다 기관 설문 조사를 통한 알레르기 질환의 연령별 임상 양상 (Clinical manifestations patterns of allergic disease in Korean children under the age of 6 : multi-center study)

  • 목 적 : 알레르기 질환이 알레르기 행진의 임상 양상을 보이며 이를 이해하는 것이 예방에 중요 한 것은 널리 알려진 사실이다. 그러나 국내에서 알레르기 진행에 대한보고는 미미 한 실정이다. 방 법 : 2006년 5월 1일부터 30일 까지 서울에 위치한 4개 대학병원의 소아 알레르기 클리닉을 방문한 6세 미만 환아의 보호자들을 대상으로 설문 조사를 시행하였다. 결 과 : 총 229명의 환아 중 여자는 122명 남자는 107명이였다. 2세 이하에서는 아토피피부염(79%)이 가장 흔했고 뒤이어 천식(13%)과 알레르기비염(13%)의 순이었다. 2-4세에서는 알레르기비염(38%), 천식(37%), 아토피피부염(19%)의 순이었다. 4-6세에서는 천식(72%)이 가장 흔했으며 알레르기비염(64%)과 아토피피부염(21%)의 순이었다. 73%에서 가족력이 있었고 이중 아버지 알레르기 병력이 가장 흔했다. 알레르기비염을 가진 환아의 50%에서 천식, 30%에서 아토피피부염을 경험하였고 천식을 가진 환아의 57%가 과거에 아토피피부염을 가지는 알레르기 행진을 볼 수 있었다. 알레르기 질환의 심각도 조사에서 부모들은 영아기에는 천식을, 유아기에는 아토피피부염을 가장 심각한 알레르기 질환으로 생각하였다. 결 론 : 국내에서 6세 미만의 소아에서 알레르기 행진이 진행한다는 것을 알 수 있었다.

  • 본 연구는 정신장애인의 정신질환 발병 이후 생애경험을 '생애사적 진행과정구조'에 주목하여 재구성함으로써 발병이후 위기의 진행과 전환의 과정을 이해하고자 한다. 정신장애인 5명이 연구에 참여했으며, 수집된 자료는 Sch$\ddot{u}$tze의 생애사적 이야기식 인터뷰(narrative interview)분석에 의하여 이루어졌고 위기 상황의 전환을 중심으로 '발병과 혼란단계'와 '집중적 치료 재활단계로' 나누어 재구성하였다. 전자는 '질환에 압도당함, 고통스런 삶의 반복과 감내, 가족으로부터의 분리', 후자는 '정신보건법 제정과 지역사회정신보건서비스의 확대, 사회적 역할변화를 통한 자신의 재발견, 생애사적 잠재성을 깨닫게 해준 중요한 타인, 지역사회통합의 장애물 경험, 가족관계의 재통합, 발병이전의 삶에 대한 고찰을 통한 질병경험의 재해석, 미래에 대한 기대'라는 주제로 분석했다. 그리고 두 단계를 전체적으로 비교분석하였다.

치매(痴?)에 대한 문헌적(文獻的) 고찰(考察) (A Bibliographical Study on Dementia)

  • This thesis, deduced from studying eastern and western medical records, deals with geriatric demedtia in modern society. The result were obtained as follows : 1. Dementia is a kind of chronic, progressive, degenerative disease. The chief expression and pathogenic change of the disease is organic: e.g., extensive change such as cerebrum - atrophy, and denaturalization result. in such a situation intellectual capacities and the ability to enjoy daily life deteriorate trenendously. 2. A basic internal cause of the disease is Defficiencies of the heart, liver and kidneys. An exterior cause is an Excessiveness of the 'Dam'(痰), 'Blood Stasis', 'Fung'(風) and 'Fire'. In a Western Medical view, the reason for dementia is due to the onset of Alzheimer's disease and Brain anemia resulting from Multi - infarction or some other reason. If the white - matter of the brain is injured, then dementia easily to results. 3. Disease symptoms result in troubles in intellectual functions : e.g., memory, orientation, intelligence, judgement, common sence and calculating abilities. 4. The proper therapeutic treatment depends on the causes. When the Deficiency is serious, Fortification (heart, liver. and kidney deficiency) is applied and Decrease is follow. When Excessiveness of wrong is serious, the Decrease is tried before the supplement measure is used depending on the deficiency, which generally is used together with 'Fortify Right - Decreace Wrong'. 5. If the disease wasn't caused by some mental reason, it's difficult to be cured of the disease. When the degree of the disease is light and it doesn't continue for a long time, the therapeutic treatment can block the disease's progress and improve the patient's symptoms.

알츠하이머병 치매 환자의 인지재활: 현실감각훈련(ROT)의 적용과 효과 (Improving Cognitive Abilities for People with Alzheimer's Disease: Application and Effect of Reality Orientation Therapy (ROT))

  • Healthcare providers in Korea are using conservative pharmacological treatment for Alzheimer's disease (AD) to delay the progress of the disease or to mitigate its behavioral and neurological symptoms. However, there is a growing need for interventions using practical non-pharmacologic treatment, as the effects of pharmacological treatments has faced limitations. This research provided a cognitive rehabilitation program to 3 AD patients and used a multiple baseline design across subjects to examine the effects. Performing reality orientation therapy (ROT) for 1 cycle (4 weeks) resulted in a slight increase in accuracy and responsiveness on an orientation task, mainly with patients with mild cases of AD. Also, in the sub-domain of the Korean-Mini Mental Status Examination performed to examine changes in cognitive ability, there were minimal changes in place orientation. In functional communication, however, there were no significant differences before and after the intervention. In conclusion, we found that ROT was an effective intervention for improving accuracy and responsiveness in the orientation of patients with mild cases of AD. In future studies, the effect of non-pharmacological interventions can be evaluated more reliably by examining the interaction effects of sample size, length of the intervention, outcome measurements, and pharmacological intervention.

알츠하이머병의 최신지견 (Recent Advances in Diagnosis and Treatment of Alzheimer's Disease)

  • Alzheimer's disease (AD) is a neurodegenerative disorder in which neuronal loss causes cognitive decline and other neuropsychiatric problems. It can be diagnosed based on history, examination, and appropriate objective assessments, using standard criteria such as the Diagnostic and Statistical Manual of Mental Disorders or the National Institute of Neurological and Communicative Disorders and Stroke and the Alzheimer's Disease and Related Disorders Association (NINCDS-ADRDA). Brain imaging and biomarkers are making progress in the differential diagnoses among the different disorders. The cholinesterase inhibitors, donepezil, rivastigmine and galantamine and N-methyl-D-aspartate receptors antagonist memantine are approved by the US Food and Drug Administration for AD. Recently some acetylcholinesterase inhibitors gained approval for the treatment of severe AD and became available in a higher dose formulation or a patch formulation. Optimal care in AD is multifactorial and it should include early diagnosis and multidisciplinary care with pharmacological and nonpharmacological interventions including exercise interventions, cognitive interventions and maintenance of social networks.

『상한론(傷寒論)』 변병진단체계(辨病診斷體系)에 근거하여 회역탕(回逆湯) 투여 후 호전된 한포진 1례 (A Case Report of Dyshidrotic Eczema treated by Hoiyeok-tang based on Shanghanlun Provisions)

  • Objective : The purpose of this study is to report the improvement of patient with Dyshidrotic Eczema treated by herbal medication based on Shanghanlun disease pattern identification diagnostic system. Methods : According to 'Disease Pattern Identification Diagnostic System based on Shanghanlun Provisions', the patient diagnosed as 'Jueyinbing Huoluan' number 388 provision and took herbal medication Hoiyeok-tang for 28days. The changing symptom progress of Dyshidrotic Eczema was evaluated by DLQI(Dermatology Life Quality Index) and VAS(Visual Analogue Scale). Results : The DLQI score changed 24 to 0 and VAS changed 8 to 0. Conclusions : Patient who suffered from Dyshidrotic Eczema caused by concentrating on sophisticated work using her eyes and hands was treated by Hoiyeok-tang in Shanghanlun. Skin disease on hands or feet can be closely related to conception of 手足厥 in Shanghanlun. Further studies are needed to validate the conception of 手足厥 and the effects of Hoiyeok-tang.

Single-cell and spatial transcriptomics approaches of cardiovascular development and disease

  • Recent advancements in the resolution and throughput of single-cell analyses, including single-cell RNA sequencing (scRNA-seq), have achieved significant progress in biomedical research in the last decade. These techniques have been used to understand cellular heterogeneity by identifying many rare and novel cell types and characterizing subpopulations of cells that make up organs and tissues. Analysis across various datasets can elucidate temporal patterning in gene expression and developmental cues and is also employed to examine the response of cells to acute injury, damage, or disruption. Specifically, scRNA-seq and spatially resolved transcriptomics have been used to describe the identity of novel or rare cell subpopulations and transcriptional variations that are related to normal and pathological conditions in mammalian models and human tissues. These applications have critically contributed to advance basic cardiovascular research in the past decade by identifying novel cell types implicated in development and disease. In this review, we describe current scRNA-seq technologies and how current scRNA-seq and spatial transcriptomic (ST) techniques have advanced our understanding of cardiovascular development and disease.

Congenital heart disease in the newborn requiring early intervention

  • Although antenatal diagnostic technique has considerably improved, precise detection and proper management of the neonate with congenital heart disease (CHD) is always a great concern to pediatricians. Congenital cardiac malformations vary from benign to serious conditions such as complete transposition of the great arteries (TGA), critical pulmonary and aortic valvular stenosis/atresia, hypoplastic left heart syndrome (HLHS), obstructed total anomalous pulmonary venous return (TAPVR), which the baby needs immediate diagnosis and management for survival. Unfortunately, these life threatening heart diseases may not have obvious evidence early after birth, most of the clinical and physical findings are nonspecific and vague, which makes the diagnosis difficult. High index of suspicion and astute acumen are essential to decision making. When patent ductus arteriosus (PDA) is opened Widely, many serious malformations may not be noticed easily in the early life, but would progress as severe acidosis/shock/cyanosis or even death as PDA constricts after few hours to days. Ductus dependent congenital cardiac lesions can be divided into the ductus dependent systemic or pulmonary disease, but physiologically quite different from each other and treatment strategy has to be tailored to the clinical status and cardiac malformations. Inevitably early presentation is often regarded as a medical emergency. Differential diagnosis with inborn error metabolic disorders, neonatal sepsis, persistent pulmonary hypertension of the newborn (PPHN) and other pulmonary conditions are necessary. Urgent identification of the newborn at such high risk requires timely referral to a pediatric cardiologist, and timely intervention is the key in reducing mortality and morbidity. This following review deals with the clinical presentations, investigative modalities and approach to management of congenital cardiac malformations presenting in the early life.

A Binary Classifier Using Fully Connected Neural Network for Alzheimer's Disease Classification

  • Early-stage diagnosis of Alzheimer's Disease (AD) from Cognitively Normal (CN) patients is crucial because treatment at an early stage of AD can prevent further progress in the AD's severity in the future. Recently, computer-aided diagnosis using magnetic resonance image (MRI) has shown better performance in the classification of AD. However, these methods use a traditional machine learning algorithm that requires supervision and uses a combination of many complicated processes. In recent research, the performance of deep neural networks has outperformed the traditional machine learning algorithms. The ability to learn from the data and extract features on its own makes the neural networks less prone to errors. In this paper, a dense neural network is designed for binary classification of Alzheimer's disease. To create a classifier with better results, we studied result of different activation functions in the prediction. We obtained results from 5-folds validations with combinations of different activation functions and compared with each other, and the one with the best validation score is used to classify the test data. In this experiment, features used to train the model are obtained from the ADNI database after processing them using FreeSurfer software. For 5-folds validation, two groups: AD and CN are classified. The proposed DNN obtained better accuracy than the traditional machine learning algorithms and the compared previous studies for AD vs. CN, AD vs. Mild Cognitive Impairment (MCI), and MCI vs. CN classifications, respectively. This neural network is robust and better.

  • Allergic contact dermatitis (ACD) is an allergen-specific T-cell-mediated inflammatory response, albeit with unclear pathogenesis. Exosomes are nanoscale extracellular vesicles secreted by several cell types and widely distributed in various biological fluids. Exosomes affect the occurrence and development of ACD through immunoregulation among other ways. Nevertheless, the role of exosomes in ACD warrants further clarification. This review examines the progress of research into exosomes and their involvement in the pathogenesis, diagnosis, and treatment of ACD and provides ideas for exploring new diagnostic and treatment methods for this disease.
