery. Conclusions: Dokhwaljihwang-tang can be used for enhanced recovery after cancer surgery.

A Case of Persistent Hiccup Healed by Soyangin Herbal Medicines (소양인 연속성 딸꾹질 환자의 치험례)

  • 1. Objectives Hiccup is one of common symptoms clinically and caused by various etiologies. Persistent hiccup is defined as hiccup lasting more than 48 hours. We have tried researching effect of herbal medicines(Dokhwaljihwang-tang, Yangkyuksanhwa-tang). 2. Methods To heal the persistent hiccup, we used Dokhwaljihwang-tang and Yangkyuksanhwa-tang. 3. Results A Soyangin patient diagnosed to have persistent hiccup was, treated with Soyangin herbal medicines (Dokhwaljihwang-tang and Yangkyuksanhwa-tang), and change of his symptom & general condition was managed. 4. Conclusions The constitutional treatment with Soyangin herbal medicines(Dokhwaljihwang-tang, Yangkyuksanhwa-tang) may have an effect on management of persistent hiccup. Further study on management of this disease is needed.

  • Objectives The purpose of this study is to give an account of the effects of the Traditional Korean herbal medicine therapy which is based on the Sasang constitutional medicine on psoriasis Methods We diagnosed her as Soyangin(少陽人) based on her nature and emotion, physical chracteristics, symptoms. We thought Leaky Gut Syndrome is related to Soyangin Food Stagnation, stuffiness and fullness(食滯痞滿). So we applied Dokhwaljihwang-tang(獨活地黃湯) which is used to Soyangin Food Stagnation, stuffiness and fullness because her symptoms recurred as a result of Food Stagnation(食滯) after eating meat. The improvement of the skin condition was observed by Psoriasis Area and Severity Index (PASI) score Results Psoriasis patient who was treated with Soyangin Dokhwaljihwang-tang showed improvement in Psoriasis symptom. This case study describes the effectivenss on psoriasis symptoms by using Soyangin Dokhwaljihwang-tang. Conclusions The PASI score of the patient reduced from 10.8 to 1.2 after three months of treatment. According to the result, we concluded that Dokhwaljihwang-tang had a good effect on psoriasis.

A Case Study of Polyarthritis Treated with Dokhwaljihwang-tang (독활지황탕으로 호전된 다발성 관절염 환자 치험 1례)

  • Objectives The purpose of this study is to report a case of improving various symptoms such as polyarthritis, dyspnea, fatigue, heat sensation. In addition, the promising explanation on the perspective of the heat and cold syndrome could be elicited. Methods Dokhwaljihwang-tang was prescribed for an outpatient for 3 weeks without any other therapy such as acupuncture, moxibustion and Western medicine. To evaluate the results of this treatment, polyarthritis was assessed by Visual Analogue Scale(VAS). The patient's general condition such as digestion and defecation discomfort is assessed by the progress notes. Results&Conclusions Most of the various symptoms have improved by about 80 percent. It is more important to observe defecation, urine, cold symptoms and condition of hands and feet synthetically.

A Cancer Patient Suffering from Chemotherapy-Induced Anorexia and Nausea Treated by Herbal Medicine (Dokhwaljihwang-tang-gamibang) Resulted in Improved Quality of Life: A Case Report (항암치료 후 발생한 식욕부진, 오심에 대하여 독활지황탕 투여 후 삶의 질 개선 증례 보고 1례)

  • Despite a considerable improvement in antiemetic chemotherapy and the use of antiemetic prophylaxis for decades, there are still numerous cancer patients suffering from anorexia and nausea after chemotherapy, which forces them to postpone or refuse treatment. This case report is about a 66-year-old female patient diagnosed with malignant lymphoma of the small intestine who underwent right hemicolectomy. After the tumor and the colon adjacent to it were surgically removed, the patient continued to undergo chemotherapy. Chemotherapy-induced anorexia and nausea appeared but were well treated with administration of Dokhwaljihwang-tang-gamibang. During the treatment, no adverse effect related to this Korean medicine was reported. The outcome was evaluated by the functional assessment of anorexia/cachexia therapy (FAACT); the total score increased from 85 to 130 within 12 days, and the symptoms were alleviated. This improvement might suggest the feasibility of Dokhwaljihwang-tang-gamibang to help cancer patients suffering from chemotherapy-induced anorexia and nausea.

  • Objectives This study reports a case of rectal cancer patient treated with Dokhwaljihwang-tang. Methods The patient's subjective symptoms such as pain, sleeping difficulty, frequent defecation and dysuria were observed and the change of the quality of life(QOL) was evaluated using Functional Assessment of Cancer-Colon (FACT-C). Results After the treatment, symptoms such as pain, sleep disorder, defecation, and urination difficulty ameliorated and the score of the FACT-C improved. Conclusions A rectal cancer patient suffering from adverse consequences from the low anterior resection and the pain induced by the metastases showed the improvement of general condition and the quality of life after treated with Dokhwaljihwang-tang.

  • 1. Objectives This case study describes a successful treatment process of a Soyangin patient with somatoform autonomic dysfunction symptoms using Soyangin therapeutic measures, including administration of Dokhwaljihwang-tang (獨活地黃湯) and Sibyimijihwang-tang (十二味地黃湯). 2. Methods The patient was treated with several constitutionally discriminated medicinal agents (獨活地黃湯, 十二味地黃湯) and acupunctural therapy accompanied by simultaneous western medical management. The visual analogue scale (VAS) was applied to assess the severity of flushing. 3. Results and Conclusions As flushing was one of the chief complaints, the patient was treated with Soyangin therapeutic measures including Dokhwaljihwang-tang and Sibyimijihwang-tang. The patient responded positively to the therapy, but further studies are anticipated for more definitive conclusions.

A Study on Personality and Ordinary Symptoms according to Soyangin's Major Constitutional Medicine (소양인(少陽人) 주요(主要) 처방(處方) 간의 성격(性格) 및 소증(素證) 차이(差異)에 관한 연구(硏究))

  • Objectives The purpose of this study was to survey the Personality and Ordinary Symptoms of Soyangin and to compare the tendency of their personality and symptoms according to Soyangin's major prescription. Methods 331 Soyangin's personality and ordinary symptoms data registered to SCIB(Sasang Constitutional Information Bank) were used. All data were analyzed with Chi square test. Some data were ranked and analyzed with Kruskal-Wallis test and Mann-Whitney test(with Bonferroni correction). Chi square and Kruskal-Wallis test's significant P value was 0.05. Mann-Whitney test's significant P value was 0.005. Results & Conclusions In Chi square test, there is difference in personality and symptoms in Soyangin's major prescription as followed, the degree of masculinity and femininity, the degree of patience, the condition of stool, the degree of appetite, good or bad in digesting, amount of meal, the speed of eating, amount of drinking water, the temperature of drinking water, coldness and hotness in abdomen or not. In Kruskal-Wallis test and Mann-Whitney test, Yanggyuksanhwa-tang is more masculine than Dokhwaljihwang-tang, Hyeongbangsabaek-san, Hyeongbangjihwangtang. Dokhwaljihwang-tang is less appetite than Yanggyuksanhwa-tang and Hyeongbangsabaek-san. Yanggyuksanhwa- tang is more fast in eating than Hyeongbangjihwang-tang.

The Effects of Dokhwaljihwang-tang Intravenous Pharmacopuncture on Cisplatin-Induced Emesis and Gastrointestinal Mobility Disorder in Rats

  • Objectives : The objective of this study was to evaluate the effect of Dokhwaljihwang-tang (DJT) intravenous pharmacopuncture on cisplatin-induced emesis and gastric mobility disorder in Wistar rats. Methods : Thirty rats were randomly divided into six groups and cisplatin was administered to all groups except the normal group. The cisplatin group (n=5) received a cisplatin injection only. The saline group (n=5) was injected with cisplatin followed by 0.4 mL of saline. Groups DJT-1, DJT-2, and DJT-3 were injected with cisplatin, followed by 0.315 g/kg, 0.104 g/kg, and 0.034 g/kg of DJT, respectively. Body weight, food intake, and kaolin intake of rats were measured 12 h, 24 h, and 36 h after cisplatin injection. Residual food in the stomach was measured 48 h after cisplatin injection. Results : There was no significant difference in weight. The food intake was not significantly different 12 h after cisplatin administration. All groups except the normal group showed significantly decreased food intake after 24 h. After 36 h, food intake was not significantly different between groups DJT-1, DJT-2, and DJT-3 and the normal group. The kaolin intake of groups DJT-1 and DJT-2 was significantly decreased at 12 h and 24 h after cisplatin injection. Kaolin intake and residual food in the stomach were significantly decreased in groups DJT-1, DJT-2, and DJT-3. Conclusion : In a Wistar rat model, DJT intravenous pharmacopuncture is suggested to be effective for cisplatin-induced emesis and gastric motility disorder. In the future, it is necessary to study the mechanism and chemical composition of each individual constitutive drug.

Research on the Usage Frequency of Sasang Constitutional Herbal Formula with Case Record Form (증례기록지를 이용한 체질처방 사용빈도 조사)

  • Objectives : The aim of this study was to survey the usage frequency of Sasang constitutional herbal formula in the clinlcal field. Methods : We searched the case report forms from 2031 subjects in the 11 oriental hospitals and 10 oriental clinic. Subjects were diagnosed by the Sasang Constitution specialist in oriental hospital, who had remarkable improvement in their chief complain after Sasang Constitution medication. We counted the usage frequency of each constitutional herbal formula. Results and Conclusions : In TE type, Cheongsimnyeonja-tang, Yeoldahanso-tang, Jowiseungcheong-tang, Taeeumjowi-tang were used in high frequency. In SE type, various herbal formula were used in a similar frequency. In SY type, Yanggyeoksanhwa-tang, Hyeongbangjihwang-tang, Dokhwaljihwang-tang were used in high frequency. Additionally, the use of specific herbal formula was concentrated in SY type.
