A Case of Postural Orthostatic Tachycardia Syndrome Showing Nonspecific Chronic Dizziness (비특이적 만성 어지럼증을 보인 기립성빈맥증후군 1예)

  • Postural orthostatic tachycardia syndrome (POTS) is characterized by increased heart rate with preserved blood pressure on orthostatic stress. Many patients with postural orthostatic tachycardia syndrome can be misdiagnosed as neurosis, chronic fatigue or anxiety disorder. We report a patient with POTS who presented chronic dizziness and fatigue. In approaching to a patient with orthostatic or nonspecific chronic dizziness, the postural heart rate as well as blood pressure should be checked not to miss the diagnosis.

Bal Mong Ja Acupuncture Treatment of Sudden Sensory Hearing Loss with Tinnitus and Dizziness (영추 발몽자를 이용한 돌발성 난청의 치험 1례)

  • Objectives : The purpose of this study is to report clinical effects of korean medical treatment based on Bal Mong Ja on a patient with sudden sensory hearing loss. Methods : In this study, we used acupuncture based on Bal Mong Ja on a patient with sudden sensory hearing loss. We also carried out treatments like herbal medication and moxibustion therapy etc. The progress of symptoms were evaluated by THI(Tinnitus Handicap Index), DHI(Dizziness Handicap Inventory), VAS(Visual Analogue Scale) once a week. Results : After 7 weeks, there were significant improvements of auditory acuity and THI, DHI, VAS. Conclusions : Acupuncture based on Bal Mong Ja is effective for relieving symptoms of sudden sensory hearing loss with tinnitus and dizziness.

  • Aircraft accidents caused by dizziness of aviation workers are so common that they account for 5% to 10% of the total, and in most cases, have fatal consequences. Therefore, it is essential to diagnose and treat the dizziness of aviation workers properly. Common diseases caused by abnormal vestibular function include benign paroxysmal positional vertigo, Meniere's disease, and vestibular neuritis. In this paper, we first proposed an overview of these diseases' pathophysiology, diagnosis, and treatment. Also, we evaluated the importance of these diseases in aerospace medicine and presented aerospace medical dispositions for aviation medical examiners.

Acute Dizziness during Treatment of Trigeminal Neuralgia Improved after Administration of Jaeumkunbi-tang: A Case Report (삼차신경통 치료 중 발생한 급성 현훈이 자음건비탕(滋陰健脾湯) 복용 후 호전된 환자 증례 1례)

  • Objectives: This study is aimed to describe the effects of using a traditional Korean herbal medicine, Jaeumkunbi-tang, on acute dizziness and gait disturbance that arose during treatment for trigeminal neuralgia. Methods: We closely observed one female patient who had been hospitalized complaining of trigeminal neuralgia and acute onset of dizziness. The patient was treated using Korean medical treatments, such as Oyaksungi-san, Jaeumkunbi-tang, acupuncture, and moxibustion. We evaluated the patient's condition using the visual analog scale (VAS) at the Department of Hepato-Hemopoietic System, Kyung Hee University Korean Medicine Hospital in June 2019 for 12 days. Results: The patient's dizziness was decreased after treating with Jaeumkunbi-tang for five days as measured by the VAS score. Conclusions: Jaeumkunbi-tang appears to be effective for controlling dizziness.

  • Dizziness is one of common diseases clinically, it is defined as a hallucination or an illusion of motion that causes sensation disorder of circumstance, and described as circulatory. rotatory leaning. shaking sensation. In particular, benign paroximal positional vertigo(BPPV) is one of peripheral vertigo, it causes dizziness due to debris which has collected within a part of the inner ear. Chemically, debris are small crystals of calcium carbonate. They are derived from structures in the ear called 'otoliths' that have been damaged by head injury, infection, or other disorder of the inner ear, or degenerated because of advanced age. The symptoms of BPPV include dizziness or vertigo, lightheadedness, imbalance, and nausea, Activities which bring on symptoms will vary in each person, but symptom are almost always precipitated by a position change of the head or body. As for treatment of vertigo, it is differentiated as excess in the upper and deficiency in the lower(上實下虛) and treated in oriental medicine and are used to stability. antihistaminics . anticolinergics . vestibule control drug of GABA system in western medicine. Moreover, Dix-hallpike maneuver is applicated in diagnosis and treatment of BPPV patients. A case of dizziness patient suggested benign paroximal positional vertigo who is diagnosed as weakly dizziness(虛暈)showed prominent improvement by Jaeumkunbitang-gamibang(滋陰建脾湯) and Dix-hallpike maneuver.

  • In general, dizziness is caused by various situations, but among them, symptoms due to dysfunction of the motor system belonging to the nervous system are the most severe, accompanied by nausea and vomiting. Treatment of these dizziness includes drug therapy, surgical therapy, and rehabilitation. Drug therapy and surgery are generally performed in vest rehabilitation training, which is a rehabilitation therapy because of the risk of aftereffects. The vestibular rehabilitation training includes eye training, posture stabilization training, and walking training. Among them, walking training is performed in a certain space under the supervision of a doctor or a professional therapist, so that the time and space burden is increased. In order to solve this problem, we implemented gait training contents which can be used for rehabilitation training by using the augmented reality technology. It is expected that it can be utilized as dizziness rehabilitative contents which can be used in medical environment through clinical tests for patients with dizziness.

Laboratory Management Status of Vestibular Function Test in Korea (국내 전정기능검사실의 운영 현황)

  • Objectives: This study was conducted to assess the current management status of the vestibular function test laboratories in Korea. Methods: Questionnaire about the management status of the vestibular function test laboratories was sent by email to the entire members of the Korean Balance Society. The contents of questionnaire included situation of employees who perform the tests, the types of vestibular function tests and equipment, frequency of the test and types of dizziness related questionnaires. Results: Forty-nine hospitals and clinics responded. All the 49 respondents answered that they have videonystagmography. Spontaneous nystagmus analysis by videonystagmogrphy was the most frequently tests for patients with dizziness. Questionnaires for dizziness were used by 27 respondents (55.1%) for initial evaluation of the dizziness patients. The Korean version of dizziness handicap inventory was the most frequently used dizziness related questionnaire. Conclusions: We analyzed the current management status of vestibular function test laboratories to comprehend the present condition of the vestibular function test. We think that these results will help to provide a standard for laboratory operations and prepare for the education, focusing on high-demand tests.

A Case of Delayed Facial Palsy and Dizziness in Miller-Fisher Syndrome (Miller-Fisher 증후군에서 나타난 지연형 안면마비 및 현훈 치험 1례)

  • Objective : Miller-Fisher syndrome(MFS) is a type of acute inflammatory polyneuropathy, a disease characterized by abnormal muscle coordination, absence of tendon reflexes, and paralysis of the eye muscle. This study is to report a case of delayed facial palsy and dizziness in Miller-Fisher syndrome, treated with traditional Korean medicine. Methods : A 57-year-old male was diagnosed with Miller-Fisher syndrome accompanied by neurological symptoms such as right facial palsy and dizziness. He received traditional Korean medicine treatment such as acupuncture(including pharmacopuncture, needle embedding therapy, electroacupuncture) and herbal medicine for 17 days. Results : Each neurological symptoms were improved after traditional Korean medicine. In case of facial palsy, House Brackmann Grading System was reduced form grade 3 to grade 2, and Sunnybrook Scale was improved from 39 to 55. Also, other symptoms such as dizziness and headache was also improved. Conclusions : This case report suggests that traditional Korean medicine may have a role in treating neurological symptoms such as facial palsy and dizziness in patient diagnosed with Miller-Fisher syndrome.

A Study of the Differences in Subjective Visual Vertical Between the Elderly and Young Adults and Balance, Dizziness, and Gait Changes (노인과 젊은 성인의 주관적 시수직의 차이와 그에 따른 균형, 어지럼증 및 보행 변화 연구)

  • Purpose: Balance and gait dysfunction caused by aging affect elderly individuals' independent life, which, in turn, can reduce their overall quality of life. The purpose of this study is to compare the differences in the vestibular function of healthy elderly and young adults based on the subjective visual vertical (SVV) test as well as to compare and analyze the gait ability between these two groups to study the differences and association between vestibular, dizziness, and balance ability. Methods: The subjects were 18 young and 16 elderly adults with no neurological or musculoskeletal damage. To evaluate vestibular function, a subjective visual vertical test was performed. To evaluate the gait function, the step time, step length, stride length, stance phase ratio, and swing phase ratio were measured. Balance was evaluated using the Berg Balance Scale (BBS), and dizziness was evaluated using a dizziness handicap inventory (DHI). Results: There were significant differences in the SVV, BBS, and DHI between the young and elderly adults (p < 0.05). The gait variables of the older adults were all significantly different (except for the swing phase ratio) than those of the young adults (p < 0.05). As the result of correlation analysis, the SVV values of the young adults showed a significant negative correlation with step length and stride length (p < 0.05), while the SVV values of the elderly adults only showed a significant positive correlation with the DHI (p < 0.05). Conclusion: The elderly appeared to show a decrease in vestibular function when compared to the young adults, and it is thought that walking and balance function declined, while dizziness increased. Moreover, it is believed that these results can be used as basic data for vestibular rehabilitation in the future.

A Case Report of Dizziness with Postural Hypotension Improved by Korean Medicine Treatment (한의 치료로 호전된 기립성 저혈압을 동반한 어지럼증 환자 치험 1례)

  • Background: Postural hypotension refers to a drop in systolic blood pressure of 20 mmHg or diastolic blood pressure of 10 mmHg or more within three minutes after standing up from lying down. Symptoms of postural hypotension not only include dizziness and blurred vision but also fatigue, cognitive decline, leg weakness, headache, and, in severe cases, fainting. Postural hypotension is a phenomenon that occurs in about 6% of the total population, and in Korea, the number of patients with postural hypotension is continuously increasing. Both pharmacological and non-drug therapies, which are treatment methods for this disease, do not show a satisfactory symptom improvement effect. Case summary: A 65-year-old male patient who visited the hospital complaining of dizziness and paresthesia due to postural hypotension was treated with acupuncture, moxibustion, and herbal medicine. For symptom evaluation, the numeric rating scale (NRS) of each symptom was used, and blood pressure change according to posture was measured. After treatment, the NRSs of dizziness and dysesthesia were decreased, and blood pressure changes according to posture was improved. Conclusion: This case report suggests that Korean medicine treatment, including acupuncture, electro-acupuncture, moxibustion, and herbal medicine, can be an effective treatment for dizziness with postural hypotension.
