The Applicability of the United States' Chronic Disease Self-Management Program (CDSMP) to Korean Adults (미국 만성질환자가관리프로그램(CDSMP)의 성공 사례와 국내 적용가능성)

  • Objectives: The current study reviews the implementation and evaluation of the Chronic Disease Self-Management Program (CDSMP) in the United States (U.S.) to illustrate the program's potential contribution to improving health among Korean adults with chronic conditions while saving healthcare costs. Methods: This study examines existing literature on the history, theoretical background, essential elements, and delivery outcomes of CDSMP with special focus on the successes and challenges to be faced in the implementation of CDSMP to Koreans with chronic conditions. Results: CDSMP is designed to empower people with chronic conditions to develop skills necessary for medical, social role, and emotional management of chronic conditions. Recent studies show the utility of CDSMP in achieving the Triple Aim health reform goals (i.e., better care, better health, better value). Lessons learned from the U.S. experience emphasize the importance of establishing evidence-based studies, collaborating with community partners, and diversifying funding sources to make CDSMP more sustainable. Conclusion: The current study demonstrates the replicability of CDSMP and potential for expansion in Korea. More concerted efforts among academia, government, and communities are needed to deliver CDSMP to Korean adults and identify its effectiveness within the Korean context in terms of meeting the Triple Aim goals of better care, better health, and better value.

Blood Lipid Levels, Nutrient Intakes and Health-Related Lifestyles of Industrial Male Workers According to Apolipoprotein E Polymorphisms (Apolipoprotein E 다형성에 따른 사업장 근로자의 혈중 지질농도, 영양소 섭취 및 건강관련 생활습관)

  • The purpose of this study was to investigate the association among nutrient intakes and health-related lifestyles with cardiovascular disease risk assessed by blood lipid profile according to Apolipoprotein E genotypes. Middle-aged industrial male workers who had completed their annual medical examination were recruited and data of 675 subjects who finished the nutrient survey were used in the analysis. Anthropometric parameters, dietary assessment (FFQ), health-related lifestyles and blood profiles were used for statistical analyses. Apo E genotype groups were classified into the following three genotypes: Apo E2 group (including E2/E2, E2/E3, E2/E4), Apo E3 group (including E3/E3), Apo E4 group (including E3/E4, E4/E4). The frequency of Apo E2, E3, and E4 allele were 13.3%, 75.0% and 11.7% respectively. There were no significant differences in the anthropometric parameters depending on different Apo E genotypes. Also, no significant differences in the nutrient intakes were found according to the genotype groups. The nutrient intakes of all subjects were similar to or higher than the level of KDRIs (Dietary Reference Intakes For Koreans) except for intakes of calcium (67.44% of KDRIs), vitamin A (73.83% of KDRIs) and vitamin $B_2$ (78.02% of KDRIs). Also, there were no significant differences of health-related lifestyles according to Apo E genotype groups. As for the lipid profiles, Apo E4 group had significantly higher total and LDL-cholesterol concentrations than the Apo E2 group (p < 0.05). We confirmed that plasma total and LDL-cholesterol concentrations were greatly influenced by Apo E genotypes. However, nutrient intakes and health-related lifestyles were not associated with Apo E genotypes.

Imported Parasitic Diseases (해외유입 기생충 감염질환)

  • Recently, peoples of travelling to endemic area of parasitit diseases are rapidly increased and the imported parasitic diseases by tourists have become a public health problem. Here author describess briefly about the imported parasitic in Korea. The 15 kinds of parasitic diseases, I.e., malaria, babesiosis, cutaneous leishmaniasis, visceral leishmaniasis, ancylostomiasis, cutaneous larva migrans, angiostrongylosis, gnathostomiasis,loiasis, heterophyiasis, urinary schistosomiasis, hydatis disease, pentastomiasis, cutaneous myiasis and syngamosis were imported during last thirty years. The most prevalent imported parasitic disease was malaria. Indigenous vivax malaria has been eradicated since 1970s. However imported malaria(1970~1985) was reported 107 cases of patient with a history of travel abroad. Futhermore a case of reemerging vivax malaria was patient were occurred in 2000.These parasitic disease are sometimes overlooked or misdiagnosed. There is a need to concern about travel medicine and imported parasitic diseases.

Prevalence of Zoonotic Trematode Metacercariae in Freshwater Fish from Gangwon-do, Korea

  • The infection status of zoonotic trematode metacercariae was investigated in a total of 2,293 freshwater fish collected from 11 rivers or streams in 9 administrative regions of Gangwon-do, Korea for 5 years (2009-2013). All fish were collected by netting methods and examined using the artificial digestion methods. Clonorchis sinensis metacercariae were detected in 4 fish species, i.e., Pungtungia herzi, Squalidus japonicus coreanus, Acheilognathus rhombeus, and Ladislabia taczanowskii, from only Hantangang in Cheorwon-gun. Metagonimus spp. metacercariae were found in 1,154 (50.3%) fish and their average number per infected fish was 55.8. Among the positive fish species, especially Tribolodon hakonensis from Namdaecheon in Yangyang-gun and Plecoglossus altivelis from Osipcheon in Samcheok-si were most heavily infected. Centrocestus armatus metacercariae were detected in 611 (26.7%) fish and the average metacercarial burden per infected fish was 1,032. Two chub species, Zacco platypus and Zacco temminckii were highly and heavily infected with C. armatus metacercariae in almost all regions surveyed. Echinostoma spp. metacercariae were also found in 24 fish from a few localities, but their numbers per fish infected were very low. From the above results, it is confirmed that the metacercariae of intestinal flukes, especially Metagonimus spp. and C. armatus, were heavily infected, while C. sinensis metacercariae were rarely found in fish from Gangwon-do, Korea.

Pathology and virus distribution in the lymphoid tissues of chicks co-infection with H9N2 Avian influenza and Newcastle disease virus (저병원성 조류인플루엔자와 뉴캐슬 바이러스의 복합감염에 따른 닭 림프조직 병변의 특성 및 바이러스 검출)

  • Low pathogenic avian influenza (LPAI; H9N2) and Newcastle disease (ND) are economically important poultry diseases in Korea. In this study, we investigated pathological features and virus distribution in the lymphoid tissues of chicks experimentally infected with H9N2 and/or ND virus. Six-weeks-old SPF chickens were divided into 4 groups, Control (C), H9N2 (E1), NDV (E2), and H9N2+NDV (E3). E1 group was challenged with 0.1 ml A/Kr/Ck/01310/01 (H9N2) $10^{5.6}$ $EID_{50}$ intranasally, E2 group was challenged with 0.5 ml KJW (NDV) $10^{5.0}{\sim}10^{6.0}$ $ELD_{50}$ intramuscularly, and E3 group was challenged with H9N2, followed 7 days later by NDV. In histopathological examination, E1 group showed depletion and necrosis in bursa of Fabricius, thymus, cecal tonsil, and spleen, whereas E2 and E3 groups were noted severe lymphocyte depletion and necrosis with destruction of lymphoid organs structures. In TUNEL assay, apoptotic bodies were detected in lymphoid organs of all experimental groups, which was most severe in E3 group. H9N2 and ND viruses were predominantly detected in cecal tonsil of E1, E2, and E3 groups by PCR and immunohistochemistry (ICH). In conclusion, co-infection of H9N2 with NDV caused severe pathologic lesions and apoptosis in lymphoid tissues compared to single infections.

Current Status of Genomic Epidemiology Reseach (유전체 역학연구의 동향)

  • Genomic epidemiology is defined as 'an evoking field of inquiring that uses the systematic application of epidemiologic methods are approaches in population-based studies of the impact of human genetic variation on health and disease (Khoury, 1998)'. Most human diseases are caused by the intricate interaction among environmental exposures and genetic susceptibility factors. Susceptibility genes involved in disease pathogenesis are categorized into two groups: high penetrance genes (i.e., BRAC1, RB, etc.) and lour penetranoe genes (i.e., GSTs, Cyps, XRCC1, ets.), and low penetrance susceptibility genes has the higher priority for epidemiological research due to high population attributable risk. In this paper, the summarized results of the association study between single nucleotide polymorphisms (SNPs) and breast cancer in Korea were introduced and the international trends of genomic epidemiology research were reviewed with an emphasis on internee-based case-control and cohort consortium.

Factors Affecting the Quality of Life of Family Caregivers of Patients with Parkinson Disease (파킨슨병 환자 가족의 삶의 질에 미치는 영향요인)

  • Purpose: The purpose of this study was to investigate the factors affecting the quality of life of family caregivers of patients with Parkinson disease in Korea. Methods: A total of 162 family caregivers of patients with Parkinson disease who were visiting the D hospital in B city completed questionnaires, including general and disease-specific characteristics, family burden, patient-caregiver relationship satisfaction, family support and quality of life. Data were analyzed with t-test, ANOVA with $Scheff{\acute{e}}$ test, Pearson's correlation coefficient, and multiple regression analysis using SPSS 23.0. Results: The average score of the quality of life was $85.54{\pm}17.74$ (range: 26~130) among family caregivers of patients with Parkinson disease. Stepwise multiple regression analysis revealed that fifty percent of the quality of life was explained by family support, family burden, and patient-caregiver relationship satisfaction. Conclusion: Family support, family burden, and patient-caregiver relationship satisfaction were identified as factors affecting the quality of life of family caregivers of patients with Parkinson disease. Therefore, it is necessary to develop a program to enhance family support and reduce family burden to improve the quality of life of family members caring for patients with Parkinson disease.

Improvement of Memory Impairment by L-Theanine Through Inhibition of Acetylcholinesterase Activity in Mice (마우스에서 L-Theanine의 기억력 회복능 및 Acetylcholinesterase 활성 억제)

  • Acetylcholinesterase (AChE) plays a role in the progression of Alzheimer's disease (AD). In this study, we examined the improving effect of L-theanine, a major amino acid in Japanese green tea (Camellia sinensis) on the scopolamine (1 mg/kg/mouse)-induced memory dysfunction in mice. Treatment with L-theanine (2, 4 mg/kg/mouse p.o.) in the drinking water for 7 days reversed the scopolamine-induced latency time and distance in the water maze test, latency time in the passive avoidance test, and inhibited AChE activity. This study suggests that L-theanine may be a useful agent for prevention of progression of AD.

Analysis of Three Years of Airmen Medical Certificate for Preparing Aviation-related Health Promotion Plan (항공종사자 건강증진활동계획에 반영되어야 할 근거자료: 3년간의 항공신체검사 결과 분석)

  • In accordance with ICAO SARPs, the states should implement appropriate aviation-related health promotion for airmen subject to an aviation medical examination to reduce future medical risks to flight safety. We aimed to analyze the results of the aviation medical examination to use as objective evidence for establishing an aviationrelated health promotion plan. The results of the final issuance of airmen medical certificate from 2015~2017 were collected from the e-sky computer system. The 30 cases (0.3%) in 2015, 22 cases (0.2%) in 2016, 38 cases (0.3%) in 2017 were unfit for flight duty. The 34 unfit out of the total 90 cases were renewal physical examinations. The cardiovascular disease was most commonly in 11 cases, followed by 8 tumors, 4 psychiatric diseases, 3 ophthalmic diseases, 2 respiratory diseases, 2 neurologic diseases, 2 otolaryngological diseases, 1 endocrine disease and 1 digestive disease. The results of this study can be used as objective data in aviation-related health promotion plan. The health promotion activities based on objective data can contribute to improving aviation safety by improving the lifestyles of airmen.
