Erectile Dysfunction in Men With Adult Congenital Heart Disease: A Prevalent but Neglected Issue

  • Background and Objectives: For adult men with congenital heart disease (ACHD), data on erectile dysfunction (ED) is limited. We aimed to assess the frequency of ED, its role in patient-physician communication and to identify parameters predicting ED. Methods: Male ACHD ≥18 years registered at the German National Register for Congenital Heart Defects were invited to participate in an online questionnaire about sexual health. Participants with presumed ED according to International Index of Erectile Function Score were compared to patients without ED. Results: The 371 patients responded to the questionnaire (83% with moderate to highly complex ACHD). The 43% presented with more than mild ED. When ED was present, patients complained about general anxiety to be sexually active more often (p<0.05) and underwent sexual activity less frequently compared to those without ED (p<0.05). Age ≥40 years (odds ratio [OR], 3.04; p=0.002), being single (OR, 6.82; p<0.0001), anxiety to be sexually active (OR, 2.64; p=0.0002) and psychiatric disease (OR, 4.33; p<0.0007) emerged as independent predictors for ED. Overall, patients sought medical advice in 6.7% of cases, whilst 29.6% would appreciate an active approach by the physician to address this sensitive topic. Conclusions: ED is affecting one third to one half of male ACHD according to a questionnaire-based analysis. Older age, being single, fear of sexual activity due to ACHD and psychiatric disorder emerged as independent predictors for ED. These parameters can easily be assessed to identify patients at risk. ED should be addressed proactively by health professionals.

  • Objective: Patients with chronic obstructive pulmonary disease (COPD) may experience reduced physical activity and quality of life (QoL) due to decreased pulmonary function. The purpose of this study was to investigate the level of pulmonary function, physical activity, and QoL of COPD patients. Design: Cross-sectional observational study. Methods: This study examined the published data of the Korea National Health and Nutrition Examination Survey in 2015-2019. Among 39,759 subjects who participated for 5 years, data from 151 patients diagnosed with COPD were analyzed separately. For the pulmonary function, the results of forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), FEV6, forced expiratory flow 25-75%, and peak expiratory flow were observed. Physical activity was identified as frequency and duration. For the QoL, EQ-5D-3L evaluation results were examined, and the frequency and index of the Korean version were investigated. Results: In pulmonary function, all variables were found to be lower than age and weighted matched normal values. COPD patients showed to perform very low levels of high/medium physical activity and sitting time was confirmed to be more than 8 hours a day. In QoL, it was found that the highest reporting rate of some problems was the "pain and discomfort" and "mobility". Conclusions: It was found that COPD patients showed that the prevalence of circulatory disease was relatively high, lowered pulmonary function, and QoL. These can be improved through regular physical activity, and it is thought that this can be achieved through optimization of pulmonary rehabilitation.

A Case Study of a Crohn's Disease Patient Treated with Oriental Medical Treatment (한방 치료로 호전된 크론병 치험 1예)

  • Objectives : The purpose of this study is to report the effect of Oriental medical treatment on a patient with Crohn's disease. Methods : The patient was treated with acupuncture, herb remedy and moxibustion for about 2 weeks. We evaluated the results of the treatment by observing the patient's symptoms and through the Crohn's disease activity index(CDAI) score. Results : After Oriental medical treatment the patient's symptoms such as emesis, heartburn and abdominal pain considerably reduced. The patients CDAI score decreased from 190.8 to 134.2. Conclusions : These results support that Oriental medical treatment can be a help in relieving symptoms of Crohn's disease.

  • This investigation was undertaken to determine the relationship between the amount of polymorphonuclear leukocyte(PMN) enzyme myeloperoxidase(MPO) in gingival crevicular fluid(GCF) collected from active or control site and gingival disease status described by clinical indices(gingival index, papillary bleeding index, pocket depth, periotron unit). The results were as follows : 1. MPO activity/site was greater at active sites than at control sites. 2. According to increasing the clinical parameters, MPO/sites was higher statistically (P<0. 01, P<0.05). 3. High MPO(unit/site) groups was higher statistically than low MPO(unit/site) groups in various clinical parameters. 4. Correlation coefficients between MPO(unit/site) and GI, MPO($unit/{\mu}l$ GCF) and periotron unit were 0.4782, -0.5901, respectively.

Effects of an Individual Health Counseling Program for Community: Health One-stop Service Program (지역사회 대상의 개인별 건강상담서비스 프로그램 중재 효과: 건강원스톱서비스 사업)

  • Objectives: The aim of the study was to identify the effects of a community-level individual health counseling program for community. Methods: Data included baseline demographics, blood pressure, blood sugar, waist circumference, total cholesterol and health behavior index(body mass index, dietary practice guidelines score, physical activity, high-risk drinking) collected at public health centers in Chungnam province from January to September, 2011. Data obtained from the individual health counseling program in Chungnam province were analyzed using Wilcoxon Signed Rank Test and McNemar Test. Results: After the individual health counseling intervention, the results of health measurement index; systolic blood pressure, diastolic blood pressure, total cholesterol, waist circumference decreased in the health risk group, while total cholesterol and waist circumference decreased in the disease management group. Health behavior change in both groups. Body mass index, moderate physical activity, dietary practice guidelines scores were improved. Conclusions: These results indicate that the individual health counseling program for community was effective in improving health behaviors and status. The results demonstrate that step-by-step counseling program development and intervention studies are needed.

Effect of Hwangyeonhaedok-tang on Experimental Mouse Colitis Induced by Dextran Sulfate Sodium (황련해독탕(黃連解毒湯)이 Dextran Sulfate Sodium 유도 마우스 대장염에 미치는 영향)

  • Objectives : Hwangyeonhaedok-tang(HHDT) has been traditionally used for various clinical symptoms associated with gastrointestinal disorder, cardiovascular diseases, and inflammation in the Oriental medicine. However, little is known for antioxidant and anti-inflammatory effects of HHDT on dextran-sulfate sodium(DSS)-induced colitis in mice. Methods : In this study, we investigated an antioxidant and anti-inflammatory effects of HHDT on DSS-induced colitis in mice. An experimental colitis was induced by daily treatment with 5% DSS. HHDT was orally administrated the various concentrations(25-100 mg/kg, body weight/day) for 7 days with one time per day. Results : HHDT reduced significantly clinical sign of DSS-induced colitis, including body weight loss, shorten colon length, disease activity index(DAI), and histological colon injury. HHDT also inhibited significantly serum NO and prostaglandine $E_2(PGE_2)$ productions in DSS-induced colitis mice. Furthermore, HDDT increased significantly an superoxide anion(SOD), catalase, and glutathione peroxidase(GPx) activity of the colon tissue in DSS-induced colitis mice. Conclusions : These results suggest that HHDT administration could reduce significantly the clinical signs and inflammatory mediators, and increase antioxidant activity in DSS-induced colitis model mice and is a good candidate for further evaluation as an effective anti-ulcerative agent.

Pulsatilla koreana Ameliorates Ddextran Sulfate Sodium-induced Ccolitis in Mice

  • Ulcerative colitis (UC) is an inflammatory bowel disease, which is a chronic gastrointestinal disorder. Pulsatilla koreana (P. koreana) is a perennial plant that grows around Korea and it has various pharmacological effects such as anti-cancer and anti-inflammatory activity. However, the regulatory effects of P. koreana in intestinal inflammation are not yet understood. This study attempted to determine the effect of P. koreana in dextran sulfate sodium (DSS)-induced colitis in mice. The colitis mice were induced by drinking water containing 5% DSS for 7 days. The results showed that mice treated with DSS showed remarkable clinical signs, including weight loss, and reduced colon length. Administration of P. koreana attenuated DSS-induced the weight loss, colon shortening and Disease activity index in mice. Additionally, P. koreana inhibited the cyclooxygenase-2 and prostaglandin $E_2$ levels in DSS-treated colon tissues. These results provide experimental evidence that P. koreana might be a useful therapeutic medicine for patients with UC.

Cognitive Impairment in the Patients with Mildly Active Systemic Lupus Erythematosus (경증 전신성 홍반성 루프스 환자의 인지기능장애)

  • This study was designed to determine whether cognitive impairment was evident in patients with SLE. Also, it aimed to examine the association of cognitive impairment with other clinical variables. The subjects consisted of 20 patients with mildly active SLE and 20 healthy controls. Methods : A total of 20 SLE patients and 20 normal controls completed a computerized neuropsychological test battery using Vienna Test System. These included Cognitrone test, Continuous attention test, Corsi block tapping test, Standard progressive matrices. Also, neuro-behavioral cognitive status examination was done. The symptom severity of depression was measured with Beck Depression Inventory, Hamilton Depression Rating Scale, and current medications were documented. Disease activity was rated using the SLE diasease activity index (SLEDAI). Results : SLE patients had poorer performance than normal controls on the tests of Cognitrone, attention, nonverbal IQ and memory, independent of age, education, disease activity, steroid use and depression status. Conclusion : Cognitive dysfunction was not uncommon in ambulatory SLE patients as measured by standardized neuropsychological tests. It seemed to occur independently of various clinical variables. These findings would suggest that cognitive dysfunction in SLE may be explained by reflecting subclinical central nervous system(CNS) involvement, rather than coexisting psychological distress due to chronic illness or side effect of medication.

Association between soluble forms of the receptor for advanced glycation end products and periodontal disease: a retrospective study

  • Objectives: Periodontitis is the most common chronic disease that causes tooth loss and is related to systemic diseases such as cardiovascular disease and diabetes. An objective indicator of the current activity of periodontitis is necessary. Soluble forms of the receptor for advanced glycation end products (sRAGE) are markers that reflect the status of inflammatory diseases. In this study, the relationship between sRAGE and periodontitis was analyzed to determine whether it can be used to diagnose the current state of periodontitis. Patients and Methods: Eighty-four patients without any systemic diseases were diagnosed with periodontitis using three classifications of periodontitis. Demographics and oral examination data such as plaque index (PI), bleeding on probing (BOP) index, and probing pocket depth (PPD) were analyzed according to each classification. In addition, correlation and partial correlation between sRAGE and the values indicating periodontitis were analyzed. Results: In each classification, the level of sRAGE tended to decrease if periodontitis was present or severe, but this change was not statistically significant. sRAGE and periodontitis-related variables exhibited a weak correlation, among which the BOP index showed a relatively strong negative correlation (ρ=-0.20). Based on this, on analyzing the correlation between the BOP index and sRAGE in the group with more severe periodontitis (PPD≥5 mm group, severe group of AAP/CDC [American Academy of Periodontology/Centers for Disease Control and Prevention], periodontitis group of López), the correlation further increased (ρ=-0.23, -0.40, -0.50). Partial correlation analysis of the sRAGE and BOP index showed a stronger negative correlation (ρ=-0.36, -0.55, -0.45). Conclusion: sRAGE demonstrated a tendency to decrease upon increased severity of periodontitis according to the classifications used. Above all, the correlation with the BOP index, which reflects the current state of periodontitis, was higher in the group with severe periodontitis. This indicates that the current status of periodontitis can be diagnosed through sRAGE.

Adult Physical Activity and Health Related Quality of Life : National Big Data Utilization (7th National Health and Nutrition Survey) (성인의 신체활동과 건강관련 삶의 질 : 국가빅데이터를 중심으로)

  • The purpose of this study was to investigate the relationship between physical activity and health-related quality of life of adults using the 7th National Health and Nutrition Survey. The study was conducted with 11,211 adults, and the health-related quality of life was defined using the EuroQol group's EQ-5D and physical activity using GPAQ. Data analysis was performed using the SAS 9.4 program, the general characteristics and degree of physical activity of the subject, Chi-square for KEQ-5D index, and Logistic Regression Analysis for the relationship between physical activity and quality of life. As a result of the study, the general characteristics of the subjects were marital status, educational status, occupation, smoking, alcohol consumption, economic status, stress, chronic disease, chronic disease treatment, physical activity due to leisure and physical activity due to occupation, depending on gender. There was a difference (p<0.05). As for the quality of life related to physical activity and health, the quality of life was significantly lower by 37% in the 'minimum physical activity group' of occupational physical activity (p<0.05). The results of this study are expected to be provided as basic data for physical activity-related health policy establishment and physical activity programs.
